2. SplitEE: Early Exit in Deep Neural Networks with Split Computing

Abstract

Deep Neural Networks (DNNs) have drawn attention because of their outstanding performance on various tasks. However, deploying full-fledged DNNs in resource-constrained devices (edge, mobile, IoT) is difficult due to their large size. To overcome the issue, various approaches are considered, like offloading part of the computation to the cloud for final inference (split computing) or performing the inference at an intermediary layer without passing through all layers (early exits). In this work, we propose combining both approaches by using early exits in split computing. In our approach, we decide up to what depth of DNNs computation to perform on the device (splitting layer) and whether a sample can exit from this layer or need to be offloaded. The decisions are based on a weighted combination of accuracy, computational, and communication costs. We develop an algorithm named SplitEE to learn an optimal policy. Since pre-trained DNNs are often deployed in new domains where the ground truths may be unavailable and samples arrive in a streaming fashion, SplitEE works in an online and unsupervised setup. We extensively perform experiments on five different datasets. SplitEE achieves a significant cost reduction ($>50\%$) with a slight drop in accuracy ($<2\%$) as compared to the case when all samples are inferred at the final layer. Abstract

In recent years, there have been a growing number of small and large molecules that could be used to treat diseases of the central nervous system (CNS). Nose-to-brain delivery can be a potential option for the direct transport of molecules from the nasal cavity to different brain areas. This review aims to provide a compilation of current approaches regarding drug delivery to the CNS via the nose, with a focus on biologics. The review also includes a discussion on the key benefits of nasal delivery as a promising alternative route for drug administration and the involved pathways or mechanisms. This article reviews how the application of various auxiliary agents, such as permeation enhancers, mucolytics, in situ gelling/mucoadhesive agents, enzyme inhibitors, and polymeric and lipid-based systems, can promote the delivery of large molecules in the CNS. The article also includes a discussion on the current state of intranasal formulation development and summarizes the biologics currently in clinical trials. It was noted that significant progress has been made in this field, and these are currently being applied to successfully transport large molecules to the CNS via the nose. However, a deep mechanistic understanding of this route, along with the intimate knowledge of various excipients and their interactions with the drug and nasal physiology, is still necessary to bring us one step closer to developing effective formulations for nasal–brain drug delivery. 15. The preparation of solid core drug delivery system (SCDDS) via supercritical processing of fatty acids

Abstract

The aim of this study is to develop a novel drug delivery system for biologics i.e. proteins and peptides using a solid core drug delivery system (SCDDS). Both, 0.5 μm and 1.0 μm silica particles were used as a model core material and were coated with different ratios of fatty acid. A proportional increase in particle size was observed with increasing ratio of fatty acid in the formulation. Coating of silica particles with smaller chain fatty acids (lauric and myristic acid) resulted in a higher increase in particle size compared to the longer chain fatty acids i.e. palmitic acid and stearic acid. Bovine serum albumin was used as a model drug. SCDDS preparation included absorption of BSA on silica (BSA-Si) followed by coating of the BSA-Si particles with fatty acid via supercritical processing. In order to determine the best system to achieve maximum absorption of BSA, isotherms were obtained in different media i.e. water, 0.15 M NaCI solution and a citrate-phosphate buffer at pH 4.0, 4.7, 5.0, and 7.0. Results showed an increase in the amount of BSA absorbed was observed with the increasing specific surface area of silica particles. Isotherms demonstrated that maximum absorption of BSA can be achieved at or close to the iso-electric point (IEP) of the protein. A SCDDS was prepared by absorbing BSA on silica at pH 5.0 in citrate/phosphate buffer and coating these particles with fatty acid in the ratio (fatty acid:silica) of 0.1:1, 0.25:1 and 0.5:1. Release studies were conducted in phosphate buffer saline at pH 7.4. The release of BSA was fastest and highest from SCDDs prepared using 0.5 μm silica with a specific surface area of 4.4 m2/g. SCDDS prepared using 1.0 μm silica with a specific surface area of 2.2 m2/g provided slowest BSA release. BSA release from the SCDDS was also dependent on the chain length of fatty acid used and its ratio in the formulation. The release of BSA from SCDDS prepared with lauric acid was fastest and highest whilst stearic acid formulations showed the slowest release amongst all systems.

Secukinumab, a selective interleukin (IL)-17A inhibitor, is approved for use in adult and paediatric psoriasis, psoriatic arthritis, ankylosing spondylitis and non-radiographic axial spondyloarthritis. The aim of this study was to report the long-term safety of secukinumab in pooled data from 28 clinical trials and a post-marketing safety surveillance in psoriasis, psoriatic arthritis and ankylosing spondylitis patients. Analyses included 12,637 secukinumab-treated patients, corresponding to 15,063, 5,985 and 3,527 patient-years of exposure in psoriasis, psoriatic arthritis and ankylosing spondylitis patients, respectively. Abstract

Aim: To determine whether combined laboratory and ultrasonography results can be used to select patients for biliary tract imaging (BTI) or intervention. Background: Despite ongoing research, selection of patients with suspected CBD stone (CBDS) for BTI or direct intervention without imaging is still a subject of debate. Methods: Patients aged≥18 with symptomatic gallstone disease (SGD) who underwent MRCP over 3 years (2014-2017) were divided into the following cohorts: Group A: Normal liver enzymes with normal CBD diameter; Group B: Normal liver enzymes with dilated CBD; Group C: Isolated rise of liver enzymes with normal CBD diameter; Group D: Isolated rise of liver enzymes with dilated CBD; Group E: Hyperbilirubinemia with normal CBD diameter; Group F: Hyperbilirubinemia with dilated CBD. Binary logistic regression models were constructed for analyses. Results: Overall, 1022 patients were included. The frequency of CBDS was 7.2% in Group A; 3.8% in Group B; 6.3% in Group C; 22% in Group D; 24.2% in Group E; 47.4% in Group F.
