Abstract

1 september 2019, Item does not contain fulltext, The International Alliance for Biological Standardization organized the second workshop on human challenge trials (HCT) in Rockville, MD, in September 2017. The objective of this meeting was to examine the use of HCT, in response to the continuing human suffering caused by infectious diseases, preventable by the development of new and improved vaccines. For this, the approach of HCT could be valuable, as HCT can provide key safety, tolerability, immunogenicity, and efficacy data, and can be used to study host-pathogen biology. HCT can generate these data with speed, efficiency and minimal expense, albeit not with the same level of robustness as clinical trials. Incorporated wisely into a clinical development plan, HCT can support optimization or down-selection of new vaccine candidates, assuring that only the worthiest candidates progress to field testing. HCT may also provide pivotal efficacy data in support of licensure, particularly when field efficacy studies are not feasible. Many aspects of HCT were discussed by the participants, including new and existing models, standardization and ethics. A consensus was achieved that HCT, if ethically justified and performed with careful attention to safety and informed consent, should be pursued to promote and accelerate vaccine development.

Abstract

We studied cardiac complications in young persons with severe acute respiratory syndrome coronavirus 2. In a prospective cohort of 127 Military Health System beneficiaries 0–22 years old, 3.1% had cardiac abnormalities, all resolved within 6 months. Our findings support guidelines against routine cardiac screening in mild COVID-19 cases without cardiac symptoms.

Abstract

Shigellosis is a leading cause of diarrhea worldwide prompting vaccine development The Shigella flexneri lnvaplex 50 is a macromolecular complex containing lpaB, lpaC, and LPS, formulated from an aqueous extract of virulent Shigella delivered via nasal administration. Preclinical vaccine testing demonstrated safety, immunogenicity and efficacy. An open-label dose-escalating phase 1 study evaluated a 3-dose (2-week intervals) regimen via nasal pipette delivery. Thirty-two subjects were enrolled into one of four vaccine dose groups ( 10, 50, 240, or 480 micrograms). The vaccine was well tolerated with minor short-lived nasal symptoms without evidence of dose effect. Antibody-secreting cell (ASC) responses were elicited at doses equal 50 micrograms with the highest lgG ASC, Invaplex 50 (100%) and S.flexneri 2a LPS (71%), as well as. serologic responses (43%) occurring with the 240 micrograms dose. FecallgA responses, Invaplex 50 (38.5%) and LPS (30.8%) were observed at doses =240 micrograms. The lnvaplex 50 nasal vaccine was safe with encouraging mucosal immune responses. Abstract

Medical preparation of travelers to overseas locations is an important part of military medical care. We reviewed pre-travel records of patients attending the travel clinic at the National Naval Medical Center (NNMC) and used a post-travel questionnaire to determine the most frequent medical problems associated with international travel. Among 1,416 individuals who received pre-travel care at NNMC, there were 760 (54%) males and 656 (46%) females, the median patient age was 48 years, the most common reason for travel was pleasure, and the median duration of travel was 21 days. The most common destinations were Asia (27%), Africa (15%), Europe (13%), Central America/Caribbean (12%), and South America (11%). The median number of immunizations prescribed was three. Malaria chemoprophylaxis was prescribed to 45%. The average cost of vaccines and medications to medically prepare a traveler was $67. Among 271 (82%) who returned the post-travel questionnaire, the most common illnesses reported were diarrhea (23%) and upper respiratory infections (19%); medical treatment was sought by 9%. Properly informed, military physicians can provide a valuable service at a reasonable cost to reduce the risk of travel-acquired medical problems and illnesses.

42. Mothers' and fathers' views of the interdependence of their relationships with their infant: a systems perspective on early family relationships.

Abstract

This study examined the interrelatedness of mother-infant and father-infant relationships as they develop over the first 4 months postpartum as well as the dynamics used by the couple to balance these relationships. First-time mother-father couples (n = 18) were interviewed separately at 1, 6, and 16 weeks postpartum using the Parent-Infant Relationship Interview. The data were analyzed using in-depth qualitative strategies. The parents' core themes of their early family relationships ranged from an undifferentiated unit at 1 week, to being a highly disorganized unit at 6 weeks, to a more integrated unit at 16 weeks. These results suggest that one should be thinking of early family relationships and parenting in terms of 'messy processes' out of which new ways of being together are created. This disorganization plays a fundamental role in the establishment of early family relationships and warrants further empirical and clinical attention. [ABSTRACT FROM AUTHOR]

Abstract

Pregnancy planning is becoming an important issue in preconception health promotion. Unfortunately, its definition and measurement remain imprecise. This article will develop an operational definition and identify essential components of the concept of pregnancy planning. Six steps, from Walker and Avant's concept analysis procedure were used. Data were collected through a literature review, interviews with key informants, and three focus groups. The concept analysis yielded a conceptual framework comprising three essential components: attitude, timing, and sexual behavior. Pregnancy planning is defined as the adoption of an attitude centered on conception, including sexual behaviors (proceptive or contraceptive) and timing. Moreover, pregnancy planning is not a clear-cut phenomenon with questions that can be answered simply with 'yes' or 'no'. Rather, it is a dynamic process that evolves according to contextual factors. From these results, an instrument evaluating the intensity of pregnancy planning can be developed for epidemiological research and promotional purposes. [ABSTRACT FROM AUTHOR]

49. The Health Insurance Portability and Accountability Act: security and privacy requirements.

Abstract

The security and privacy requirements of the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and their implications for pharmacy are discussed. HIPAA was enacted to improve the portability of health care insurance for persons leaving jobs. A section of the act encourages the use of electronic communications for health care claims adjudication, mandates the use of new standard code sets and transaction sets, and establishes the need for regulations to protect the security and privacy of individually identifiable health care information. Creating these regulations became the task of the Department of Health and Human Services. Regulations on security have been published for comment. Regulations on privacy and the definition of standard transaction sets and code sets are complete. National identifiers for patients, providers, and payers have not yet been established. The HIPAA regulations on security and privacy will require that pharmacies adopt policies and procedures that limit access to health care information. Existing pharmacy information systems may require upgrading or replacement. Costs of implementation nationwide are estimated to exceed $8 billion. The health care community has two years from the finalization of each regulation to comply with that regulation. The security and privacy requirements of HIPAA will require pharmacies to review their practices regarding the storage, use, and disclosure of protected health care information.
