5. First-line treatment of advanced non-small-cell lung cancer with docetaxel and cisplatin: A multicenter phase II study

Abstract

Purpose: To evaluate the efficacy and safety of the docetaxelcisplatin combination in patients with advanced non-small-cell lung cancer (NSCLC). Patients and methods: Chemotherapy-naive patients with histologically confirmed, measurable stage IIIB or IV NSCLC, a World Health Organization (WHO) performance status of 0-2 and adequate bone marrow renal, hepatic and cardiac function were eligible for the study. Patients received docetaxel (100 mg/m(2)) as an one-hour infusion on day 1 and cisplatin (80 mg/m(2)) as a 30-min infusion with appropriate hydration on day 2. Granulocyte colony-stimulating factor (G-CSF; 150 mu g/m(2), SC) was given on days 3 to 13. Treatment was repeated every three weeks. Results. Fifty-three patients were enrolled (25 with stage IIIB and 25 with stage IV). One complete and 23 partial responses were observed (overall response rate (OR): 45%; 95% CI: 34.1%-61.8%). The response rate was 57% and 32% in patients with stages IIIB and IV disease (P = NS). The median time to progression was 36 weeks and the median survival 45 weeks; the one-year survival was 48%. Grade 3-4 neutropenia occurred in 23 patients, 15 of whom were hospitalized for neutropenic fever; two patients died of sepsis. Grade 2 neurotoxicity was observed in six patients and grade 3 in five patients; grade 3 fatigue occurred in seven patients, grade 3-4 mucositis in four patients and grade 3-4 diarrhea in six patients. Mild allergic reactions and oedema were observed in five and four patients, respectively. The median dose intensity was 30 mg/m(2)/week for docetaxel and 24 mg/m(2)/week for cisplatin, corresponding to 91% and 89% of the specified protocol doses, respectively. Conclusions: The docetaxel-cisplatin combination is an active regimen in advanced NSCLC, but hematologic toxicity remains high despite the prophylactic use of G-CSE.

Abstract

Background. To evaluate the tolerance and efficacy of the combination of paclitaxel and gemcitabine as salvage treatment in patients with advanced non-small-cell lung cancer (NSCLC). Patients and methods. Forty-nine patients with measurable NSCLC (PS 0-1: 80%; stage IV: 84%) who progressed or failed first-line chemotherapy were enrolled. Prior chemotherapy was cisplatin-based with (n = 20) or without (n = 22) docetaxel and docetaxel-vinorelbine (n = 7). Patients received gemcitabine (900 mg/m(2) i.v.; days 1 and 8) and paclitaxel (175 mg/m(2); day 8) every three weeks; G-CSF (150 mu g/m(2)/day s.c.; days 9-15) was given prophylactically to all patients. Results: One (2%) complete and eight (16%) partial responses were achieved (overall response 18%; 95% CI: 4%-24%); 14 patients (29%) had stable disease and 26 (53%) progressive disease. Six responses were observed in 17 patients who responded to first-line chemotherapy. The median duration of response was seven months, the median TTP eight months and the median survival 11 months. The one-year survival rate was 37%. Grade 3-4 neutropenia occured in six (12%) patients, grade 2-3 neurotoxicity in 16 (32%) and grade 2-3 asthenia in 25 (51%). Other toxicities were mild. Conclusions. The paclitaxel-gemcitabine combination is a well-tolerated and relatively active salvage regimen in patients with NSCLC and it merits further investigation.

Abstract

CASE REPORT A 12-year-old male castrated Domestic Shorthair cat was presented with a 3-month history of dysuria and a right anal sac abscess. A CT scan revealed a large right-sided anal sac mass and likely metastatic disease within the thorax. The mass was surgically resected and a diagnosis of anal sac adenocarcinoma was made on histopathological examination. The disease followed a highly aggressive course despite surgical debulking with lymph node removal and adjuvant chemotherapy. Postmortem histopathological examination showed Wallerian degeneration of the sciatic nerve, but a metastatic lesion to this nerve root or spinal cord could not be confirmed. CLINICAL SIGNIFICANCE Feline anal sac adenocarcinoma is a rare disease and thought to follow a less aggressive course than its canine counterpart. We report a substantial prevalence study in symptomatic pet dogs of important zoonotic parasitic enteric infections. A total of 4526 dogs which had a faecal sample submitted to a diagnostic laboratory in the UK between 2003 and 2005 were included in the study. The most common parasite was Giardiaspp., which was found in 3804526 dogs (8.4, 95 CI 7.6-9.2). Surprisingly, Cryptosporidiumspp. infection was detected in only 294526 (0.6, 95 CI 0.4-0.9). Abstract

Hypercalcaemia in active pulmonary tuberculosis - more often encountered during treatment - is a well known laboratory finding (Abbasi et al. 1979, Need et al. 1980, Kitrou et al. 1982). However the mechanism (s) of this hypercalcaemia has not been elucidated. It has been suggested (Abbasi et al. 1979, Kitrou et al. 1982) that vitamin D overactivity might be involved, as in sarcoidosis (Papapoulos et al. 1979). The finding that serum inorganic phosphorus concentration is increased parallel to calcium (Abbasi et al. 1979, Kitrou et al. 1982) favours this hypothesis. It was found, however, that intestinal calcium absorption decreases significantly during treatment of tuberculosis (Singhellakis et al. 1983). Abstract

High-grade and metastatic canine mast cell tumors carry a guarded prognosis because of their unpredictable biologic behavior. An ideal chemotherapy regime is yet to be established. The aim of this study was to review the efficacy and toxicity of combination vinblastine and toceranib for high-grade and metastatic mast cell tumors. Twenty-eight dogs were categorized with either high-grade, lymph node metastasis or Stage IV disease. Demographics, disease, and treatment variables were compared between categories (Kruskal-Wallis test for continuous data and Fisher's Exact test for categorical data). Survival times and progression-free intervals (PFI) were calculated and compared between groups (log rank test). The PFI was 310 d [95% confidence interval (CI): 155 to 1425] and overall survival was 373 d (95% CI: 226 to 1219). There was no difference between disease categories for PFI ( P = 0.9) or survival ( P = 0.5). The protocol was well-tolerated with increased liver enzyme activity and gastrointestinal toxicity most frequently observed. Abstract

Background: Congenital and inherited myopathies in dogs are faithful models of human muscle diseases and are being recognized with increasing frequency. In fact, canine models of dystrophin deficient muscular dystrophy and X-linked myotubular myopathy are of tremendous value in the translation of new and promising therapies for the treatment of these diseases. We have recently identified a family of Australian Rottweilers in which male puppies were clinically affected with severe muscle weakness and atrophy that resulted in early euthanasia or death. X-linked myotubular myopathy was suspected based on the early and severe clinical presentation and histopathological changes within muscle biopsies. The aim of this study was to determine the genetic basis for myopathy in these dogs and compare and contrast the clinical presentation, histopathology, ultrastructure, and mutation in this family of Rottweiler dogs with the previously described myotubular myopathy in Labrador retrievers., Results: Histopathology, histochemistry, and ultrastructural examination of muscle biopsies from affected Rottweiler puppies were consistent with an X-linked myotubular myopathy. An unusual finding that differed from the previously reported Labradors and similar human cases was the presence of excessive autophagy and prominent autophagic vacuoles. Molecular investigations confirmed a missense mutation in exon 11 of MTM1 that was predicted to result in a non-functional phosphatase activity. Although the clinical presentations and histopathology were similar, the MTM1 p.(Q384P) mutation is different from the p.(N155K) mutation in exon 7 affecting Labrador retrievers with X-linked myotubular myopathy., Conclusions: Here we describe a second pathogenic mutation in MTM1 causing X-linked myotubular myopathy in dogs. Our findings suggest a variety of MTM1 mutations in dogs as seen in human patients. The number of MTM1 mutations resulting in similar severe and progressive clinical myopathy and histopathological changes are likely to increase as canine myopathies are further characterized.

Abstract

An 8-year-old, mixed-breed dog with preputial epitheliotropic lymphoma was initially treated with cyclophosphamide, vincristine, and prednisolone. A short-term partial response was followed by disease progression after 4 weeks. Recombinant human interferon alpha-2a was administered starting at week 7. The interferon therapy resulted in rapid resolution of clinical signs and a 10-week disease-free interval. The lymphoma recurred at 17 weeks and did not respond to rescue chemotherapy. Additional oral lesions were treated with localized radiotherapy followed by increased dosages of interferon. This additional interferon treatment resulted in another 12 weeks of stable disease.

Abstract

This study reports the prevalence of Cryptosporidium, Giardia and Isospora species in cats showing signs of gastrointestinal disease. Records from a United Kingdom commercial diagnostic laboratory between December 2003 and December 2005 were reviewed. Of 1355 cats, Cryptosporidium species oocysts were found in 13 cats (1%), Giardia species trophozoites in 74 (6%), and Isospora felis oocysts in 46 (3%). In a second group of 48 cats, prevalence of Giardia species was 15% using an immunoassay for detection of antigen compared to 4% detected with microscopy. Prevalence of Giardia (9%) and Isospora (9%) species was higher in cats less than 6 months old. Gender and breed did not affect prevalence. There was a trend for Cryptosporidium and Isospora species infections to be detected in late autumn and early winter. Regional differences in prevalence were not detected. None of these organisms show a characteristic pattern of clinical signs. This study demonstrates that enteric protozoal infection is common in domestic cats showing signs of alimentary disease.

Abstract

Questionnaires regarding the perceptions of chemotherapy and its impact on the quality of life (QoL) of their cat were received from owners of 31 cats treated for lymphoma between 2002 and 2006 with COP (cyclophosphamide, vincristine, prednisolone) chemotherapy. The QoL scores prior to the onset of cancer (mean 9.5, range 6-10) were significantly higher than the ratings given after the onset of cancer but before commencement of chemotherapy (mean 3.9, range 1-9.4). The QoL scores during chemotherapy (mean 6.3, range 1-10) were also significantly lower than prior to the onset of cancer, but significantly higher during treatment than prior to starting treatment. Adverse effects were experienced by 27 (87%) cats during the course of chemotherapy. Twenty-five (83%) of clients were happy they treated their cat and 27 owners (87%) would treat another cat. The results suggest that COP chemotherapy is perceived by owners to be tolerated by cats.

Abstract

One hundred and three ever-married women with newly diagnosed Chronic Obstructive Pulmonary Disease (COPD), who have never smoked, and 179 ever-married women who were visiting friends or relatives at the same hospital during the same period and have never smoked, were interviewed regarding the smoking habits of their husbands. There was statistically marginally significant difference between the COPD cases and the controls with respect to their husband's smoking habits. The association was irregular with respect to daily number of cigarettes smoked but there was a smooth dose response curve with respect to life long total number of cigarettes smoked, with women whose husband smoked more than 300 thousand cigarettes having a relative risk of 1.8 (90% confidence interval of 0.9-3.6) compared to women whose husband has never smoked. These findings, and converging related evidence, indicate that exposure to environmental tobacco smoke may be a risk factor for the development of COPD.

Abstract

Serum calcium was prospectively studied in 50 consecutive patients with active pulmonary tuberculosis. Twenty-four of them (48%) developed hypercalcaemia during an observation period of at least 8 weeks. Maximal increase in serum calcium (corrected for serum albumin) occurred three weeks after initiation of treatment, by which time 28% of the patients were hypercalcaemic. The increase in serum calcium was followed by a spontaneous remission. Only two patients developed symptoms related to hypercalcaemia, which promptly responded to steroid administration. No patient received vitamin D supplements before or during the study. No correlation could be found between hypercalcaemia and either the presence of acid-fast bacilli in the sputum or the season of the year. There was a trend for higher serum calcium values in the patients with the more severe radiographic changes on admission. Hypercalcaemia in patients with pulmonary tuberculosis seems to be triggered by chemotherapy. However, the mechanism(s) by which anti-tuberculosis treatment affects calcium metabolism remains uncertain.
