4. The Perspectives of Students Undertaking Masters' Degrees by Coursework on Career Development Learning

Abstract

Master's degree by coursework students have been identified as a "forgotten" cohort by the Queensland University of Technology (QUT) Careers and Employment service. Traditionally, these students have been included in undergraduate career development activities. Master's degree students are arguably a specialised group due to the advanced learning implicit in many of their degrees. A brief survey was designed to better understand how to assist QUT master's degree students with career development activities. Students were emailed an invitation to complete a brief online survey, asking for their assistance in planning appropriate career development activities for their cohort. Questions included reasons for undertaking a master's degree and preferred topics and times for career development activities. A total of 615 students completed the survey. This brief report presents the results of the survey and highlights suggestions for career development practitioners and services. (Contains 3 tables.)

11. Active compression-decompression: a new method of cardiopulmonary resuscitation

Abstract

A mechanical device that allows doctors to decompress the chest actively during cardiopulmonary resuscitation (CPR) rather than passively may be more effective in restoring heart function in patients who are in cardiac arrest. This device was used on 10 patients in cardiac arrest who had received conventional care for at least 10 minutes. Each patient was randomized to receive either CPR with an active compression-decompression (ACD) device or traditional CPR for two minutes; then they were switched to the alternate method. Blood pressure in eight patients increased significantly when they were switched from CPR to ACD-CPR. Doppler echocardiography during ACD-CPR showed that blood flow from the heart increased, compared to CPR. The echocardiogram also showed that the mitral valve, which controls the flow of blood into the left ventricle, opened much more widely during the relaxation phase of ACD-CPR, compared to CPR. ACD-CPR restored a stable rhythm in three of the patients, but none survived., Objective.--To described and compare with standard cardiopulmonary resuscitation (CPR) in humans a new form of CPR that involves both active compression and active decompression of the chest. Design.--Patients in cardiac arrest in whom standard cardiac life support failed were randomized to receive 2 minutes of either standard or active compression-decompression (ACD) CPR using a custom, hand-held suction device, followed by 2 minutes of the alternate technique. The ACD device was applied midsternum and used to perform CPR according to the guidelines of the American Heart Association: 80 compressions per minute, compression depth of 3.8 to 5 cm, 50% duty cycle, and constant-volume ventilation. Mechanical Thumper CPR was also compared in five patients. End-tidal carbon dioxide ([ETCO.sub.2]) concentrations and hemodynamic variable were measured. Transesophageal Doppler echocardiography was used to assess contractility, the velocity time integral(an analogue of cardiac output), and diastolic myocardial filling times. Abstract

Purpose: To review the historical evolution and rationale for the development of new techniques of cardiopulmonary resuscitation (CPR). Data Sources: English-language studies published after 1960 were identified by computer and manual search using MEDLINE and Index Medicus. Historical references were obtained through a HISTLINE search. Additional information was acquired from personal files and bibliographies of existing literature. Study Selection: Critical review with emphasis on study size, methods, and reproducibility of results. Results: Survival after in-hospital cardiac arrest and institution of CPR is approximately 10% to 15%. Investigation of the physiology of blood flow during CPR has led to the conclusion that flow may occur because of direct cardiac compression or thoracic pump forces. Based on these observations, several new techniques of CPR have been introduced. Interposed abdominal compression, pneumatic vest, and active compression-decompression resuscitation have been shown to improve cardiopulmonary hemodynamics in animal models and humans after cardiac arrest. Only interposed abdominal compression CPR has been shown to improve long-term survival in human subjects after in-hospital cardiac arrest. Although these techniques have shown promising results in animal studies and a limited number of clinical trials, none has gained widespread use. Conclusions: Improved methods of CPR are now available. Selective use of CPR in the hospital and community training in the use of these new adjunctive techniques should have the greatest impact on improved survival after sudden cardiac arrest. Abstract

To determine the frequency and distribution of pneumonia in an intensive care unit (ICU), we retrospectively examined the records of 1,854 consecutive ICU admissions between January 1987 and April 1990. A total of 266 patients met criteria for pneumonia (unilateral or bilateral infiltrate by chest roentgenogram, plus 2 of the following: leukocyte count > 10 x 10[9] per liter, temperature > 38.5 ° C, or culture of blood or sputum positive for pathogens). Pneumocystis carinii pneumonia in patients infected with the human immunodeficiency virus was the most frequent cause (28%) precipitating an ICU admission in this series of patients. Streptococcus pneumoniae (13%), Staphylococcus aureus (8%), Haemophilus influenzae (4%), and viruses (4%) were also commonly observed. Overall mortality was 20%. An APACHE II score of greater than 24, the need for intubation, and the presence of P carinii were predictive of increased mortality. Age, sex, and length of stay did not predict final results. Abstract

Adenosine was administered to test the hypothesis that it would maximize preexcitation and facilitate noninvasive localization of accessory pathways in 22 patients with suspected accessory pathway-mediated tachycardias. Twelve-lead electrocardiograms and 2-dimensional echocardiograms were recorded at baseline and during adenosine-augmented ventricular preexcitation to localize the accessory pathway. Phase analysis was performed on digitized 4-chamber and short-axis views using a first harmonic Fourier transformation. At baseline, 15 patients had manifest preexcitation. In 14 of these patients (93.3%), preexcitation became more prominent after adenosine. Four patients without preexcitation at baseline clearly had it after adenosine. In patients who had preexcitation in response to adenosine, the electrocardiogram correctly identified the accessory pathway locations in 18 of 19 patients at a regional level and was incorrect in 1 of 19 patients. Echocardiographic phase analysis correctly identified the accessory pathway location in all 17 patients, who had technically adequate studies, at a regional level. In conclusion, administration of adenosine accentuates preexcitation, allowing for more accurate electrocardiographic and echocardiographic accessory pathway localization. Abstract

A woman’s vaginal pH has many implications on her health and it can be a useful tool in disease diagnosis and prevention. For that reason, the further examination of the relationship between the human vaginal pH and microbiota is imperative. In the past several decades, much has been learned about the physiological mechanisms modulating the vaginal pH, and exogenous/genetic factors that may influence it. A unified, coherent understanding of these concepts is presented to comprehend their interrelationships and their cumulative effect on a woman’s health. In this review, we explore research on vaginal pH and microbiota throughout a woman’s life, vaginal intermediate cell anaerobic metabolism and net proton secretion by the vaginal epithelial, and the way these factors interact to acidify the vaginal pH. This review provides foundational information about what a microbiota is and its relationship with human physiology and vaginal pH. We then evaluate the influence of physiological mechanisms, demographic factors, and propose ideas for the mechanisms behind their action on the vaginal pH. [ABSTRACT FROM AUTHOR]

Abstract

Turbulence and mean flow measurements were acquired downstream of a normal, supersonic (M=1.6), two-dimensional, slot injection into Mach 3.0 flow (Re/m = 15x10 to the 6th power). Two non-adiabatic cases (Tti/Tt = 1.11 and Tti/Tt = 0.93) were studied. Multiple overheat anemometry and conventional mean flow analysis, as well as shadowgraph and schlieren photography, were utilized to study the turbulent mixing layer. The mean flow data included the Pitot, cone-static, and Mach number profiles. The measured turbulence data included shear stresses, mass flux, total temperature turbulence intensities, and turbulent heat fluxes. The temperature and density variation in the flow provided a better understanding of the mixing process. The turbulence mass flux and shear results highlighted the importance of compressibility within the mixing region. The optical analysis showed the physical phenomena of the flow as well as the associated turbulent structure that existed in the mixing layer. (AN)
