Background: Surgical intervention is a crucial treatment for hepatic cystic echinococcosis. However, the choice between radical and conservative surgery remains controversial. This study aimed to compare the perioperative outcomes and hospitalization costs between radical and conservative surgery for hepatic cystic echinococcosis. Method: A retrospective cohort study was conducted on patients undergoing surgical treatment at the First People's Hospital of Kashi Prefecture from July 1, 2012, to October 1, 2023. Propensity score-matching analysis was utilized to mitigate patient selection bias between the two surgical groups. Result: Of the 434 patients included, 324 underwent conservative surgery and 110 underwent radical surgery. After propensity score-matching, 182 conservative surgery and 102 radical surgery patients were compared. Radical surgery patients experienced longer operative time, higher intraoperative blood loss, increased blood transfusion, and higher costs compared to conservative surgery patients. However, no differences were observed in short-term outcomes, including overall morbidity, death, bile leak, effusion, pulmonary infection, incision infection, intestinal obstruction, ICU stay, abdominal drainage time, and postoperative hospital stays. Conclusion: This study suggests that radical surgery is associated with greater surgical complexity and higher hospitalization costs, while it doesn't offer a significant short-term advantage. Conservative surgery may be a viable option in resource-limited settings or for patients unsuitable for complex procedures. Further research with long-term follow-up is needed to determine the optimal approach. Background Posthepatectomy liver failure remains a potentially life-threatening complication after hepatectomy. Soluble suppression of tumourigenicity 2 is an injury-related biomarker. The aim of the study was to assess soluble suppression of tumourigenicity 2 elevation after hepatectomy and whether it can predict posthepatectomy liver failure. Methods This was a single-centre retrospective study including all patients who underwent a liver resection between 2015 and 2019. Plasma concentrations of soluble suppression of tumourigenicity 2 were measured before surgery and at postoperative days 1, 2, 5 and 7. Posthepatectomy liver failure was defined according to the International Study Group of Liver Surgery and the morbidity rate was graded according to the Clavien–Dindo classification. Results A total of 173 patients were included (75 underwent major and 98 minor resection); plasma levels of soluble suppression of tumourigenicity 2 increased from 43.42 (range 18.69–119.96) pg/ml to 2622.23 (range 1354.18–4178.27) pg/ml on postoperative day 1 (P < 0.001). Postoperative day 1 soluble suppression of tumourigenicity 2 concentration accurately predicted posthepatectomy liver failure ≥ grade B (area under curve = 0.916, P < 0.001) and its outstanding performance was not affected by underlying disease, liver pathological status and extent of resection. The cut-off value, sensitivity, specificity, positive predictive value and negative predictive value of postoperative day 1 soluble suppression of tumourigenicity 2 in predicting posthepatectomy liver failure ≥ grade B were 3700, 92%, 85%, 64% and 97% respectively. Soluble suppression of tumourigenicity 2high patients more frequently experienced posthepatectomy liver failure ≥ grade B (64.3% (n = 36) versus 2.6% (n = 3)) and Clavien–Dindo IIIa higher morbidity rate (23.2% (n = 13) versus 5.1% (n = 6)) compared with soluble suppression of tumourigenicity 2low patients. Conclusions Soluble suppression of tumourigenicity 2 may be a reliable predictor of posthepatectomy liver failure ≥ grade B as early as postoperative day 1 for patients undergoing liver resection. Objective: To explore the incidence, distribution characteristics and related symptoms of gastroesophageal reflux disease (GERD) as well as its related risk factors in Kashgar, Xinjiang., Methods: From March 2020 to October 2020, a questionnaire survey was conducted among 5,080 permanent residents aged 18-80 years in Kashgar using cluster sampling and stratified sampling methods. The content included basic information, accompanying symptoms and diseases, living, customs and eating habits, and Gastroesophageal Reflux Disease Questionnaire score, etc. Results: The prevalence of GERD in Kashgar was 23.4% (1187/5080), and the proportions of patients with reflux symptoms lasting 1 day, 2-3 days and 4-7 days within a week were 12.5%, 6.4% and 4.4%, respectively. The proportion of patients showing symptoms in the GERD group was significantly higher than that in the non-GERD group ( P <0.05). The proportion of people who are overweight or obese, take alcohol drink, eat, constipate, or take various chronic disease drugs in the GERD group is higher than that in the non-GERD group. The proportion of people in the GERD group who often eat sweet foods, pickled products, roasted products, spicy foods and meat, or drink coffee, acidic beverages, and cold drinks was higher than that in the non-GERD (P<0.05). The proportion of people in the GERD group who regularly consume fish, milk, eggs, vegetables, and fruits was significantly lower than that in non-GERD group (P<0.05). Logistic regression analysis found that Uyghur nationality (for the Han nationality), age (for the 30-39 years group), drinking, overeat, constipation, and frequent medication were risk factors ( P <0.05, OR>1). Multivariate logistic regression analysis found that sweets, baked products, cold drinks, and spicy foods were independent risk factors ( P <0.05, OR>1).
