Telerehabilitation such as secure videoconferencing offers one solution to this disparity in rural care delivery, but incorporating the perspectives of health practitioners and patients is important when developing new sustainable care models.Patients and methods: This study investigated the experiences of practitioners and patients during a novel interprofessional model of assessment where an urban-based physical therapist used videoconferencing to virtually join a rural nurse practitioner and a rural patient with CBD. Patient surveys and semi-structured interviews of practitioners and patients were analyzed quantitatively and qualitatively.Results: Most patients were “very satisfied” (62.1%) or “satisfied” (31.6%) with the overall experience, and “very” (63.1%) or “somewhat (36.9%) confident” with the assessment. Thematic analysis of interviews revealed that this novel assessment method identified: access to care for CBD, effective interprofessional practice, enhanced clinical care for CBD, and technology considerations.Conclusion: Patient satisfaction with the telerehabilitation model of care was high. Patients and practitioners reported their experiences were impacted by access to care, interprofessional practice, enhanced care for CBD and technology. Abstract

Hydrogen Sulfide (H2S) is produced in manure storage facilities and released during manure agitation. Exposure to this gas presents health risks to workers at daily exposure of greater than 10 parts per million (ppm). Exposure levels on BC dairy farms are unknown. The aim of this study was to document peak H2S exposure levels to operators and bystanders during manure agitation on British Columbia dairy farms to determine if action levels were exceeded. Farms were evaluated at two time points during manure agitation. Both operator exposure and potential exposure sampling were undertaken. Peak hydrogen sulfide levels were measured and categorized as greater than or less than the ceiling level (10 ppm) and action level (5 ppm). Associations between H2S levels above the ceiling level and farm factors were assessed. Measures were recorded at 43 dairy farms with 91 total measures recorded. Action levels were exceeded in 30% of operator measures and 64% of potential exposure measures while 20% of operator and 53% of potential peak H2S measures exceeded 10 ppm. Manure storage facilities were evenly distributed between indoor and outdoor locations with under-barn (43%) and outdoor storage (47%) most common. Sawdust was the most prominent bedding type (65%). Tractor operated propeller was the most common type of agitation equipment (62%). Manure temperature remained a significant predictor in the multivariate model for operator peak exposure. Exposure to H2S above the action level commonly occurs on BC Dairy farms. Given that Worksafe BC requires exposure control plans for H2S levels above 5 ppm, and that on dairy farms, measures of operator and potential H2S exposures were recorded above these levels, best practices to reduce potential H2S exposure are necessary to reduce possible operator and bystander exposure. Abstract

Mobile farm machinery operators are at a high risk of injury when entering (ingress) and exiting (egress) the cabs of such machinery due to slips and falls. Safety organizations and equipment manufacturers have delivered a consistent message: operators are to egress machines facing in, toward the access path, and maintain three points of contact at all times. This study used a laboratory-based model of a mid-sized agricultural tractor to determine adherence to best practices for safety and the effect of acute wholebody vibration exposure on compliance. The majority of 19 experienced operators (16 male, 3 female) performed machinery egress facing out from the cab because descending while facing in toward the machine took 2.5 s longer. Maintaining at least three points of contact during egress was observed for only approximately 30% of egress duration, but was as high as approximately 41% for participants who self-selected the facing-in orientation. Exposure to 1 h of whole-body vibration did not change points of contact behavior nor trial duration when performing egress while facing out. Overall, the model cab used in this study had safety features similar to a real-world machine, indicating that there may be opportunities in access path or cab door design to promote increased points of contact use. Future work is needed to accurately assess three-dimensional movement patterns and external forces for disease and injury risk models. [ABSTRACT FROM AUTHOR]

16. Predicting Directly Measured Trunk and Upper Arm Postures in Paper Mill Work From Administrative Data, Workers' Ratings and Posture Observations

Abstract

Objectives: A cost-efficient approach for assessing working postures could be to build statistical models for predicting results of direct measurements from cheaper data, and apply these models to samples in which only the latter data are available. The present study aimed to build and assess the performance of statistical models predicting inclinometer-assessed trunk and arm posture among paper mill workers. Separate models were built using administrative data, workers' ratings of their exposure, and observations of the work from video recordings as predictors.Methods: Trunk and upper arm postures were measured using inclinometry on 28 paper mill workers during three work shifts each. Simultaneously, the workers were video filmed, and their postures were assessed by observation of the videos afterwards. Workers' ratings of exposure, and administrative data on staff and production during the shifts were also collected. Linear mixed models were fitted for predicting inclinometer-assessed exposure variables (median trunk and upper arm angle, proportion of time with neutral trunk and upper arm posture, and frequency of periods in neutral trunk and upper arm inclination) from administrative data, workers' ratings, and observations, respectively. Performance was evaluated in terms of Akaike information criterion, proportion of variance explained (R2), and standard error (SE) of the model estimate. For models performing well, validity was assessed by bootstrap resampling.Results: Models based on administrative data performed poorly (R2 = 15%) and would not be useful for assessing posture in this population. Models using workers' ratings of exposure performed slightly better (8% = R2 = 27% for trunk posture; 14% = R2 = 36% for arm posture). The best model was obtained when using observational data for predicting frequency of periods with neutral arm inclination. 17. Observer variability in posture assessment from video recordings: The effect of partly visible periods

Abstract

© 2016 Elsevier Ltd Observers rank partly visible postures on video frames differently than fully visible postures, but it's not clear if this is due to differences in observer perception. This study investigated the effect of posture visibility on between-observer variability in assessments of trunk and arm posture. Trained observers assessed trunk and arm postures from video recordings of 84 pulp mill shifts using a work sampling approach; postures were also categorized as ‘fully’ or ‘partly’ visible. Between-worker, between-day, and between-observer variance components and corresponding confidence intervals were calculated. Although no consistent gradient was seen for the trunk, right upper arm posture showed smaller between-observer variance when all observers rated a posture as fully visible. This suggests that, partly-visible data, especially when observers disagree as to the level of visibility, introduces more between-observer variability when compared to fully visible data. Some previously-identified differences in daily posture summaries may be related to this phenomenon.

18. Full-Shift Trunk and Upper Arm Postures and Movements among Aircraft Baggage Handlers

Objectives: The present study assessed full-shift trunk and upper arm postural exposure amplitudes, frequencies, and durations among Swedish airport baggage handlers and aimed to determine whether exposures differ between workers at the ramp (loading and unloading aircraft) and baggage sorting areas. Methods: Trunk and upper arm postures were measured using inclinometers during three full work shifts on each of 27 male baggage handlers working at a large Swedish airport. Sixteen of the baggage handlers worked on the ramp and 11 in the sorting area. Variables summarizing postures and movements were calculated, and mean values and variance components between subjects and within subject (between days) were estimated using restricted maximum likelihood algorithms in a one-way random effect model. Results: In total, data from 79 full shifts (651h) were collected with a mean recording time of 495min per shift (range 319-632). On average, baggage handlers worked with the right and left arm elevated > 60° for 6.4% and 6.3% of the total workday, respectively. The 90th percentile trunk forward projection (FP) was 34.1°, and the 50th percentile trunk movement velocity was 8° s -1 . For most trunk (FP) and upper arm exposure variables, between-subject variability was considerable, suggesting that the flight baggage handlers wer e not a homogeneously exposed group. A notable between-days variability pointed to the contents of the job differing on different days. Peak exposures ( > 90°) were higher for ramp workers than for sorting area workers (trunk 0.6% ramp versus 0.3% sorting; right arm 1.3% ramp versus 0.7% sorting). Conclusions: Trunk and upper arm postures and movements among flight baggage handlers measured by inclinometry were similar to those found in other jobs comprising manual material handling, known to be associated with increased risks for musculoskeletal disorders. Abstract

BACKGROUND: Aromatase inhibitors are more effective than is tamoxifen in prevention of breast-cancer recurrence, but at the expense of increased musculoskeletal side-effects, such as carpal tunnel syndrome. The aim of this study was to assess risk factors and the prognostic value of musculoskeletal symptoms during treatment with the steroidal aromatase inhibitor exemestane or with tamoxifen after 2-3 years of tamoxifen. METHODS: In the Intergroup Exemestane Study, postmenopausal women treated for early invasive breast cancer who remained disease free and on treatment after 2-3 years of tamoxifen were randomised to switch to exemestane or to continue tamoxifen for the remainder of the 5-year period of endocrine treatment. The primary endpoint for this retrospective analysis was occurrence of carpal tunnel syndrome and any musculoskeletal events, analysed in the safety population, which consisted of all patients who had received any trial treatment. As well as case-report forms, questionnaires were distributed retrospectively to gain more details of cases of carpal tunnel syndrome. The relation between musculoskeletal symptoms reported by 6 months from randomisation and survival from 9 months onwards was assessed by Cox proportional hazards models. The trial is registered, number ISRCTN11883920. It has completed accrual and follow-up is continuing for enrolled participants. FINDINGS: After a median follow-up of 91·0 months (IQR 83·0-99·2), carpal tunnel syndrome had been reported for 66 (2·8%) of 2319 patients in the exemestane group compared with 13 (0·6%) of 2338 in the tamoxifen group (odds ratio [OR] 5·23, 99% CI 2·39-11·49; p

23. Partly visible periods in posture observation from video: Prevalence and effect on summary estimates of postures in the job

Abstract

© 2015 Elsevier Ltd and The Ergonomics Society. This paper investigated the extent to which observers rated clearly visible postures on video differently from partly visible postures, and whether visibility affected full-shift posture summaries. Trunk and upper arm postures were observed from 10,413 video frames representing 80 shifts of baggage handling; observers reported postures as fully or only partly visible. Postures were summarized for each shift into several standard metrics using all available data, only fully visible frames, or only partly visible frames. 78% of trunk and 70% of upper arm postural observations were inferred. When based on all data, mean and 90th percentile trunk postures were 1.8° and 5.6° lower, respectively, than when based only on fully visible situations. For the arm; differences in mean and 90th percentile were 0.7° and 8.2°. Daily posture summaries were significantly influenced by whether partly visible postures are included or not.

Abstract

Objective: This study was conducted to compare the collection of endocervical specimens by endocervical brush, curette, and a combined curette and brush technique.Methods: Women underwent colposcopy with endocervical curettage using one of 3 collection methods.Results: The endocervical brush produced equivalent amounts of tissue and endocervical cells compared to the curette alone or combined techniques. More squamous and glandular atypia and SIL/AIS were found when a brush was used, but a statistically significant difference was not noted. The brush alone produced a significantly greater percentage of samples that were insufficient for diagnosis and more specimens without stromal components. The brush with the curette as a combined technique provided no improvement in amounts of tissue, endocervical cells/clusters, or amount of stroma retrieved.Conclusion: Each technique has advantages and disadvantages in terms of what types of components are collected and what diagnosis may be determined from the sample taken.
