The participants were 91 patients with pressure ulcers in the Home Health Care Unit and 91 caregivers. The data collection tool, consisted of questions regarding sociodemographic data prepared by researchers and questioning the knowledge of caregivers’ knowledge about pressure ulcer. The Barthel Index, Pressure Ulcer Scale for Healing, Braden Risk Assessment Scale, and Care Assessment Inventory provided by family members were used to assess the level of dependence in patients’ daily living activities.Results: The patients’ average age was 77.21± 13.26 years and 58.2% of them were women. The caregivers’ average age was 53.32± 11.45 years and 83.5% of them were women. A statistically significant increase was observed between knowledge scores before and after training (p< 0.001). Indicating that the training positively affects the caregivers’ level of knowledge. Factors such as a private room, caregiver support, caregiver competence, and caregiving difficulties were significantly related to changes in knowledge scores.Conclusion: Training programs for caregivers on pressure ulcer handling in home health care patients are effective and should be planned from a broader perspective to include caregivers within the family.Keywords: home care services, pressure sore, patient care, health education

Serratia marcescens idrar yolu enfeksiyonları, pnömoni, sepsis, menenjit gibi enfeksiyonlara ve hastane kaynaklı enfeksiyonlara da neden olabilen fırsatçı Gram negatif bir bakteridir. En sık çocuk yenidoğan ve yoğun bakım ünitelerinde görülmektedir. Birçok antibiyotiğe doğal dirençlidir.Bu çalışmada Göztepe Prof. Dr. Süleyman Yalçın Şehir Hastanesi Tıbbi Mikrobiyoloji Laboratuvarı’na 2016-2022 yılları arasında çeşitli kliniklerden gönderilmiş örneklerden izole edilen S. marcescens suşlarının piperasilin-tazobaktam, seftriakson, seftazidim, sefepim, meropenem, ertapenem, gentamisin, amikasin, siprofloksasin, ve trimetoprim-sülfametoksazol antibiyotiklerine direnç durumunun belirlenmesi ve direnç oranının yıllar içindeki değişiminin retrospektif olarak incelenmesi amaçlanmıştır.Çalışmaya hastaneye ayaktan başvuran ve hastanede yatan hastaların örneklerinden izole edilen S. marcescens suşları dahil edilmiştir. Örnekler rutin mikrobiyolojik standartlara göre işlenmiş, mikroorganizmaların tanımlanması ve antibiyotik duyarlılıkları otomatize sistemler kullanılarak yapılmıştır.S. marcescens olarak tanımlanan 416 izolatın 239’u servis ve yoğun bakım ünitelerinde yatan hastalardan (%58), 177’si (%42) poliklinik hastalarından gelen örneklerde saptanmıştır. S. marcescens %42 oranıyla en çok üriner sistem örneklerinde izlenmiş, %20’si trakeal aspirat örneklerinden, %13’ü kan kültüründen, %10’u apse aspiratı ve yara sürüntüsü örneklerinden, %5’i balgam, %4’ü doku, %3’ü kateter, %2’si bronkoalveolar lavaj ve %1’i diğer örneklerden izole edilmiştir.Çalışmamızda izole edilen S. marcescens suşlarının en duyarlı olduğu antibiyotiklerin aminoglikozit grubu antibiyotiklerden amikasin (>%98) ve gentamisin (>%96), karbapenemlerden meropenem (>%98) ve ertapenem (>%97) olduğu saptanmıştır. En dirençli olduğu antibiyotiğin ise %15.4 direnç oranıyla seftriakson olduğu bulunmuştur.Sonuç olarak ciddi enfeksiyonlara neden olabilen ve fırsatçı bir patojen olan S. marcescens’in antibiyotik direnç profilinin belirlenmesi tedavi yönetiminde yol gösterici olması ve yapılacak diğer çalışmalara ışık tutması açısından önemlidir.

AIM: To retrospectively evaluate the reported cases of HACEK group bacteria at Istanbul Göztepe Prof. Dr. Süleyman Yalçın City Hospital from 2017 to 2023. BACKGROUND: HACEK group bacteria, including Haemophilus parainfluenzae, Aggregatibacter spp., Cardiobacterium hominis, Eikenella corrodens, and Kingella kingae, are recognized endocarditis pathogens that are difficult to culture. These bacteria inhabit the digestive, respiratory, and oropharyngeal microbiota, causing a wide range of opportunistic infections. METHOD: Microbiological samples were inoculated on 5% sheep blood agar, chocolate agar, and chromogenic agar (all from bioMérieux, Marcy-l'Étoile, France) and incubated under appropriate conditions. Species-level identification of cultured bacteria was performed using MALDI-TOF MS (Vitek MS, bioMérieux, France), and antimicrobial susceptibility testing was conducted using E-test strips (bioMérieux, France). RESULTS: Over the 7-year period, HACEK bacteria were reported in 30 patients aged 0-76 years. C. hominis was not detected. The sample diversity ranged widely from sterile body fluids to sputum and abscesses (Table 1). Antimicrobial susceptibility was reported for only 4 samples, and polymicrobial growth was detected in 20 samples. CONCLUSION: The data presented may not fully reflect the true prevalence of HACEK bacteria due to the challenges in culturing these organisms. The current literature is sparse, with most reports from Turkey being limited to individual case presentations. The optimal treatment for HACEK infections remains undetermined due to limited data and the absence of clinical breakpoints. Extensive research is critically needed to enhance our understanding of these pathogens.

Science and technology parks are organizations that aim to increase the welfare and competitiveness of the society. According to the data of the Ministry of Industry and Technology, there are 89 technoparks in our country. Technopark firms have the ability to produce new technology. Among the basic founding objectives of technoparks, there are qualifications such as ensuring university-industry cooperation and providing collaborative environments that support innovation for companies. In this study, the effect of the collaborative environment provided by technoparks to companies on the innovation capacity of companies was investigated. Meta analysis technique was used as a research method. In the meta-analysis method, new and generalizable results are obtained by applying statistical analysis to the quantitative data obtained from different studies. A total of 6 studies detected were included in the meta-analysis within the scope of this study. The total number of samples in the studies included in the analysis was 2103. According to meta analysis results, the studies included in the research were in the heterogeneous form. The overall effect size of these studies was determined to be high according to the classification made for the correlation coefficient. In addition, the positivity of the effect size indicates that the collaborative approach of technopark companies has a positive effect on innovation. However, the fact that the companies are within the metropolitan boundaries or employing more than 100 personnel has not been found to have a distinctive effect on their collaborative attitudes

Primary congenital glaucoma (PCG) is characterised by increased intraocular pressure, and variants in ANGPT1, or SVEP1 have been identified as risk alleles. Here, the authors show that deletion of these genes induces glaucoma in mice, and that activation of ANGPT1-TEK signaling ameliorates disease progression in mouse models.

INTRODUCTION: Treatment of Acinetobacter baumannii infections has become increasingly diffucult, due to the high resistance rates against commonly administreted antibiotics. The aim of this study is to detect the antibiotic resistance of A.baumannii strains isolated from clinical specimens in years 2015-2018 METHODS: Antibiotic resistance of 1148 A.baumannii strains isolated from various clinical specimens between January 1, 2015 and December 31, 2018 was investigated retrospectively. Bacteria were identified using MALDI-TOF-MS (Biomerieux Inc., France) system. Antibiotic susceptibility of the strains were performed with Vitek 2 (Biomerieux Inc., Fransa) automated system. RESULTS: In 1148 of the clinical specimens A.baumannii was isolated. Total antibiotic resistance of isolated strains within these four years were determined as 83.5% for piperacillin/tazobactam, 81.6% for ceftazidime, 81.7% for meropenem, 80.5% for ciprofloxacin, 46.6% for amikacin, 50.4% for gentamicin, 59% for trimethoprim-sulfamethoxazole, 22% for tigecycline and 1.8% for colistin. Colistin resistance was found to be 0.8%, 0%, 2.9% and 3.2% for 2015, 2016, 2017 and 2018, respectively. Tigecycline resistance was determined to be 23.5%, 12.8%, 18.9% and 29.8%, respectively. DISCUSSION AND CONCLUSION: In our study, colistin and tigecycline were the most effective antibiotics against A.baumanniis trains.However, it is noteworthy that tigecycline resistance increased over the years and colistin resistance was determined. Because of these increase rates, treatment options for clinicians are decreasing day by day. Therefore, it was thought that it is important to examine its own antibiotic resistance profile by each hospital in order to guide clinician for the empiricial treatment of serious infections which can be life threatening especially like Acinetobacter infections.

Background: mecA is a predefined gene causing methicillin resistance in Staphylococcus aureus (S. aureus) isolates; however, it has been shown that some methicillin-resistant S. aureus (MRSA) strains do not carry this gene. Recently, in isolates found to be MRSA-positive but mecA-negative, a new resistance gene called mecC, which is a homolog of mecA, has been reported. This study aimed to investigate the mecC and mecA genes in MRSA strains isolated from different geographic regions in Turkey. Methods: The sample of the study consisted of 494 MRSA strains isolated from seven geographical regions in Turkey between 2013 and 2016. The strains were obtained from 17 centers, comprising 13 university hospitals, three education and research hospitals, and one state hospital. Methicillin resistance in S. aureus strains was determined using the agar disk diffusion method with a cefoxitin disk and the agar dilution method with oxacillin. The mecC and mecA genes in MRSA strains was investigated by Polymerase Chain Reaction (PCR). Results: Of the MRSA strains investigated, 47.9% were isolated from intensive care units. Concerning sample type, 36.7% were detected in the respiratory tract (tracheal aspirate, sputum, etc.), 24.8% in blood, 18.7% in skin and soft tissues, 9.3% in nasal swabs, 5.4% in urine, 4.1% in ears, and 1% in sterile body fluid. Using PCR, mecC was not identified in any of the S. aureus strains isolated from different clinical microbiology laboratories. mecA gene positivity was found in 315 of the MRSA strains (63.8%). Staphylococcal Cassette Chromosome mec (SCCmec) type was identified in 232 strains (46.9%), of which 136 (58.7%) were type II, 75 (32.4%) were type IV, 12 (5.1%) were type IIIb, six (2.5%) were type I, and three (1.3%) were type III. Conclusion: This is the first multi-centered study to investigate MRSA strains isolated from different regions in Turkey. The mecC gene was not detected in any of the MRSA strains. In this paper, we introduce the concept of D-direction curve and C- direction curve of a given curve using the alternative frame –N,C,W} in Euclidean 3-space. Furthermore, we define a (N,C) plane curve, a (N,W) plane curve and a (C,W) plane curve of a given curve, and give some results related to them. Finally, we present some illustrated examples for new associated curves.

Nephrotic syndrome is one of the most common glomerular diseases in children and can be classified on the basis of steroid responsiveness. While multiple genetic causes have been discovered for steroid resistant nephrotic syndrome, the genetics of steroid sensitive nephrotic syndrome remains elusive. Mutations in Epithelial Membrane Protein 2 (EMP2), a member of the GAS3/PMP22 tetraspan family of proteins, were recently implicated as putative monogenic cause of steroid sensitive nephrotic syndrome. We investigated this hypothesis by developing Emp2 reporter and knockout mouse models. In lacZ reporter mice (engineered to drive expression of the enzyme β-galactosidase under the control of the endogenous murine Emp2 promoter), Emp2 promoter activity was not observed in podocytes but was particularly prominent in medium- and large-caliber arterial vessels in the kidney and other tissues where it localizes specifically in vascular smooth muscle cells (vSMCs) but not in the endothelium. Strong Emp2 expression was also found in non-vascular smooth muscle cells found in other organs like the stomach, bladder, and uterus. Global and podocyte-specific Emp2 knockout mice were viable and did not develop nephrotic syndrome showing no evidence of abnormal glomerular histology or ultrastructure. Altogether, our results do not support that loss of function of EMP2 represent a monogenic cause of proteinuric kidney disease. However, the expression pattern of Emp2 indicates that it may be relevant in smooth muscle function in various organs and tissues including the vasculature.

Objective: To detect the subtype characterization and drug-resistant mutations in HIV-1 strains after the refugee movement from Syria to Turkey between 2011 and 2014 in south east border lines. Methods: A total of 65 patients were included in this study, of which 57 (88%) patients were antiretroviral therapy-naive patients. HIV-1 RNA was detected and quantified by real-time PCR assay. HIV-1 subtypes and circulating recombinant forms (CRFs) were identified by phylogenetic analysis (neighbor-joining method), and drug-resistant mutations were analyzed. Results: Three major HIV groups were indicated. Two of these groups were located in subtype B. The other group showed heterogeneity. Subtype B (48/65, 73.8%), followed by CRFs (12/65, 18.5%) was the most common strain. Subtype of CRFs consisted of CRF01_AE (9/65, 13.8%) and CRF02_AG (3/65, 4.6%). Subtype C (1/65, 1.5%), sub-subtypes A1 (2/65, 3.1%) and F1 (2/65, 3.1%) were also detected with low prevalence. The rate of overall primary antiretroviral resistance was 4.9% (3/61). Drug-resistant rate for non-nucleoside reverse transcriptase inhibitors was 4.9%. The thymidine analogue mutation rate was 13.1% (8/61). Conclusions: HIV molecular epidemiology studies are necessary to determine transmission patterns and spread. Subtype B and CRF01_AE, CRF02_AG are the most prevalent strains in the south-east of Turkey. However, subtype C, sub-subtypes A1 and F1 are of low prevalence but persist in the south-east of Turkey. In the near future, changing of HIV epidemiology will be possible in Turkey due to migration movement in border lines and resistance testing will play an important role in HIV management.

All developments taking place in computers, internet, music product formats and audio device technologies have an impact on the location of music product sales, the amount and method of such sales, the people in the music industry and even the industry model itself. In regards to the music industry, which is one that is extremely open to such changes, people’s attitudes towards devices for listening to music and copyright piracy are highly variable. As in the case of worldwide sales, Turkish sales of music products have also been negatively influenced by pirate sales, and the largest group of the buyers are the youth population, therefore resulting in a need of investigation of the attitude of Turkish youth towards these changes and piracy. Hence, the aim of this study is to pinpoint differences in the choice of devices used for listening to music in young women and men, and to investigate their points of view towards physical and digital piracy.

In this study, it was aimed to determine the water-borne and food-borne boron accumulation in the liver and muscle tissues of Gold Fish (Carassius auratus Linnaeus, 1758). For each treatment, 12 individuals were. The water-borne boron treatments were applied as boron acid concentration of 1 mg/L, 10 mg/L and 20 mg/L in the aquarium water, while the food-borne boron treatments were prepared food contained the defined levels of boron (1 mg/kg, 5 mg/kg and 10 mg/kg) as boric acid. The boron levels in the tissues were determined by an ICP-MS procedure. The maximum boron concentration was found in the 20mg/L water borne boron treatment in the liver tissue (1.78±0.02 mg/kg). In the water-borne boron treatments, the maximum Transfer Factor (TF) was found in the 20mg/L boron concentration, and TF values were increased when the boron concentrations were decreasing. In the 1 mg/kg food-borne boron treatment, TF was found as 0, and increasing concentration of boron in the food caused an increase in TF reached about 0.06. This study suggested that the target organ for boron accumulation is the liver rather than the muscles and the accumulation of food-borne boron is lower when comparing water-borne boron.

Today economic units supply their long-term fund demands from capital markets. Firms can obtain funds to issue stocks and bonds in stock exchanges in accordance with market conditions. Particularly going public stocks in primary market are traded in secondary market due to market conditions. Investors in these markets desire to generate earnings from movement of prices and actually it is the main objective in capital markets. In this context, investors want to obtain information about prospective price changes in order to achieve their earning objective. However many time, it is not possible to get such an information because of uncertainty and risks in markets. Individual investors settle some strategies about stock exchange by the help of their personal experiences, observations and information, with an aim to overcome this uncertainty. It is important to decide accurate time of taking and selling stocks. It is not possible to estimate accuracy of timing. The same difficulty appears in strategy process, either. In this study, adequate timing decision of individual investors in stock exchange was examined by a simulation application. Investors’ possible strategies were determined. These strategies applied the closing values of sectoral indexes defined in Istanbul Stock Exchange and advisable strategies for each sector were determined.

Undernutrition in hospitalized patients is a significant healthcare burden worldwide. Depression is a major global health problem characterized by symptoms such as altered mood, loss of interest and pleasure, and impairment in cognitive and physical functions. Patients hospitalized for medical conditions have a higher risk of depression than the general population. The purpose of this study was to explore the relationship between undernutrition risk and depression risk in hospitalized patients. The design of this study was an observational cross-sectional analytic. A questionnaire form created by the investigator including sociodemographic data, dietary patterns, presence of chronic diseases and anthropometric measurements, Nutritional Risk Screening and Patient Health Questionnaire-2 were applied to the patients. All quantitative data were expressed as the median and interquartile range and qualitative data were expressed as numbers and percentage. Mann-Whitney U test and Kruskal-Wallis test were used to analyze nonparametric data; Chi-square and Fisher Exact test were used to analyze qualitative data. P < .05 was considered statistically significant. A total of 308 patients who met the criteria were reached. Undernutrition was detected in 28.6% of the participants, and depression was present in 37.3%. A statistically significant relationship was found between the risk of undernutrition and the risk of depression, with individuals without a risk of depression being found to have a lower risk of undernutrition. (P < .001). The risk of undernutrition was found to increase with advancing age. Moreover, individuals with higher values for arm muscle area, arm muscle circumference, mid-upper arm circumference, triceps skinfold thickness, calf circumference, and body mass index had a lower risk of undernutrition. No relationship was found between gender and the risk of depression. However, a statistically significant relationship was observed between age and the risk of depression, indicating that older patients had a higher risk of depression. In this study, a statistically significant relationship was found between the risk of undernutrition and the risk of depression in hospitalized patients. The aim of this study is to compare different methods due to the difficulties in identifying coryneform bacteria to species level and to determine antibiotic resistance profiles. Isolates identified as Turicella otitidis ( n :45) by VITEK 2 Compact and Corynebacterium mucifaciens ( n :1) by matrix-assisted laser desorption ionization time-of-flight mass spectrometry (MALDI-TOF MS), isolated from blood and catheter cultures between 2015 and 2017 were included in the study. For identification of the isolates, conventional tests and 16S rDNA sequence analysis were performed. Antibiotic susceptibilities of the isolates were determined by Etest. The isolates identified as T. otitidis with VITEK 2 Compact could not be identified by MALDI-TOF MS and described as C. mucifaciens / Corynebacterium afermentans spp. by 16S rDNA sequence analysis. One isolate identified as C. mucifaciens by MALDI-TOF MS could not be identified with VITEK 2 Compact and described as C. mucifaciens by 16S rDNA sequence analysis and conventional methods. All isolates ( n :45) described as C. mucifaciens / C. afermentans spp. by 16S rDNA sequence analysis were identified as C. afermentans subsp. afermentans with conventional methods. All 45 isolates identified as C. afermentans subsp. afermentans were resistant to penicillin, erythromycin, and clindamycin and were susceptible to vancomycin and daptomycin, whereas 31 (69%) were resistant to trimethoprim-sulfamethoxazole (TMP-SXT). The isolate identified as C. mucifaciens was susceptible to penicillin, vancomycin, daptomycin, and TMP-SXT; it was resistant to erythromycin and clindamycin. In this study, we reported 45 C. afermentans isolates misidentified as T. otitidis in routine laboratory processes. To our knowledge, this is the first study to include the highest number of C. afermentans blood isolates.

Objectives: This study aims to investigate the rate of type D personality in Turkish patients with fibromyalgia (FM) and evaluate its associations with clinical parameters of FM as well as its effects on health-related quality of life (HRQoL)., Patients and Methods: The study included 100 patients with FM (14 males, 86 females; mean age 34.6±7.4 years; range, 22 to 49 years) fulfilling 1990 American College of Rheumatology diagnostic criteria and 50 healthy controls (9 males, 41 females; mean age 32.6±6.5 years; range, 21 to 50 years). Type D personality was assessed using the type D scale-14 (DS-14). FM disease severity was determined by Fibromyalgia Impact Questionnaire (FIQ), functional status by Stanford Health Assessment Questionnaire (HAQ), and HRQoL by Nottingham Health Profile (NHP). Severity of pain and fatigue were measured by visual analog scale (VAS)., Results: The frequency of type D personality was 33% in FM patients and 12% in controls (odds ratio=3.612, 95% confidence interval 1.398-9.333) (p=0.006). Type D FM patients scored higher in tender point count (TPC), FIQ, HAQ, VAS-pain and all NHP subgroups except energy (p<0.01). Type D personality was found to be correlated with FIQ, TPC, HAQ, VAS-pain and NHP subgroups except energy (p<0.01)., Conclusion: Based on our findings, assessment of personality characteristics of patients with FM may hold the key for the treatment of the disease. Our objective was to evaluate the performance of matrix-assisted laser desorption ionization time-of-flight mass spectrometry (MALDI-TOF MS) for identification of the Chryseobacterium and Elizabethkingia spp. isolated from pediatric patients at Hacettepe University Hospital using 16S rRNA gene sequencing as the gold standard and to determine the antimicrobial susceptibility patterns of the isolates and baseline characteristics of patients. All stored Chryseobacterium and Elizabethkingia spp. isolated from various clinical specimens (sputum, blood, and urine) of pediatric patients at Hacettepe University Hospital between 2012 and 2016 were included in this study. Minimum inhibitory concentrations of 10 antimicrobial agents were determined by Etest for all isolates. To determine the baseline characteristics of patients, medical records of all patients were retrospectively reviewed. In total, 18 isolates of Chryseobacterium spp. (16 C. indologenes, 2 C. gleum) and 5 isolates of Elizabethkingia spp. (3 E. meningoseptica, 2 E. anophelis) were identified by 16S rRNA sequencing. MALDI-TOF MS correctly identified 19 (82.6%) isolates to the species level. The quinolones (ciprofloxacin and levofloxacin), trimethoprim/sulfamethoxazole and piperacillin/tazobactam showed the highest spectrum of activity against the overall collection of isolates. Cystic fibrosis (CF) was the underlying disease in 81.8% of patients. To our knowledge, this study includes the largest number of Chryseobacterium spp. isolated from clinical specimens of pediatric patients in Turkey. In this study, we also report the first clinical isolate of E. anophelis in Turkey. Since, the majority of strains were isolated from patients with CF; larger, prospective clinical studies are needed to establish whether chryseobacteria could be considered as an emerging opportunistic pathogen in patients with CF.

Pantoea agglomerans is an environmental Gram-negative bacterium that rarely is responsible for the infections in humans but it is often a causative factor of a number of occupational diseases. This study evaluated the clinical and microbiological characteristics and pathogenicity of P. agglomerans in children. We retrospectively reviewed microbiological test results for all children (1 month old to 18 years old) who were admitted to our pediatric hospital between January 2000 to June 2015 and had positive clinical specimen cultures for P. agglomerans. Isolates were identified using conventional tests and the BBL Crystal E/NF ID or MALDI-TOF MS systems. Antibiotic susceptibilities were evaluated using the Kirby-Bauer disc diffusion method. We identified fifteen positive cultures from 14 patients with confirmed infections. The positive specimens included pus, urine, tracheal aspirate, blood, and central venous line samples that yielded P. agglomerans. The median patient age was 8.8 years (range: 1.5 months to 16.5 years), and all patients had underlying comorbidities. Five patients had medical devices, and two devices were removed. The most common P. agglomerans infections involved wound infections (35.7%), pneumonia (21.4%), and urinary tract infections (21.4%). Three patients had concomitant infections (Enterococcus faecium, Pseudomonas aeruginosa, and Aspergillus fumigatus). Five patients had anemia. Three patients (21.4%) died, and all three had carbapenem-resistant P. agglomerans that was detected after the first week of hospitalization; two cases involved pneumonia, which was ineffectively treated. P. agglomerans infections may be life-threatening, especially in young patients with pneumonia. Hospital-acquired P. agglomerans may have different pathogenicity and clinical features, compared to community-acquired P. agglomerans, although further studies are needed to understand the drug-resistance patterns in this bacterium., (Copyright © 2017 The Authors.
