6. Choc toxique staphylococcique menstruel en France : ce que vous devez savoir.

Abstract

En France, environ trente cas de choc toxique staphylococcique menstruel sont déclarés chaque année. Cette pathologie sévère peut entraîner une hospitalisation en unité de réanimation et, plus rarement, le décès si elle n'est pas traitée rapidement. Les premiers symptômes incluent fièvre, frissons, éruption cutanée et troubles digestifs. Ce syndrome survient chez des femmes durant leurs menstruations, utilisant des protections périodiques intravaginales (tampon, coupe menstruelle, etc.) et présentant une colonisation vaginale par Staphylococcus aureus producteur de la toxine du choc toxique staphylococcique (TSST-1 pour Toxic shock syndrome toxin- 1). Cette toxine superantigénique, produite au niveau vaginal, active de façon non spécifique le système immunitaire, entraînant une libération massive de cytokines pro-inflammatoires. Le diagnostic de ce syndrome repose sur une association de critères cliniques et biologiques. Le diagnostic biologique implique l'isolement d'une souche de S. aureus producteur de TSST-1, généralement sur un prélèvement vaginal. Le retrait de la protection périodique intravaginale, l'administration d'une bêtalactamine anti-staphylococcique et d'un antibiotique antitoxinique comme la clindamycine permettent de réduire la mortalité et les séquelles associées à cette maladie. Around 30 cases of menstrual staphylococcal toxic shock syndrome are reported each year in France. This is a severe illness that, if not rapidly treated, can result in hospitalization in intensive care and, and, less commonly, death. The initial symptoms include fever, chills, rash and digestive disorders. This syndrome occurs during menstruation, in women who use intravaginal sanitary protection (tampon, menstrual cup, etc.) and who have a vaginal carriage of Staphylococcus aureus producing toxic shock syndrome toxin 1 (TSST-1). This superantigenic toxin, produced in the vagina, non-specifically activates the immune system, leading to the massive release of pro-inflammatory cytokines. Diagnosis of this syndrome is based on a combination of clinical and biological criteria. Biological diagnosis typically involves the isolation of a TSST-1-producing strain of S. aureus , which is usually obtained from a vaginal swab. The removal of intravaginal protection, administration of an anti-staphylococcal beta-lactam antibiotic and an antitoxin such as clindamycin can reduce mortality and sequelae of the disease. Abstract

The objective of this study was to characterize Staphylococcus aureus isolates recovered from the nasal samples of healthy pet cats in Algiers province. A total of 138 nasal swabs were collected. Antimicrobial susceptibility was conducted using the disk-diffusion method and the VITEK-2 susceptibility system. Whole genome sequencing was performed to identify multiple-locus sequence typing, antimicrobial and virulence genes. Staphylococcus aureus isolates were detected in 23 cats. Among these, 11 were methicillin-resistant S. aureus (MRSA) (one isolate/sample). Three sequence types (ST6, ST5, and ST1) were identified in MRSA, with the predominance of ST6 (n  = 7). Seven distinct STs [ST398, ST97, ST15, ST7, ST291, ST5043, and a new ST, (ST9219)] were detected in methicillin-sensitive S. aureus. All MRSA isolates harbored the mec A gene and SCC mec -type-IVa. MRSA exhibited resistance to tetracycline [ n  = 3/ tet (L) and tet (M); n  = 1/ tet (K)], kanamycin-tobramycin [ n  = 3/ ant (4′)- Ia), amikacin-kanamycin (n  = 1/ aph (3′)- IIIa ], and erythromycin-clindamycin [ n  = 1/ erm (C)]. Seven S. aureus isolates were multidrug resistant. All the isolates were negative for lukS/lukF -PV and tst- 1 genes, while 20 isolates were IEC-positive. This study revealed a diversity of genetic lineages in S. aureus strains isolated from nasal samples of pet cats, including multidrug-resistant and toxigenic strains. Abstract

Background Necrotizing soft tissue infections (NSTIs) caused by group A Streptococcus (GAS) and occasionally by Staphylococcus aureus (SA) frequently involve the deep fascia and often lead to muscle necrosis. Methods To assess the pathogenicity of GAS and S. aureus for muscles in comparison to keratinocytes, adhesion and invasion of NSTI-GAS and NSTI-SA isolates were assessed in these cells. Bloodstream infections (BSI-SA) and noninvasive coagulase-negative staphylococci (CNS) isolates were used as controls. Results NSTI-SA and BSI-SA exhibited stronger internalization into human keratinocytes and myoblasts than NSTI-GAS or CNS. S. aureus internalization reached over 30% in human myoblasts due to a higher percentage of infected myoblasts (>11%) as compared to keratinocytes ( Conclusions Our findings suggest a factor in NSTI-SA severity is the strong invasiveness of S. aureus in muscle cells, a property not shared by NSTI-GAS isolates.

Abstract

Objectives To describe two linezolid-resistant MRSA strains carrying the cfr (B) gene detected in the French National Reference Centre for staphylococci. Methods Two linezolid-resistant MRSA strains isolated from cystic fibrosis patients in two different French hospitals in 2017 and 2019 were examined to explore the mechanisms of linezolid resistance. Antimicrobial susceptibility was tested using broth microdilution and gradient strips. The genetic determinants of linezolid resistance were assessed by a multiplex PCR targeting cfr / cfr (B), optrA and poxtA genes, by amplification and sequencing of individual 23S rRNA genes and by WGS using both Illumina and Nanopore technologies. Results The two MRSA strains were resistant to linezolid but susceptible to tedizolid, and PCR-positive for cfr / cfr (B). The WGS analysis indicated that they belonged to two different STs (ST8-MRSA-IV and ST5382-MRSA-IV) and that they both harboured the cfr (B) gene on the same 9.7 kb Tn 6218 -like chromosomal transposon, a finding only previously reported in Enterococcus sp. and Clostridioides difficile. Conclusions To the best of our knowledge, this is the first description of the presence of cfr (B) in staphylococci, more specifically in linezolid-resistant MRSA strains. This finding illustrates the risk of horizontal intergenus transfer of oxazolidinone resistance genes in Staphylococcus aureus and highlights the need to monitor such emergence in this species. A total of 1320 prospective stool samples collected on FecalSwab™ (Cary-Blair medium) were screened using BD Max™ CDIFF assay (BDM). Results were compared to the routine algorithm including immunochromatographic testing for glutamate dehydrogenase (GDH) and toxin A/B (tox) using C. DIFF QUIK CHEK COMPLETE® followed by Xpert C. difficile assay in case of GDH+/tox- results. The prevalence of toxigenic C. difficile was estimated to 5.3% (n=70/1320) using the routine algorithm but to 7.4% (n = 98/1320) using BDM. The overall agreement between BDM and the routine algorithm was 97.4%. After resolution of discrepant results between BDM and the algorithm using Xpert C. difficile assay (unless already perform) and toxigenic culture performed by the French National Reference Laboratory, the overall agreement was increased to 99.4%, demonstrating the excellent performances of the test.

Abstract

Background: Phage therapy a promising antimicrobial strategy to address antimicrobial resistance for infections caused by the major human pathogen Staphylococcus aureus. Development of therapeutic phages for human use should follow pharmaceutical standards, including selection of strictly lytic bacteriophages with high therapeutic potential and optimization of their production process. Results: Here, we describe three novel Silviavirus phages active against 82% of a large collection of strains (n = 150) representative of various methicillin-susceptible and -resistant S. aureus clones circulating worldwide. We also investigated the optimization of the efficiency and safety of phage amplification protocols. To do so, we selected a well-characterized bacterial strain in order to (i) maximize phage production yields, reaching phage titres of 1011 PFU/mL in only 4 h; and (ii) facilitate phage purity while minimizing the risk of the presence of contaminants originating from the bacterial host; i.e., secreted virulence factors or induced temperate phages. Conclusions: In sum, we propose a quality-by-design approach for the amplification of broad-spectrum anti-S. aureus phages, facilitating the subsequent steps of the manufacturing process; namely, purification and quality control.
