Abstract

The National Archives of the United Kingdom has a leadership role for archives in England, representing a very broad range of memory institutions, most with limited digital capacity. This paper outlines the multi-strand approach underway to build capacity across this network of archives. The paper focuses on the aims of different intervention approaches and the involvement of the archives community and its stakeholders in different elements of delivery. The paper finishes with preliminary analysis of impact and known risks of this approach.

7. Studies relating to diabetic pregnancy : effect of hyperglycaemia upon carbohydrate metabolism and insulin on growth in the rat embryo

Abstract

Congenital abnormalities are up to four times more frequent in diabetic pregnancy than in the general population. Human and animal studies suggest several factors which may be involved in diabetic embryopathy, one of which is maternal hyperglycaemia. Although most clinical studies have reported on insulin treated diabetic pregnancy, (associated with higher serum insulin levels on average), few suggest that lack of insulin as encountered in poorly controlled or undiagnosed metabolically decompensated diabetic pregnancy may contribute to diabetic embryopathy. The first part of this thesis aims to establish whether rat embryos can metabolise excess glucose, (as encountered in maternal hyperglycaemia). Findings have shown that they are unable to do this and it is postulated that the unmetabolized glucose is free to exert any teratological effects. Enzyme kinetic studies show that glucokinase, (active at high glucose concentrations) is inactive during organogenesis. The role of insulin in rat embryonic growth and development was also assessed by culture of rat embryos in insulin deficient rat serum. This showed that embryonic growth and development was abnormal such serum. Supplementation with insulin, restored the ability of the insulin deficient serum to support normal growth and development. Culture of rat embryos in guinea pig serum known to contain a biologically inactive insulin for rat systems also resulted in abnormal rat embryonic growth and development which was largely restored by the supplementation of such serum with a biologically active insulin. Insulin was not detected by sensitive immunocytochemical methods before 13 days gestation. Thus insulin is required for normal embryonic development, but the earliest appearance of rat embryonic insulin was not until the much later stages of organogenesis. This would suggest that the embryo is dependent upon maternal insulin for normal development during most of organogenesis.

Abstract

The National Weather Service sponsored a workshop on aviation weather on 10-12 December 1991, in Kansas City, Missouri. The theme of the workshop was the improvement of service to the aviation community through the application of technology and advanced forecast techniques. The 150-plus people who attended the workshop included a cross section of operational forecasters, pilots, research meteorologists, and representatives of the aviation industry. The workshop included sessions on user requirements, operational procedures, and the impacts of new technology on the forecast products. There were also four 'hands-on' laboratory sessions where participants produced various types of aviation weather products. The interaction between the user community and working-level forecasters made the workshop a unique event.

Abstract

Congenital anomalies occur up to four times more frequently in diabetic pregnancy than in the nondiabetic population. Although past work has shown that maternal hyperglycemia and hyperketonemia may increase embryonic abnormalities, recent experimental evidence suggests that low insulin levels may also contribute to diabetic embryopathy. This study investigated the effects of guinea pig serum (whose insulin is inactive in rat systems) on rat embryonic growth and development in culture. Supplementation of guinea pig serum with pork insulin at low (1 ng/ml) and high (5 ng/ml) physiological concentrations and insulinlike growth factors (IGF) I and II were also studied. Culture of rat embryos from the early headfold stage in guinea pig serum resulted in poor embryonic growth and development with a 92% rate of anomalies. Supplementation of guinea pig serum with zinc-binding pork insulin significantly improved rat embryonic growth and development (46% anomaly rate) especially between the first 5 and 21 h of the period of organogenesis. This evidence supports our most recent findings that low insulin levels, as encountered in untreated diabetic pregnancy, may contribute to the increased risk of congenital abnormality. Insulin at low physiological concentrations improved growth, whereas higher physiological concentrations were required to increase growth and development. IGF-I or IGF-II supplementation improved rat embryonic growth and development but failed to match that of the controls, indicating that other growth factors including insulin may also be required. Patient satisfaction at emergency departments can be improved by reductions in waiting time. Traditional methods require registration and triage before seeing the doctor with senior emergency physicians mainly engaged in treating serious cases. We examine a radical change in workflow pattern on waiting time by placing a senior emergency physician with the triage nurse and examining the impact of treating simple cases upfront with discharge on the waiting times for stretcher cases.
