The coronavirus disease 2019 (COVID-19) pandemic is unlike anything seen before by modern science-based medicine. Health systems across the world are struggling to manage it. Added to this struggle are the effects of social confinement and isolation. This brings into question whether the latest guidelines are relevant in this crisis. We aim to support urologists in this difficult situation by providing tools that can facilitate decision making, and to minimise the impact and risks for both patients and health professionals delivering urological care, whenever possible. We hope that the revised recommendations will assist urologist surgeons across the globe to guide the management of urological conditions during the current COVID-19 pandemic. Forty-four supracondylar and intercondylar fractures of the femur treated with 95 degree angulated condylar plates and Dynamic Condylar Screws (DCS) were retrospectively evaluated. AII patients were treated by open reduction and internal fixation. The follow-up periods ranged between 1.5-11.2 years (mean, 4 years). Fractures united 2 to 6 months clinically and 2.5 to 7 months radiologically in the final controls. Clinically 35 patients (79.6%) had excellent/good results nine patients (20.4%) had fair/failure results. Radiologically 39 patients (88.6%) had excellent/good results. Five patients (11.4%) had fair/failure results. Of the patients who were treated with 95 degree angulated condylar plates, clinically, 15 (75%) had excellent/good results and 5(25%) fair/failure; and of the patients who were treated with DCS, 20 (83.5%) had excellent/good results and 4 (16.5%) had fair/failure results. We conclude that both L -plate and DCS can be applied in the treatment of isolated supracondylar femur fractures. With the application of DCS, better outcomes can also be anticipated in the remaining fractures., 41 olgunun 44 suprakondiler ve interkondiler femur kınğı açık redüksiyon ve internal fiksasyon yöntemiyle tedavi edildi. 20 olguda 95 derece açılı L-plak, 34 olguda ise Dinamik Kondiler Çivili Plak (DCS) kullanıldı. 0lgular 1.5 ile 11.2 yıl arasında takip edildi. Klinik olarak kaynama süresi 2-6 ay, radyolojik olarak ise 2.5-7 ay arasındaydı. Olgular son kontrollerinde klinik ve radyolojik olarak değerlendirildi. Klinik olarak %79.6 ''çok iyi ve iyi'', %20.4 ''orta ve yetersiz'' radyolojik olarak da; %88.6 ''çok iyi ve iyi '', % 11.4 ''orta ve yetersiz'' sonuç alındı. Klinik olarak L-plak uygulayarak tedavi ettiğimiz olguların 15'inde (%75) ''çok iyi ve iyi'', S'inde (%25) 'orta ve yetersiz', DCS kullanarak tedavi ettiğimiz olguların ise 20'sinde (%83.5) ''çok iyi ve iyi'', 4'ünde (% 16.5) ''orta ve yetersiz'' sonuç alındı. Sonuç olarak, izole suprakondiler femur kırıklarında hem L-plak hem de DCS kullamlabileceği, diğer kırıkların tedavisinde ise DCS kullanılarak daha iyi sonuçların alınabileceğini düşünüyoruz.

In this study, our aim was to determine the success of extending extremity by the method of callotasis, using external fixator with ''bilateral frame'' following osteotomy and also to investigate the effect of the level of osteotomy on to the results whether applying it on the metaphysis or on diaphysis. We applied the osteotomy on 6 dogs at the level of metaphysis and on 6 dogs at the level of diaphysis. After waiting for 10 days, we have carried out distraction about 0.25 mm long in evety twelve hours throughout 40 days. We took roentgenograms at the third, fourth, seventh and tenth weeks of osteotomy. At the end of tenth week, we made macroscopic and radiographic evaluation of 12 femurs. Later, we examined six of these femurs histologicall (three of them were metaphyseal and three of them were diaphyseal) and the other six femurs biomechanically (three of them were metaphyseal and three of them were diaphyseal). The average Iength increase was 2 cm (11%) and there was no significant difference between these two osteotomies. But the roentgenographic, histological and biomechanical evaluations were more successful in the favor of the metaphyseal osteotomy. We were successful to unite all of the bones without any complications. We may suggest according to our findings which we have mentioned above, that the metaphyseal osteotomy technique will give better results than diaphyseal osteotomies on extending extremities with the method of callotasis. Furthermore we think that callotasis is a reliable method with lesser complications in the operations of extending extremities, Çalışmamızda; osteotomiyi takiben ''bilateral freymli'' eksternal fiksatör kullanarak kallotasis yoluyla ekstremite uzatma işleminin başarı durumunun belirlenmesi ve bu uzatma işleminde, osteotominin metafiz ya da diafizden yapılmasının, sonuçlara etkisinin araştınlması amaçlandı. Osteotomiden hemen sonra, 3, 5, 7 ve 10 haftalarda röntgen filmleri çekildi. 10 haftalık deney sonunda 12 femur önce makroskopik ve radyolojik olarak incelendi. Daha sonra bunlardan 6'51 (3'ü metafizer, 3'ü diafizer) histolojik, altısıda (3'ü metafizer, 3'ü diafizer) biyomekanik incelemeye alındı. Uzatma sonrası kazanılan ortalama uzunluk 2 cm (%11) olup, metafizer veya diafizer osteotomi yapılan gruplar arasında belirgin bir fark yoktu. Fakat rontgenografik, histolojik ve biyomekanik inceleme sonuçları, metafizer osteotomi yapılan grup lehine daha başarılı idi. Tüm osteotomilerde yeterli kaynama temin edildi ve önemli bir komplikasyon görülmedi Biz yukarıdaki bulguları gözönüne alarak, kallotasis metoduyla yapılacak olan ekstremite uzatmalarındaki; metafizer osteotomi tekniğinin diafizer osteotomilere göre daha başarılı sonuçlar vereceğini ve ekstremite uzatma ameliyatlarında kallotasis işleminin çok az komplikasyonları olan, güvenilir bir metod olduğunu düşünmekteyiz.

Distally-based fasciocutaneous flaps from sural region were used to reconstruct defects around the lower third of the leg, the ankle, and the foot in three patients. Successful soft tissue coverage was achieved in all patient. The results show that this flap can be used as a reliable alternative to the other flaps to repair defects around the lower third of the leg, the ankle, and the foot., Bacağın 1/3 distal kısmı, ayak bileği ve ayakta yumuşak doku defekti olan üç hastada rekonstrüksiyon amacıyla sural bölgeden distal pediküllü fasyokutanöz flep kullanıldı. Hastaların tamamında yumuşak doku defektleri başarılı bir şekilde kapatıldı. Bulgularımıza göre bu flep, bacağın 1/3 distal bölgesi, ayak bileği ve ayaktaki yumuşak doku defektlerinin rekonstrüksiyonunda diğer fleplere alternatif olarak kullanılabilir.

The purpose of this study was to determine the effect of head injury on fracture healing. Between January 1989-December 1990 a group of 34 patients who had diaphyseal fracture with head injuries and the control group of 15 patients who had only diaphyseal fracture admitted to our clinic was the subject of this study. We were led to the conclusion that rapidly fracture healing and excessıve callus formation the patient with head injuried group might be because of higher excretion of growth hormone then normal., Bu çalışmamızın amacı, kafa travmasımn kırık iyileşmesi üzerine etkisini belirlemekti. Ocak 1989-Aralık 1990 tarihleri arasında kliniğimize başvuran kafa travması ve beraberinde diafiz kırığı olan 34 hastadan oluşan grup ile, kafa travması olmaksızın diafiz kırığı olan 15 hastadan oluşan kontrol grubu bu çalışmamızın konusunu teşkil etmiştir. Hastaların tamamında kırıktan bir ve üç hafta sonra kan serumunda büyüme hormonu seviyelerine bakıldı. Radyolojik olarak kırık iyileşme cevabı ölçüldü. Kafa travması olan hastalarda travma sonrası Bilgisayarlı Beyin Tomografisi (BBT) yapılarak, travmanın beyinde yaptığı hasarın ciddiyeti tespit edildi. Kafa travması olan gruptaki serum büyüme hormonu değerleri ve kırık iyileşme cevabı kontrol grubuna göre daha yüksek bulundu. Kafa travması beraberinde diafiz kırığı olan hasta grubundaki hızlı kırık iyileşmesi ve aşırı kallus oluşumuna, bu hastalarda normalde fazla büyüme hormonu salınımının neden olabileceği kanaatine varıldı.

The surface properties of the polymeric nanoparticulate drug delivery systems play a key role on the biological behaviour that is shown in the organism. By the modification of the surface properties of these systems, targeting potential and cellular uptake levels are increased, blood circulation times are prolonged, especially the accumulation in the tumor tissues is improved to higher levels.
