20. Separate lives? : confronting the marginalization of young people in Middle England

Abstract

This thesis considers the way the young people of Middle England Eastleigh are located in narratives of the 'other' and defined in opposition to the adult population. This categorisation is reinforced through their experiences of subordination in many spheres. Eastleigh, 'the town that's too good to be true', gives off the aura of well-enough-off contentment and presents itself as a good place to live and bring up children. There is a good community feeling where much is seen to be done for the marginalized, especially its young people. The Council prides itself with funding provisions for and consulting with young people. Statutory and voluntary organisations work constantly to provide what they deem is necessary to enable the young to take their (conforming) place within Eastleigh society, so that the good image is perpetuated. The young people, however, have a different story to tell. Using the voices of some of those young people this thesis explores the processes whereby young people become subordinated in a community where the portrayal is of cohesiveness. Central to the study are the programmes designed to divert young people from crime and integrate them into the community. The young people, who become involved, however, often meet attempts to mould, contain or control them in their formative years in directions they do not seek. This study found that young people increasingly have to learn to negotiate their way through the ambiguity of their position but that the initiatives provide useful support for some young people some of the time. The young people of Eastleigh have views and values which seldom fit with those of their elders, who tend to see all young people as non-conforming, and there is constant friction between the two. An uneasy fermentation continues to highlight areas where all is not as it seems. There is an intergenerational divide promoting the physical distancing of young people from adults and emphasizing a lack of communication and trust. The model image by which Eastleigh is portrayed is far from the reality experienced in the lives of its young people.

24. 'Whoops, I Forgot David': Children's Perceptions of the Adults Who Work in Their Classrooms

Abstract

This article reports on the findings of a small-scale study into the perceptions of 78 primary school children regarding the adults in their classrooms. The data show that children easily differentiate between their own class teacher and other adults, but report a substantial overlap between the activities of teachers and teaching assistants. Some express the difference in terms of status rather than role. Accounts call into question the notion that teaching assistants "help" rather than teach and that there is a clear division of labour between them and teachers. Teachers and assistants are seen as working in an interdependent way, with each making a significant contribution to children's learning. The difficulties of using children's language as evidence are considered and it is concluded that the notion of a "remodelled" primary school workforce needs to take into account the ways in which teachers and assistants maintain fluid working relationships. 25. Quantitative Statements in Standards of Three Accrediting Bodies: APA, CACREP, and CSWE.

Abstract

The accreditation manual of the Council for Accrediation of Counseling and Related Education Programs (CACREP) contains a statement that, whenever possible, criteria for assessing standards should be qualitative rather than quantitative. The study reported in this paper was conducted to gather baseline data on the extent to which accrediting bodies have included quantitative terms in standards related to program goals, structures, and processes. Quantitative statements were defined as any reference in an accreditation standard where a numerical value could be attached. Minimum hours, minimum degree levels, numbers of required semesters or quarters, ratios of students to faculty, and percent of increase in funding for library materials are examples of terms that met the criteria for being identified as quantitative. The accreditation manuals of the American Psychological Association (APA), the Council on Social Work Education (CSWE), and CACREP were examined to determine the presence of quantitative statements in the standards of these bodies. A model which included 47 categories of accrediting information revealed that, when compared to APA and CSWE, CACREP had more categories which included quantitative information and was more expansive on quantitative statements within each category. Abstract

Summary Background Heterogeneity is a major obstacle to developing effective treatments for patients with primary Sjogren's syndrome. We aimed to develop a robust method for stratification, exploiting heterogeneity in patient-reported symptoms, and to relate these differences to pathobiology and therapeutic response. Methods We did hierarchical cluster analysis using five common symptoms associated with primary Sjogren's syndrome (pain, fatigue, dryness, anxiety, and depression), followed by multinomial logistic regression to identify subgroups in the UK Primary Sjogren's Syndrome Registry (UKPSSR). We assessed clinical and biological differences between these subgroups, including transcriptional differences in peripheral blood. Patients from two independent validation cohorts in Norway and France were used to confirm patient stratification. Data from two phase 3 clinical trials were similarly stratified to assess the differences between subgroups in treatment response to hydroxychloroquine and rituximab. Findings In the UKPSSR cohort (n=608), we identified four subgroups: Low symptom burden (LSB), high symptom burden (HSB), dryness dominant with fatigue (DDF), and pain dominant with fatigue (PDF). Significant differences in peripheral blood lymphocyte counts, anti-SSA and anti-SSB antibody positivity, as well as serum IgG, κ-free light chain, β2-microglobulin, and CXCL13 concentrations were observed between these subgroups, along with differentially expressed transcriptomic modules in peripheral blood. Similar findings were observed in the independent validation cohorts (n=396). Reanalysis of trial data stratifying patients into these subgroups suggested a treatment effect with hydroxychloroquine in the HSB subgroup and with rituximab in the DDF subgroup compared with placebo. Interpretation Stratification on the basis of patient-reported symptoms of patients with primary Sjogren's syndrome revealed distinct pathobiological endotypes with distinct responses to immunomodulatory treatments. Our data have important implications for clinical management, trial design, and therapeutic development. Similar stratification approaches might be useful for patients with other chronic immune-mediated diseases. Abstract

Public Health England (2013) survey data indicates that while the place of death is geographically uneven across England, given a choice, many older people nearing end of life would prefer to die at home. There is, however, a growing critique that policies designed to support home death fail to understand the needs and preferences of older people and the impact on family carers. Such policies also make assumption about within whose home the home death takes place. Hence, there are major gaps in our understanding of firstly, where and how care work undertaken by family members within domestic settings takes place; and secondly, how it can create tensions between home and care that fundamentally disrupt the physical and socio-emotional meaning of home for family carers, impacting on their sense of home post-death. This can have consequences for their own well-being. In this paper we draw on interview data from our ‘Unpacking the Home’ study to elicit an in-depth understanding of how facilitating a home death can create an ambiguity of place for family carers, where the issues faced by them in caring for a dying older person at home, and the home death itself, can fundamentally reshape the meaning and sense of home.

Background: Older people aged 80 and over are increasingly providing end of life care to spouses at home, and often do so for long periods of time, whilst also trying to manage their own illnesses and disabilities. Little of the research on older spousal carers has focussed on the oldest carers, hence the needs of this particular population are not fully known. Objective: To explore the experiences of the ‘oldest carers’ in caring for a dying spouse at home. Methods: Secondary analysis was undertaken on a subset of data from a larger qualitative interview study; this dataset comprised 17 interviews from participants aged 80 or over. Framework analysis methods were used, with items derived from the thematic analysis of the main study. Results: The oldest carers in this subset demonstrated high levels of resilience, and the ability to adapt to their caring role. Caring until death was accepted as an integral part of the commitment made to their partner as part of the ‘wedding contract’. Carers felt they benefitted from the support provided by family, friends and care services; however their own care needs were not always recognised by health and social care services. Conclusions: These findings underscore the complexity of the oldest carers’ experiences and challenges in times of illness and end of life. Healthcare professionals should be alerted to the myriad ways caregiving is enacted in serious illness and seek opportunities for developing supportive interventions specifically for older carers.
