In the framework of the archaeometric research on Apulian Late Antique glass, a collection of 24 glass vessels (third to seventh centuries ad) and production indicators found at Herdonia (Foggia, Italy) was examined, by means of SEM-EDS, EMPA, ICP-MS and LA-ICP-MS. A fragment of crucible was further investigated by SEM-EDS and EMPA. The results obtained were discussed together with another glass collection from Herdonia previously investigated. The full set of 48 samples was made of siliceous sands mixed with plant ashes (two samples: a dark green slab and a colourless beaker/lamp) or natron (all the other 46 samples). The latter were assigned to the following compositional groups: HIMT, Levantine 1, RNCBGY1, RNCBGY2, RC/LAC-Sb, intermediate HIMT/RNCBGY1, intermediate RNCBGY1/HIMT, intermediate Levantine/RNCBGY2 and two other groups, the CaO-rich HIMT and the CaO-rich/Na2O poor HIMT, which showed a prevailing Adriatic distribution and a close similarity to the weak HIMT glass group. Most Herdonia samples were similar to the HIMT productions, and thus of likely Egyptian origin, while a smaller number of samples referred to the Levantine productions. [ABSTRACT FROM AUTHOR]

A collection of 30 ceramic samples, 16 of coarse wares and 14 of fine painted wares, have been investigated by optical microscopy, scanning electron microscopy, inductively coupled plasma optical emission spectrometry, inductively coupled plasma mass spectrometry and neutron activation analysis. Further samples of clayey sediments, both locally outcropping and found within the settling tank, have been submitted to the same analytical techniques for comparison with the ceramic collection. The results demonstrated that local clayey sediments were used as received for the production of coarse wares. The same raw materials were sieved and/or refined by decantation for the production of fine painted wares, which, in fact, provided results that were perfectly comparable with those for the clayey raw materials found within the settling tank. The Faragola productions were distinguished from the neighbouring Apulian productions according to petrographic features (the presence of leucite-bearing volcanic rocks and Mn-rich wads) and their bulk chemical composition. Late Antique coarse cooking wares and painted fine wares found at Herdonia (second half of the fourth century to mid-fifth century ad) and Canusium (late sixth century to early seventh century ad) have been chemically and mineralogically characterized. A total of 74 samples (40 of coarse ware and 34 of fine painted ware) was investigated through optical microscopy, scanning electron microscopy, X-ray powder diffraction, inductively coupled plasma optical emission spectrometry, inductively coupled plasma mass spectrometry, neutron activation analysis and X-ray fluorescence. A new statistical method, namely the classification tree methodology, was used for the treatment of geochemical data. The characterization of the Herdonia and Canusium assemblages was combined with a review of earlier results obtained for San Giusto and Posta Crusta, in order to get an insight on Late Antique ceramic trades in northern Apulia. It appears possible to reconstruct a production pattern organized at multiple production sites, both rural and urban, that exploited similar raw material deposits, specialized in certain productions, and commercialized products at different geographical scales. Imports from outside northern Apulia may be identified for coarse wares. A likely area of production is difficult to establish; however, the northern Adriatic coast and the area of Greece may be suggested. [ABSTRACT FROM AUTHOR]

SEM–EDS, EMPA, ICP–MS and ICP–OES were applied to the study of three sectilia panels made of red, orange and yellow glass slabs. In order to determine the chemical state of Cu, XAS was also performed. The results indicate that, with the exception of the red samples, all slabs were made by mixing a siliceous sand with natron. The red slabs show significant compositional differences with respect to K2O and MgO contents, probably linked to the use of plant ashes as a source of alkali. The red, orange and yellow slabs are coloured by metallic copper, cuprite and Pb antimonates, respectively. The comparison between the chemical composition of the Faragola samples and several glass reference groups did not provide conclusive evidence of provenance. [ABSTRACT FROM AUTHOR]

Background: 11% of new cancer diagnoses occur in the emergency department. Historically, these diagnoses disproportionately affect underserved patient populations and are associated with poor outcomes. This is an observational study of the Rapid Assessment Service (RAS) program, which aims to provide timely outpatient follow-up and facilitate a diagnosis for patients discharged from the emergency department with suspected malignancies., Methods: We performed a retrospective chart review of 176 patients who were discharged from the emergency department with RAS clinic follow up between February 2020 and March 2022. We manually chart reviewed 176 records in order to determine the average time to RAS clinic appointment, average time to diagnosis, and the final diagnosis based on biopsy., Results: 163 of 176 patients (93%) discharged to RAS received reliable follow-up care. 62 of the 176 patients (35%) followed up in the RAS clinic with a mean of 4.6 days. 46 of the 62 patients (74%) who followed up in the RAS clinic were ultimately diagnosed with a new cancer, with a mean time to diagnosis of 13.5 days. Background: The purpose of this study was to determine the impact of bariatric surgery on employment status in underserved, unemployed patients with severe obesity., Methods: A retrospective review of all unemployed severely obese patients seen in our urban safety-net bariatric surgery program was performed. Preoperative patient questionnaires and medical records were reviewed to evaluate patient employment status at the time of initial evaluation by the multidisciplinary bariatric surgery team. Follow-up data was obtained on all available patients (including those who did not undergo surgery), including weight and employment status. A standardized telephone questionnaire was administered to supplement details regarding employment. Changes in employment status and body weight were determined in both groups., Results: Here, 193 unemployed severely obese patients were evaluated by the multidisciplinary obesity team. The vast majority of patients (>80%) were minorities (primarily Hispanic) and publicly insured. Seventy-two underwent bariatric surgery and 121 did not. Twenty-four percent of the surgical patients and 9% of the non-surgical patients had acquired full-time employment at least one year postoperatively (p = 0.043). There was a 10-point body mass index reduction in the surgical group, compared to 1-point reduction in the non-surgical group after one year., Conclusions: Bariatric surgery may improve employment status in an unemployed severely obese patient cohort. Future research in this area should collect detailed prospective data on employment prior to surgery and assess changes longitudinally to provide a more complete picture of the impact of bariatric surgery on employment.

Glucose obtained from unprocessed blood samples can decrease by 5%-7% per hour due to glycolysis. This study compared the impact of glucose degradation on measured glucose values by examining two different collection methods. For the first method, blood samples were collected in tubes containing sodium fluoride (NaF), a glycolysis inhibitor. For the second method, blood samples were collected in tubes containing a clot activator and serum gel separator and were centrifuged to separate the serum and plasma 20 minutes after sample collection. The samples used in the two methods were collected during the same blood draw and were assayed by the clinical laboratory 2-4 hours after the samples were obtained. A total of 256 pairs of samples were analyzed. The average glucose reading for the centrifuged tubes was significantly higher than the NaF tubes by 0.196 ± 0.159 mmol/L (P < 0.01) or 4.2%. This study demonstrates the important role collection methods play in accurately assessing glucose levels of blood samples collected in the field, where working environment may be suboptimal. Therefore, blood samples collected in the field should be promptly centrifuged before being transported to clinical labs to ensure accurate glucose level measurements.

Chondral injury in the knee is a unique challenge to the orthopaedic surgeon. Given the high probability of progression to knee arthrosis, the treatment of symptomatic cartilage defects of the knee has become an important surgical intervention in young, active patients. The demand for an alternative to prosthetic resurfacing has driven the trend towards biologic resurfacing and joint preservation. Osteochondral allografts are composed of hyaline cartilage attached to subchondral bone and are suited for large osteochondral lesions. This allograft tissue must be harvested, processed, and stored appropriately to reduce the risks of graft failure and potential complications. With appropriate indications and surgical techniques, osteochondral allografts have been shown to have good long-term graft survival and patient outcomes.

Objectives: To determine the point prevalences of metabolic syndrome (MetS) and its components among healthy weight, overweight, and obese inner-city public high school students, to compare the prevalences of MetS when using 2 different definitions (one with the impaired fasting glucose [IFG] level and the other with a homeostasis model assessment of insulin resistance [HOMA-IR] of 3.99 or higher to define the glucose regulation component), and to compare the degree to which HOMA-IR and fasting glucose level are associated with the other MetS components., Design: Cross-sectional analysis., Setting: Two New York City public high schools, from April 2008 through August 2011., Participants: Convenience sample of 1185 high school youth, comprising predominantly Hispanic and African American students from low-income households, participating in The Banishing Obesity and Diabetes in Youth Project, a medical screening and education program., Main Outcome Measures: Prevalences of the following individual MetS components: IFG threshold, HOMA-IR, hypertension, central adiposity, hypertriglyceridemia, and low high-density lipoprotein cholesterol. Rates of MetSIFG and MetSHOMA-IR were also assessed., Results: MetSIFG and MetSHOMA-IR point prevalences were both 0.3% in the healthy weight group; they were 2.6% and 5.9%, respectively, in the overweight group and were 22.9% and 35.1%, respectively, in the obese group (P < .05 for both). An IFG threshold of 100 mg/dL or higher was found in 1.0% of participants, whereas a HOMA-IR of 3.99 or higher was found in 19.5% of participants., Conclusions: An elevated HOMA-IR is much more sensitive than an IFG threshold in identifying adolescents with metabolic dysregulation. Using a HOMA-IR threshold of 3.99 identifies more youth with MetS than using an IFG threshold of 100 mg/dL. In addition to increasing the sensitivity of MetS detection, HOMA-IR has a much higher association with the other MetS components than the IFG threshold and may better reflect a unified underlying pathologic process useful to identify youth at risk for disease.
