2. Teaching and learning about food : a study of curriculum change in nutrition education in primary schools

Abstract

The development of nutrition education as part of the school curriculum is considered in the first section of this thesis. The second part focuses on an action research project which was part of in-service courses for primary teachers at the Institute of Education. Teaching and learning about food as part of science and health education provided the context for the research. Each of the seventy teachers involved in the project interviewed three children in their class to explore the ideas which children held about the foods they ate, how they grouped food items, and the types of food choices which they made, as well as the reasons for those choices. Data from the interviews were used as a means of helping teachers to reflect on teaching and learning and to explore their own ideas about food and health. Reflection on practice was encouraged through discussion and the writing of reports based on the interviews. The findings from the interviews indicate that children's ideas, particularly about nutrients, are frequently different from accepted scientific views. There is also evidence that children choose foods which they like, rather than those which they consider to be healthy.Analysis of the reports written by teachers indicates that the interviews have helped to alert teachers to the range of ideas held by children and to increase their own understanding about diet and health. There is evidence that the process of reflection and writing has helped teachers to consider and to develop teaching strategies which are consistent with constructivist learning theory. The findings from this study have implications for teaching and learning about food in science and health education in primary schools. The information about children's ideas about food and health has importance also for policy and practice at an individual, school and national level.

As part of the study, the students investigated secondary students' understanding of health. Surveys and interviews were effective in collecting student teachers' views. The Association for Science Education's (ASE's) project aimed to increase understanding of surroundings and everyday activities through the exchange of information between students in different areas of Europe. The ASE deals with the administration and provides materials suitable for use by 14-16 year olds.

Abstract Background The preparation of HTA reports requires a great deal of time, effort and resource, and there is a desire to improve efficiency, avoid duplication of effort and facilitate the transfer of knowledge between countries. This is of particular importance for countries with more limited resources which have less capacity to produce their own reports. The aim of this study was to investigate the extent of duplication of published Health Technology Assessment (HTA) reports, on the same technology, for the same indication; using positron emission tomography (PET) for lung cancer and Hodgkin's disease as a case study. This was done in order to assess the potential usefulness of a toolkit developed to aid in the adaptation of HTA reports from one context or country to another. Methods A systematic search of the National Institute for Health Research (NIHR) CRD HTA database was conducted in June 2008 in order to identify full HTA reports containing information on the use of PET for lung cancer and Hodgkin's disease, written in English, and readily available on the web. The contents of the reports identified were then examined to assess the extent of duplication of content between reports and potential for the use of the toolkit. Results From 132 records of HTA reports about PET, 8 reports were identified as fulfilling all the criteria set, and therefore demonstrating potential duplication of effort. All these reports covered four similar domains, technology use, safety, effectiveness and economic evaluation. Five of the reports also considered organisational aspects. Conclusions There was some duplication of effort in the preparation of HTA reports concerned with the use of PET for lung cancer and Hodgkin's disease. This is an example of where resource could have been conserved and time saved by the use of a toolkit developed to aid in the adaptation of HTA reports from one context to another.

Abstract

It is important that children have a 'common-sense' understanding of diet and health and the promotion of healthy lifestyles through health education programs is an essential component of their school life. A comparative study of schoolchildren in Greece and the UK investigates their understanding and knowledge of fat as part of their diet and the relationship between fat, diet and health. The findings suggest that both sets of children are aware of food's role in maintaining health but the link between diet and health is ill understood, particularly the relationship between dietary fat and heart disease.

Abstract

A small-scale study has been carried out of children at a primary school in southern England, and the context in which they eat school meals, which affected their diet. Children tend to be subject to stress at meal times with pressure to make rapid choices and eat fast. They have little choice over what they eat and this includes children eating packed lunches, whose parents often make the decision over contents. There is little attempt to integrate their school meals with teaching on food and nutrition in the school.

This monograph is an edited collection of some of the papers presented at the conference on "New Developments in Nutrition," held in London in 1983. One of the strengths of the conference was the diverse background of the participants, who represented more than thirty different countries. This diversity of interest and expertise is reflected in the texts in which those engaged in nutrition education examine, from a number of perspectives, a wide range of important issues in nutrition education. Discussions are included on the effectiveness of traditional nutrition education activities which have taken place outside the education sectors, and experiences are reviewed which have included extensive experimentation with new and different approaches, attitudes, and behaviors. OBJECTIVE:\ud To assess the cost effectiveness of different management strategies for urinary tract infections.\ud \ud DESIGN:\ud Cost effectiveness analysis alongside a randomised controlled trial with a one month follow-up.\ud \ud SETTING:\ud Primary care.\ud \ud PARTICIPANTS:\ud 309 non-pregnant adult women aged 18-70 presenting with suspected urinary tract infection.\ud \ud INTERVENTIONS:\ud Patients were randomised to five basic management approaches: empirical antibiotics, empirical delayed (by 48 hours) antibiotics, or targeted antibiotics based on either a high symptom score (two or more of urine cloudiness, smell, nocturia, dysuria), dipstick results (nitrite or leucocytes and blood), or receipt of a positive result on midstream urine analysis.\ud \ud MAIN OUTCOME MEASURE:\ud Duration of symptoms and cost of care.\ud \ud RESULTS:\ud Management with targeted antibiotics with midstream urine analysis was more costly over the period of one month. Costs for the midstream urine analysis and dipstick management groups were pound37 and pound35, respectively; these compared with pound31 for immediate antibiotics. Cost effectiveness acceptability curves suggested that if avoiding a day of moderately bad symptoms was valued at less than pound10, then immediate antibiotics is likely to be the most cost effective strategy. For values over pound10, targeted antibiotics with dipstick testing becomes the most cost effective strategy, though because of the uncertainty we can never be more than 70% certain that this strategy truly is the most cost effective.\ud \ud CONCLUSION:\ud Dipstick testing with targeted antibiotics is likely to be cost effective if the value of saving a day of moderately bad symptoms is pound10 or more, but caution is required given the considerable uncertainty surrounding the estimates.
