This article explores the implementation of Radically Open Dialectical Behavior Therapy (RO DBT) in a University or College Counseling Setting (UCCS). The authors review the rationale for including RO DBT in a UCCS and the process of implementing the treatment at their center. This paper evaluates this newly implemented treatment by examining clinician experience, feasibility, and adaptability of RO DBT including Words of Wisdom regarding future implementations of RO DBT in a UCCS.

Abstract Background Walking is the primary and preferred mode of exercise for older adults. Walking to and from public transit stops may support older adults in achieving exercise goals. This study examined whether density of neighborhood public transit stops was associated with walking for exercise among older adults. Methods 2018 National Health and Aging Trends Study (NHATS) data were linked with the 2018 National Neighborhood Data Archive, which reported density of public transit stops (stops/mile2) within participants’ neighborhood, defined using census tract boundaries. Walking for exercise in the last month was self-reported. The extent to which self-reported public transit use mediated the relationship between density of neighborhood public transit stops and walking for exercise was examined. Covariates included sociodemographic characteristics, economic status, disability status, and neighborhood attributes. National estimates were calculated using NHATS analytic survey weights. Results Among 4,836 respondents with complete data, 39.7% lived in a census tract with at least one neighborhood public transit stop and 8.5% were public transit users. The odds of walking for exercise were 32% higher (OR = 1.32; 95% confidence interval: 1.08, 1.61) among respondents living in a neighborhood with > 10 transit stops per mile compared to living in a neighborhood without any public transit stops documented. Self-reported public transit use mediated 24% of the association between density of neighborhood public transit stops and walking for exercise. Conclusions Density of neighborhood public transit stops was associated with walking for exercise, with a substantial portion of the association mediated by self-reported public transit use. Increasing public transit stop availability within neighborhoods may contribute to active aging among older adults.

Background: Understanding how and where children accumulate their physical activity (PA) and sedentary minutes throughout the day has important implications for behavioral interventions. Purpose: The purpose of this study was to examine the duration and intensity of habitual PA and sedentary time (SED) at and away from school among preadolescent children. Methods: Data from twenty-one children ages 7-11 (57% girls; 57% nonwhite; 24% overweight/obese) were included in this analysis. Proportion of time spent in PA, SED, and SED bouts were examined on school days (in-school and out-of-school) and non-school days via accelerometry. Results: When comparing school time with non-school time on school days, children accumulated a higher proportion of light PA while in-school [49.3% (i.e. 5 min 18 s) vs. 39.6% (i.e. 3 h 53 min), p = 0.01]. While at school, the proportion of time spent in SED was less [46.3% (i.e. 3 h 7 min) vs. 56.3% (i.e. 5 h 32 min), p = 0.01] and fewer children spent time engaged in prolonged sitting compared to out-of-school [uninterrupted SED for 30 min 52.4% (i.e. 11 children) vs. 85.7% (i.e. 18 children), p = 0.02]. Discussion: These findings suggest children are proportionately most active and least sedentary when at school, yet children only accumulated approximately 18 min of moderate-to-vigorous PA in this environment. Translation to Health Education Practice: These preliminary findings can help to inform physical activity programming designed to promote life-long physical activity behaviors in children.

Rural areas are experiencing a dynamic, multidirectional change. It is the result of impact of many processes, including rapid, often spontaneous urbanisation and structural transformation in the agricultural sector and temporary and permanent movements of the population. Changes in the spatial structures of the countryside, on the one hand, influenced by European programmes and measures, linked to the processes of modernisation and organisation of rural settlements and economic systems. On the other hand, rapid economic development and spontaneous investment are resulting in the expansion of the urban forms of land use – not only in the immediate surroundings of the cities and towns, but also in remote, peripheral areas dominated until recently by farming. The paper presents the genesis of contemporary land use in rural areas in Poland, linking the countryside structures with the intensity and forms of residency and user economies in the rural areas. The main drivers of change, significantly affecting the emerging forms of farming villages, are processes of intensive suburbanisation and semi-urbanisation, the phenomenon of progressive peripherisation of large areas in different parts of the country, the depopulation of the countryside and profound changes in the demographic structure of the population, improvement of infrastructure and transport accessibility and economic revival of villages. There are also important issues related to advantages such as natural and cultural values of the village. The aim of the article is to present the main directions of functional and spatial transformations of rural areas in Poland from the beginning of the 21st century. The main hypothesis is that the development of rural areas in Poland proceeds in two directions: Urban “colonization”, which involves the invasion of both single- and multi-family housing in the surroundings of traditional villages and in areas previously used for agricultural purposes, is becoming increasingly widespread; this follows restructuring and diversification (spatial and functional) in urbanized rural areas. Much indicates that the rational development and use of rural areas will be one of the main challenges of Poland in the near future.

It is well-established that the post-seismic slip results from the combined contribution of seismic and aseismic processes. However, the partitioning between these two modes of deformation remains unclear due to the difficulty of inferring detailed and robust descriptions of how both evolve in space and time. This is particularly true just after a mainshock when both processes are expected to be the strongest. Using state-of-the-art sub-daily processing of GNSS data, along with dense catalogs of aftershocks obtained from template-matching techniques, we unravel the spatiotemporal evolution of post-seismic slip and aftershocks over the first 12 h following the 2015 Mw 8.3 Illapel, Chile, earthquake. We show that the very early post-seismic activity occurs over two regions with distinct behaviors. To the north, post-seismic slip appears to be purely aseismic and precedes the occurrence of late aftershocks. To the south, aftershocks are the primary cause of the post-seismic slip. We suggest that this difference in behavior could be inferred only a few hours after the mainshock. We finish by showing that this information can potentially be obtained very rapidly after a large earthquake, which could prove to be useful in forecasting the long-term spatial pattern of aftershocks.

Abstract Background Corneal dystrophies are a group of rare, inherited disorders that are usually bilateral, symmetric, slowly progressive, and not related to environmental or systemic factors. The majority of publications present the advanced form of the disease with a typical clinical demonstration. The initial signs and symptoms of different epithelial and stromal corneal dystrophies are not specific; therefore, it is very important to establish the early characteristic corneal features of these disorders that could guide the diagnostic process. Case presentation The main purpose of this study was to report the differential diagnosis of a pediatric patient with bilateral anterior corneal involvement suspected of corneal dystrophy. An 8-year-old male patient presented with asymptomatic, persistent, superficial, bilateral, diffuse, anterior corneal opacities. Slit lamp examination results were not specific. Despite the lack of visible stromal involvement on the slit lamp examination, corneal analysis based on confocal microscopy and optical coherence tomography revealed characteristic features of macular corneal dystrophy (MCD). The diagnosis of MCD was confirmed by CHST6 gene sequencing. The early corneal characteristic features of MCD, established based on the findings of this case report, include corneal astigmatism (not specific), diffuse corneal thinning without a pattern of corneal ectasia (specific), and characteristic features on confocal microscopy (specific), including multiple, dark, oriented striae at different corneal depths. Conclusions The clinical examination should be complemented with corneal imaging techniques, such as confocal microscopy and optical coherence tomography. In patients suspected of corneal dystrophy, genetic testing plays an important role in establishing the final diagnosis.

Background Appropriate conceptualization and measurement of disability are critical for population-focused resource allocation and policy development. Self-reported and performance-based measures of functioning have been used to represent disability. Variation in environmental context or self-perception of ability may influence self-reports; however, performance-based measures that attempt to control environmental context may not accurately capture real-world aspects of functioning. This study examined the agreement between self-report and performance-based measures of functioning within 4 domains among older adults. Methods Cross-sectional data from the 2021 National Health and Aging Trends Study was used. Self-reported and performance-based measures of functioning were assessed for vision, hearing, mobility, and memory domains. We examined the diagnostic characteristics of performance-based versus self-reported measures using sensitivity, specificity, and receiver operating characteristics curves. Differences in the agreement of these measures across sociodemographic groups were investigated using logistic regression. Results Among 2 442 respondents 71 years and older (mean 78.5 ± 5.3, 56% female participants), performance measures of hearing and mobility had high sensitivity (89% and 91%, respectively) and low/moderate specificity (36% and 63%, respectively). The sensitivity and specificity of vision measures were 71%. Memory measures had high specificity (89%) and low sensitivity (28%). Performance-based discrimination ranged from 0.59 (memory) to 0.78 (mobility). Agreement varied across sociodemographic factors. Conclusions Performance measures diverge from self-reported functioning among older adults. Discordance may reveal opportunities for environmental intervention where participants' performance does not capture the full extent of barriers in their daily lives. Additional research is needed to investigate individual and environmental factors which could explain the observed differences. [ABSTRACT FROM AUTHOR]

Sieć handlu detalicznego w Polsce od początku transformacji ulega ciągłym zmianom strukturalnym, ilościowym oraz jakościowym. W strukturze sieci detalicznej pojawiają się nowe formaty handlu wielkopowierzchniowego, takie jak parki i galerie handlowe, obserwowany jest intensywny rozwój sieci dyskontowych, stosujących strategię niskich cen oraz selekcję oferty asortymentowej. Tradycyjne, małe sklepy coraz częściej działają w ramach sieci zintegrowanych, które pozwalają im na redukcję kosztów funkcjonowania. Istotne znaczenie dla rozwoju sieci detalicznej ma handel elektroniczny i nowe technologie wspierające procesy dystrybucyjne. Sytuacja zmienia się najszybciej w dużych ośrodkach miejskich, ale z czasem analogiczne zmiany zachodzą też w małych miastach i na obszarach wiejskich. Duże i średniej wielkości miasta, jako atrakcyjne rynki zbytu, były naturalnym polem dla wzmożonej aktywności inwestycji handlowych. Od kilku lat inwestorzy rozpoczęli ekspansję do małych miast, które stanowią 76% liczby miast ogółem w Polsce. Celem artykułu jest określenie wpływu detalicznych sieci handlowych, w szczególności dyskontowych, na rozwój małych miast oraz otaczających je obszarów wiejskich. W 2018 r. przeprowadzono badania pilotażowe w wybranych małych miastach i na obszarach wiejskich w województwie śląskim, położonych w strefie oddziaływania aglomeracji górnośląskiej. Do badań zakwalifikowano małe miasta, w których funkcjonują sieci dyskontowe. Dla pełniejszej oceny oddziaływania handlu sieciowego na rozwój lokalny zinwentaryzowano rozmieszczenie placówek handlowych w badanych jednostkach.

Abstract Background Although previous research has highlighted the association between the built environment and individual health, methodological challenges in assessing the built environment remain. In particular, many researchers have demonstrated the high inter-rater reliability of assessing large or objective built environment features and the low inter-rater reliability of assessing small or subjective built environment features using Google Street View. New methods for auditing the built environment must be evaluated to understand if there are alternative tools through which researchers can assess all types of built environment features with high agreement. This paper investigates measures of inter-rater reliability of GigaPan®, a tool that assists with capturing high-definition panoramic images, relative to Google Street View. Methods Street segments (n = 614) in Pittsburgh, Pennsylvania in the United States were randomly selected to audit using GigaPan® and Google Street View. Each audit assessed features related to land use, traffic and safety, and public amenities. Inter-rater reliability statistics, including percent agreement, Cohen’s kappa, and the prevalence-adjusted bias-adjusted kappa (PABAK) were calculated for 106 street segments that were coded by two, different, human auditors. Results Most large-scale, objective features (e.g. bus stop presence or stop sign presence) demonstrated at least substantial inter-rater reliability for both methods, but significant differences emerged across finely detailed features (e.g. trash) and features at segment endpoints (e.g. sidewalk continuity). After adjusting for the effects of bias and prevalence, the inter-rater reliability estimates were consistently higher for almost all built environment features across GigaPan® and Google Street View. Conclusion GigaPan® is a reliable, alternative audit tool to Google Street View for studying the built environment. GigaPan® may be particularly well-suited for built environment projects with study settings in areas where Google Street View imagery is nonexistent or updated infrequently. The potential for enhanced, detailed imagery using GigaPan® will be most beneficial in studies in which current, time sensitive data are needed or microscale built environment features would be challenging to see in Google Street View. Furthermore, to better understand the effects of prevalence and bias in future reliability studies, researchers should consider using PABAK to supplement or expand upon Cohen’s kappa findings.

Purpose: This study aimed to evaluate the ocular surface parameters of post-COVID-19 patients when compared to healthy controls. Methods: Patients after symptomatic SARS-CoV-2 infection, as confirmed by a PCR test of their nasopharyngeal swab sample, were enrolled. Complete ophthalmic examination, including visual acuity test, intraocular pressure measurement (IOP), slit-lamp examination, tear osmolarity test, central corneal thickness, endothelial cell number measurements, non-invasive keratograph break-up time (NIKBUT), meniscus height, and the Schirmer’s test were performed and compared with the controls. Results: It must be noted that there were 36 COVID-19 patients and 25 control subjects included in the study. Eye itching and burning (19%) were the most common symptoms of COVID-19 infection, followed by a subjective decrease in vision (17%), conjunctivitis and eye pain were present in 11%, and 6% of the patients had episcleritis. The mean time from initial infection was 6.5 ± 3.9 (range 1–24 weeks). Meniscus height was not significantly changed between the COVID-19 (0.34 ± 0.13 mm) group and the control (0.33 ± 0.12 mm, p = 0.88) group. In addition, the NIKBUT-1 (p = 0.88; 7.22 ± 4.60 s and 6.91 ± 4.45 s) and NIKBUT average (p = 0.91, 12.30 ± 5.86 s and 11.77 ± 4.97 s) test results showed no significant change either. Neither was a significant result found in the IOP (p = 0.17, 14.56 ± 2.10 mmHg and 14.11 ± 1.96 mmHg); the Schirmer test (p = 0.18, 20.22 ± 7.92 mm and 20.02 ± 7.17 mm); Tosm (p = 0.16, 294.42 ± 54.51 mOsm/dL and 299.33 ± 5.65 mOsm/dL); CCT (p = 0.06, 549.15 ± 28.98 vs. 539.21 vs. 29.08 µm); nor the endothelial cell density (p = 0.07, 2516.64 ± 287.61 vs. 2454.21 ± 498.60 cells/mm2). Conclusions: Through this study it was not revealed that there were any significant differences between the post-COVID group and control group in the objective measurements of ocular surface conditions, when performed after the acute phase of COVID-19. The exact incidence and mechanism of ocular findings, especially dry eye disease, in correlation with SARS-CoV-2 requires further research.
