1. Vanishing in Fractal Space: Thermal Melting and Hydrodynamic Collapse

Abstract

Fractals emerge everywhere in nature, exhibiting intricate geometric complexities through the self-organizing patterns that span across multiple scales. Here, we investigate beyond steady-states the interplay between this geometry and the vanishing dynamics, through phase-transitional thermal melting and hydrodynamic void collapse, within fractional continuous models. We present general analytical expressions for estimating vanishing times with their applicability contingent on the fractality of space. We apply our findings on the fractal environments crucial for plant growth: natural soils. We focus on the transport phenomenon of cavity shrinkage in incompressible fluid, conducting a numerical study beyond the inviscid limit. We reveal how a minimal collapsing time can emerge through a non-trivial coupling between the fluid viscosity and the surface fractal dimension.

3. One-loop matrix element emulation with factorisation awareness

Abstract

In this article we present an emulation strategy for one-loop matrix elements. This strategy is based on the factorisation properties of matrix elements and is an extension of the work presented in arXiv:2107.06625. We show that a percent-level accuracy can be achieved even for large multiplicity processes. The point accuracy obtained is such that it dwarfs the statistical accuracy of the training sample which allows us to use our model to augment the size of the training set by orders of magnitude without additional evaluations of expensive one-loop matrix elements., Comment: 20 pages, 10 figures, minor corrections for v2

Abstract

Finding the equivalent resistance of an infinite ladder circuit is a classical problem in physics. We expand this well-known challenge to new classes of network topologies, in which the unit cells are much more entangled together. The exact analytical results there can still be obtained with elementary methods. These topology classes will add layers of complexity and much more diversity to a very popular kind of physics puzzles for teachers and students.

6. Infinite AC Ladder with a 'Twist'

Abstract

The infinite AC ladder network can exhibit unexpected behavior. Entangling the topology brings even more surprises, found by direct numerical investigation. We consider a simple modification of the ladder topology and explain the numerical result for the complex impedance, using linear algebra. The infinity limit of the network's size corresponds to keeping only the eigenvectors of the transmission matrix with the largest eigenvalues, which can be viewed as the most dominant modes of electrical information that propagate through the network.

Abstract

Positive displacement compressors are essential in many engineering systems, from domestic to industrial applications. Many studies have been devoted to providing more insights into the workings and proposing solutions for performance improvements of these machines. This study aims to present a systematic review of published research on positive displacement compressors of various geometrical structures. This paper discusses the literature on compressor topics, including leakage, heat transfer, friction and lubrication, valve dynamics, port characteristics, and capacity control strategies. Moreover, the current status of the application of machine learning methods in positive displacement compressors is also discussed. The challenges and opportunities for future work are presented at the end of the paper.

Abstract

Background: Literature suggests colonic resection and primary anastomosis (RPA) instead of Hartmann’s procedure (HP) for the treatment of left-sided colonic emergencies. We aim to evaluate the surgical options globally used to treat patients with acute left-sided colonic emergencies and the factors that leading to the choice of treatment, comparing HP and RPA. Methods: This is a prospective, international, multicenter, observational study registered on ClinicalTrials.gov. A total 1215 patients with left-sided colonic emergencies who required surgery were included from 204 centers during the period of March 1, 2020, to May 31, 2020. with a 1-year follow-up. Results: 564 patients (43.1%) were females. The mean age was 65.9 ± 15.6 years. HP was performed in 697 (57.3%) patients and RPA in 384 (31.6%) cases. Complicated acute diverticulitis was the most common cause of left-sided colonic emergencies (40.2%), followed by colorectal malignancy (36.6%). Severe complications (Clavien-Dindo ≥ 3b) were higher in the HP group (P < 0.001). 30-day mortality was higher in HP patients (13.7%), especially in case of bowel perforation and diffused peritonitis. 1-year follow-up showed no differences on ostomy reversal rate between HP and RPA. (P = 0.127). A backward likelihood logistic regression model showed that RPA was preferred in younger patients, having low ASA score (≤ 3), in case of large bowel obstruction, absence of colonic ischemia, longer time from admission to surgery, operating early at the day working hours, by a surgeon who performed more than 50 colorectal resections. Conclusions: After 100 years since the first Hartmann’s procedure, HP remains the most common treatment for left-sided colorectal emergencies. Treatment’s choice depends on patient characteristics, the time of surgery and the experience of the surgeon. RPA should be considered as the gold standard for surgery, with HP being an exception.

Abstract

The optimization process of compressors is usually regarded as a ‘black-box’ problem, in which the mathematical form underlying the relationship between design parameters and the design objective is impractical and costly to be obtained. To solve the ‘black-box’ problem, Bayesian optimization has been proven as an accurate and efficient method. However, the application of such a method in the design of compressors is rarely discussed, particularly no work has been reported in terms of the positive displacement type compressor. Therefore, this paper aims to introduce the Bayesian optimization to the design of positive displacement compressors through the optimization process of the novel limaçon compressor. In this paper, a two-stage optimization process is presented, in which the first stage optimizes the geometric parameters as per design requirements and the second stage focuses on revealing an optimum setting of port geometries that improves machine performance. A numerical illustration is offered to prove the validity of the presented approach.

Abstract

The thrusters and propulsion propellers systems, as well as the operating situations, are all well-known nonlinearities which are caused less accuracy of the dynamic positioning system (DPS) of vessels in the path planning control process. In this study, to enhance the robust performance of the DPS, we proposed a robust adaptive fuzzy control model to reduce the effect of uncertainty problems and disturbances on the DPS. Firstly, the adaptive fuzzy controller with adaptive law is designed to adjust the membership function of the fuzzy controller to minimize the error in path planning control of the vessel. Secondly, the H ∞ performance of robust tracking is proved by the Lyapunov theory. Moreover, compared to the other controller, a simulation experiment comprising two case studies confirmed the efficiency of the approach. 24. Development and validation of a model for predicting 18-month mortality in type 2 myocardial infarction.

Abstract

Background: Despite the poor prognosis in patients with type 2 myocardial infarction (MI), no prospective data on risk stratification exists. The aim of this study was to develop and validate a model for prediction of 18-month mortality of among patients with type 2 MI (T2MI) and compare its performance with GRACE and TARRACO scores.Methods: The prospective observational study included 712 consecutive patients diagnosed with MI undergoing coronary angiography <24 h between January 2017 and December 2018. Diagnosis of T2MI was adjusted according to Third universal definition. A prognostic model was developed by using Bayesian approach and logistic regression analysis with identifying predictors for mortality. The model was validated by bootstrap validation. Comparison performance between scores using Delong test.Results: T2MI was identified in 174 (24.4%) patients. The median age of patients was 69 years, 52% were female. The mortality rate was 20.1% at 18 months. Prior MI, presence of ST elevation, hemoglobin level at admission, Charlson comorbidity index and were independently associated with 18-month mortality. The model to predict 18-month mortality showed excellent discrimination (optimism corrected c-statistic = 0.822) and calibration (corrected slope = 0.893). GRACE and TARRACO scores had moderate discrimination [c-statistic = 0.748 (95% CI 0.652-0.843) and 0.741, 95% CI 0.669-0.805), respectively] and inferior compared with model (p = 0.043 and 0.037, respectively).Conclusions: The risk of mortality among T2MI patients could be accurately predicted by using common clinical characteristics and laboratory tests. 26. A Proof of Concept for Applying the Radicchi Index (hf) to Compare Academic Productivity and Scientific Impact Among Medical Specialties

Abstract

Many indices have been developed to assess the impact of scientific publications by investigators, disciplines, and institutions. The h-indexhas emerged as a leading tool in the assessment of the productivity of authors. Differences in publication and citation opportunity among specialties create inappropriate conclusions when the h-indexis used to compare authors across different disciplines. An alternative, the Radicchi index, hf, has been proposed to assess the impact of publications across disciplines. We curated a database of all articles published from 2002 to 2015 from the 3 highest impact factor medical journals: New England Journal of Medicine (NEJM), the Lancet, and the Journal of American Medical Association (JAMA). With this database, articles were categorized into medical subspecialties. We calculated the respective h-indexand Radicchi index of each specialty. We found that the Radicchi index eliminated variability associated with publication and citation opportunity between different specialties when compared to the h-index. The Radicchi index is a useful measure of scientific impact and productivity that advances the h-indexby allowing interspecialty comparisons. There remains a need to define a researcher's specialty designation especially if he/she conducts multidisciplinary research.

Abstract

Background Children living with HIV (CLHIV) on prolonged antiretroviral therapy (ART) are at risk for lipid and glucose abnormalities. Prevalence and associated factors were assessed in a multicentre, Asian longitudinal paediatric cohort.Methods CLHIV were considered to have lipid or glucose abnormalities if they had total cholesterol ≥200 mg/dL, high-density lipoprotein (HDL) ≤35 mg/dL, low-density lipoprotein (LDL) ≥100 mg/dL, triglycerides (TG) ≥110 mg/dL, or fasting glucose >110 mg/dL. Factors associated with lipid and glucose abnormalities were assessed by logistic regression.Results Of 951 CLHIV, 52% were male with a median age of 8.0 (interquartile range [IQR] 5.0–12.0) years at ART start and 15.0 (IQR 12.0–18.0) years at their last clinic visit. 89% acquired HIV perinatally, and 30% had ever used protease inhibitors (PIs). Overall, 225 (24%) had hypercholesterolemia, 105 (27%) low HDL, 213 (58%) high LDL, 369 (54%) hypertriglyceridemia, and 130 (17%) hyperglycemia. Hypercholesterolemia was more likely among females (versus males, aOR 1.93, 95% CI 1.40–2.67). Current PIs use was associated with hypercholesterolemia (current use: aOR 1.54, 95% CI 1.09–2.20); low HDL (current use: aOR 3.16, 95% CI 1.94–5.15; prior use: aOR 10.55, 95% CI 2.53–43.95); hypertriglyceridemia (current use: aOR 3.90, 95% CI 2.65–5.74; prior use: aOR 2.89, 95% CI 1.31–6.39); high LDL (current use: aOR 1.74, 95% CI 1.09–2.76); and hyperglycemia (prior use: aOR 2.43, 95% CI 1.42–4.18).Conclusion More than half and one-fifth of CLHIV have dyslipidemia and hyperglycemia, respectively. Routine paediatric HIV care should include metabolic monitoring. The association between PIs use and dyslipidemia emphasizes the importance of rapidly transitioning to integrase inhibitor-containing regimens.

29. Budget impact of pasireotide for the treatment of Cushing’s disease, a rare endocrine disorder associated with considerable comorbidities

AbstractObjectives:Cushing’s disease (CD) is a rare condition with a prevalence of roughly 39 cases per million in the general population. Healthcare costs are substantial for CD patients with either untreated or inadequately controlled disease. This study assesses the 3-year budget impact of pasireotide on a US managed care health plan following pasireotide (Signifor) availability.Methods:Two scenarios were evaluated to understand the differences in costs associated with the introduction of pasireotide. The first scenario evaluates the budget impact of pasireotide from the perspective of an entire health plan (total budget impact) and the second from the perspective of the pharmacy budget (pharmacy budget impact). Both scenarios evaluate the annual incremental budget impact with and without pasireotide. Scenario 1 includes costs for medical procedures, drug therapies, monitoring, surgical complications, comorbidities for patients with controlled or uncontrolled CD, and adverse events. Procedures include transsphenoidal surgery, bilateral adrenalectomy, radiotherapy and radiosurgery. Drugs include pasireotide (indicated for CD), mifepristone (indicated to control hyperglycemia secondary to hypercortisolism in patients with Cushing’s syndrome) as well as several off-label treatments (ketoconazole, cabergoline, mitotane). Scenario 2 considers costs solely from the perspective of a health plan pharmacy. Costs are in 2013.Results:The estimated total budget impact is 0.0115 per-member per-month (PMPM) in the first year following FDA approval, 0.0184 in the second year, and 0.0194 in the third year. Introduction of pasireotide is expected to increase the pharmacy budget by 0.0257 PMPM in the first year, 0.0363 in the second year, and 0.0360 in the third year.Limitations:Model inputs rely on the small body of literature available for Cushing’s disease.Conclusions:Cushing’s disease is severe disease with debilitating comorbidities and substantial healthcare costs when untreated or inadequately controlled. The inclusion of pasireotide in a health plan formulary appears to have only a small impact on the total health plan or pharmacy budget.

Abstract

Background The World Health Organization (WHO) recommends boosted protease inhibitor (bPI)-based HAART after failing non-nucleoside reverse transcriptase inhibitor (NNRTI) treatment. We examined outcomes of this regimen in Asian HIV-infected children.Methods Children from five Asian countries in the TREAT Asia Pediatric HIV Observational Database (TApHOD) with =24 weeks of NNRTI-based HAART followed by =24 weeks of bPI-based HAART were eligible. Primary outcomes were the proportions with virological suppression (HIV RNA<400 copies/ml) and immune recovery (CD4+T-cell percentage [CD4%]=25% if age <5 years and CD4+T-cell count =500 cells/mm3if age =5 years) at 48 and 96 weeks.Results Of 3,422 children, 153 were eligible; 52% were female. At switch, median age was 10 years, 26% were in WHO stage 4. Median weight-for-age z-score (WAZ) was -1.9 (n=121), CD4% was 12.5% (n=106), CD4+T-cell count was 237 cells/mm3(n=112), and HIV RNA was 4.6 log10copies/ml (n=61). The most common bPI was lopinavir/ritonavir (83%). At 48 weeks, 61% (79/129) had immune recovery, 60% (26/43) had undetectable HIV RNA and 73% (58/79) had fasting triglycerides =130 mg/dl. By 96 weeks, 70% (57/82) achieved immune recovery, 65% (17/26) had virological suppression, and hypertriglyceridaemia occurred in 66% (33/50). Predictors for virological suppression at week 48 were longer duration of NNRTI-based HAART (P=0.006), younger age (P=0.007), higher WAZ (P=0.020) and HIV RNA at switch <10,000 copies/ml (P=0.049).Conclusions In this regional cohort of Asian children on bPI-based second-line HAART, 60% of children tested had immune recovery by 1 year, and two-thirds had hyper-lipidaemia, highlighting difficulties in optimizing second-line HAART with limited drug options.

31. Evaluation of a DNA Microarray, the Check-Points ESBL/KPC Array, for Rapid Detection of TEM, SHV, and CTX-M Extended-Spectrum β-Lactamases and KPC Carbapenemases

ABSTRACTExtended-spectrum ß-lactamases (ESBLs) and Klebsiella pneumoniaecarbapenemases (KPC carbepenemases) have rapidly emerged worldwide and require rapid identification. The Check-Points ESBL/KPC array, a new commercial system based on genetic profiling for the direct identification of ESBL producers (SHV, TEM, and CTX-M) and of KPC producers, was evaluated. Well-characterized Gram-negative rods (Enterobacteriaceae, Pseudomonas aeruginosa, Acinetobacter baumannii) expressing various ß-lactamases (KPC-2, SHV, TEM, and CTX-M types) were used as well as wild-type reference strains and isolates harboring ß-lactamase genes not detected by the assay. In addition, phenotypically confirmed ESBL producers isolated in clinical samples over a 3-month period at the Bicetre hospital were analyzed using the Check-Points ESBL/KPC array and by standard PCR. The Check-Points ESBL/KPC array allowed fast detection of all TEM, SHV, and CTX-M ESBL genes and of the KPC-2 gene. The assay allowed easy differentiation between non-ESBL TEM and SHV and their ESBL derivatives. None of the other tested ß-lactamase genes were detected, underlining its high specificity. The technique is suited for Enterobacteriaceaebut also for P. aeruginosaand A. baumannii. However, for nonfermenters, especially P. aeruginosa, a 1:10 dilution of the total DNA was necessary to detect KPC-2 and SHV-2a genes reliably. The Check-Points ESBL/KPC array is a powerful high-throughput tool for rapid identification of ESBLs and KPC producers in cultures. It provided definitive results within the same working day, allowing rapid implementation of isolation measures and appropriate antibiotic treatment. It showed an interesting potential for routine laboratory testing.

Abstract

We have tested the hypothesis that a fermented beverage from the macroalgae Hydropuntia eucheumatoidesexhibits antioxidant and enzymatic activity. The macroalga was hydrolyzed (maximum hydrolysis yield: 78%) with a mixture of the enzymes viscozyme and lactozyme. Then, the hydrolyzate was fermented with Lactobacillus caseiand Saccharomyces boulardii. This beverage contained oligosaccharide prebiotics. The lactic acid, acetic acid, ethanol, methanol, cell count, pH, and heavy metal content of the beverage were determined. All tested heavy metals were either not detected (eg, As) or within the US Food and Drug Administration limits (eg, Fe).

Abstract

This paper reviews IBM's 193 nm resist development efforts, placing an emphasis on the systems employing fluoroalcohol as an acidic group. Polymethacrylates were initially selected as a platform for their good transparency at 193 nm and then the emphasis on dry etch resistance prompted the work on all‐norbornene systems. Carboxylic acid was employed in these resist systems as an acid group, which resulted in significant swelling during aqueous base development. Replacement of the carboxylic acid with fluoroalcohol has produced second generation 193 nm resist systems, the good dissolution behavior of which is offered by the fluoroalcohol group. The fluoroalcohol‐based 193 nm resists in this study include cycloolefin‐SO2, all‐norbornene, trifluoromethylacrylate‐norbornene (and vinyl ether), methacrylate, and silsesquioxane systems. 36. Time of Flowering of Pea (Pisum sativum L.) as a Function of Leaf Appearance Rate and Node of First Flower

Abstract

In order to assess the influence of environmental conditions on time of flowering of pea (Pisum sativum L.), a serial sowing trial was conducted over 2 years at Dijon, France, on two winter cultivars Frisson and Frilene. Time of flowering was analysed according to two variables: the leaf appearance rate RL and the node of first flower NI.RL was linearly related to temperature (r2 = 0·94). The base temperature was 2°C for both varieties. Growth rate accounted for the residual variability of RL . Photoperiod and temperature acted on NI in an additive way. Frilene, the later genotype, was more responsive than Frisson.A model for predicting time of flowering based upon these results is proposed. Tobacco nitrate reductase (NR) produced in yeast retains cytochrome c reductase activity, but not NR activity. Biochemical data suggest that the haem and FAD domains are functional, and that the molybdenum cofactor (MoCo) domain is inactive owing to the absence of MoCo in yeast. The native form of the produced NR is dimeric. Thus MoCo is not involved in NR dimerization in higher plants, contrary to current assumptions.

40. A rule-based expert system for the automatic classification of DNA “ploidy” histograms measured by the CAS 200 image analysis system

Abstract

DNA “ploidy” histogram interpretation is one of the most important sources of variation in DNA image cytometry and is influenced by multiple technical factors such as scaling, selection of peaks, and variable classification criteria. A rule-based expert system was developed to automate and eliminate subjectivity from this interpretative process. Ninety-eight Feulgen stained histologic sections from patients with breast, colon, and lung cancer were measured with the CAS 200 image analysis system (Becton Dickinson, Santa Clara, CA); they included diploid (n = 42), aneuploid (n = 46), tetraploid (n = 7), and multiploid (n = 3) examples. The data was converted from listmode format into ASCII with the aid of CELLSHEET software (JVC Imaging, Elmhurst, IL). Individual microphotometric nuclear measurements were sorted to one of 64 bins based on DNA index. The 64 bins were then divided into 5 semi-arbitrarily defined ranges: hypodiploid, diploid, aneuploid, tetraploid, and hypertetraploid. The nuclear percentages in each range were calculated with EXCEL 4.0 (Microsoft, Redmond, WA). The histograms were divided into 2 equal sets: training and testing. The data from the training set were used to develop 16 IF-THEN rules to classify the histograms into diploid, aneuploid, or tetraploid. A macro was programmed in EXCEL to automate all these operations. The rule-based expert system classified correctly 45/50 histograms of the training set. Two tetraploid histograms were classified as aneuploid. Three multiploid histograms were classified as tetraploid. All histograms in the testing set were correctly classified by the expert system. 42. Ipsilateral ureteroureterostomy for ureteral duplication anomalies: predictors of adverse outcomes.

Abstract

Ipsilateral ureteroureterostomy (UU) has gained popularity for treating ureteral duplication (UD) associated with obstruction or vesicoureteral reflux (VUR). Several studies have demonstrated its high success rate and low morbidity. This study aimed to identify the predictors of adverse outcomes associated with UU. A retrospective chart review was conducted for patients with UD treated with UU at the study institution. Patients with less than 6 months of follow-up were excluded. Charts were reviewed for demographics, pre-operative imaging, surgical indications, operative notes, and follow-up data. Pre-operative and intra-operative variables were tested for association with adverse outcomes including unplanned surgery, worsening hydronephrosis, and febrile UTI recurrence. A total of 35 patients underwent UU for UD between 2009 and 2017 at the study institution. The majority (33/35) had a distal anastomosis. A concomitant re-implantation of the recipient ureter was performed in 9 (25.7%) patients. After a median follow-up of 36.7 months, adverse outcomes developed in 9 patients (25.7%). No further surgery was required in 88.6% of the subjects. Adverse outcomes were associated with female gender (p = 0.048) and hydronephrosis of both upper and lower moieties (p = 0.015) and were more likely associated with ectopic ureteroceles (50%, p = 0.074) and having a concomitant re-implant performed (44.4%, p = 0.19) (table). In this study, the low re-operation rates of UU for treating UD in patients with ectopic ureters or intravesical ureteroceles was demonstrated. The majority of patients with ectopic ureteroceles treated with UU and concomitant ureteral re-implantation had worsening hydronephrosis of both moieties and recurrent febrile UTI in the absence of mechanical obstruction. Interestingly, some of them had progression of hydronephrosis after excision of the upper pole. While worsening of hydronephrosis of the recipient moiety after UU was historically attributed to yo-yo reflux, most of the study patients did not have worsening hydronephrosis despite having distal anastomosis. Therefore, mechanical obstruction should be ruled out, and any underlying bladder dysfunction should be aggressively managed in patients with worsening hydronephrosis or recurrent UTI after UU before considering additional surgery. Ureteroureterostomy is a successful option for UD with a low risk of adverse events in patients with ectopic ureters or intravesical ureteroceles. However, it should be cautiously considered in patients with hydronephrosis of both moieties, with ectopic ureteroceles, or if a concomitant re-implant is needed. [ABSTRACT FROM AUTHOR]

Abstract

IntroductionWe propose to perform near full-length deep sequencing of HIV-1 genomes from approximately 100 newly-diagnosed cases in San Francisco in the previous year (2016), using stored specimens from the clinical genotyping service that serves the majority of HIV cases in San Francisco.MethodsStored, frozen blood plasma from approximately100 newly-diagnosed cases in San Francisco over the previous year will be analysed. Four amplification products covering most of the HIV-1 genome will be derived by RT-PCR adapted from existing protocols. These protocols have been shown to be successful for the near full length analysis of approximately 85% of specimens having viral load greater than 10 000 copies per ml, which represent the majority of newly diagnosed people in San Francisco. Briefly, four overlapping regions spanning all reading frames of the HIV genome will be amplified in a published one-step RT-PCR strategy. Pooled amplicons will be purified and sequenced on an Illumina HiSeq instrument operated by GENEWIZ.ResultsThis work is ongoing and we have generated approximately half of the near full length HIV genomes for this project. This data and techniques will be presented as part of the results. ConclusionDeep sequencing provides better resolution for characterising the duration of infection. Viral diversity increases over time on average and can be estimated from the frequency of sequence ambiguities in population sequences. However, sequence ambiguities can occur due to dual infection, which can occur early in infection, and make the virus population sequence appear older.
