7. Multiparametric in situ mRNA hybridization analysis to predict disease recurrence in patients with colon carcinoma

Abstract

We examined the expression level of several genes that regulate different steps of metastasis in formalin-fixed, paraffin-embedded archival specimens of primary human colon carcinomas from patients with at least 5 years of follow-up. The expression of epidermal growth factor receptor, basic fibroblast growth factor, type IV collagenase, E-cadherin, and multidrug resistance (mdr-1) was examined by a colorimetric in situ mRNA hybridization technique concentrating on reactivity at the periphery of the neoplasms. The in situ hybridization technique revealed inter- and intratumor heterogeneity for expression of the metastasis-related genes. The expression of basic fibroblast growth factor, collagenase type IV, epidermal growth factor receptor, and mdr-1 mRNA was higher in Dukes's stage D than in Dukes' stage B tumors. Among the 22 Dukes' stage B neoplasms, 5 specimens exhibited a high expression level of epidermal growth factor receptor, basic fibroblast growth factor, and collagenase type IV. Clinical outcome data (5-year follow-up) revealed that all 5 patients with Dukes' stage B tumors developed distant metastasis (recurrent disease), whereas the other 17 patients with Dukes' stage B tumors expressing low levels of the metastasis-related genes were disease-free. Multivariate analysis identified high levels of expression of collagenase type IV and low levels of expression of E-cadherin as independent factors significantly associated with metastasis or recurrent disease. More specifically, metastatic or recurrent disease was associated with a high ratio (> 1.35) of expression of collagenase type IV to E-cadherin (specificity of 95%). Collectively, the data show that multiparametric in situ hybridization analysis for several metastasis-related genes may predict the metastatic potential, and hence the clinical outcome, of individual lymph-node-negative human colon cancers.

21. Quantifying the position and steepness of radiation dose-response curves.

Abstract

Radiation dose-response curves are of fundamental importance both in practical radiotherapy and as the basis of more theoretical considerations concerning the potential benefit to be gained from modified dose-fractionation schedules or of the effects of dosimetric and biological variability. The steepness of the dose-response curve is a key parameter and quantitative measures of steepness derived from clinical data are strongly needed. Unfortunately, there are many ambiguities associated with quantifying the steepness of radiation dose-response curves and these are identified and discussed in the present paper. The following problems are reviewed. (1) In the literature, various descriptors of 'steepness' are reported. We focus on the normalized dose-response gradient, gamma , and the dose-response slope, theta . The mathematical properties and the relationship between these are discussed. (2) Steepness estimates depend on the mathematical model used to describe the dose-response relationship. Three standard formulations are considered: the Poisson, the logistic and the probit dose-response model. The magnitude of the model dependence is influenced by the range of the empirical dose-response data available, and is most pronounced for data concentrated around very low or very high response levels. (3) Reparametrizations of the standard models in terms of position and steepness are given, and it is pointed out that some previously published formulas are only approximations. (4) The method of analysis can influence the steepness estimate. An analysis of a specific data set shows that the use of the leastsquares method rather than the preferred maximum likelihood method may influence both the steepness estimate and its confidence interval. (5) Dose-response data generated with a fixed number of fractions rather than a fixed dose per fraction will produce steeper dose-response curves. The approximation involved in describing such a set of dose-response data by a position and a single steepness parameter is discussed. (6) The importance of specifying the statistical uncertainty of the steepness estimate is stressed. All of these problems are illustrated by a practical example, in which dose-response data from the literature are re-analysed. [ABSTRACT FROM AUTHOR]

Abstract

Objective: To evaluate serum beta-2 microglobulin (beta-2M) and other prognostic indicators in previously untreated low-grade lymphoma.Design: Cohort study of 80 patients with uniformly treated low-grade lymphoma, followed for a median of 21 months. These 80 patients, all of whom had serum beta-2M drawn within 2 weeks before starting therapy, were derived from a cohort of 119 previously untreated patients entered into one of three clinical trials.Setting: Tertiary referral cancer center.Patients: Eighty previously untreated stage I to IV patients (mean age, 55 years).Intervention: Treatment was given according to Ann Arbor stage: Patients in stage IV were treated with CHOP-bleomycin and maintained on interferon therapy; those in stage III received CHOP-bleomycin and radiotherapy; and those in stages I and II received COP-bleomycin and radiotherapy.Measurements: Outcome was determined by assessing complete remission rate and time to treatment failure. Univariate and multivariate analyses were used.Results: The complete remission rate for patients with a beta-2M level of 3.0 mg/L or greater was 36% compared with 71% for those with a level of less than 3.0 mg/L. Using multivariate analysis that tested beta-2M as a continuous variable, it was selected as the most significant factor for complete response. The adjusted odds ratio was 0.285 (95% CI, 0.101 to 0.809). The Ann Arbor stage had marginal significance (adjusted odds ratio, 0.435; CI, 0.150 to 1.263). For time to treatment failure, beta-2M was the only variable retained in the multivariate model. At 42 months, no patient with a beta-2M level of 3.0 mg/L or greater was projected to be in remission as compared with 85% of patients with a beta-2M level of less than 3.0 mg/L.Conclusions: The serum beta-2M level is a good predictor of complete response and time to treatment failure. A larger number of patients should be studied to clarify the role of other potentially independent variables such as stage and age. [ABSTRACT FROM AUTHOR]

42. Marketing: applications in a military health care setting.

Abstract

Military health care leaders must recognize the importance of satisfied consumers. As part of this recognition, the focus of military medicine must change from a coercive-power to a reward-power system. This change highlights the need for business practices such as marketing. Encouraging military health care administrators to learn and understand the applications of the marketing variables will enhance demand management and health care delivery for beneficiaries. This paper describes some applications of marketing variables, informs the military health care administrator about the process of marketing, and describes the utility of marketing in the current paradigm shift in military health care delivery.
