Abstract

OBJECTIVES: To examine whether The Ending PyJama (PJ) Paralysis campaign, focused on increasing in-hospital physical activity, affects objectively measured sedentary behavior and physical activity patterns and if these are associated with changes in physical and functional performance in geriatric rehabilitation inpatients. DESIGN: Quasi-experimental study. SETTING AND PARTICIPANTS: Within the REStORing health of acutely unwell adulTs (RESORT) observational, longitudinal cohort of geriatric rehabilitation inpatients, the Ending PJ Paralysis campaign was implemented on 2 out of 4 wards. METHODS: Objectively measured sedentary behavior and physical activity were measured by an inertial sensor (ActivPAL4) for 1 week, comparing control (non-PJ) and intervention (PJ) groups using linear mixed models. Mean sedentary behavior and physical activity measures and their association with physical and functional performance changes were investigated by linear regression analyses, stratified by low vs high performance at admission using the median as a cut-off. RESULTS: A total of 145 (n = 68 non-PJ and n = 77 PJ) inpatients with a mean age of 83.0 (7.7) years (55.9% female inpatients) were included. The median nonupright time was 23.1 [22.1-23.6] and 23.0 [21.8-23.6] hours/day for non-PJ and PJ groups, respectively. Objectively measured sedentary behavior and physical activity measures did not significantly change over measurement days and were independent of the Ending PJ Paralysis campaign. For inpatients with low performance at admission, lower sedentary behavior [B(SE) -0.013 (0.005) to -0.157 (0.045), P < .01] and higher physical activity [B(SE) 0.033 (0.007) to 0.814 (0.200), P < .01] measures were associated with improved physical performance. In addition, lower sedentary behaviour [B(SE) = -0.058 (0.024), P < .05 and higher physical activity [B (SE) 0.060 (0.024) to 0.683 (0.182), P < .05] were associated with improved instrumental functional performance. The colorectal cancer screening programme was introduced gradually from 2014. Since 2019, all people aged 55 to 75 have been invited to participate once every two years. In 2021, more than 1.6 million people took part in the screening programme, and colorectal cancer was detected in 2,700 participants, with an early stage of colorectal cancer being detected in 17,000 participants. At this stage, the screening programme has been running for too short a time to be able to scientifically demonstrate that it leads to less mortality from colorectal cancer; however, there are clear indications in that direction. Under the current circumstances, the benefits (prevention of colorectal cancer and associated mortality) outweigh the risks of the screening programme (screening test that detects no abnormalities but is taxing and causes concern, and cases of cancer that are missed). The Council therefore recommends that no modifications be made to the regular programme at this time.The Council does, however, recommend conducting a regional pilot population screening programme with one-off screening of people around 50 years of age. This will allow a review of to what extent such a programme would provide health benefits and whether the benefits would outweigh the risks. Based on the results, a decision can be made as to whether the age limit of the programme should be lowered. One promising development that may improve the screening programme in the long term is customised screening. As such, the Council recommends that this be reviewed. In addition, the Council recommends continuing investment in increasing participation rates.

Abstract

Due to the prevalence of skin cancer, the State Secretary for Health, Welfare and Sport requested that the Health Council of the Netherlands review whether introducing a screening programme for skin cancer would be expedient. A key requirement underpinning the introduction of a screening programme is that its usefulness has been established and that its benefits outweigh any disadvantages, which, the Council has determined, is not the case for skin cancer. There is no scientific evidence that a programme of this type would be beneficial. Any added value offered by a screening programme is expected to be low, given that many cases are already detected at an early stage and the mortality rate is low.Care for people with a suspicious lesion on their skin usually starts at the GP, who carries out a risk assessment and will either treat the lesion themselves or refer the patient to a dermatologist. People suspected to have a familial or hereditary predisposition to melanoma are offered a periodic skin examination and genetic testing. Survival rates for skin cancer are high with the current detection and treatment strategies for suspicious lesions.Excessive exposure to sunlight and sunburn in combination with a light skin tone are the principal risk factors for skin cancer. Behaviour can therefore significantly reduce the risk of skin cancer (e.g. not staying in the sun for long periods of time, covering the skin and using a high factor sun cream). The Steering Committee for Skin Cancer Care in the Netherlands (Stuurgroep Huidkankerzorg Nederland) and RIVM are working on more intensive information provision. The Health Council of the Netherlands emphasises the importance of behavioural change and information provision. In the future, apps for suspicious skin lesions may play a role in the detection of skin cancer, however, this requires more development and research.

13. Longitudinal Changes in Muscle Mass, Muscle Strength, and Physical Performance in Acutely Hospitalized Older Adults

Abstract

OBJECTIVES: Acute hospitalization may lead to a decrease in muscle measures, but limited studies are reporting on the changes after discharge. The aim of this study was to determine longitudinal changes in muscle mass, muscle strength, and physical performance in acutely hospitalized older adults from admission up to 3 months post-discharge. DESIGN: A prospective observational cohort study was conducted. SETTING AND PARTICIPANTS: This study included 401 participants aged ≥70 years who were acutely hospitalized in 6 hospitals. All variables were assessed at hospital admission, discharge, and 1 and 3 months post-discharge. METHODS: Muscle mass in kilograms was assessed by multifrequency Bio-electrical Impedance Analysis (MF-BIA) (Bodystat; Quadscan 4000) and muscle strength by handgrip strength (JAMAR). Chair stand and gait speed test were assessed as part of the Short Physical Performance Battery (SPPB). Norm values were based on the consensus statement of the European Working Group on Sarcopenia in Older People. RESULTS: A total of 343 acute hospitalized older adults were included in the analyses with a mean (SD) age of 79.3 (6.6) years, 49.3% were women. From admission up to 3 months post-discharge, muscle mass (-0.1 kg/m2; P = .03) decreased significantly and muscle strength (-0.5 kg; P = .08) decreased nonsignificantly. The chair stand (+0.7 points; P < .001) and gait speed test (+0.9 points; P < .001) improved significantly up to 3 months post-discharge. At 3 months post-discharge, 80%, 18%, and 43% of the older adults scored below the cutoff points for muscle mass, muscle strength, and physical performance, respectively. CONCLUSIONS AND IMPLICATIONS: Physical performance improved during and after acute hospitalization, although muscle mass decreased, and muscle strength did not change. 15. Trends in Neuromotor Fitness in 10-to-12-Year-Old Dutch Children: A Comparison Between 2006 and 2015/2017

Abstract

Children with a low level of neuromotor fitness are less skilled to participate in sports activities. Moreover, lower levels of neuromotor fitness are related to adiposity, lower cardiovascular health, and poor self-esteem in children. The aim of this paper was to determine neuromotor fitness in 10–12-year-old Dutch children over a 10-year period. Test scores measured in 2015/2017 (N = 533 in 2015, N = 941 in 2017) were compared with scores of same-aged children measured in 2006 (N = 1986). Neuromotor fitness was assessed using theMOPER fitness test battery, including speed and agility, strength, flexibility, and coordination and upper-limb speed. Data were analyzed using multilevellinear regression models and tobit regression analyses in case of skewed distributions with an excess of zeros. Analyses were stratified by age and gender, and adjusted for level of urbanization. Children in 2015/2017 performed significantly worse on speed and agility (b = 0.8 to 1.1 s), significantly better on coordination/upper-limb speed (b = −1.0 to −0.6 s), and–except for 12-year-old girls–significantly worse on flexibility vs. childrenin 2006 (b = −3.4 to −1.8 cm). Additionally, upper-body strength was significantly worse among 10-year olds (b = −3.2 to −2.5 s) while leg strength was significantly worse among 11-year-olds in 2015/2017 vs. 2006 (b = −1.8 to −1.7 cm). Trunk strength was worse among 11- and 12-year old boys (b = 1.1 to 1.2 s). In line with a previously observed downward trend in neuromotor fitness among children (1980–2006), we found worse scores on speed and agility, and flexibility in 2015/2017 vs. 2006, stressing the need for interventions aimed at improving neuromotor fitness in order to promote physical activity and future health.

Abstract

Objective: Attention deficit hyperactivity disorder (ADHD), autism spectrum disorder (ASD), and obsessive-compulsive disorder (OCD) are common neurodevelopmental disorders that frequently co-occur. The authors sought to directly compare these disorders using structural brain imaging data from ENIGMA consortium data. Methods: Structural T1-weighted whole-brain MRI data from healthy control subjects (N=5,827) and from patients with ADHD (N=2,271), ASD (N=1,777), and OCD (N=2,323) from 151 cohorts worldwide were analyzed using standardized processing protocols. The authors examined subcortical volume, cortical thickness, and cortical surface area differences within a mega-analytical framework, pooling measures extracted from each cohort. Analyses were performed separately for children, adolescents, and adults, using linear mixed-effects models adjusting for age, sex, and site (and intracranial volume for subcortical and surface area measures). Results: No shared differences were found among all three disorders, and shared differences between any two disorders did not survive correction for multiple comparisons. Children with ADHD compared with those with OCD had smaller hippocampal volumes, possibly influenced by IQ. Children and adolescents with ADHD also had smaller intracranial volume than control subjects and those with OCD or ASD. Adults with ASD showed thicker frontal cortices compared with adult control subjects and other clinical groups. No OCD-specific differences were observed across different age groups and surface area differences among all disorders in childhood and adulthood. Conclusions: The study findings suggest robust but subtle differences across different age groups among ADHD, ASD, and OCD. ADHD-specific intracranial volume and hippocampal differences in children and adolescents, and ASD-specific cortical thickness differences in the frontal cortex in adults, support previous work emphasizing structural brain differences in these disorders.

Abstract

Objective: Brain imaging communities focusing on different diseases have increasingly started to collaborate and to pool data to perform well-powered meta- and mega-analyses. Some methodologists claim that a one-stage individual-participant data (IPD) mega-analysis can be superior to a two-stage aggregated data meta-analysis, since more detailed computations can be performed in a mega-analysis. Before definitive conclusions regarding the performance of either method can be drawn, it is necessary to critically evaluate the methodology of, and results obtained by, meta- and mega-analyses. Methods: Here, we compare the inverse variance weighted random-effect meta-analysis model with a multiple linear regression mega-analysis model, as well as with a linear mixed-effects random-intercept mega-analysis model, using data from 38 cohorts including 3,665 participants of the ENIGMA-OCD consortium. We assessed the effect sizes and standard errors, and the fit of the models, to evaluate the performance of the different methods. Results: The mega-analytical models showed lower standard errors and narrower confidence intervals than the meta-analysis. Similar standard errors and confidence intervals were found for the linear regression and linear mixed-effects random-intercept models. Moreover, the linear mixed-effects random-intercept models showed better fit indices compared to linear regression mega-analytical models. Conclusions: Our findings indicate that results obtained by meta- and mega-analysis differ, in favor of the latter. In multi-center studies with a moderate amount of variation between cohorts, a linear mixed-effects random-intercept mega-analytical framework appears to be the better approach to investigate structural neuroimaging data.

30. MELK Inhibition in Diffuse Intrinsic Pontine Glioma

Abstract

Purpose: Diffuse intrinsic pontine glioma (DIPG) is a highly aggressive pediatric brain tumor, for which no effective therapeutic options currently exist. We here determined the potential of inhibition of the maternal embryonic leucine zipper kinase (MELK) for the treatment of DIPG.Experimental Design: We evaluated the antitumor efficacy of the small-molecule MELK inhibitor OTSSP167 in vitro in patient-derived DIPG cultures, and identified the mechanism of action of MELK inhibition in DIPG by RNA sequencing of treated cells. In addition, we determined the blood-brain barrier (BBB) penetration of OTSSP167 and evaluated its translational potential by treating mice bearing patient-derived DIPG xenografts.Results: This study shows that MELK is highly expressed in DIPG cells, both in patient samples and in relevant in vitro and in vivo models, and that treatment with OTSSP167 strongly decreases proliferation of patient-derived DIPG cultures. Inhibition of MELK in DIPG cells functions through reducing inhibitory phosphorylation of PPAR?, resulting in an increase in nuclear translocation and consequent transcriptional activity. Brain pharmacokinetic analyses show that OTSSP167 is a strong substrate for both MDR1 and BCRP, limiting its BBB penetration. Nonetheless, treatment of Mdr1a/b;Bcrp1 knockout mice carrying patient-derived DIPG xenografts with OTSSP167 decreased tumor growth, induced remissions, and resulted in improved survival.Conclusions: We show a strong preclinical effect of the kinase inhibitor OTSSP167 in the treatment of DIPG and identify the MELK-PPAR? signaling axis as a putative therapeutic target in this disease. Abstract

Low cardiorespiratory fitness and inactivity are strong health predictors associated with excessive fatness. The objective of this study was to determine the associations between physical activity (PA) and cardiorespiratory fitness and body fatness in South African adolescents. A cross-sectional study was performed with a subsample of 149 school-going adolescents (58 boys and 91 girls) with a mean age of 15.7 ± 0.8 years whom were participating in the Physical Activity and Health Longitudinal Study (PAHLS). Body mass, height, and skinfolds were assessed according to the standards of the International Society for the Advancement of Kinanthropometry (ISAK). Subsequently, body mass index (BMI) and percentage of body fat (%BF) were calculated. Cardiorespiratory fitness was derived from predicted VO2max, and PA assessed by combined heart rate and accelerometry (ActiHeart®, CamNTech). Physical activity was classified according to activity categories. Out of 149 participants, 100 (67.1%) had normal body mass; 26 (17.5%) were underweight and 23 (15.4%) were overweight. Seventeen (18.7%) girls and six (10%) boys were overweight or obese. The prevalence of underweight in boys was 29.3% and 9.9% in girls. Abstract

Latent growth models are often used to measure individual trajectories representing change over time. The characteristics of the individual trajectories depend on the variability in the longitudinal outcomes. In many medical and epidemiological studies, the individual health outcomes cannot be observed directly and are indirectly observed through indicators (i.e. items of a questionnaire). An item response theory or a classical test theory measurement model is required, but the choice can influence the latent growth estimates. In this study, under various conditions, this influence is directly assessed by estimating latent growth parameters on a common scale for item response theory and classical test theory using a novel plausible value method in combination with Markov chain Monte Carlo. The latent outcomes are considered missing data and plausible values are generated from the corresponding posterior distribution, separately for item response theory and classical test theory. These plausible values are linearly transformed to a common scale. A Markov chain Monte Carlo method was developed to simultaneously estimate the latent growth and measurement model parameters using this plausible value technique. It is shown that estimated individual trajectories using item response theory, compared to classical test theory to measure outcomes, provide a more detailed description of individual change over time, since item response patterns (item response theory) are more informative about the health measurements than sum scores (classical test theory). 40. One-year stability of cardiovascular diseases risk factors in Portuguese youngsters.

Abstract

The purposes of the present study were to analyze the stability of biological risk factors for CVD (blood pressure [BP], percentage of fat mass [%FM], total cholesterol [TC]) and one behavioral/life style risk factor (physical activity [PA]), as well as to study the stability of biological risk factors clustering. The sample comprised 325 males and 367 females, aged 8-15 years old. Participants were classified as being 'at risk' according to age and sex adjusted 4th quartile or the first for PA. Three biological risk factors (%FM, SBP and TC) showed higher stability (one-year interval; r = 0.9-0.5) than DBP (r = 0.3) and PAI (r = 0.3). It can be concluded that 46% of subjects with two and/or three biological risk factors at the first measurement remained with the same number of biological risk factors in the second measurement. Abstract

This longitudinal study evaluates the relationship of food intake and physical activity with biological maturation of 200 boys and girls during adolescence and young adulthood. The subjects were followed during 9 years from ages 12 to 22 years, with repeated measurements at ages 13, 14, 15, 16, and 21. Biological maturation was estimated four times between ages 12 and 17 as skeletal age by radiographs of the left hand and wrist. Daily nutritional intake (macro- and micronutrients) was assessed with a cross-checked dietary history method. Daily physical activity was assessed through structured interview, whereby average weekly time spent in activity was used to assign a weighted activity score. The 107 girls and 93 boys were divided into three maturity groups: early maturers, late maturers, and average maturers. It was concluded that in both sexes, late maturation seemed to coincide with a higher energetic food intake and a slightly higher activity pattern than early maturation during adolescence.
