- Abstract
Spectral K-edge subtraction (SKES) is an imaging technique that takes advantage of the sharp rise in the mass attenuation coefficient of specific elements within an object at their K-edge to produce separate and quantifiable distributions of each element. In this paper, a high-sensitivity and wide bandwidth SKES imaging system for computed tomography applications on biological samples is presented. X-ray images are acquired using a wide and continuous energy spectrum that encompasses the absorption edges of the target materials. System characterization shows that high energy resolution (approximately 3 × 10 − 3 ) and unprecedented large energy bandwidth (around 15%) are achieved over a field-of-view of several centimeters. Imaging results obtained on contrast elements relevant for biomedical applications, namely silver, iodine, xenon, and barium, demonstrate the system sensitivity to concentrations down to 0.5 mg ml−1. The achievement of a large energy bandwidth allowed the simultaneous imaging of the K-edges of iodine, xenon, and barium and provided an accurate concentration estimation and distinction of co-localized contrast elements, leading the way for future simultaneous cardiovascular (iodine), pulmonary (xenon), and gastrointestinal/inflammatory (barium) imaging applications.

After administration, patients underwent subsequent injections according to PRN criteria in case of edema relapse, but not earlier than 4 months after the previous treatment. Patients were evaluated at baseline, within 15 days of injection, and every month thereafter. During all visits, best-corrected visual acuity (BCVA) was recorded; central retinal thickness (CRT), type of edema, presence of SND, and presence and number of HRFs were evaluated using swept-source optical coherence tomography (SS-OCT) 3D. Treatment outcome was defined as changes in BCVA, CRT, SND and HRFs at 12 (T12) and 24 (T24) months compared with baseline (T0).Results: The study enrolled 24 eyes of 18 patients. The mean duration of follow-up was 18± 6.6 months; for all eyes, T12 data were available, while follow-up reached T24 for 12 eyes. BCVA improved significantly and CRT decreased significantly during treatment; the edema was no longer detectable in 13/24 eyes at T12 and 8/12 eyes at T24. No patient presented SND at T12 and T24, and the mean number of HRFs decreased significantly during treatment. Results with CRT and HRFs correlated with BCVA at 12 and 24 months. No significant adverse events were observed.Conclusion: In patients with DME, the intravitreal dexamethasone implant was effective and safe in improving both functional and tomographic parameters. X-ray phase contrast imaging (XPCI) techniques are sensitive to refraction (differential-phase) and small-angle X-ray scattering (dark-field) signals, not measurable with conventional absorption imaging techniques. Among XPCI techniques, edge illumination (EI), grating interferometry (GI), and speckle-based imaging (SBI) make use of wavefront markers, such as absorbing masks with periodical apertures or random diffusers, to encode refraction and dark-field signals induced by the sample. The Unified Modulated Pattern Analysis (UMPA) provides an algorithmic solution to extract the transmission, refraction, and dark-field images from EI, GI, and SBI datasets where the wavefront marker is directly resolved by the employed detection system. In its original implementation, UMPA has been designed for XPCI techniques sensitive to refractions along two axes. This work presents a modified version of the algorithm to extend its applicability to all the existing XPCI techniques that use wavefront markers with sensitivity to refraction limited along one direction (UMPA-1D). The algorithm, written in C++ and Cython and parallelized with OpenMP, enables fast reconstruction times that are particularly convenient for large tomographic datasets. The validity of the UMPA-1D has been demonstrated using both simulated images and real acquisitions with an EI setup in beam-tracking mode.

- Abstract
Purpose: To test reliability and reproducibility of ESASO morphologic OCT-based classification of diabetic maculopathy (DM). Methods: This is a multi-center cross-sectional study including a coordination center (CC) and 18 participating centers (PCs). After instruction on the correct use of ESASO Classification, the validation process was carried out in two consecutive stages. In the first retrospective phase, we evaluated the concordance between PCs and CC in the staging of OCT images collected during PCs’ daily activity (608 images). In a second prospective phase, we analyzed the inter-observer agreement of staging assigned by each PCs to OCT images selected by the CC (22 images). Results: The overall concordance achieved in the retrospective phase was 89.8% (Kappa = 0.83 (95% CI: 0.78–0.87); p<0.0001). In 99.5% of cases, concordance did not differ by more than one stage. In the prospective phase, PCs reached an inter-operator agreement of 93.0% (Krippendorff's Alpha = 0.953, 95% CI: 0.929–0.977, p<0.0001). Any discrepancy among the 22 images was within one stage. Conclusion: The results achieved in this study confirm that ESASO OCT-based Classification can be considered as an easy and reproducible method to stage DM during clinical practice. A diffused use of a common and validated method to describe the progression of retinal damage in DM may offer several clinical and scientific advantages.

- Abstract
Nutritional deficiencies are common in inflammatory bowel diseases (IBD). In patients, magnesium (Mg) deficiency is associated with disease severity, while in murine models, dietary Mg supplementation contributes to restoring mucosal function. Since Mg availability modulates key bacterial functions, including growth and virulence, we investigated whether the beneficial effects of Mg supplementation during colitis might be mediated by gut microbiota. The effects of dietary Mg modulation were assessed in a murine model of dextran sodium sulfate (DSS)-induced colitis by monitoring magnesemia, weight, and fecal consistency. Gut microbiota were analyzed by 16S-rRNA based profiling on fecal samples. Mg supplementation improved microbiota richness in colitic mice, increased abundance of Bifidobacterium and reduced Enterobacteriaceae. KEEG pathway analysis predicted an increase in biosynthetic metabolism, DNA repair and translation pathways during Mg supplementation and in the presence of colitis, while low Mg conditions favored catabolic processes. Thus, dietary Mg supplementation increases bacteria involved in intestinal health and metabolic homeostasis, and reduces bacteria involved in inflammation and associated with human diseases, such as IBD. These findings suggest that Mg supplementation may be a safe and cost-effective strategy to ameliorate disease symptoms and restore a beneficial intestinal flora in IBD patients.

- Abstract
Purpose: In less than one and a half year, the COVID-19 pandemic has nearly brought to a collapse our health care and economic systems. The scientific research community has concentrated all possible efforts to understand the pathogenesis of this complex disease, and several groups have recently emphasized recommendations for nutritional support in COVID-19 patients. In this scoping review, we aim at encouraging a deeper appreciation of magnesium in clinical nutrition, in view of the vital role of magnesium and the numerous links between the pathophysiology of SARS-CoV-2 infection and magnesium-dependent functions. Methods: By searching PubMed and Google Scholar from 1990 to date, we review existing evidence from experimental and clinical studies on the role of magnesium in chronic non-communicable diseases and infectious diseases, and we focus on recent reports of alterations of magnesium homeostasis in COVID-19 patients and their association with disease outcomes. Importantly, we conduct a census on ongoing clinical trials specifically dedicated to disclosing the role of magnesium in COVID-19. Results: Despite many methodological limitations, existing data seem to corroborate an association between deranged magnesium homeostasis and COVID-19, and call for further and better studies to explore the prophylactic or therapeutic potential of magnesium supplementation. Conclusion: We propose to reconsider the relevance of magnesium, frequently overlooked in clinical practice. Therefore, magnesemia should be monitored and, in case of imbalanced magnesium homeostasis, an appropriate nutritional regimen or supplementation might contribute to protect against SARS-CoV-2 infection, reduce severity of COVID-19 symptoms and facilitate the recovery after the acute phase.

21. L'ecomuseo del grano e del pane: un percorso progettuale centrato sul design

The feast of St. Joseph, as well as an important and heartfelt religious celebration, is in Salemi above all the "feast of the loaves" ,beautiful achievements of high artistic craftsmanship, whose wisdom is handed down in the families and in the associations of women who for many months work to produce the enormous quantity of small and large forms of bread, to cover the votive constructions that welcome the ritual of dinner.Thus the idea of proposing an "Ecomuseum of wheat and bread" in the Jesuit College was born, enhancing and re-opening a space of great historical-architectural interest to the public. As part of the consolidated collaboration of the Municipality of Salemi with the Department of Architecture, professors and students worked on a strategic design project for museum set-up and communication, also open forms of participation and sharing with the various territorial actors, in a sort of 'social conversation' with the protagonists and skills in the field. A collaboration with Polimi Desis_Lab of the Milan Polytechnic concerning the contribution of the 'design of services for social innovation' was also initiated on these aspects ,Settore ICAR/13 - Disegno Industriale - Abstract
La festa di S. Giuseppe, oltre che un’importante e sentita ricorrenza religiosa, è a Salemi soprattutto la “festa dei pani”; bellissime realizzazioni di alto artigianato artistico, la cui sapienza si tramanda nelle famiglie e nelle associazioni di donne che per mesi lavorano alla produzione dell’enorme quantità delle piccole e grandi forme di pane, per rivestire le costruzioni votive che accolgono il rito della cena. Nasce quindi l’idea di proporre un “Ecomuseo del grano e del pane” nel Collegio dei Gesuiti, valorizzando e riaprendo al pubblico uno spazio di grande interesse storico-architettonico. Nell’ambito della consolidata collaborazione del Comune di Salemi con il Dipartimento di Architettura, docenti e studenti hanno lavorato ad un progetto di design strategico per l’allestimento e la comunicazione museale, aperto anche forme di partecipazione e condivisione con i diversi attori territoriali, in una sorta di ‘conversazione sociale’ con i protagonisti e le competenze in campo. Su questi aspetti è stata avviata anche una collaborazione con il Polimi Desis_Lab del Politecnico di Milano inerente l’apporto del ‘design dei servizi per l’innovazione sociale’.

- Abstract
Thanks to their high detection efficiency and low intrinsic noise, direct conversion X-ray Photon Counting Detectors (XPCDs) are particularly suitable for low dose imaging applications such as mammography. The photon counting technology can implement also two or more acquisition thresholds to perform single-shot spectral imaging with one polychromatic source. However, XPCDs with thick high Z crystal sensor and small pixel size are affected by the charge sharing effect. This issue can induce multiple counts from a single interaction reducing both spectral and spatial resolutions. In a pure counting mode, multiple counts from charge sharing can be partially or totally removed by increasing the acquisition threshold at the cost of a loss of efficiency. Thus, the performances of XPCDs can be threshold-dependent. New generation XPCDs implement solutions to cope with charge sharing issue. If the charge sharing is properly corrected, no multiple counts are recorded and the performances (detection efficiency, spectral and spatial resolutions) of the acquisition system are expected to be independent from the acquisition threshold. This work presents a study of a Pixirad-1 detection system based on a CdTe Schottky sensor and PIXIE-III readout system. This chip can be configured in three different operation modes, two of which implement solutions to remove multiple counts. Using monochromatic radiation, the performances of these different operation modes have been compared trough a measure of the integral spectra and the Signal to Noise Ratio (SNR) as function of the acquisition threshold.

- Abstract
K-edge subtraction (KES) imaging is a technique able to map a specific element such as e.g. a contrast agent within the tissues, by exploiting the sharp rise of its absorption coefficient at the K-edge energy. Whereas mainly explored at synchrotron radiation sources, the energy discrimination properties of modern x-ray photon counting detectors (XPCDs) pave the way for an implementation of single-shot KES imaging with conventional polychromatic sources. In this work we present an x-ray CT imaging system based on the innovative Pixie-III detector and discrete reconstruction. The results reported here show that a reliable automatic localization of Barium (above a certain concentration) is possible with a few dozens of tomographic projections for a volume having an axial slice of 512 [Formula: see text] 512 pixels. The final application is a routine high-fidelity 3D mapping of a specific element ready for further morphological quantification by means of x-ray CT with potential promising applications in vivo.

- Abstract
Hypomagnesemia is very commonly observed in cancer patients, most frequently in association with therapy with cetuximab (CTX), a monoclonal antibody targeting the epithelial growth factor receptor (EGFR). CTX-induced hypomagnesemia has been ascribed to renal magnesium (Mg) wasting. Here, we sought to clarify whether CTX may also influence intestinal Mg absorption and if Mg supplementation may interfere with CTX activity. We used human colon carcinoma CaCo-2 cells as an in vitro model to study the mechanisms underlying Mg transport and CTX activity. Our findings demonstrate that TRPM6 is the key channel that mediates Mg influx in intestinal cells and that EGF stimulates such influx; consequently, CTX downregulates TRPM6-mediated Mg influx by interfering with EGF signaling. Moreover, we show that Mg supplementation does not modify either the CTX IC50 or CTX-dependent inhibition of ERK1/2 phosphorylation. Our results suggest that reduced Mg absorption in the intestine may contribute to the severe hypomagnesemia that occurs in CTX-treated patients, and Mg supplementation may represent a safe and effective nutritional intervention to restore Mg status without impairing the CTX efficacy.

Item does not contain fulltext, Breast computed tomography (BCT) is an emerging application of X-ray tomography in radiological practice. A few clinical prototypes are under evaluation in hospitals and new systems are under development aiming at improving spatial and contrast resolution and reducing delivered dose. At the same time, synchrotron-radiation phase-contrast mammography has been demonstrated to offer substantial advantages when compared with conventional mammography. At Elettra, the Italian synchrotron radiation facility, a clinical program of phase-contrast BCT based on the free-space propagation approach is under development. In this paper, full-volume breast samples imaged with a beam energy of 32 keV delivering a mean glandular dose of 5 mGy are presented. The whole acquisition setup mimics a clinical study in order to evaluate its feasibility in terms of acquisition time and image quality. Acquisitions are performed using a high-resolution CdTe photon-counting detector and the projection data are processed via a phase-retrieval algorithm. Tomographic reconstructions are compared with conventional mammographic images acquired prior to surgery and with histologic examinations. Results indicate that BCT with monochromatic beam and free-space propagation phase-contrast imaging provide relevant three-dimensional insights of breast morphology at clinically acceptable doses and scan times.

- Abstract
Derangement of magnesium homeostasis underlies the pathophysiology of many diseases, including cancer. Recent advances support the view that aberrant expression of Mg2+ channels and other Mg2+ homeostatic factors may affect many hallmarks of cancer. The seminal idea of magnesium as a key regulator of cell proliferation has been enriched by novel intriguing findings that link magnesium and Mg2+ transporters to distinctive and complementary capabilities that enable tumour growth and metastatic dissemination. In this review, we examine the evidence on the involvement of members from the TRPM, CNNM and SCL41 protein families in cancer progression, and discuss their potential as therapeutic targets.

- Abstract
The last decades have witnessed a greater appreciation of the importance of magnesium for human health, but the relationship between magnesium and cancer development remains controversial. Here we review the current knowledge on the cellular and molecular mechanisms that underlie the manifold effects of magnesium during tumor progression. A complex picture emerges where the positive consequences of low magnesium availability on tumor growth seem to be counterbalanced by negative outcomes in the very early and late stages of tumorigenesis; a concurrent immune-inflammatory response appears to contribute throughout the natural history of a tumor. We also discuss the debated interaction between magnesium status and the response to therapy and the potential application of the TRPM7 magnesium channel as a prognostic marker and a therapeutic target. A deeper understanding of magnesium homeostasis through the interaction of fundamental and clinical researchers is key to develop new strategies of cancer prevention and treatment.

In vivo, low Mg and immune system activation seem to cooperate to promote endothelial dysfunction. We therefore evaluated whether exposure of human endothelial cells to low Mg in vitro determines oxidative stress features. We therefore measured intracellular reactive oxygen species (ROS) by dichlorofluorescein (DCF) fluorescence after Mg deprivation with or without treatment with H2O2 While we did not observe any alteration of DCF-detectable intracellular ROS under basal conditions, we show that, early after exposure to low Mg (2 h), endothelial cells are more sensitive to the oxidant action of H2O2 than the controls cultured in physiologic concentrations of Mg. This increase of ROS in Mg deprived cells is transient and followed by a stable reduction of DCF-fluorescence below the levels measured in the controls. We also evaluated oxidative DNA damage and observed higher 8-hydroxy-deoxyguanine levels early (2 h) after Mg deprivation in respect to the controls, both in basal conditions and after treatment with H2O2 Mg deficiency in vivo associates with the onset of an inflammatory response leading to increased circulating levels of cytokines, which trigger an oxidative response in endothelial cells. We here show that exposure to IL-1 and IL-6 significantly increased the levels of DCF-detectable ROS in cells cultured in physiologic concentrations of Mg, but not in Mg-deprived cells. We conclude that low Mg transiently leads to pro-oxidant effects. We suggest that different molecules, including pro-inflammatory cytokines, might be involved in promoting endothelial dysfunction.

Retroperitoneal schwannomas are rare tumours originating from Schwann cells of peripheral nerve sheaths. Their clinical presentation is often delayed as they grow to a large size in a non-restrictive space, such as the retroperitoneum, before any clear symptomatology is manifested. Furthermore, the symptoms may mimic different diseases and be aspecific. The preoperative diagnosis is often unclear as no pathognomonic radiological features are known. Fine needle biopsy is not diagnostic due to tumour cell pleomorphism. We report the case of a 51-year-old woman presenting with unusual symptoms such as severe constipation, bowel distension, excess flatus, postprandial fullness and abdominal pain due to left colon compression by a large retroperitoneal tumour. A complete resection of the mass combined with sparing of the surrounding tissues was carried out through a midline laparotomy. Microscopic evaluation and immunohistochemistry documented a benign retroperitoneal schwannoma. Postoperatively, complete resolution of abdominal symptoms and no major complications were observed. At 28 months' follow-up no local recurrence was found. To the best of our knowledge, this is a rare case of retroperitoneal schwannoma with definite abdominal symptoms and with sudden onset of severe constipation.

A 71-year-old man affected by left hemidiaphragmatic agenesis developed late severe constipation and occasional episodes of bowel obstruction. At left subcostal laparotomy, the stomach, transverse colon, splenic flexure, and spleen were located in the left hemithorax. Repair was performed with a 2-mm-thick expanded polytetrafluoroethylene (Gore-Tex) patch secured in place circumferentially as a new diaphragmatic dome. No early major complications and no recurrence at 34 months' follow-up were observed. To the best of our knowledge, this is the oldest treated patient with a true hemidiaphragmatic agenesis and is the eighth case reported in the literature. The use of the ePTFE soft tissue patch, thanks to its strength and pliability, affords good anatomical and functional repair.
