Abstract

Introduction: Favorable long-term outcomes of endoscopic submucosal dissection (ESD) for early remnant gastric cancer (ERGC) have been reported in single-center studies from advanced institutions. However, no studies have examined the long-term outcomes using a multicenter database. This study aimed to investigate the long-term outcomes of the aforementioned approach using a large multicenter database. Methods: This retrospective multicenter cohort study included 242 cases with 256 lesions that underwent ESD for ERGC between April 2009 and March 2019 across 12 centers. We investigated the long-term outcomes of these patients with the Kaplan-Meier method, and the relationship between curability, additional treatment, or hospital category, and the survival time was evaluated using the log-rank test. Results: During the median follow-up period of 48.4 months, the 5-year overall survival rate was 81.3%, and the 5-year gastric cancer-specific survival rate was 98.1%. The survival time of patients of endoscopic curability (eCura) C-2 without additional surgery was significantly shorter than the corresponding of patients of eCura A/B/C-1 and eCura C-2 with additional surgery. There was no significant difference in either overall survival or gastric cancer-specific survival rate between the high-volume and non-high-volume hospitals. Conclusion: The gastric cancer-specific survival of ESD for ERGC using a multicenter database was favorable. ESD for ERGC is widely applicable regardless of the hospital case volume. Abstract

Objectives: Peroral endoscopic myotomy (POEM) is an effective treatment for esophageal motility disorders including achalasia and its variants. However, some surgeons have encountered challenging cases. This study aimed to develop a risk‐scoring system to predict challenging cases of POEM. Methods: Consecutive patients who underwent POEM between April 2015 and March 2020 at our hospital were included in this single‐center retrospective study. Challenging cases of POEM were defined as patients with any of the following: (i) procedure time ≥90 min; (ii) mucosal perforation; (iii) pneumothorax; and (iv) major bleeding. A risk‐scoring system for predicting challenging cases was developed based on multivariate logistic regression and internal validation was performed using the bootstrap method. Clinical usefulness was evaluated using a decision curve analysis. Results: Of the 467 patients, 59 (12.6%) had challenging POEM. A risk‐scoring system consisted of four variables: duration of symptoms ≥5 years (assigned score, 1 point), antithrombotics use (1 point), manometric diagnosis of achalasia variants (2 points), and dilation grade 3 (2 points). Our scoring system showed satisfactory discrimination (area under the receiver operating characteristic curve, 0.69; 95% confidence interval [CI] 0.61–0.77) and calibration (slope, 0.99; 95% CI 0.65–1.35). The decision curve analysis demonstrated its clinical usefulness. Conclusions: We established a risk‐scoring system to predict challenging cases of POEM. Abstract

Adult T cell leukemia (ATL; uncontrolled growth of white blood cells) is caused by the human T cell leukemia virus (HTLV-I). It is not clear how the virus causes this disease. While HTLV-I is found in all ATL cells, it does not cause disease in all individuals it infects; instead, most remain healthy carriers of the HTLV-I virus. Why only some individuals progress to ATL or other associated diseases is unknown. There is evidence of a higher rate of HTLV-I in family members of ATL patients, suggesting transmission of the infection within the family or an effect of genetic backgrounds shared by blood relatives. Research has shown that a gene called the tax gene may potentially induce leukemia. Based on the assumption that this is true, and that ATL frequently occurs in families, a study was developed to test the prevalence of tax proteins (by testing for antibodies to tax proteins) in HTLV-I infected people, compared with matched (for age) HTLV-I-positive blood donors. It was found that direct offspring of ATL patients produced more antibodies than their HTLV-I-positive blood donor matches. This suggests possible differences in time or route of transmission of HTLV-I infection, or differences in the strain of virus within families, compared with strains among non-blood-relatives. Abstract

The immune system of patients who are infected with human T-cell leukemia virus type I (HTLV-I) and have developed adult T-cell leukemia (ATL) is suppressed. However, the status of the immune system in carriers of HTLV-I, who are healthy and have no disease symptoms, is not certain. Previous studies have shown that the immune reaction to the tuberculin skin test, which measures delayed-type hypersensitivity (DTH), is suppressed in HTLV-I carriers in Japan, where HTLV-I infection is endemic. The suppression is more pronounced in individuals who are elderly, aged 60 years or older. Other types of DTH, reaction to purified protein derivative (PPD) of tuberculin and phytohemagglutinin (PHA), were examined in 56 patients, aged from 62 to 93 years old. The subjects were hospitalized with diseases that did not cause a suppression of the immune system. Twenty-two (39 percent) had antibodies to HTLV-I, indicating infection, but none had symptoms of disease caused by HTLV. There was an increased level of nonreactivity to PPD and PHA with increasing age. The degree of nonreactivity was greater in carriers than in non-carriers. Eighty-eight percent (15 out of 17) of HTLV-I carriers were nonreactive to both antigens, compared with 60 percent (15 out of 25) of non-carriers. Among the carriers who reacted to the antigens, the degree of reaction to PPD was reduced, but reaction to PHA was not reduced. Abstract

Based on the concept of the adenoma‐carcinoma sequence, most colorectal cancers are considered to arise from conventional adenomas. However, recent studies suggested that a subset of colorectal cancers develop through the serrated neoplastic pathway. It has also been documented that serrated polyps can rapidly transform into invasive cancers even when they are small in size. We now describe a case of a sessile serrated adenoma/polyp which had been followed up for 4 years but eventually showed rapid transformation into an advanced cancer accompanied by a remarkable morphological change within only 13 months. Retrospective genetic and epigenetic analyses showed microsatellite instability, CpG island methylator phenotype‐positive, and BRAF mutation in the lesion, suggesting the tumor had developed through the serrated neoplastic pathway. This case may provide valuable information about the natural history of sessile serrated adenoma/polyps which eventually progress to advanced cancers. [ABSTRACT FROM AUTHOR]

40. Sensitivity of Polymerase Chain Reaction Assay for Rickettsia tsutsugamushiin Patients' Blood Samples

Abstract

We developed a nested polymerase chain reaction (PCR) method to detect Rickettsia tsutsugamushi(R. tsutsugamushi) DNA and determined its sensitivity. Primers were selected from the DNA sequence of the 58‐kDa group‐specific antigen gene of the Karp strain. The target sequence of rickettsial DNA was detectable as the band corresponding to 88 bp in 1.0 μgof the DNA extracted from BS‐C‐1 cells infected with R. tsutsugamushi. Rickettsia‐specific bands were observed not only for the homologous Karp strain, but also for four heterologous strains: two other reference strains (Gilliam and Kato) and two prototype strains prevalent in Miyazaki district (Irie and Hirano). The minimum copy number detectable by this method was estimated to be five rickettsiae. All of nine peripheral blood mononuclear cell samples from patients with tsutsugamushi disease who were seen 2‐11 days after disease onset tested positive for rickettsial DNA. The PCR assay method presented here could be a specific diagnostic tool for tsutsugamushi disease, especially in its early acute stage.
