The quality of bicycle path surfaces significantly influences the comfort of cyclists. This study evaluates the effectiveness of smartphone sensor data and smart bicycle lights data in assessing the roughness of bicycle paths. The research was conducted in Hasselt, Belgium, where various bicycle path pavement types, such as asphalt, cobblestone, concrete, and paving tiles, were analyzed across selected streets. A smartphone application (Physics Toolbox Sensor Suite) and SEE.SENSE smart bicycle lights were used to collect GPS and vertical acceleration data on the bicycle paths. The Dynamic Comfort Index (DCI) and Root Mean Square (RMS) values from the data collected through the Physics Toolbox Sensor Suite were calculated to quantify the vibrational comfort experienced by cyclists. In addition, the data collected from the SEE.SENSE smart bicycle light, DCI, and RMS computed results were categorized for a statistical comparison. The findings of the statistical tests revealed no significant difference in the comfort assessment among DCI, RMS, and SEE.SENSE. The study highlights the potential of integrating smartphone sensors and smart bicycle lights for efficient, large-scale assessments of bicycle infrastructure, contributing to more informed urban planning and improved cycling conditions. It also provides a low-cost solution for the city authorities to continuously assess and monitor the quality of their cycling paths.

Traffic congestion negatively affects the economy, human well-being, safety, and environment in both developed and developing countries. In the past, numerous studies have been undertaken to assess congestion costs. This research focuses on quantifying the individual costs incurred due to traffic congestion and the effects of socio-demographic characteristics on the individual congestion of private motorized vehicle users in the context of an urban metropolitan (Lahore) in Pakistan. Further, the study has also proposed individual traffic congestion cost (ITCC) analytics to estimate the costs related to traffic congestion. Also, statistical tests, such as the ANOVA and T -test, have been employed to determine the impact of socio-demographic characteristics on individual congestion costs. The primary data was collected from the target population by filling out a questionnaire in the study area. With a confidence level of 95% and a sampling error of 6%, this study used a sample size of 282 participants. The study findings showed that the ITCC for the respondents ranged from 17.29 rupees per day to 1136.88 rupees per day. The ANOVA and the t -test indicate that all socio-demographic variables affect the calculation of ITCC. Furthermore, this research showed that car-owning respondents faced higher individual congestion costs than motorbike-owning respondents. The anticipated contribution of this study will aid private motorized vehicle users in estimating their individual congestion costs and managing their expenses. In addition, it will encourage individuals to modify their travel habits to make more efficient use of transportation for their daily tasks.

Promoting bicycling and making it attractive requires appropriate infrastructure. Sociodemographic characteristics, frequency and experiences of bike use, and purpose of bicycle trips can affect preferences towards bicycle infrastructure facilities in urban areas. Hence, this study aims to explore the heterogeneity in the perceived importance of bicycle infrastructure facility attributes in various cyclist groups based on gender, age, weekly biking frequency, daily cycling distance, cycling experience, and bicycle trip purpose. Data were collected from bicycle users through a questionnaire disseminated via social media platforms and QR code brochures distributed in Hasselt, Belgium. A 5-point Likert-type ordinal scale was used to collect data on the perceived importance of bicycle infrastructure facility indicators. The Technique for Order Preference by Similarity to Ideal Solution (TOPSIS) was used to rank the indicators. At the same time, Mann–Whitney U and Kruskal–Wallis tests were utilized to verify the heterogeneity among the groups. The findings reveal that bicycle infrastructure, i.e., bicycle lanes or paths, is the most critical variable, while the slope was considered the least important. No heterogeneity was found regarding the importance of bicycle infrastructure indicators based on gender. However, heterogeneity was observed based on age, daily bicycle use, cycling experience, weekly bicycle use, and bicycle trip purpose. The findings of this research help urban and transport planners develop improvement strategies for the city’s existing bicycling facilities and prioritize future developments by considering various cyclist groups’ preferences.

This study explores the potential of online traffic safety education for adolescent motorcyclists in Pakistan. An e-learning platform, “Route 2 School” (R2S), was developed focusing on traffic knowledge, situation awareness, risk detection, and risk management. Male students (14–18 years) who commute to school by motorcycle were divided into an experimental group (EG) and a control group (CG), both completing pre- and post-measurement questionnaires. The EG showed significant improvement in knowledge, risk detection, and risk management compared to the CG, but not in situation awareness. Participants reported increased traffic safety awareness and suggested adding more interactive elements. The R2S platform’s scores revealed better performance in risk detection and risk management modules than situation awareness. Time spent on modules varied, with situation awareness requiring the most time. Adolescents expressed satisfaction with the platform, acknowledging its role in increasing traffic awareness. This study provides initial insights into the effectiveness of online traffic safety education in Pakistan, highlighting the potential to address the lack of comprehensive traffic safety education in schools. Further research and stakeholder engagement are recommended to integrate such platforms into formal education, potentially reducing traffic-related injuries among adolescent motorcyclists in developing countries.

Bicycle use is associated with health benefits due to increased physical activity. Encouraging cycling in cities requires the establishment of supportive infrastructure. Various assessment methods have been developed to evaluate bicycle infrastructures' safety, comfort, and efficiency. This scoping review provides an overview of the methods used to assess bicycle infrastructure, as reported in relevant studies. A comprehensive literature search was conducted in three scientific databases (Web of Science, Scopus, and Google Scholar) using the PRISMA guideline extension for scoping the reviews. The retrieved articles were screened, coded, and synthesized according to the eligibility criteria. Fifty-five articles met the criteria and were included in the scoping review. The assessment methodologies primarily focused on four aspects: vibration or roughness index, Bicycle Level of Service (BLOS), Bikeability Index (BI), and Bicycle Safety Index (BSI). Questionnaires (evaluation platforms), bicycles, GIS, and video cameras were the most commonly used equipment/resources. Roughness index assessments relied on objective data, such as acceleration values, and some studies validated their findings using cyclists' subjective comfort perception. On the other hand, subjective data were predominantly used for BLOS assessment. The BIs present a more comprehensive analysis of bicycles by including more components of bicycle infrastructure design. Methodologies have been developed to evaluate various aspects of the bicycle infrastructure. However, selecting appropriate methods for specific contexts cannot be undermined. This review article provides a helpful guide on selecting an appropriate methodology for the unique characteristics of the study area that enhances the effectiveness of bicycle infrastructure evaluation.

The Government of Pakistan has established clean energy transition goals in the national Alternative and Renewable Energy (ARE) Policy. The goal of this policy is to increase the 30% capacity of green energy in total energy mix by 2030. In this regard, the aim of this study is to develop a de-carbonization plan for achieving net zero emissions through the deployment of a green energy system for the period 2021 to 2040 by incorporating the ARE policy targets. The Low Emissions Analysis Platform (LEAP®) software is used for finding the unidirectional causality among gross domestic product, population within the country, energy demand, renewable energy production and CO2 emissions for Pakistan. The results revealed that energy production of 564.16 TWh is enough to meet the energy demand of 480.10 TWh with CO2 emissions of 22.19 million metric tons, having a population of 242.1 million people and GDP growth rate of 5.8%, in the year 2040 in Pakistan. The share of green energy production is 535.07 TWh, which can be utilized fully for meeting energy demand in the country, and almost zero emissions will produce till 2040. CO2 emissions produced by burning natural gas were 20.64 million metric tons in 2020, which then reduced to 3.25 million metric tons in 2040. CO2 emissions produced by burning furnace oil are also reduced from 4.19 million metric tons in 2020 to 2.06 million metric tons in 2040. CO2 emissions produced by burning coal were 24.85 million metric tons in 2020, which then reduced to 16.88 million metric tons in 2040. Energy demand is directly related to the population and GDP of the country, while renewable utilization is inversely proportional to carbon emissions. The declining trend of carbon emissions in Pakistan would help to achieve net zero emissions targets by mid-century. This technique would bring prosperity in the development of a clean, green and sustainable environment.

Due to the recent climate change, organizations all over the globe are developing plans for reducing carbon emissions by developing clean energy technologies and energy efficient devices. However, the path for transition to green energy system is still unclear and in general, the representation of green energy supply for transition pathways is limited. Therefore, this study outlines a plan for getting Swedish energy sector completely carbon neutral by 2050. The approach can also be applicable to the majority of nations worldwide. Computer based simulations are performed on Energy PLAN software for making clean, green and sustainable energy system that can balance every component of entire energy system during the study period 2022 to 2050. This study takes into account the sustainable use of renewable sources for all economic sectors as well as the interchange of energy with nearby nations under the two scenarios. Additionally, the energy system works in tandem with other industries to create a fully carbon-free environment. The results revealed that, 50% de-carbonization is possible till 2035 and 100% de-carbonization is possible till 2050. This enables a discussion of how ambitious 10-year goals might serve as a first step toward the mid-century elimination of fossil fuels from the energy sector.

Background: As COVID-19 spread across the globe, the new variants continued to emerge, leading to a rapid surge in the cases and thereby overwhelming the testing laboratories. As Rapid antigen test kits became available, they complimented the reverse transcription polymerase chain reaction (RT-PCR) based testing and thereby moderately disburdening the testing laboratories. However, to increase the testing capacities, the regulatory authorities approved these rapid kits with little scrutiny. Therefore, validation of rapid antigen testing kits becomes essential before they are used as viable alternatives to RT-PCR based testing. Aims and Objectives: The aim of this study was to compare the sensitivity and specificity of OSKIT SARS-CoV-2 corona antigen test (Oscar Medicare Pvt. Ltd., India) to that of COVID-19 ONE-STEP RT-PCR KIT (Meril Diagnostics Pvt. Ltd., India). Materials and Methods: In this prospective study, spanning the peak of third wave in India (Between December 1 and January 31, 2022), 242 specimens were collected and analyzed by comparing their results both by OSKIT SARS-CoV-2 Corona Antigen Test and COVID-19 ONE-STEP RT-PCR KIT. The various clinico-epidemiological attributes of the patients were taken into consideration and used to analyze the results. Results: Rapid antigen tests (RAT) was positive in 44 individuals, giving an overall sensitivity of 67.31%. Pakistan is facing an annual electricity shortfall of 5000 MW since 2005 due to lack of effective planning on harnessing indigenous energy resources and dependence on imported fuels. In this regard, a qualitative analysis is presented to address the ongoing energy crises and to suggest the development of domestic energy assets and recommends a policy framework to meet electricity goals. Status of energy in Pakistan is presented in the context of energy supply and demand and discusses the major issues such as electricity theft and circular debt. The reasons are identified for power system degradation, energy crises and their potential impacts on the economy of Pakistan is discussed. Further, domestic energy assets are evaluated for their capacity of power production, including solar, wind, geothermal, biomass, tidal, hydro, coal, and natural gas. The main variables of Pakistan’s energy status are power system restructuring, power system planning and policies, electricity theft, energy crisis and circular debt. The key challenges for policy development are also addressed using possible approaches and used these approaches as input in making policy framework by considering the domestic energy resource through incorporating the policy actors, policy criteria, and policy tools. It is found that Pakistan has a renewable potential of 3425.796 GW and a non-renewable potential of 104.883 GW to meet the future energy demand. It is suggested to exploit domestic energy assets for power production, optimize energy planning and policy to find a secure and sustainable energy option for Pakistan.

Background and Objectives: Vulvovaginal candidiasis (VVC) is one of the most frequent reasons for gynecological consultations. Candida albicans is responsible in the majority of cases. Lately, VVC caused by non-albicans Candida spp. (NAC), which are resistant to routinely used antifungals, is on the rise. This study was designed to determine the prevalence of Candida in patients suffering from vaginitis and to assess the predisposing factors along with identification of Candida species and evaluation of their susceptibility profile. Materials and Methods: High vaginal swabs were collected from 225 women. Sample processing consisted of Gram stain and culture onto Sabouraud’s dextrose agar and HiChrom Candida Differential agar. Isolates were identified and speciated using VITEK2 Compact System. Susceptibility testing was done using VITEK2 AST-Y S08 cards and disc diffusion. Results: Candida spp. were isolated from 94 (41.8%) of the cases. C. albicans was the predominant species (71.6%) followed by other NAC spp. (28.4%). Pregnancy and diabetes were the most frequently implicated risk factors (67.1% and 44.4%). High resistance was observed in NAC spp. as opposed to C. albicans to all antifungal agents tested. Conclusion: Empirical therapy with routinely used antifungals can be initiated for C. albicans. In the case of NAC spp., identification should be followed by susceptibility testing.

Introduction: On March 11th, 2020, the World Health Organisation (WHO) declared the outbreak of the novel coronavirus disease caused by the Severe Acute Respiratory Syndrome Corona Virus 2 (SARS-CoV-2) virus as a pandemic. This recently discovered β-coronavirus spread instantaneously across mainland China due to human-to-human transmission and crossed international borders aided by intercontinental travel. In most nations, the logarithmic growth of the cases very quickly overwhelmed the healthcare system which led to the overcrowding of the hospitals and led to a sudden surge in Hospital-Acquired Infections (HAIs). Implementation of contact precautions was implemented to control cross-infection. Aim: To determine the effect of Coronavirus Disease-2019 (COVID-19) on the prevalence of HAIs with special emphasis on Staphylococcus aureus (S. aureus). Materials and Methods: This three-year retrospective study (September 2018-August 2021) was undertaken at an apex tertiary care institute in Northern India. Wound swabs were collected and processed in the laboratory for the presence of aerobic bacterial isolates. S. aureus was identified using conventional methods and antimicrobial sensitivity was performed by the Kirby-Bauer disc diffusion method. Data was entered in Microsoft Excel and later analysed in International Business Machines (IBM) Statistical Package for the Social Sciences (SPSS) version 22.0. Results: A steady increase in the isolation of Methicillin-Resistant Staphylococcus aureus (MRSA) was noted during the study period (60.5% in 2018 to 78.1% in 2021). A statistically significant increase was noted in the detection of MRSA after the onset of the COVID-19 pandemic (p=0.018) despite the reduced number of surgeries conducted in the institution and rigorous execution of contact precautions. Conclusion: There was an increase in the rate of MRSA isolation during the study period. The increase was significantly affected by the onset of COVID-19. To contain the spread of MRSA, novel methods including preoperative screening of patients undergoing elective surgeries and periodic screening of hospital staff need to be implemented along with standard infection control precautions at all times.

Introduction: Timely diagnosis and isolation of cases is of paramount importance to contain the spread of a pandemic. The Coronavirus Disease-2019 (COVID-19) has emerged as a major health problem that needs concerted efforts for mitigation and control. Real-time Reverse Transcription Polymerase Chain Reaction (RT-PCR), the gold-standard diagnostic modality, has high cost and can be performed in special laboratories. Rapid Antigen Tests (RAT) has been developed to serve as an alternative and is recommended to use at point-of-care testing. Aim: To compare the case detection rate of RAT and RT-PCR and the possible role they may play in the pandemic mitigation efforts. Materials and Methods: In this retrospective study, all the samples collected during a nine-months period were analysed. Depending upon the criteria, either a RAT or RT- PCR was done on the samples. Data was analysed using descriptive statistics (frequencies, mean, standard deviation, and percentages). Results: A total of 8,29,745 samples were tested during the study period among which number of positive samples was 19,414 giving an overall positivity rate of 2.34% (0.20% to 12.58%). RAT positivity was 1.58% while RT-PCR gave a positivity of 4.26. Total number of positive cases identified by RAT and RT-PCR were 9,382 and 10,032, respectively. Conclusions: RAT is a low-cost alternative to the expensive RT-PCR with the added advantage of giving accurate and timely results. This can be a game changer especially in low-resource settings, which have witnessed a increase in the spread of COVID-19 during the latter part of the pandemic.

Atrial flutter (AFL) is a rare arrhythmia in neonatal and infantile period. Potentially, AFL might lead to severe morbidity and even a fatal outcome. However it can have a good prognosis with early diagnosis and prompt appropriate treatment. Synchronized electrical cardioversion or transoesophageal atrial overdrive pacing is recommended in both stable and unstable cases. Antiarrhythmic drugs can be tried in stable newborns though the sinus rhythm takes some time to return to normal.Excellent prognosis has been noted in newborns and infants having AFL after sinus rhythm has been restored. The risk of recurrence is low and prolonged antiarrhythmic therapy is less likely to be required. However follow-up after discharge is essential to look for any possibility of recurrent arrhythmia and possible untoward effects of treatment.

Objective: To assess the immediate and short-term efficacy and safety of transcatheter closure of cardiac defects with Lifetech™ Konar-MFO in a Tertiary Cardiac Care Center. Study Design: Descriptive cross-sectional study. Setting and Duration of Study: Armed Forces Institute of Cardiology/National Institute of Heart Diseases, Rawalpindi Pakistan, from Mar 2019 to Dec 2021 Methodology: Patients of all ages and both gender undergone transcatheter closure of a cardiac defect with Konar-MFO from Jan 2021 to Dec 2021 were included in the study through non-probability consecutive sampling and approval by the institutional ethical review committee was taken prior to data collection. Data of patients undergone transcatheter closure with Konar-MFO from Mar 2019 to Dec 2020 was extracted retrospectively to assess the immediate and short-term efficacy and safety of the occluder. SPSS version-23 was used for data entry and analysis. Results: A total of 138 patients had transcatheter closure of a cardiac defect with Lifetech™ Konar-MFO during the study period. Case of VSD were 124(89.9%), PDA 9(6.5%), post-operative cases were 4(2.9%) along with 1(0.7%) case of coronary artery fistula (CAF) to the right ventricle. Out of 138 patients, 73(52.9%) were females while 65(47.1%) were males. The mean age was 9.3 ±8.1 years with a range of 6 months to 36 years. Mean fluoroscopy time was 13.68±9.74 min. The procedure was successful in 137(99.27%) cases. The device embolized in 2(1.45%) cases. Conclusion: In selected cases of VSD, PDA and in some post-operative cases the occlusion with Lifetech™ Konar-MFO is safe and efficacious with the added benefits of softness and versatility of the approach.

Patent ductus arteriosus (PDA) is a common cardiac anomaly making 5%-10 % of all congenital heart diseases. The incidence increases in premature, very low birth weight neonates. Transcatheter closure is a standard procedure for treating PDA.However, in pre-term and very low birth weight neonates it presents special problems thus becoming a limitation in performing the procedure. This case report describes a PDA closure in a 1.3 Kg infant in Armed Forces Institute of Cardiology,Rawalpindi.

Recent increase in deployment of trans-catheter devices such as septal or ductal occluding coils and devices has been a possible risk factor for infective endocarditis, predominantly in the initial post deployment period before endothelialization has ensued. Though a long-term study of trans-catheter Atrial Septal Defect (ASD) closure revealed no cases of Infective Endocarditis, some case reports of endocarditis in trans-catheter device closure of ASD, VSD and Patent ductus arteriosus(PDA) advocate that residual defect after device occlusion may be a factor in the risk for Infective Endocarditis. We herein describe the case of a 9-month-old female patient who underwent device closure for peri-membranous ventricular septal defect (VSD) and developed fungal infective endocarditis in early post occlusion period which was confirmed on blood culture. Despite treatment with appropriate antifungal as per sensitivity, clinical condition of the patient deteriorated and it was decided to manage her with surgical explanation of device and closure of defect.

Objective: To evaluate the visual outcome after clear lens-extraction (CLE)/refractive lens exchange (RLE) and implantation of intraocular lens (IOL) in myopic eyes. Study Design: A descriptive study. Place and Duration of Study: This study was conducted in CMH Multan from Jan 2014 to Aug 2015 and CMH Peshawar from Sep 2015 to Dec 2016. Patients and Methods: Eighty eyes of 40 patients had clear lens extraction by Phacoemulsification and aspiration followed by foldable intraocular lens implantation through 2.75mm clear corneal incision. There were seventeen (42.5%) male patients i.e. 34 eyes and twenty three (57.5%) female patients i.e. 46 eyes. Mean age of patients was 26.8 years. Myopia of -9.00 D to -20.00 D with one year stability were included in the study. Pre op retina was examined in all cases with special emphasis to retinal degenerative changes. All surgeries were performed by the same surgeon, under topical anaesthesia using the same make Phacoemulsification unit. All surgeries were uneventful and patients were followed up for six months after surgery. Results: Best spherical corrected visual acuity improved by 11.5% postoperatively i.e. 59 eyes (73.75%) had vision of 6/18 or better which increased to 68 eyes (85%) after surgery. Postoperative spherical correction was between - 0.50 to -1.50 D and there were no vision threatening complications. Conclusion: In experienced hands clear lens extraction with foldable intraocular lens implantation is safe and effective way to treat high myopia that is not suitable for laser refractive surgery.

In this work we have designed, modeled and simulated a 32-channel DWDM (Dense Wavelength Division Multiplexing) system using OptiSystem photonic simulator. The system is tested for external transmitter with NRZ (Non-Return Zero) encoder using Mech-Zehnder modulator, which accepts input from CW (Continuous Wave) laser array and low pass Bessel filter that supports PIN photodiode at receiver end. System performance for optical channels has been compared by varying channel length of EDFA (Erbium Doped Fiber Amplifier) and Raman amplifiers to identify its impact in the sense of eye diagram, QF (Quality Factor) and minimum BER (Bit Error Rate). Experimental setup was done by varying channel lengths of 60, 100, 120, 150 and 170Kms. It was found that EDFA has better performance in the sub-low ranges of 60Kms, whereas Raman offers better performance at wider channel lengths i.e. more than 100Kms. The proposed work can be extend for the avoidance of amplifier utilization at wider ranged.
