26. Symptom burden and health-related quality of life in chronic kidney disease: A global systematic review and meta-analysis

Abstract

Background The importance of patient-reported outcome measurement in chronic kidney disease (CKD) populations has been established. However, there remains a lack of research that has synthesised data around CKD-specific symptom and health-related quality of life (HRQOL) burden globally, to inform focused measurement of the most relevant patient-important information in a way that minimises patient burden. The aim of this review was to synthesise symptom prevalence/severity and HRQOL data across the following CKD clinical groups globally: (1) stage 1-5 and not on renal replacement therapy (RRT), (2) receiving dialysis, or (3) in receipt of a kidney transplant. Methods and findings MEDLINE, PsycINFO, and CINAHL were searched for English-language cross-sectional/longitudinal studies reporting prevalence and/or severity of symptoms and/or HRQOL in CKD, published between January 2000 and September 2021, including adult patients with CKD, and measuring symptom prevalence/severity and/or HRQOL using a patient-reported outcome measure (PROM). Random effects meta-analyses were used to pool data, stratified by CKD group: not on RRT, receiving dialysis, or in receipt of a kidney transplant. Methodological quality of included studies was assessed using the Joanna Briggs Institute Critical Appraisal Checklist for Studies Reporting Prevalence Data, and an exploration of publication bias performed. The search identified 1,529 studies, of which 449, with 199,147 participants from 62 countries, were included in the analysis. Studies used 67 different symptom and HRQOL outcome measures, which provided data on 68 reported symptoms. Random effects meta-analyses highlighted the considerable symptom and HRQOL burden associated with CKD, with fatigue particularly prevalent, both in patients not on RRT (14 studies, 4,139 participants: 70%, 95% CI 60%-79%) and those receiving dialysis (21 studies, 2,943 participants: 70%, 95% CI 64%-76%). A number of symptoms were significantly (p < 0.05 after adjustment for multiple testing) less prevalent and/or less severe within the post-transplantation population, which may suggest attribution to CKD (fatigue, depression, itching, poor mobility, poor sleep, and dry mouth). Quality of life was commonly lower in patients on dialysis (36-Item Short Form Health Survey [SF-36] Mental Component Summary [MCS] 45.7 [95% CI 45.5-45.8]; SF-36 Physical Component Summary [PCS] 35.5 [95% CI 35.3-35.6]; 91 studies, 32,105 participants for MCS and PCS) than in other CKD populations (patients not on RRT: SF-36 MCS 66.6 [95% CI 66.5-66.6], p = 0.002; PCS 66.3 [95% CI 66.2-66.4], p = 0.002; 39 studies, 24,600 participants; transplant: MCS 50.0 [95% CI 49.9-50.1], p = 0.002; PCS 48.0 [95% CI 47.9-48.1], p = 0.002; 39 studies, 9,664 participants). Limitations of the analysis are the relatively few studies contributing to symptom severity estimates and inconsistent use of PROMs (different measures and time points) across the included literature, which hindered interpretation. Conclusions The main findings highlight the considerable symptom and HRQOL burden associated with CKD. The synthesis provides a detailed overview of the symptom/HRQOL profile across clinical groups, which may support healthcare professionals when discussing, measuring, and managing the potential treatment burden associated with CKD. 27. The management of ambidexterity : an intellectual capital approach

Abstract

In this thesis I propose that the literature on ambidexterity does not fully explore the detailed practices by which organisations and managers may achieve both exploitation and exploration. A systematic review identifies that studies have focused principally at the organisation-level, and there is a lack of both empirical and theoretical work at the micro-level of analysis highlighting how ambidexterity may be achieved in practical, complex, working structures. The research addresses these micro-mechanisms in the context of the management of projects, a suitable area in that it can be considered as using defined processes together with the flexibility to overcome particular issues that arise. The contribution of the thesis is that it presents an insight into the management of ambidexterity in such an environment, and identifies how multiple knowledge resources are utilised, together with the underlying managerial practices. The level of analysis is the project (specifically, IT-services projects in a major multinational organisation), using the manager as the unit of analysis. The research question is ‘How is ambidexterity achieved at the level of the project?’ This is an opportunity to explore a practical as well as a theoretical gap, in an increasingly important area of business operations. The first stage of the research examines the managerial role in terms of intellectual capital, using a variety of projects. This shows that the sub-components of IC (human, social and organisational/project capital) can each be understood as having co-existing, orthogonal, exploitative and exploratory elements, an important extension of existing theory. The forms of intellectual capital are interwoven not only with each other, but also with the processes of exploitation and exploration, and to conceive of them as separate is an insufficient theorisation. The findings from the qualitative approach are used to investigate the duality of these concepts and bring greater clarity to our understanding of their operationalisation. .This is followed by eight case studies, each using between three and five managerial respondents, together with project data, used to develop a more fine-grained understanding of ambidexterity in a wide range of industrial settings. This shows different managerial configurations (including ‘distributed’ and ‘point’ ambidexterity – an addition to current theory), together with five key managerial practices to enable project-level ambidexterity, identified in the context of project complexity, critical events and constraints.

Abstract

Effective integration of the Regular and Reserve Armed Forces is essential to operational effectiveness, but evidence suggests that this remains problematic. Past research has focused on the professional values of Regulars and the perceptions that this group holds about Reservists. In this study, we argue that it is necessary to consider the perceptions of both Regulars and Reservists to truly understand the barriers to integration between these elements of Defence. This study investigates what Regular and Reserve Royal Marines see as the important constructs related to each group, through the use of repertory grid technique with 18 Regulars and 16 Reservists. Not only did the Regulars and Reservists in this study see different constructs as important, they also ascribed different constructs to each group. These differences are potentially problematic when aiming to integrate the Regular and Reserve Armed Forces.

47. Tenant environmental performance and property investment : the use of environmental management systems in reducing risk

Within the property research community consideration of environmental risk in relation to a tenant's polluting activities has received little attention. Whilst the profession has begun to address environmental issues through various research initiatives, the study of current land uses causing environmental problems and, m particular, the consequences for property investors, is at a very early stage. Through the literature review, and the major empirical project undertaken in the course of this research, significant advancements have been made in this area. It has been possible to establish the following: that the property investment portfolios of financial institutions and property investment companies are unlikely to be completely devoid of properties occupied by tenants capable of causing environmental damage; where such damage occurs an increasing body of opinion suggests that landlords could be held criminally liable for fines and/or statutory clean up costs; even where the tenant has sole liability (perhaps because the liabilities arise from activities the tenant has undertaken at another site) there are income security repercussions for the property investor. Site specific case studies, where industrial properties were inspected by the researcher and discussed with environmental auditors, played an important part in obtaining the information to support these findings. Consequently, improvements in the environmental performance1 of tenants occupying their properties, possibly, through the implementation of an environmental management system (EMS), will provide investors with a less risky property investment vis a vis other similar property investment opportunities occupied by tenants displaying low levels of environmental awareness. The empirical work within this thesis also found that property investors are beginning to acknowledge the concept of EMSs by considering it in the stock selection process. The research introduces a new consideration into the catalogue of property investment risks, namely, the environmental performance of the tenant and the role of environmental management systems therein. This strengthens the existing academic literature on property investment risk, and as such provides an original and significant contribution to this field of knowledge.
