1. Depression and Recovery after Hip Fracture in Community-Dwelling Older Adults

Abstract

The impact of depression on functional recovery in older adults following hip fracture is unclear. We aimed to examine the association between depression and 4-month functional recovery of older inpatients with hip fracture. We conducted a longitudinal cohort study on older hip fracture patients admitted to an Orthogeriatric Unit between January 2021 and February 2022 within the multicenter “Gruppo Italiano di Ortogeriatria” network. Depression was assessed retrospectively from patient medical history. Poor functional status was a Cumulated Ambulation Score <4 after 4 months. The sample included 154 patients (72.1% females, mean age 81.9). A history of depression was reported in 25.3% of participants. Depression was independently associated with higher odds of poor functional outcome (OR = 2.94, 95%CI: 1.15 - 7.85). Depression predicts a poorer functional recovery after hip fracture. The identification and treatment of depression might promote better physical recovery in orthogeriatric patients.

Abstract

Background The impact of falls on cognitive function is unclear. We explored whether injurious falls are associated with cognitive decline in older adults, and evaluated the role of changes in psychological and physical health as mediators of such association. Methods This prospective study involved 2267 community-dwelling participants in the Swedish National study on Aging and Care in Kungsholmen (≥60 years). Data on injurious falls (ie, falls requiring medical attention) during each 3-year time interval of follow-up were obtained from national registers. Assessment of cognitive function (Mini-Mental State Examination), depressive mood (Montgomery-Åsberg Depression Rating Scale), and physical performance (walking speed) were carried out every 3 or 6 years over a 12-year follow-up. The association between falls and cognition was estimated through linear mixed-effects models, and the mediating role of changes in depressive mood and physical performance was tested using mediation analysis. Results After adjusting for potential confounders, individuals who experienced injurious falls had a greater annual decline in Mini-Mental State Examination in the subsequent time interval (β = −1.49, 95% CI: −1.84; −1.13), than those who did not. The association increased with the occurrence of ≥2 falls (β = −2.13, 95% CI: −2.70; −1.56). Worsening of walking speed and depressive mood explained around 26% and 8%, respectively, of the association between falls and cognitive decline. Conclusions Injurious falls are associated with greater cognitive decline, and this association is partly mediated by worsening of physical performance and, in a lesser extent, of depressive mood. These findings suggest that physical deficits and low mood are potential therapeutic targets for mitigating the association between falls and cognitive decline.

9. Sleep disturbances and the speed of multimorbidity development in old age : results from a longitudinal population-based study

Abstract

Background: Sleep disturbances are prevalent among older adults and are associated with various individual diseases. The aim of this study was to investigate whether sleep disturbances are associated with the speed of multimorbidity development among older adults. Methods: Data were gathered from the Swedish National study of Aging and Care in Kungsholmen (SNAC-K), an ongoing population-based study of subjects aged 60+ (N = 3363). The study included a subsample (n = 1189) without multimorbidity at baseline (< 2 chronic diseases). Baseline sleep disturbances were derived from the Comprehensive Psychiatric Rating Scale and categorized as none, mild, and moderate–severe. The number of chronic conditions throughout the 9-year follow-up was obtained from clinical examinations. Linear mixed models were used to study the association between sleep disturbances and the speed of chronic disease accumulation, adjusting for sex, age, education, physical activity, smoking, alcohol consumption, depression, pain, and psychotropic drug use. We repeated the analyses including only cardiovascular, neuropsychiatric, or musculoskeletal diseases as the outcome. Results: Moderate–severe sleep disturbances were associated with a higher speed of chronic disease accumulation (ß/year = 0.142, p = 0.008), regardless of potential confounders. Significant positive associations were also found between moderate–severe sleep disturbances and neuropsychiatric (ß/year = 0.041, p = 0.016) and musculoskeletal (ß/year = 0.038, p = 0.025) disease accumulation, but not with cardiovascular diseases. Results remained stable when participants with baseline dementia, cognitive impairment, or depression were excluded. Conclusion: The finding that sleep disturbances are associated with faster chronic disease accumulation points towards the importance of early detection and treatment of sleep disturbances as a possible strategy to reduce chronic multimorbidity among older adults.

Abstract

Le dinamiche strutturali cui sono soggette le città meridionali tendono a configurare spazi di vita svantaggiati dove la tradizionale dicotomia centro/periferia è data più che dalla distanza fisica da un centro da una, ben più profonda, distanza sociale. Si sta conducendo una sperimentazione didattica sul tema della riqualificazione delle periferie urbane di Palermo assumendo come campo di applicazione contesti locali che presentano caratteri morfologici e sociali abbastanza diversificati tali da offrire un quadro esaustivo delle diverse forme che la marginalità assume nel sistema urbano. La sperimentazione ha interessato le aree del fiume Oreto e i quartieri Zen, Borgo Nuovo e Malaspina, curando aspetti tecnici del progetto intrecciati alla sfera comunicativa dell’interazione con gli abitanti. Emerge la potenzialità offerta ai processi di rinnovo urbano dal riconoscimento che la differenziazione costituisce un valore e può agire da elemento propulsore per lo sviluppo della città.

15. Inclusion of Trehalose (TRH) into liposomes to regulate uptake of this cryoprotectant into human hepatocytes

Abstract

Purpose. Problems with the limited availability of human hepatocytes for cell transplantation may be overcome by improving the efficiency of their cryopreservation. TRH-loaded liposomes could enhance epatocyte viability by regulating the intracellular uptake of the cryoprotectant [1]. Methods. TRH-loaded liposomes were prepared by film hydration method (EPC, sodium cholate, 200mM TRH solution) [2]. Liposomal size was determined by dynamic light scattering. Intraliposomal TRH content was measured using the Megazyme spectrophotometric method. After incubation (1-5 h, 37°C, 5% CO2, 95% RH) of human hepatocytes with TRH-loaded liposomes or equivalent TRH solution, intracellular sugar content was determined. Cell viability was evaluated by trypan blue exclusion. Results. Size analysis (320.2 nm diameter) and distribution (93.40.4%) suggested formation of highly homogeneous, reproducible Small Unilamellar Vesicles (SUVs). Encapsulation efficiency was 332% w/w. After incubation of hepatocytes with loaded liposomes, intracellular TRH content was about 0.1 mg/million of cells and the cell viability (858%) was unvarying and comparable to the control. By contrast, after incubation with TRH solution, hepatocyte viability dropped to 485%. Conclusions. TRH-loaded SUVs seem a useful tool in controlling intracellular uptake of the cryoprotectant. Abstract

Background: Tacrolimus is a substrate of cytochrome P-450 (CYP) 3A enzyme and of the drug transporter ABCBl. We have investigated the effects of possible relevant CYP3A5 and ABCBl single nucleotide polymorphisms (SNPs) present in both donors and recipients on tacrolimus blood levels achieved in a population of 32 Caucasian liver transplant patients. Material/Methods: At 1, 3 and 6 months after transplantation, tacrolimus doses (mg/kg/day) and trough blood levels (C0) were determined. Polymerase chain reaction followed by restriction fragment length polymorphism analysis was used for gen-otyping CYP3A5*3 [6986A>G] as well as ABCBl at exons 21 [2677G>T] and 26 [3435C>T]. Results:87.5% of the population showed a CYP3A5*3/*3 genotype. For the ABCBl SNPs, in the case of 3435C>T the total frequency observed for the allelic variant was 50%. For the 2677G>T, the total frequency of the allelic variant was 12.5%, lower than in other Caucasian populations and without any significant linkage with 3435C>T. At 3 and 6 months after transplantation, tacrolimus dose requirements were significantly higher in patients receiving a liver with one copy of the *1 al-lele compared to those homozygous for the *3 allele (0.111±0.057 vs. 0.057±0.030 [P

Abstract

In ischaemic stroke, thrombolytic drugs speed the recanalisation of intracerebral arteries. The effects of aspirin are not known. A trial was conducted to determine whether, separately or together, streptokinase and aspirin have clinical benefits in acute ischaemic stroke similar to those in acute myocardial infarction. 622 patients with acute ischaemic stroke within 6 hours of symptom onset were randomised with a 2x2 factorial design to (i) a 1-hour intravenous infusion of 1 . 5 MU streptokinase, (ii) 300 mg/day buffered aspirin for 10 days, (iii) both active treatments, or (iv) neither. Early results raised a question whether the trial should be continued. Abstract

Small angle neutron and X-ray scattering (SANS and SAXS) are used to investigate the monomer–aggregate transition of fluorocarbon–hydrocarbon diblock copolymers in supercritical carbon dioxide. SANS data are analyzed using a polydisperse sphere core–shell model. Synchrotron SAXS data have been collected by profiling the pressure at different temperatures, and critical micellization densities have been obtained for a series of diblock solutions. Finally pressure jump experiments, combined with synchrotron SAXS, have revealed two steps in the dynamics of the formation of the aggregates.

Abstract

Industrial applications of supercritical carbon dioxide (scCO(2)) rely upon the rather selective and easily adjustable solvent ability Of CO2. CO2 near the critical point is a poor solvent for high molecular weight (HMW) hydrocarbon polymers, while it is a very good solvent for amorphous fluorinated polymers. By increasing the pressure, CO2 becomes a good solvent even for HMW hydrogenated chains. Specially engineered amphiphilic di-block copolymers, with CO2-philic and CO2-phobic portions, are expected to undergo trough a monomer-aggregate transition when the solvent density of the scCO(2) changes. Here small-angle neutron scattering (SANS) results are reported for a block copolymer dissolved in liquid and supercritical CO2. Time-resolved small-angle X-ray scattering (TR-SAXS) results are also reported.

Abstract

Small angle X-ray and neutron scattering (SAXS and SANS) are used to obtain structural information on the aggregation behavior of block-copolymers dissolved in supercritical CO2. The SANS technique is used to provide a detailed structural model for the micellar aggregates, which form below the critical micellization density (CMD), that we defined in our previous work. The SAXS technique (with a synchrotron source) is used to provide the first experimental information concerning the kinetic features of both formation and decomposition of such aggregates as soon as pressure jumps are applied to the solutions across the CMD. Abstract

Small angle neutron (SANS) and X-ray (SAXS) Scattering measurements on aggregate formation of block copolymers in water and in near-critical and supercritical CO2 are reported here. Time Resolved SAXS (TR-SAXS) has also been performed in the supercritical region. Experiments have been carried out for a series of different thermodynamic conditions, changing the solvent density by profiling the pressure at constant temperature. A sharp transition between monomers dissolved as random coils and micelles characterized by a solvo-philic shell and a solvo-phobic core occurs when the solvent density reaches the critical micellization value, This is easily shown in the case of scCO(2). Abstract

Ultra small angle neutron scattering instruments have recently covered the gap between the size resolution available with conventional intermediate angle neutron scattering and small angle neutron scattering instruments on one side and optical microscopy on the other side. Rocks showing fractal behavior in over two decades of momentum transfer and seven orders of magnitude of intensity are examined and fractal parameters are extracted from the combined USANS and SANS curves. © 2000 American Institute of Physics. Abstract

In this paper we report a SANS investigation of micelle formation by fluorocarbon-hydrocarbon block copolymers in supercritical CO[sub 2](scCO[sub 2]) at 313K. A sharp unimer-micelle transition is obtained due to the tuning of the solvating ability of scCO[sub 2] by profiling pressure. At high pressure the copolymer is in a monomeric state with a random coil structure. By lowering the pressure aggregates are formed with the hydrocarbon segments forming the core and the fluorocarbon segments forming the corona of spherical aggregates. This aggregate-unimer transition is driven by the gradual penetration of CO[sub 2] molecules toward the core of the aggregate and is critically related to the density of the solvent, thus suggesting the definition of a critical micellization density (CMD). © 2000 American Institute of Physics. 50. Experimental evaluation of the efficacy of sanitation procedures in operating rooms.

Abstract

BACKGROUND: There remains much debate on how to define an adequate sanitation protocol in hospital environments. METHODS: The efficacy of a sanitation protocol in the operating room (OR) of a modern hospital was evaluated by measuring bacterial load on different types of finishing materials of all internal surfaces (ie, walls, floors, and furnishings). Samples were obtained before cleaning and over the subsequent 24 hours. A total of 2124 microbiological samples were collected using RODAC plates and sterile swabs. RESULTS: The data demonstrate a very significant postsanitation reduction of bacterial load on floors and furnishings; however, no significant data on walls were obtained, because of the low levels of initial contamination (1.50 to 5.98 cfu/100 cm2). The increase in postsanitation bacterial load over time was greater on smooth materials than on porous materials, on which a further reduction in contamination was seen. The study outcomes were confirmed by simulation experiments in which different materials were contaminated with a predetermined bacterial load and then subjected to the sanitation protocol. These simulation experiments were carried out both in vitro and in an eddy-flux testing room that simulated a full-scale OR similar (in terms of architectonic systems) to a real setting. CONCLUSION: Our data demonstrate that the spatial (vertical/horizontal) disposition of materials affects the initial contamination level, which is always much lower on vertical surfaces than on horizontal ones.
