Purpose In the randomized phase III trial CeTeG/NOA-09, temozolomide (TMZ)/lomustine (CCNU) combination therapy was superior to TMZ in newly diagnosed MGMT methylated glioblastoma, albeit reporting more frequent hematotoxicity. Here, we analyze high grade hematotoxicity and its prognostic relevance in the trial population. Methods Descriptive and comparative analysis of hematotoxicity adverse events ≥ grade 3 (HAE) according to the Common Terminology of Clinical Adverse Events, version 4.0 was performed. The association of HAE with survival was assessed in a landmark analysis. Logistic regression analysis was performed to predict HAE during the concomitant phase of chemotherapy. Results HAE occurred in 36.4% and 28.6% of patients under CCNU/TMZ and TMZ treatment, respectively. The median onset of the first HAE was during concomitant chemotherapy (i.e. first CCNU/TMZ course or daily TMZ therapy), and 42.9% of patients with HAE receiving further courses experienced repeat HAE. Median HAE duration was similar between treatment arms (CCNU/TMZ 11.5; TMZ 13 days). Chemotherapy was more often discontinued due to HAE in CCNU/TMZ than in TMZ (19.7 vs. 6.3%, p = 0.036). The occurrence of HAE was not associated with survival differences (p = 0.76). Regression analysis confirmed older age (OR 1.08) and female sex (OR 2.47), but not treatment arm, as predictors of HAE. Conclusion Older age and female sex are associated with higher incidence of HAE. Although occurrence of HAE was not associated with shorter survival, reliable prediction of patients at risk might be beneficial to allow optimal management of therapy and allocation of supportive measures. Background The CeTeG/NOA-09 trial assessed in a randomized phase 3 setting, whether combined treatment of lomustine together with temozolomide was superior to temozolomide treatment alone in newly diagnosed MGMT (O(6)-methylguanine-DNA-methyltransferase) promoter methylated glioblastoma patients. Survival was significantly improved from 31.4 months with temozolomide alone to 48.1 months with the combination of lomustine plus temozolomide. In view of this encouraging data - suggesting this combination could have a significant impact on the survival of newly diagnosed glioblastoma patients - we were curious to assess safety and efficacy of this regimen under real-life conditions. Material and Methods We collected clinical and radiographic data from adult newly diagnosed MGMT promoter methylated IDH (isocitrate dehydrogenase) wildtype glioblastoma patients treated at five neuro-oncology centers in Germany. As a requirement for inclusion, first-line treatment with lomustine plus temozolomide had to be performed for at least six weeks (one course). The available radiographic data was independently reviewed by an experienced board-certified neuro-radiologist. Results In total, 70 patients were included. Median progression-free survival of the full cohort was 14.4 months and median overall survival was 36.0 months. Patients who received TTFields (Tumor Treating Fields) treatment for eight weeks or longer together with the combination of lomustine plus temozolomide (n=22, 31%) had a prolonged progression-free survival compared to those patients who received TTFields treatment less than eight weeks or did not receive treatment with TTFields (n=48, 69%) (21.5 months versus 11.2 months; HR: 2.118, 95% CI: 1.245-3.605; p=0.0105). In a multivariable Cox regression analysis the use of TTFields for eight weeks or longer together with the combination of lomustine plus temozolomide as well as the application of at least five courses of CeTeG therapy emerged as independent prognostic factors for progression-free survival and overall survival. Pseudoprogression occurred in n=16 (33%) of the patients. We observed no treatment related deaths and high-grade hematotoxicity in n=31 (44%) of the patients. Conclusion The results from this multicentric trial that investigated newly diagnosed MGMT promoter methylated IDH wildtype glioblastoma under real-life conditions indicate toxicity and survival estimates comparable to the CeTeG/NOA-09 trial. The use of TTFields for at least eight weeks in combination with this regimen was independently associated with extended progression-free and overall survival.

OBJECTIVE: Tumor Treating Fields (TTFields) in combination with adjuvant temozolomide (TMZ) as therapy for newly diagnosed glioblastoma (GBM) patients showed significantly increased progression-free survival (PFS), overall survival (OS) and long-term survival rates in the EF-14 phase III trial. In the trial, patients were randomized after completion of radiochemotherapy, and TTFields therapy was initiated in combination with adjuvant chemotherapy. In preclinical settings, TTFields synergistically enhanced efficacy of radiotherapy in GBM, hypothetically by inhibiting DNA damage repair in irradiated cells. The presented phase I/II trial will evaluate safety and feasibilty of TTFields initiated prior and concomitant to combined radiochemotherapy in newly diagnosed GBM. METHODS: In arm A of this prospective multi-center trial, seven eligible patients with newly diagnosed GBM will be enrolled initially. Provided that treatment is tolerated well, enrollment will continue for up to 20 patients. Patients will be subjected to TTFields after complete wound-healing following surgery. TTFields treatment will be continued throughout radiochemotherapy and adjuvant chemotherapy for six cycles. In total, patients will receive TTFields therapy for approximately nine months. In arm B, elderly patients with a reduced KPS (50 or 60) will be treated with postsurgical TTFields therapy followed by TTFields therapy concomitant to hypofractionated radiotherapy with 40 Gy for three weeks. In line with arm A, TTFields therapy will continue beyond radiotherapy throughout adjuvant chemotherapy for a total of nine months. In the first stage, six patients will be accrued. Under the provision of an acceptable safety profile, seven additional patients will be accrued for a total of thirteen patients. RESULTS: The primary endpoint of the trial is safety and tolerance based on the frequency of a set of predefined treatment-limiting toxicities (TLTs). Secondary endpoints consist in particular of PFS, OS, radiologic response and frequency of adverse events. First experiences will be presented. CONCLUSION: The objective of this trial is to demonstrate that the administration of TTFields therapy prior and concomitant to radiotherapy and adjuvant chemotherapy is feasible and safe. Moreover, first data obtained on efficacy may serve as a basis for a potential randomized phase III trial.

Background: Although it is known that upfront complete surgical resection is associated with improved survival in glioblastoma, studies are lacking to confirm whether this holds true for the unfavorable patient population of O-6-methylguanine-DNA methyltransferase (MGMT) non-methylated glioblastoma. Here, we investigated the association of survival with the extent of resection in the GLARIUS study, a randomized phase II trial of bevacizumab/irinotecan (BEV/IRI) versus standard temozolomide (TMZ) in MGMT non-methylated glioblastoma. Methods: Patients included in the modified intention-to-treat (ITT, n=170) population were stratified by extent of resection (partial vs. complete) as determined by early (>72h) postoperative contrast-enhanced MRI. In a Kaplan-Meier analysis, we compared overall survival (OS) between the groups in each treatment arm. A Cox regression analysis was used to detect whether complete resection was of prognostic value and independent of canonical prognostic markers, including age and Karnofsky performance score (KPS). Results: In the BEV/IRI arm, 58 patients (50%) each underwent partial (PR) and complete resection (CR). In the TMZ arm, 29 patients (55%) underwent partial and 24 (45%) complete resection. Prognostic factors, including age and KPS, were balanced between patients with PR and CR. Neither in the BEV/IRI arm (CR, median OS [mOS], 17.3 [95% CI, 15.5-21.4] versus PR, mOS, 16.5 [95% CI, 14.9-17.9]; Hazard Ratio [HR], 0.79 [95% CI, 0.53-1.20]; p=0.28) nor in the TMZ arm (CR, mOS, 18.0 [95% CI, 17.1-21.3] versus PR, mOS, 15.3 [95% CI, 10.6-21.1]; HR, 0.77 [95% CI, 0.43-1.40]; p=0.40) patients with CR derived a significant OS benefit compared to those with PR. In a multivariable Cox regression analysis, this effect did not change after accounting for canonical prognostic markers. Conclusions: In the GLARIUS trial, patients with complete resection derived no relevant survival benefit as compared with partially resected patients. However, the small sample size, in the TMZ arm in particular, limits our analysis.

Die Therapie des Glioblastoms steht kurz davor, sich grundlegend zu andern.Insbesondere bei der Therapie von uber 70-jahrigen Patienten sind molekulareMarker wie O6-Methylguanin-DNA-Methyltransferase (MGMT) entscheidend dafur, ob eine alleinige Strahlentherapie erfolgt oder eine Behandlung mit Temozolomid (TMZ) alleine- und/oder in Kombination mit Bestrahlung durchgefuhrt wird. Uber die bisherige Standardtherapie hinausgehend zeigen drei randomisierte Studien mit dem Angiogeneseinhibitor Bevacizumab eine Verlangerung der progressionsfreien Zeit, so dass evtl. in nachster Zeit auch eine zusatzliche Bevacizumabtherapie insbesondere bei Patienten mit nichtmethyliertem MGMT-Promotor eingesetzt werden kann. Im Glioblastomrezidiv stehen TMZ-Wiederaufnahme und CCNU-Therapie sowie Reoperation und/oder erneute Bestrahlung zur Verfugung. Auch hier ist der Einsatz von Bevacizumab zu diskutieren.

Neurological complications of therapeutic procedures for brain tumors are increasingly being recognized. These encompass the classic types of central and peripheral neurotoxicity, such as radiotherapy-induced leukoencephalopathy and platinum-induced neuropathy. However, the advent of novel protocols and targeted therapeutics has expanded the spectrum of neurological complications. A problem of considerable importance is pseudoprogression after radiochemotherapy with temozolomide. Among the new targeted drugs complications of therapy with bevacizumab are the subject of intense discussion. In this review article the neurotoxic potential of intrathecal chemotherapy, kinase inhibitors, immunological strategies and local therapies are summarized. Knowledge about neurological complications of brain tumor therapy procedures is important for risk assessment and patient information.

Die Meningeosis neoplastica (MN) ist eine disseminierte ZNS-Metastasierung solider Tumoren und weist eine besonders schlechte Prognose auf. Eine zeitnahe Diagnose ist haufig durch die sehr heterogene neurologische Symptomatik erschwert. Wenngleich bei den betroffenen Patienten haufig gleichzeitig solide parenchymatose Hirnmetastasen bestehen, sind bei Diagnostik und Therapie zahlreiche Besonderheiten zu beachten. Die kontrastmittelgestutzte MRT-Diagnostik, die den Spinalbereich miteinschliest, und die zytologische Diagnostik durch Liquorpunktion sind besonders bedeutsam. Therapeutisch kommen in Abhangigkeit von der Auspragung der MN mit entweder soliden leptomeningealen Raumforderungen oder zirkulierenden nichtadharenten Tumorzellen verschiedene Therapiemodalitaten wie Strahlentherapie, intrathekale und systemische Chemotherapie sowie supportive Therapiemasnahmen in Frage, wobei haufig gleichzeitig bestehende systemische Tumormanifestationen mit in das Therapiekonzept eingebunden werden sollten. Eine Entscheidungsgrundlage bietet hier die Leitlinie „Meningeosis neoplastica“ der Neuroonkologischen Arbeitsgemeinschaft (NOA) der deutschen Krebshilfe.

Therapeutic concepts for malignant gliomas increasingly target the genetically non-transformed tumor stroma rather than the tumor cells themselves. There are two particular compartments of the tumor stroma which are currently tackled: the vascular compartment by using antiangiogenic treatment with the aim of vascular normalization and the immune compartment with the aim of enhancing or inducing anti-tumor immunity. Although the vascular endothelial growth factor (VEGF) A antibody bevacizumab has not been approved for the treatment of malignant glioma in European countries, there is evidence from smaller trials of biological efficacy particularly in recurrent disease and the results of a large European phase III study testing the clinical efficacy are currently expected. Immunotherapies are on the verge of entering the clinical arena with the first randomized phase III clinical trials having already been completed. In these studies, active vaccination and checkpoint inhibitors which are approved for other tumor entities are being tested. This article provides an overview on the current antiangiogenic and immunological therapies for gliomas, summarizes the results of clinical trials and discusses further developments.

In der Behandlung von Hirntumoren kommen Operation, Strahlentherapie und Chemotherapie zum Einsatz. Zur Auswahl der Therapie mit einem sinnvollen Verhaltnis von Nutzen und Risiko fur den Patienten mussen die therapieinduzierten Komplikationen und Nebenwirkungen der einzelnen Behandlungsmodalitaten sowie ihrer Kombination und deren Auswirkung auf die Lebensqualitat der Patienten moglichst genau abgeschatzt werden. Hirntumoroperationen sind Routineeingriffe. Die Operationsmortalitat sowie das Risiko fur Nachblutungen und lokale Infektionen ist gering. Die Rate persistierender, neuer postoperativer Neurodefizite wird v. a. von der Tumorlokalisation bestimmt und mit 13–26% angegeben. Der Einsatz neuer, intraoperativer Technologien soll helfen, postoperative neurologische Ausfalle zu vermeiden. Strahlentherapie ist unter Vermeidung groser Bestrahlungsvolumina und hoher Einzeldosen ein sicheres Behandlungsverfahren mit abschatzbarem Risiko. Neben akuten und subakuten Nebenwirkungen ist besonders die Spattoxizitat in Form von Nekrose und Enzephalopathie von klinischer Bedeutung. Unter Beachtung der Toleranzgrenzen des gesunden Gehirngewebes und Einsatz moderner Bestrahlungstechniken ist das Risiko fur radiogene Spatschaden aber als gering einzustufen. Behandlung mit systemischer Chemotherapie bringt systemische Nebenwirkungen wie Ubelkeit und Erbrechen sowie gelegentlich Therapie limitierende Myelosuppression mit sich. Neben allgemeiner Toxizitat einer systemischen Chemotherapie findet sich oft eine toxische Wirkung auf das zentrale und periphere Nervensystem. Kombination von Chemo- mit Strahlentherapie kann zur Verstarkung und Verschiebung der Nebenwirkungen gegenuber der Monotherapie fuhren.

Primary central nervous system lymphomas (PCNSLs) are increasing in frequency both in immunocompetent and immunodeficient individuals. The majority of PCNSLs are high grade B cell lymphomas. In AIDS patients most tumors contain EBV genome. PCNSLs usually present as intracerebral, often deep-seated lesions half of which are multilocular. Less frequent are diffuse periventricular, exclusively leptomeningeal, ocular or spinal spread. On imaging PCNSLs show as contrast-enhancing lesions with relatively little perifocal edema. CSF protein is usually elevated. Malignant cells are present in 20-30% of cases. Demonstration of a monoclonal B cell population by immunocytology or FACS analysis may also be diagnostic. Once PCNSL is suspected extensive systemic evaluation is not useful. Instead, (stereotactic) biopsy of brain lesion(s) should be performed. Prior to biopsy, corticosteroids should be withheld as they may obscure diagnosis. Symptomatic edema or increased intracranial pressure should therefore initially be treated with osmotherapeutics. All immunodeficient patients should receive empiric anti-toxoplasmosis therapy for about 14 days prior to biopsy. AIDS patients with PCNSL survive 3 to 5 months (median) after whole brain irradiation and usually do not benefit from chemotherapy. Immunocompetent patients have a median survival of 12 to 18 months after whole brain irradiation alone, but a median survival of 33 to 43 months after combined radiochemotherapy using cytostatic drugs which penetrate the blood-brain barrier. Based on these encouraging results current concepts aim to intensify chemotherapy and to reduce or delay radiotherapy in the treatment of immunocompetent patients.

Gliomas account for about 60% of all primary CNS tumors; two-thirds of all gliomas comprise the most malignant form, glioblastoma multiforme, or glioma grade IV. Although much progress has been achieved in the treatment of other solid tumors over the last few decades, the median survival of patients with glioblastoma remains at around 12 mo after standard treatment, which includes bulk resection and irradiation, as well as chemotherapy in some cases (1). Essentially, no patient can expect to survive 5 yr. New treatment modalities like immunotherapy have been applied so far with only limited success (2). With the improvement of methods for in vivo and ex vivo gene delivery, gene therapy became a new, promising approach to glioma therapy. Gliomas appear to be a particularly good target for a gene therapy approach using locally applied vectors, as the growth of gliomas is restricted to the brain. Clinical trials are under way using retrovirus and adenovirus vectors which carry the herpes simplex virus type-1 (HSV-1) thymidine kinase gene (HSV-tk). This gene encodes a prodrug-activating enzyme, which in infected cells converts the nontoxic prodrug, ganciclovir (GCV), to its cytotoxic phosphorylated form (3-5). There is an ever-increasing list of other prodrug-activation systems that showed efficacy in culture and in preclinical studies using rodent glioma models. These include, for example, cytosine deaminase converting 5-fluorocytosine to 5-fluoro-uracil (6), cytochrome P450-2B1 converting cyclophosphamide to phosphoramide mustard (7), deoxycytidine kinase phosphorylating cytosine arabinoside (8), and the Escherichia coli guanine phosphoribosyl transferase (gpt) metabolizing 6-thioxanthine and 6-thioguanine to toxic nucleoside analogs (9). Moreover, gene therapy approaches to brain tumors include the viral transfer of immune-enhancing cytokines, particularly granulocyte/macrophage colony-stimulating factor (10), or antisense to TGF-β to glioma cells (11) used for vaccination purposes. Other approaches use the transfer of genes that modulate angiogenesis (12,13) or are involved in apoptosis like p53 (14). All aforementioned gene-transfer methods use nonreplicative viral vectors.

Seizures complicate the clinical course of80% of patients with low-grade gliomas. Patients with some tumor variants almost always have epilepsy. Diffuse low-grade gliomas (LGG) are believed to cause epilepsy through partial deafferentiation of nearby brain cortex (denervation hypersensitivity). Glioneural tumors may interfere with local neurotransmitter levels and are sometimes associated with structural abnormalities of the brain which may produce seizures. The severity of tumor associated epilepsy varies considerably between patients. Some cases may present with a first seizure. Others suffer from long-standing pharmacoresistant epilepsy. Seizure control rates of70-80% can be expected after complete tumor resections. Patients with drug-resistant epilepsy require a comprehensive preoperative epileptological work-up which may include the placement of subdural (and intraparenchymal) electrodes or intraoperative electrocorticography (ECoG) for the delineation of extratumoral seizure foci. Partial and subtotal tumor resections are helpful in selected cases, i.e. for gliomas involving the insula. In one series, 40% of patients presented for surgery with uncontrolled seizures, i.e. medical therapy alone often fails to control tumor-related epilepsy. Use of the newer (second generation) non-enzyme inducing antiepileptic drugs (non-EIAED) is encouraged since they seem to have lesser interactions with other medications (e.g. chemotherapy). Chemotherapy and irradiation may have some minor beneficial effects on the patients' seizure disorder. Overall 60-70% of patients may experience recurrent epilepsy during long-term follow-up. Recurrent seizures (not infrequently heralding tumor recurrence) after surgery continue to pose significant clinical problems.

Treosulfan is a bifunctional alkylating prodrug with activity against various solid tumors. To improve the outcome for patients with recurrent malignant glioma, we assessed the efficacy of intravenous treosulfan (6-10 g/m2 4-weekly) as salvage therapy for patients with recurrent or progressive glioblastoma (GB, n = 14) or anaplastic astrocytoma (AA, n = 2). All patients had prior involved-field radiotherapy and adjuvant nitrosourea-based chemotherapy. A total of 56 cycles were administered. Tumor responses were assessed radiologically and clinically prior to each cycle. All patients were assessable for toxicity, response and survival. There were no complete or partial responses (CR, PR). Two patients progressed after the first cycle, 14 patients had initially stable disease (SD). Median progression-free survival was 3.25 months for the GB patients. Five patients were progression-free at 6 months (30%), including the 2 AA patients. The 2 AA patients are stable at 22 months. Myelosuppression was the dose-limiting toxicity in this cohort of nitrosourea-pretreated patients. Treosulfan has modest activity in patients with recurrent malignant glioma. Further evaluation of treosulfan in chemonaive malignant glioma patients is warranted.

This study investigates elements of herpes simplex virus type 1 (HSV-1) which influence transgene expression in tetracycline-regulated expression systems.Different HSV-1 mutants were used to infect Vero cells that had been transfected with plasmids containing the luciferase gene under the control of tet-off or tet-on tetracycline-regulation systems.The baseline level of luciferase expression was elevated after infection with HSV-1 mutants lacking one or more immediate early genes encoding transactivating factors: ICP27, ICP4 and ICP0. With the tet-off system, not only was baseline expression elevated, but there was a complete loss of induction upon removal of tet when this regulatory system was brought into the cell by infection with helper virus-free amplicon vectors. Elevation of luciferase expression was also observed upon infection with the same HSV-1 mutants following transfection with a plasmid containing only a CMV minimal promoter driving luciferase (pUHC13-3). Only one HSV mutant (14Hdelta3), which bears a disruption in the transactivation domain of VP16 and is deleted for both ICP4 genes, did not increase baseline luciferase expression after transfection of pUHC13-3. The disregulating effects were dependent on virus dose and were not influenced by treatment with interferon (IFN)-alpha, which suppresses viral gene expression. Additional assays involving cotransfection of pUHC13-3 with a plasmid encoding of the HSV-1 transactivating factor ICP4 revealed that ICP4 was the most potent inducer of gene expression from the tetO/CMV minimal promoter.These results indicate that proteins encoded in the HSV-1 genome, especially the transactivating immediate early gene products (ICP4, ICP27 and ICP0) and the VP16 tegument protein can activate the tetO/ minimal CMV promoter and thereby interfere with the integrity of tetracycline-regulated transgene expression.

The versatility of HSV-1 vectors includes large transgene capacity, selective replication of mutants in dividing cells, and availability of recombinant virus (RV) and plasmid-derived (amplicon) vectors, which can be propagated in a co-dependent, 'piggyback', manner.A replication-defective piggyback vector system was generated in which the amplicon carries either of two genes essential for virus replication, IE2 (ICP27) or IE3 (ICP4), as well as lacZ; the RV is deleted in both these genes, and vector stocks are propagated in cells transfected with one of the complementary genes. In the replication-competent system, the amplicon carries the IE2 and lacZ; the RV had a large deletion in the IE2; and stocks are propagated in untransfected cells. Titers over successive passages, recombination between amplicon and RV, and the structural integrity of vector genomes were evaluated. The replication-competent system was tested for therapeutic efficacy in subcutaneous 9L gliosarcoma tumors in nude mice with activation of ganciclovir via the viral HSV-thymidine kinase gene.Both systems generated high titer amplicon vectors (about 10(7) tu/ml) and amplicon:RV ratios (0.6-3.0). No replication-competent RV was generated in either system. The replication-defective system showed low toxicity and increased packaging efficiency of amplicon vectors, as compared to single mutant RV helper virus. The replication-competent system allowed co-propagation of amplicon and RV; injection into tumors followed by ganciclovir treatment inhibited tumor growth without systemic toxicity.New replication-defective and replication-competent piggyback HSV, vector systems allow gene delivery via amplicon vectors with reduced toxicity and co-propagation of both RV and amplicon vectors in target cells, with effective tumor therapy via focal virus replication and pro-drug activation.

Immunocompetent patients with primary central nervous system lymphoma (PCNSL) present with a median age of 55 years, immunosuppressed patients with a median age of 40 years. They show a broad range of signs and symptoms. Symptoms of increased intracranial pressure and personality change are most frequent, followed in frequency by ataxia and hemiparesis. The median time from onset of symptoms to diagnosis is 3-5 months in immunocompetent patients and 2 months in immunodeficient patients. The time to diagnosis can be considerably longer in patients with slowly developing personality change or fluctuating symptoms due to spontaneous or steroid-induced remission of so-called sentinel lesions. Native CT scans show iso- or hyperdense lesions with homogenous contrast enhancement. T1-weighted MRI scans show hypointense and T2-weighted scans hyperintense lesions. The definitive diagnosis of PCNSL requires biopsy. In some cases, however, the definitive diagnosis may exclusively be made by the demonstration of malignant B-lymphocytes in the cerebrospinal fluid.

In addition to the brain parenchyma and the leptomeninges, primary central nervous system lymphoma (PCNSL) can involve the posterior parts of the eye with ocular lymphoma in up to 20% of patients and lead to systemic dissemination of lymphoma in 7-8% of patients. Ocular lymphoma is diagnosed by slit lamp examination and requires treatment by ocular irradiation. However, the clinical significance of systemic dissemination, which occurs mainly in the end stage of the disease, appears to be disputable. Extensive systemic staging at the time of initial presentation proved to be unrewarding in the vast majority of studies. So far, only 10 patients have been reported with occult systemic lymphoma at time of initial presentation, and systemic lymphoma determined the course of the disease in none of them. Thus, initial staging procedures for patients with biopsy-confirmed PCNSL can be restricted to regular blood tests, including HIV testing, and slit lamp examination of the eye.

We have established a new xenogeneic animal model of leptomeningeal metastasis (LM) by intracisternal inoculation of human CEM T-cell lymphoma into nude rats, and used it to evaluate the anti-lymphoma efficacy of an anti-CD7 ricin A chain immunotoxin (DA7). In vitro incubation with 2 microg/ml DA7 for 72 h inhibited CEM cells by 90% in a trypan blue exclusion assay. To establish its anti-lymphoma activity, one and four days after cisternal inoculation of 10(6) CEM cells, eight animals each were treated cisternally with 10 microg DA7 in 50 microl PBS or sham-treated with 50 microl PBS. Histopathologically, all eight sham-treated and five of eight DA7 treated animals showed typical features of LM with multilayers of tumor cells along the whole subarachnoid space and the ventricular walls, as well as subependymal and diffuse parenchymal tumor cell infiltration. Three DA7 treated animals were free of tumor. Two of these animals were asymptomatic long-term survivors (90 days). The third tumor-free animal suddenly died on day 51. Histology revealed viral myocarditis. Median symptom-free survival was 51 days (range 29-90+ days) in DA7 treated and 34 days (range 29-87 days) in sham-treated animals (p = 0.12, log-rank test). Histologically, no signs of neurotoxicity or systemic toxicity was found. However, DA7 treated animals showed a tendency to a slower weight increase on days 6-28 after tumor cell inoculation. Our results indicate that this model is useful in studying leptomeningeal seeding and intracisternal treatment of lymphoma. The demonstrated anti-tumor effect of DA7 treatment deserves further evaluation especially regarding the application of DA7 in early stages of LM from T-cell lymphoma.

Immunotherapeutic approaches to leptomeningeal metastasis (LM) include the intrathecal application of cytokines such as interleukin-2 (IL-2) and interferon-alpha (IFN-alpha), and lymphokine-activated killer cells (LAK cells). Results in a rodent model of leptomeningeal gliomatosis with intrathecal IL-2 application are discouraging, but some clinical improvement and clearance of neoplastic cells from CSF have been seen in patients with LM from melanoma treated with intrathecal IL-2 alone, and in patients with LM from primary brain tumors and squamous cell carcinoma of the tongue treated with intrathecal LAK cells and IL-2. The neurotoxicity of this therapy, mainly increased intracranial pressure, has been considerable but generally manageable. However, IFN-alpha caused severe neurotoxicity in form of an only partly reversible progressive vegetative state in the majority of patients. Considering the small number of patients treated with IL-2 and LAK cells, its value for the treatment of LM could only be stated by further investigation. In future, the application of recently discovered cytokines such as Fas-ligand, the continuous paracrine cytokine release by genetically modified cells, or vaccination strategies using genetically modified tumor cells might offer new immunotherapeutic approaches in LM.

Animal models of leptomeningeal metastasis (LM) should give insight into pathophysiological mechanisms and allow to evaluate new treatments including their neurotoxicity. Syngeneic models use tumor cells of mouse, rat, rabbit or guinea pig origin. Allogeneic models usually rely on human tumor cells injected into nude mice or rats. A review of the literature revealed 2 (4) different glioma, 3 medulloblastoma, 3 (3) carcinoma, 3 (1) melanoma, 1 rhabdomyosarcoma, 2 (8) leukemia and 2 (2) non-Hodgkin's lymphoma allogeneic (syngeneic) models of LM. These models have been used to study the evolution of LM and to evaluate systemic or intrathecal chemotherapy, intrathecal immunotherapy (interleukin-2, interferon-beta, uncoupled, toxin- or radionuclide-conjugated antibodies), and recently gene therapeutic approaches. On the whole, pathophysiological, therapeutic and neurotoxic findings have been well transferable to the clinical situation. Therefore, it seems rational to preclinically test new treatments in an appropriate animal model of LM before using them in patients.

Brain tumors have an immunoprivileged status which contributes to their refractoriness to treatment. In this study, immune rejection of GL261 glioma tumors in the mouse brain was achieved by subcutaneous vaccination with GM-CSF-transduced glioma cells. Cultured GL261 cells were transduced to secrete murine GM-CSF using a retrovirus vector, then irradiated, and injected subcutaneously into H-2 matched C57BL/6 mice. In prevaccination studies, the median survival time (MST) of animals vaccinated with 5 x 10(4) or 5 x 10(5) GM-CSF-transduced cells 7 days prior to intracranial injection of 10(6) nontransduced, nonirradiated GL261 cells was significantly prolonged by 45-50% compared with animals vaccinated in parallel with nontransduced, irradiated glioma cells. In treatment of established gliomas, the MST of animals, which were treated subcutaneously with 5 X 10(6) irradiated GM-CSF-transduced cells 3 days after intracranial injection of 2 x 10(4) nontransduced cells, was prolonged significantly by 36% compared with animals treated with the same number of nontransduced, irradiated cells or to sham-treated animals. In prevaccination studies, histology of brain tumors 4 days after intracranial tumor cell injection revealed infiltrates of CD8+ lymphocytes and eosinophils, the latter exclusively in animals vaccinated with GM-CSF-transduced cells, Thus, subcutaneous injection of irradiated GM-CSF-transduced glioma cells can induce a potent immune response to intracranial gliomas both as a vaccination against subsequent intracranial glioma cell implantation and for treatment of established intracranial glioma.

Primary central nervous system lymphomas (PCNSLs) are increasing in frequency both in immunocompetent and immunodeficient individuals. The majority of PCNSLs are high grade B cell lymphomas. In AIDS patients most tumors contain EBV genome. PCNSLs usually present as intracerebral, often deep-seated lesions half of which are multilocular. Less frequent are diffuse periventricular, exclusively leptomeningeal, ocular or spinal spread. On imaging PCNSLs show as contrast-enhancing lesions with relatively little perifocal edema. CSF protein is usually elevated. Malignant cells are present in 20-30% of cases. Demonstration of a monoclonal B cell population by immunocytology or FACS analysis may also be diagnostic. Once PCNSL is suspected extensive systemic evaluation is not useful. Instead, (stereotactic) biopsy of brain lesion(s) should be performed. Prior to biopsy, corticosteroids should be withheld as they may obscure diagnosis. Symptomatic edema or increased intracranial pressure should therefore initially be treated with osmotherapeutics. All immunodeficient patients should receive empiric anti-toxoplasmosis therapy for about 14 days prior to biopsy. AIDS patients with PCNSL survive 3 to 5 months (median) after whole brain irradiation and usually do not benefit from chemotherapy. Immunocompetent patients have a median survival of 12 to 18 months after whole brain irradiation alone, but a median survival of 33 to 43 months after combined radiochemotherapy using cytostatic drugs which penetrate the blood-brain barrier. Based on these encouraging results current concepts aim to intensify chemotherapy and to reduce or delay radiotherapy in the treatment of immunocompetent patients.
