OBJECTIVE: To describe the declining trend in maternal mortality observed in Mongolia from 1992 to 2007 and its acceleration after 2001 following implementation of the Maternal Mortality Reduction Strategy by the Ministry of Health and other partners. METHODS: We performed a descriptive analysis of maternal mortality data collected through Mongolia's vital registration system and provided by the Mongolian Ministry of Health. The observed declining mortality trend was analysed for statistical significance using simple linear regression. We present the maternal mortality ratios from 1992 to 2007 by year and review the basic components of Mongolia's Maternal Mortality Reduction Strategy for 2001-2004 and 2005-2010. FINDINGS: Mongolia achieved a statistically significant annual decrease in its maternal mortality ratio of almost 10 deaths per 100 000 live births over the period 1992-2007. From 2001 to 2007, the maternal mortality ratio in Mongolia decreased approximately 47%, from 169 to 89.6 deaths per 100 000 live births. CONCLUSION: Disparities in maternal mortality represent one of the major persisting health inequities between low- and high-resource countries. Nonetheless, important reductions in low-resource settings are possible through collaborative strategies based on a horizontal approach and the coordinated involvement of key partners, including health ministries, national and international agencies and donors, health-care professionals, the media, nongovernmental organizations and the general public.

Aim. To assess whether investments for medical equipments assigned by a team of experts to a mother and child health hospital located in Mongolia were correlated with structural, organizational, and educational level of its services/departments. Methods. A score was used for evaluating the level of each service/department. It was based on a 'structural area' and an 'organizational and educational area'. Destination of funds was determined by a team of experts in collaboration with the head of the service/department. Results. Thirty-three of 36 services/departments (91.6%) were evaluated. A total sum of 4,432,140 Euros to invest in medical equipment was estimated. Assigned investments were inversely correlated with the total (structural plus organizational and educational area) score ( n = 33; r = −0.59; p = 0.0002), and the specific scores for structural area ( n = 33; r = −0.46; p = 0.005) and organizational and educational area ( n = 33; r = −0.56; p = 0.0006). Conclusions. A large part of the funds for medical equipment was destined to services/departments with low organizational and educational conditions, limiting the potential effect of the aid meanwhile supporting the most in need departments. Hepatitis B virus infection is a serious public health problem. Mongolia is one of the countries with the highest rates of hepatitis B virus infection in the world. The routine immunization with the hepatitis B vaccine began nationwide in 1991. The purpose of this study was to determine the persistence of seroprotection (anti-HBs >or= 10 mlU/mL) in children 5-10 years old that were immunized with the hepatitis B vaccine as infants. In total, 438 children were selected from six health facilities in Ulaanbaatar through a multistage random sampling method. Vaccination information was confirmed by checking the vaccination records kept in the health facilities. A blood sample was obtained from each child for anti-HBs, HBsAg and anti-HBc. Of 438 children, five (1.1%) were HBsAg positive and 58 (13.2%) were anti-HBc positive. Sixty infected children were excluded and a total of 378 (86.3%) sera were evaluated. The seroprotective antibodies were detected in only one-fourth of the children at the age of ten. Titres of anti-HBs decreased significantly with age (Linear regression p = 0.01). This decrease is primarily due to the rapid decrease in children living in ger areas (p < 0.001) compared to children living in apartment areas (p = 0.152). On the other hand, children living with higher socio-economic status had more exposure to blood-borne pathogens, probably due to inappropriate health-seeking behaviors.
