Understanding SARS-CoV-2 seroprevalence among health care personnel is important to explore risk factors for transmission, develop elimination strategies and form a view on the necessity and frequency of surveillance in the future.We enrolled 4927 health care personnel working in pediatric units at 32 hospitals from 7 different regions of Turkey in a study to determine SARS Co-V-2 seroprevalence after the first peak of the COVID-19 pandemic. A point of care serologic lateral flow rapid test kit for immunoglobulin (Ig)M/IgG was used. Seroprevalence and its association with demographic characteristics and possible risk factors were analyzed.SARS-CoV-2 seropositivity prevalence in health care personnel tested was 6.1%. Seropositivity was more common among those who did not universally wear protective masks (10.6% vs 6.1%). Having a COVID-19-positive co-worker increased the likelihood of infection. The least and the most experienced personnel were more likely to be infected. Most of the seropositive health care personnel (68.0%) did not suspect that they had previously had COVID-19.Health surveillance for health care personnel involving routine point-of-care nucleic acid testing and monitoring personal protective equipment adherence are suggested as important strategies to protect health care personnel from COVID-19 and reduce nosocomial SARS-CoV-2 transmission.

- Abstract
Objectives: the aim of this study is to identify the epidemiological, clinical, and laboratory features of coronavirus disease 2019 (COVID-19) in children.Methods: a retrospective study was conducted by pediatric infectious disease specialists from 32 different hospitals from all over Turkey by case record forms. Pediatric cases who were diagnosed as COVID-19 between March 16, 2020, and June 15, 2020 were included. Case characteristics including age, sex, dates of disease onset and diagnosis, family, and contact information were recorded. Clinical data, including the duration and severity of symptoms, were also collected. Laboratory parameters like biochemical tests and complete blood count, chest X-ray, and chest computed tomography (CT) were determined. Results: there were 1,156 confirmed pediatric COVID-19 cases. In total, male cases constituted 50.3% (n = 582) and females constituted 49.7% (n = 574). The median age of the confirmed cases was 10.75 years (4.5-14.6). Of the total cases, 90 were younger than 1 year of age (7.8%), 108 were 1-3 years of age (9.3%), 148 were 3-6 years of age (12.8%), 298 were 6-12 years of age (25.8%), 233 were 12-15 years of age (20.2%), and 268 cases were older than 15 years of age (23.2%). The most common symptom of the patients at the first visit was fever (50.4%) (n = 583) for a median of 2 days (IQR: 1-3 days). Fever was median at 38.4 degrees C (38.0-38.7 degrees C). The second most common symptom was cough (n = 543, 46.9%). The other common symptoms were sore throat (n = 143, 12.4%), myalgia (n = 141, 12.2%), dyspnea (n = 118, 10.2%), diarrhea (n = 112, 9.7%), stomachache (n = 71, 6.1%), and nasal discharge (n = 63, 5.4%). When patients were classified according to disease severity, 263 (22.7%) patients were asymptomatic, 668 (57.7%) patients had mild disease, 209 (18.1%) had moderate disease, and 16 (1.5%) cases had severe disease. GİRİŞ ve AMAÇ: Bu çalışmada, kliniğimize septik artrit öntanısı ile başvuran olguların klinik ve laboratuvar bulgularının ve tedavi yanıtlarının değerlendirilmesi amaçlandı.YÖNTEM ve GEREÇLER: Ocak 2010-Ocak 2019 tarihleri arasında artrit öntanısıyla yatmış olan 111 çocuk (0-18 yaş) hasta kayıtları retrospektif incelendi. Olguların klinik, laboratuvar bulgularının değerlendirilmesi, tedavi ve prognostik özelliklerinin incelenmesi planlandı.BULGULAR: Çalışmaya alınan toplam 111 hastanın %66’sı erkek olup ortalama yaşları 91±56 (medyan 83,1-215 aralığı) ay idi. Olguların çoğunluğunu (n=62,%56) 3-10 yaş aralığındaki hastalar oluşturdu. Olguların %60’ına (n=67) septik artrit tanısı konuldu. Bu tanıyı reaktif artrit (%10), juvenil idiopatik artrit (%10), toksik/geçici sinovit (%5) ve diğer artritler takip etti. Başvuruda olguların %96’sında ağrı, %63’ünde eklem şişliği, %21’inde kızarıklık, %41’inde eklemde ısı artışı, %64’ünde hareket kısıtlılığı, %38’inde yürüyememe yakınması vardı. Sıklıkla tutulan eklemler diz (%51) ve kalçaydı (%35). Ateş yüksekliği olan olgularda septik artrit olasılığı yüksek saptandı (p=0,0001). Septik artrit dışı artritlerde ibuprofene yanıt daha fazlaydı (p=0,0001). Olguların %55’ine (n=61) ponksiyon yapıldı, %34’ü (n=38) eklem içi debridman operasyonu geçirdi. Eklem sıvı kültüründe en sık üreyen mikroorganizmalar Staphylococcus aureus ve Streptococcus pyogenes idi. Septik artrit ile septik artrit dışı olgular karşılaştırıldığında, ultrasonografi ile ölçülen efüzyon miktarı, ponksiyonla alınan sıvı miktarı septik artrit grubunda istatistiksel olarak anlamlı oranda daha fazla, CRP ve lökosit sayısı ise septik artrit grubunda daha yüksek bulundu (sırasıyla p=0,001;p=0,025;p=0,018;p=0,032). Olguların %19’unda (n=21) osteomyelit saptanıp hepsi septik artrit grubundaydıTARTIŞMA ve SONUÇ: Bu çalışmada ateşi olan, lökosit sayısı >12100/mm3, CRP>3 mg/dl üzerinde olan olgularda, ultrasonografi ile 8,5 mm ve üzerinde efüzyon ölçülen olgularda septik artrit olma olasılığı istatistiksel anlamlı bulundu. Septik artrit dışı artritlerde ibuprofen yanıtı daha fazlaydı.

- Abstract
Objective: in the early stages of any epidemic caused by new emerging pathogens healthcare personnel is subject to a great risk. Pandemic caused by SARS-CoV-2, proved to be no exception. Many healthcare workers died in the early stages of pandemic due to inadequate precautions and insufficient protection. It is essential to protect and maintain the safety of healthcare personnel for the confinement of pandemic as well as continuity of qualified healthcare services which is already under strain. Educating healthcare personnel on appropiate use of personal protective equipment (PPE) is as essential as procuring them. Material and methods: a survey is conducted on 4927 healthcare personnel working solely with pediatric patients from 32 different centers. Education given on PPE usage were questioned and analyzed depending on age, sex, occupation and region. Results: among four thousand nine hundred twelve healthcare personnel from 32 different centers 91% (n=4457) received education on PPE usage. Of those who received education only 36% was given both theoretical and applied education. Although there was no differences among different occupation groups, receiving education depended on regions. Rice husk ash is a side-product of rice production; thus, it is a cheap, abundant, and renewable material, and utilized as an enzyme carrier to immobilize Candida antarctica lipase B. In this study, Candida antarctica lipase B immobilized onto rice husk ashes was used to catalyze ring opening polymerization of 16-membered lactone, ω-pentadecalactone. In order to determine the best polymerization conditions for highest molar mass polymer, reactions were proceeded at various temperatures and time periods. The best reaction conditions were obtained as 80°C and 6 hours (Mn= 34255 g mol-1). Molecular structure of this polymer sample was confirmed via proton nuclear magnetic resonance spectroscopy (¹H-NMR) and Fourier transform infrared spectroscopy (FTIR). Thermal properties were investigated by differential scanning calorimetry (DSC) and thermal gravimetric analysis (TGA). Consequently, poly( ω-pentadecalactone) was synthesized enzymatically with rapid polymerization tendency and improved thermal properties which were quite close to low-density polyethylene. Successful polymerization results showed that Candida antarctica lipase B immobilized onto rice husk ashes may be a good alternative to commercial immobilized form, Novozyme 435, as a result of its comparable activity and low cost. Çocukluk çağı kanserlerinin birçoğunun etyolojisi net açıklanamamıştır. Son çalışmalar, anomalilerin genetik ve çevresel faktörlerin de etkisi ile kanser gelişiminde rol alabileceğini göstermiştir. Bu çalışmanın amacı, maligniteler ve konjenital anomaliler arasındaki ilişkinin araştırılması ve çeşitli kanserlere en sık eşlik eden anomalilerin belirlenmesidir. Çalışmamızda Uludağ Üniversitesi Tıp Fakültesi Çocuk Onkoloji Bilim Dalı'nda takipli, solid tümör veya lenfoması olan 506 hasta incelenmiştir. Hastalardaki malignite dağılımına bakıldığında 135 (%27) hasta sayısı ile lenfomalar birinci sıradadır. Bunu 65 (%13) hasta ile santral sinir sistemi tümörleri ve 63 (%12) hasta ile germ hücreli tümörler takip etmektedir. Hastaların %33.5'inde (n=170) anomali olduğu görülmüştür. Bu 170 hastada, 241 farklı anomali tanımlanmıştır. Bu anomalilerin %30.7'sinin göz, kulak, yüz, boyun anomalileri, %18.25'inin kardiyovasküler sistem anomalileri, %10.78'inin ürogenital sistem anomalileri, %9.12'sinin SSS anomalileri, %7.05'inin kas iskelet sistemi anomalileri, %6.27'sinin sindirim sistemi anomalileri, %2.48'inin solunum sistemi anomalileri, %15.35'inin ise diğer anomaliler grubunun anomalileri olduğu görülmüştür. Hastaların demografik özelliklerine bakıldığında; hastaların %44.6'sı (n=226) kız, %55.4'ü (n=280) erkektir. Hastaların ortalama yaşı 7.34±5.46 yıldır. Doğumdaki ortalama anne yaşı 26.83±5.17, ortalama baba yaşı ise 30.02±5.41'tir. Soygeçmişlerine bakıldığında %20.4'ünde (n=103) annede düşük veya ölü doğum öyküsü olduğu, %28.1'inde (n=142) akrabalarında kanser öyküsü olduğu ve %14.2'sinin (n=72) anne ve babası arasında akrabalık olduğu görülmüştür. Anomalisi olan hastaların %25'inin doğumdaki baba yaşının 35 yaş ve üzeri olduğu görülmüştür (p:0.004). Malignitelerde anomali görülme sıklığı incelendiğinde; SSS tümörü olan hastalarda anomali görülme sıklığı %61.5 (n=40) olarak saptanmıştır (p

OBJECTIVE: Head trauma is one of the most important emergency health problems both in the world and in our country. The objective in our study is to (i) state the correlation between the findings of bispectral index score (BIS) and computed tomography (CT), which are used to evaluate the level of consciousness of patients with isolated head trauma, and (ii) investigate objective results about the patient's level of consiousness/alertness according to the CT modality, which is used frequently. METHODS: This prospective study was carried out between 03.01.2014 and 09.01.2014 in the emergency department of Fatih Sultan Mehmet Education and Research Hospital. The average BIS scores were correlated with the Glasgow Coma Scale (GCS) point, the Canadian CT Head Rule major and minor criteria, and the pathologic findings in CT imaging. The patients' demographic features, vital signs at admission, and arrival times at the hospital were investigated. RESULTS: In our study, 64 (31.7%) patients were female, and 138 (68.3%) patients were male. The mean BIS scores were 84.99±11.20 (86.05) and 93.78±3.80 (95.05) in patients with and without CT pathologies, respectively. The correlation between CT pathology and BIS scores was statistically significant: BIS scores were lower in patients with CT pathologies (p=0.001; p<0.01). There was a statistically significant positive correlation between the BIS and GCS scores (45.6%) (p<0.05). CONCLUSION: We showed that most head traumas occur after dangerous accidents, and according to the results, we can predict that males are more frequently affected than females. There was a statistically significant positive correlation between BIS scores and GCS points. In our study, the BIS scores were statistically significantly lower in patients with CT pathology than in patients without. We can predict that if the BIS score of the patient is low, then there will be the presence of pathology on CT imaging. [ABSTRACT FROM AUTHOR]

Giriş: Bu çalışmada Bursa Uludağ Üniversitesi Tıp Fakültesi Hastanesi’nde Çocuk Yoğun Bakım Ünitesinde (ÇYBÜ) ve Yenidoğan Yoğun Bakım Ünitesinde (YDYBÜ) saptanan Sağlık Bakımı İlişkili Enfeksiyonların (SBİE) insidansı, dansitesi ve karşılıklı ilişkilerinin genel bir durum tespiti çerçevesinde değerlendirilmesi amaçlandı. Gereç ve Yöntem: Bu çalışmada Hastalık Kontrol ve Korunma Merkezleri (CDC) 2015 ve Ulusal Hastane Enfeksiyonları Sürveyans Ağı (UHESA) 2017 kriterleri dikkate alınarak 2012 - 2018 yılları arasında SBİE gelişen 91 ÇYBÜ hastası ve 158 YDYBÜ hastasının verileri retrospektif olarak incelendi. Bulgular: ÇYBÜ ve YDYBÜ de sırasıyla; SBİE hızı, ÇYBÜ’ye göre YDYBÜ’de daha yüksek (%9,6’ya karşı %14,9; p2501 g patients. HCAIs rates were found to be higher in babies with >1501 g. Conclusions: There may be differences in the rates and prevention strategies in PICU and NICU and continuous and high quality maintenance is important for infection control measures.

Objectives: the aim of this study is to identify the epidemiological, clinical, and laboratory features of coronavirus disease 2019 (COVID-19) in children.Methods: a retrospective study was conducted by pediatric infectious disease specialists from 32 different hospitals from all over Turkey by case record forms. Case characteristics including age, sex, dates of disease onset and diagnosis, family, and contact information were recorded. Clinical data, including the duration and severity of symptoms, were also collected. Laboratory parameters like biochemical tests and complete blood count, chest X-ray, and chest computed tomography (CT) were determined. Results: there were 1,156 confirmed pediatric COVID-19 cases. In total, male cases constituted 50.3% (n = 582) and females constituted 49.7% (n = 574). The median age of the confirmed cases was 10.75 years (4.5-14.6). Of the total cases, 90 were younger than 1 year of age (7.8%), 108 were 1-3 years of age (9.3%), 148 were 3-6 years of age (12.8%), 298 were 6-12 years of age (25.8%), 233 were 12-15 years of age (20.2%), and 268 cases were older than 15 years of age (23.2%). The most common symptom of the patients at the first visit was fever (50.4%) (n = 583) for a median of 2 days (IQR: 1-3 days). Fever was median at 38.4 degrees C (38.0-38.7 degrees C). The second most common symptom was cough (n = 543, 46.9%). The other common symptoms were sore throat (n = 143, 12.4%), myalgia (n = 141, 12.2%), dyspnea (n = 118, 10.2%), diarrhea (n = 112, 9.7%), stomachache (n = 71, 6.1%), and nasal discharge (n = 63, 5.4%). When patients were classified according to disease severity, 263 (22.7%) patients were asymptomatic, 668 (57.7%) patients had mild disease, 209 (18.1%) had moderate disease, and 16 (1.5%) cases had severe disease. One hundred and forty-nine (12.9%) cases had underlying diseases among the total cases; 56% of the patients who had severe disease had an underlying condition (p < 0.01). The need for hospitalization did not differ between patients who had an underlying condition and those who do not have (p = 0.38), but the need for intensive care was higher in patients who had an underlying condition (p < 0.01). Forty-seven (31.5%) of the cases having underlying conditions had asthma or lung disease (38 of them had asthma). Conclusions: to the best of our knowledge, this is one of the largest pediatric data about confirmed COVID-19 cases. Children from all ages appear to be susceptible to COVID-19, and there is a significant difference in symptomatology and laboratory findings by means of age distribution., NA

Background:Acute appendicitis is one of the challenging surgical conditions presented in the emergency departments. Clinical scoring systems were developed to reduce the negative appendectomy rate and also to avoid unnecessary diagnostic evaluation.Objectives:The primary aim was to compare the clinical adequacy of the Alvarado, Acute Inflammatory Response, and the Raja Isteri Pengiran Anak Saleha Appendicitis scores in patients with right lower quadrant pain for the diagnosis of acute appendicitis.Methods:This was a prospective and observational study. All patients over the age of 18 years who presented with a complaint of right lower quadrant pain were enrolled. The Alvarado, Acute Inflammatory Response, and Raja Isteri Pengiran Anak Saleha Appendicitis scoring systems were compared. The patients were either admitted or followed-up as out-patient. Face-to-face or telephone follow-up visits were arranged for the patients who did not have surgery and who were not admitted.Results:232 patients were included and 14 patients were excluded from the study. Of the 218 patients, 114 patients underwent surgery. Of the 114 patients, 107 patients were pathologically diagnosed with acute appendicitis. It was determined that Raja Isteri Pengiran Anak Saleha Appendicitis score was the most valuable score with 0.88 accuracy, followed by Acute Inflammatory Response (area under the curve = 0.79) and Alvarado (area under the curve = 0.71) scores.Conclusion:The accuracy of Raja Isteri Pengiran Anak Saleha Appendicitis scoring system was higher for the diagnosis of acute appendicitis than the other scores. The cut-off of the Raja Isteri Pengiran Anak Saleha Appendicitis score from a 7.5-point threshold provides a practical, non-invasive, rapid diagnostic method that increases acute appendicitis discriminative power in patients presenting with right lower quadrant pain.

Bu çalışmada, kliniğimize septik artrit öntanısı ile başvuran olguların klinik ve laboratuvar bulgularının ve tedavi yanıtlarının değerlendirilmesi amaçlandı. Ocak 2010-Ocak 2019 tarihleri arasında artrit öntanısıyla yatmış olan 111 çocuk (0-18 yaş) hasta kayıtları retrospektif incelendi. Olguların klinik, laboratuvar bulgularının değerlendirilmesi, tedavi ve prognostik özelliklerinin incelenmesi planlandı. Çalışmaya alınan toplam 111 hastanın %66’sı erkek olup ortalama yaşları 91±56 (medyan 83,1-215 aralığı) ay idi. Olguların çoğunluğunu (n=62,%56) 3-10 yaş aralığındaki hastalar oluşturdu. Olguların %60’ına (n=67) septik artrit tanısı konuldu. Bu tanıyı reaktif artrit (%10), juvenil idiopatik artrit (%10), toksik/geçici sinovit (%5) ve diğer artritler takip etti. Başvuruda olguların %96’sında ağrı, %63’ünde eklem şişliği, %21’inde kızarıklık, %41’inde eklemde ısı artışı, %64’ünde hareket kısıtlılığı, %38’inde yürüyememe yakınması vardı. Sıklıkla tutulan eklemler diz (%51) ve kalçaydı (%35). Ateş yüksekliği olan olgularda septik artrit olasılığı yüksek saptandı (p=0,0001). Septik artrit dışı artritlerde ibuprofene yanıt daha fazlaydı (p=0,0001). Olguların %55’ine (n=61) ponksiyon yapıldı, %34’ü (n=38) eklem içi debridman operasyonu geçirdi. Eklem sıvı kültüründe en sık üreyen mikroorganizmalar Staphylococcus aureus ve Streptococcus pyogenes idi. Septik artrit ile septik artrit dışı olgular karşılaştırıldığında, ultrasonografi ile ölçülen efüzyon miktarı, ponksiyonla alınan sıvı miktarı septik artrit grubunda istatistiksel olarak anlamlı oranda daha fazla, CRP ve lökosit sayısı ise septik artrit grubunda daha yüksek bulundu (sırasıyla p=0,001;p=0,025;p=0,018;p=0,032). Olguların %19’unda (n=21) osteomyelit saptanıp hepsi septik artrit grubundaydı Bu çalışmada ateşi olan, lökosit sayısı >12100/mm3 , CRP>3 mg/dl üzerinde olan olgularda, ultrasonografi ile 8,5 mm ve üzerinde efüzyon ölçülen olgularda septik artrit olma olasılığı istatistiksel anlamlı bulundu. Septik artrit dışı artritlerde ibuprofen yanıtı daha fazlaydı. The aim of this study was to evaluate the clinical and laboratory findings and treatment responses of patients with arthritis. The medical records of 111 children (0-18 years) were evaluated who were hospitalized with the diagnosis of arthritis between January 2010 and January 2019 retrospectively. The aim of this study was to evaluate the clinical and laboratory findings and to investigate the treatment and prognostic features of the patients. RESULTS: : A total of 111 patients, 66% were male and the mean age was 91±56 (median 83,1-215) months. The most of the patients (n=62,56%) were between 3-10 years of age. Septic arthritis was diagnosed in 60% (n=67) of the patients. This diagnosis was followed by reactive arthritis (10%), juvenile idiopathic arthritis (10%), toxic/transient synovitis (5%) and other arthritis. On admission, there were pain in 96%, joint swelling in 63%, redness in 21%, increased temperature of the joint in 41%, decreased range of motion in 64%, and inability to walk in 38% of the patients. The most frequently involved joints were knee (51%) and hip (35%). The possibility of septic arthritis was significantly higher in patients with high fever (p=0,0001). The response to ibuprofen was higher in non-septic arthritis (p=0,0001). Arthrocentesis was performed in 55% (n=61) of the cases and 34% (n=38) of the patients had underwent intra-articular debridement surgery. Staphylococcus aureus and Streptococcus pyogenes were the most common microorganisms growth in joint fluid culture. When septic arthritis and other arthritis cases were compared, the effusion amount, the amount of fluid taken by puncture were significantly higher and the level of CRP and leukocytes were higher in septic arthritis group (p=0,001;p=0,025;p=0,018;p=0,032,respectively). Osteomyelitis was observed in 19%(n=21) of the cases. In this study, the probability of septic arthritis was found to be statistically significant in patients with fever, leukocyte>12100/mm3, CRP>3 mg/dl, and effusion measured 8.5 mm or more by ultrasonography. Also, ibuprofen response was higher in non-septic arthritis group.

Objective: in the early stages of any epidemic caused by new emerging pathogens healthcare personnel is subject to a great risk. Pandemic caused by SARS-CoV-2, proved to be no exception. Many healthcare workers died in the early stages of pandemic due to inadequate precautions and insufficient protection. It is essential to protect and maintain the safety of healthcare personnel for the confinement of pandemic as well as continuity of qualified healthcare services which is already under strain. Educating healthcare personnel on appropiate use of personal protective equipment (PPE) is as essential as procuring them. Material and methods: a survey is conducted on 4927 healthcare personnel working solely with pediatric patients from 32 different centers. Education given on PPE usage were questioned and analyzed depending on age, sex, occupation and region. Results: among four thousand nine hundred twelve healthcare personnel from 32 different centers 91% (n=4457) received education on PPE usage. Of those who received education only 36% was given both theoretical and applied education. Although there was no differences among different occupation groups, receiving education depended on regions. Conclusion: it is essential to educate healthcare personnel appropiately nationwidely for the continuity of qualified healthcare services during the pandemic. / Giriş: yeni ortaya çıkan patojenlerin neden olduğu salgınların erken dönemleri sağlık çalışanları için büyük bir risk taşımaktadır. SARS-CoV-2’nin neden olduğu pandemide de durum farklı olmamış, alınan yetersiz önlemler ve kişisel korunma eksikliği nedenleriyle pandemi başlangıcında birçok sağlık çalışanı hayatını kaybetmiştir. Sağlık çalışanlarının koruması ve güvenli ortamda çalışmalarının devamlılığının sağlanması sağlık hizmetlerinin aksamamasının yanında salgının sınırlandırması içinde gereklidir. Kişisel koruyucu ekipmaların kullanımı ile ilgili eğitimlerin verilmesi, gerekli durumlarda güncellenmesi koruyucu ekipmanların sağlanması kadar önemlidir. Gereç ve yöntemler: otuz iki farklı merkezde çocuklara sağlık hizmeti veren 4927 sağlık personeli üzerinde anket uygulanmış, kişisel koruyucu ekipman kullanımı konusunda alınan eğitim değerlendirilmiş, yaş, cinsiyet, meslek ve bölgesel farklılıkların eğitim alma üzerine etki edip etmediği değerlendirilmiştir. Bulgular: otuz iki farklı merkezden 4927 sağlık çalışanın katıldığı çalışmada, 4457 (%91) sağlık çalışanın kişisel koruyucu ekipmaların kullanımı ile ilgili eğitim aldığı saptanmış, yalnızca %36’sının hem teorik hem de uygulamalı eğitim aldığı bulunmuştur. Meslek grupları arasında eğitim alma yönünden fark görülmezken, bölgelere göre eğitim alıp almamada anlamlı fark olduğu saptanmıştır. - Abstract
Micafungin is recommended especially in patients with liver and kidney failure and in the presence of other side effects due to antifungals apart from its known priority indications such as invasive candidiasis. The aim of this study was to evaluate the children who have received micafungin treatment. In the study, 125 children who were hospitalized in the pediatric wards and intensive care units of our hospital and had used micafungin between November 2016 and January 2019 were analyzed retrospectively. Clinical data, micafungin indication, blood values on the first and fourth days of the treatment, side effects of the drug and efficacy were evaluated. Sixty percent (75/125) of the patients were male and the mean age of all the patients were 58 ± 67 (0-215, 30) months. Approximately half of the cases (48%) had malignancy and 13% of them were premature. Sixty-two percent (n= 37) of the malignencies were hematological (27 acute lymphocytic leukemia, nine acute myeloid leukemia, one myelodysplastic syndrome) and 38% (n= 23) were oncological (six neuroblastoma, four Hodgkin lymphoma, two Non-Hodgkin's lymphoma, five sarcomas, one hepatoblastoma, five others) malignencies. The major cause of hospitalization was sepsis (53%). The patients had several risk factors like immunosuppressive therapy (n= 68, 54%), neutropenia (n= 61, 49%), central venous catheter (n= 102, 82%), nasogastric tube (n= 63, 50%), endotracheal intubation tube (n= 49, 39%), urinary catheter (n= 14, 11%) and total parenteral nutrition (n= 81, 65%). Thirteen percent (n= 16) of the cases were post-operative patients. Candida species were cultivated in 97 clinical specimens (blood, endotracheal aspirate, sputum, urine, etc.) among 23 (18%) of the patients. Thirteen (10%) of the patients had candidemia and 62% of them were non-albicans strains. In all candidemias, strains were echinocandin susceptible, and blood cultures were negative within four days. When all the patients (n= 125) were evaluated, a significant decrease in C-reactive protein, an increase in sodium, and a decrease in alanine aminotransferase were observed on the fourth day of micafungin treatment (p<0.05). A total of 39 (31%) patients underwent various antifungal treatments for median seven (1-60) days prior to micafungin treatment. Fourteen (36%) of these 39 patients, had elevated liver function tests (LFT), 10 (26%) of them had hypokalemia, and five (13%) of them had elevated renal function tests. Ten (26%) patients had antifungal-induced hypokalemia previously; and potassium levels were normalized after micafungin treatment (p= 0.0001). The patients for which micafungin treatment was chosen due to elevated liver function tests (n= 47, 38%), whether the antifungalinduced or not; alanine aminotransferase and aspartate aminotransferase levels were decreased after micafungin treatment (p= 0.0001 and p= 0.0001, respectively). Nineteen (15%) of the patients have died within the first 30 days of micafungin treatment and one of them had candidemia. No micafungin treatment related significant side effects were observed in any of the patients. Our study showed that micafungin could be a safe and effective option in pediatric cases including newborns with high liver and kidney function tests.
