Abstract

En réadaptation visuelle, l’évaluation du champ visuel au sol (ECVS) permet de mesurer la taille des scotomes en lien avec la locomotion, et de restituer au patient l’étendue de son champ visuel (CV). Les informations obtenues par l’ECVS n’ont jamais été comparées avec le test de Goldmann. L’objectif de cette étude est d’évaluer la reproductibilité de la mesure du CV dans différentes conditions (test Goldmann, ECVS manuelle et automatisée). Dix patients (51,4±14,0 ans, 4 hommes et 6 femmes) présentant un CV tubulaire, inférieur ou égal à 25° centraux, ont réalisé trois versions d’une évaluation du CV binoculaire : le test de Goldmann, l’ECVS manuelle et l’ECVS automatisée. Les deux versions de l’ECVS ont été réalisées à 1m au sol, à 5m au sol et à 5m verticalement, face au patient. Le critère de jugement principal était la surface totale perçue pour chaque test. La reproductibilité de la mesure était moyenne ou bonne pour la condition à 1m au sol (ICC=0,685 à 0,866). À l’inverse, elle était très faible entre les conditions à 5m, excepté pour les conditions ECVS automatisée et manuelle à 5m au sol (ICC=0,888). Cette étude montre une bonne reproductibilité des mesures de l’ECVS à 1m, et une reproductibilité moins bonne dans les conditions à 5m, qui peuvent s’expliquer par les difficultés dans les conditions de passation de l’ECVS. Parmi les tests, l’ECVS automatisée semble être plus fiable que l’ECVS manuelle, et reçoit la préférence des patients.

Abstract

Introduction: Among patients with indolent form of B-cell chronic lymphocytic leukemia, some of them will progress into more advanced stages. To better define this subpopulation of patients, we attempted to define some parameters capable of predicting a pejorative clinical outcome. Materials and methods: Eighty-eight previously untreated patients with B-cell chronic lymphocytic leukemia in Binet stage A were analysed to study the prognostic value of simple serological variables: soluble CD23 (sCD23), b2 microglobulin (b2m), lactate-dehydrogenase activities and albumin level. Results were compared to other conventional clinical and biological parameters by univariate and multivariate statistical analysis. Results: Our data show that: (1) among those studied, sCD23 450 u/ml was the only serological significant parameter clearly correlated with disease progression and (2) stage A’’ patients (hemoglobin level between 100 and 120 g/l and/or lymphocytosis 430.10 9 /l), axillary lymph nodes and hypogammaglobulinemia were found to be other variables associated with a pejorative outcome. These four variables enabled the establishment of a scoring system, capable of predicting disease progression since 66% of the patients with a score 42 are going to evolve into advanced stages vs 12% with a score 52. Furthermore, the time to progression is shortened when the score is increasing. Conclusion: Our findings show the prognostic relevance of a scoring system including sCD23 level. Abstract

BACKGROUND: The randomized trials that evaluate the timing and intensity of initial chemotherapy for chronic lymphocytic leukemia (CLL) have, in general, been too small to provide separately reliable results. We compared the effects on survival of the following: a) immediate versus deferred chemotherapy for early-stage CLL and b) combination chemotherapy (e.g., cyclophosphamide and vincristine plus prednisone/prednisolone [COP] or COP plus doxorubicin [CHOP]) versus single-agent chlorambucil as first-line treatment for more advanced disease. METHODS: All relevant randomized trials, whether published or not, were sought for a collaborative meta-analysis involving centralized review of the data for each patient. RESULTS: There were 2048 patients with early disease in six trials of immediate versus deferred chemotherapy (chlorambucil or chlorambucil plus prednisone/prednisolone). The 10-year survival was slightly worse (but not statistically significantly so) with immediate chemotherapy (44% versus 47% survival; difference = -3%; 95% confidence interval [CI] = -10% to 4%). There were another 2022 patients in 10 trials of combination chemotherapy versus chlorambucil, with or without prednisone/prednisolone. The 5-year survival was 48% in both cases (difference = 0%; 95% CI = -6% to 5%). A subgroup of six of these 10 trials involved an anthracycline-containing regimen but again overall survival appeared no better than with chlorambucil (anthracycline-based regimen: 325 deaths among 627 patients; chlorambucil: 306 deaths among 636 patients; death rate ratio = 1.07; 95% CI = 0.91-1.25; not statistically significant). CONCLUSIONS: In terms of survival, these trials support a conservative treatment strategy for CLL, i.e., no chemotherapy for most patients with early-stage disease, and single-agent chlorambucil as the first line of treatment for most patients with advanced disease, with no evidence of benefit from early inclusion of an anthracycline. This strategy will, however, need to be reconsidered as mature results become available from trials of other agents. Cold autoagglutinins had anti I specificity, high titers of the autoantibody (> 1:1,000) and the thermal range was relative wide. Our patient had severe haemolysis and immunosuppressive therapy with methylprednisolone and cyclophosphamide was administered. It is a question how much these immunosuppresive agents influenced the recovery, and in what extent it was a self limited disease with spontaneous recovery.

Abstract

L'effet de la canicule de l'été 2003 en France sur quatre espèces de poissons migrateurs diadromes (le saumon Atlantique - Salmo salar, l'alose - Alosa alosa, l'anguille - Anguilla anguillaet la lamproie marine - Petromyzon marinus) de la Garonne et de la Dordogne est analysé à partir des données de passage des migrateurs dans deux stations de comptage et à partir de diverses observations réalisées lors de cette épisode caniculaire. La station de comptage de la Garonne (Golfech) est située à proximité immédiate des rejets d'une centrale nucléaire alors que celle de la Dordogne est indemne de rejets thermiques. L'épisode caniculaire s'est traduit par des modifications des rythmes saisonniers de migrations, des modifications des migrations annuelles et par des mortalités variables selon les espèces considérées. La similarité des phénomènes sur la Garonne et la Dordogne laisse à penser que l'effet des rejets thermiques de la centrale nucléaire a été faible, voire inexistant.
