2. Operative and Non-Operative Hemoperitoneum in Blunt Abdominal Trauma, General Surgery Department, Sikasso Hospital

Abstract

Blunt abdominal trauma being one of the most common abdominal emergencies; they explain the patient's indication of armed expectation in the absence of any notion of hemodynamic instability. The objectives were to determine the hospital frequency, to identify the etiologies and to analyze the therapeutic modalities. The study was retrospective and prospective from January 1, 2010 to December 31, 2018 involving 45 patients treated for hemoperitoneum in the general surgery department of Sikasso hospital. All patients admitted to the department for blunt abdominal trauma with hemoperitoneum were included. We collected 45 patients whose mean age was 18.7 years (extreme 3 - 55 years). The sex ratio was 4.6 in favor of men (37 M, 8 F). We found 23 (51.12%) cases of AVP, 13 (28.89%) cases of falls from a height, 11 (7.7%) cases of aggression, hoof kick 1 (2.22%). 19 (42.22%) patients had low haemoperitoneum, 16 (35.56%) medium abundance, 10 (22.22%) high abundance. 32 (71.11%) provided non-operative treatment with a mean hospital stay of 13.7 days (range 5 to 20). 1 patient was unstable, it was activated 24 hours later. We immediately had 12 patients (26.67%) with an average hospital stay of 16.5 days (extreme 10 -24) and the postoperative course was simple in 66.67% (8 cases). We recorded 1 death (2.22%).

Abstract

In our surgical practice we have experienced abdominal trauma by stretcher. The case we are reporting to you was a 65-year-old subject who was the victim of a motor-cart type traffic accident with piercing of the abdomen. by stretcher and taken to Sikasso hospital. A laparotomy made it possible to discover mesenteric contusions, hemoperitoneum without visceral lesion. We aspirated 200ml of hemoperitoneum after extraction of the stretcher, closure of the abdominal wall. We propose to review the literature in order to discuss the therapeutic modalities.

Strangulation of the penis by a metal ring is a rare situation in urology. It requires emergency care because it can jeopardize the functional prognosis of the penis. We report the case of strangulation of the penis by a metal steel ring associated with a right inguino-scrotal hernia from which we will present the non-surgical management of this urological emergency.

Abstract

Introduction: The purpose of this study was to conduct an analysis of the diagnostic and therapeutic aspects of prostate abscess and a review of the literature. Clinical Observation: We report the case of a 24-year-old patient; admitted to our ward for acute detention associated with a febrile state at (38°C). On physical examination, the digital rectal examination could not assess the condition of the prostate due to severe pain and the rest of the examination was unremarkable. We formulated the following diagnostic hypotheses: prostate abscess, prostate cyst, acute prostatitis. Ultrasound allowed the diagnosis of a prostatic intraparenchymal cyst. The CBC reveals hyperleukocytosis and the ECBU came back positive for Escherichia coli. The diagnosis of a prostate abscess was retained after a guided echo puncture. Treatment consisted of transperineal drainage combined with antibiotic therapy suitable for ECB of pus aspirated during ultrasound-guided puncture. The evolution was favourable. Conclusion: Prostatic abscess is a rare pathology. Faced with a prostate collection in a febrile context, a puncture under ultrasound guidance could confirm the diagnosis. His treatment of choice remains endoscopic drainage and adapted antibiotic therapy.

Abstract

Cross-reinal ectopia implies that one of the two kidneys seats on the contralateral side, it is a congenital anomaly very rare. The two (2) Kidneys can be merged or not giving the superimposed kidney appearance. This anomaly is most often asymptomatic and fortuitous discovery (by imaging or reverse of an autopsy). We report observation of a 22-year-old patient carry of crossed renal ectopia with fusion discovery following of pelvian pain intermittent low intensity calmed down by paracetamol. The aim of this work was to bring the place of imaging in the therapeutic management of ectopic kidneys with review of the literature.

Abstract

Introduction:Pulmonary embolism is the sudden obliteration (total or partial)of the trunk of the pulmonary artery or one of its branches by a circulating foreign body, most often fibrino-cruoric. Objectives:To study the place of chest CT angiography in the diagnosis of pulmonary embolism Material And Method:This was a descriptive prospective study from February 1, 2020 to January 31, 2021 taking into account all patients referred during this period for clinical suspicion of pulmonary embolism to the medical clinic Marie Curie et les Etoiles. The examinations were performed with a GE 16-bar scanner in helical mode with and without injection of iodinated contrast product by an automatic injector. Results:We diagnosed 43 pulmonary embolisms out of 327 thoracic CT angiography performed by highlighting hypodense gaps inside one or more pulmonary arteries as well as certain branches of division and the sudden interruption of the opacification of certain distal arteries. The prevalence of pulmonary embolism was 13.15%. The female sex predominated with 52% (n=22) and the sex ratio was 1.16. The average age of patients in our series was 45 years with extremes (25 to 85 years). The age group of 41-50 years waspredominant. Dyspnea and chest pain were the most frequent clinical information with44.18% and 18.60% respectively. The seat of the pulmonary embolism concerned the right pulmonary arteries and the seat of the pulmonary embolism concerned the right and left pulmonary arteries in 60.46% and the right side was the most affected. Chronic Obstructive Pulmonary Disease (COPD) Pleurisy and Covid-19 were the lesions most associated with pulmonaryembolism (48.83%, 18.60% and 16.28%) Conclusion:Pulmonary embolism is a serious and life-threatening pathology. Its diagnosis of certainty is made with the thoracic CT angiography, the realization of which must not suffer from any delay. &nbsp

Abstract

Significance Despite growing evidence of the deleterious effects on ecological and human health, little is known regarding the global occurrence of pharmaceuticals in rivers. Studies assessing their occurrence are available for 75 of 196 countries, with most research conducted in North America and Western Europe. This leaves large geographical regions relatively unstudied. Here, we present the findings of a global reconnaissance of pharmaceutical pollution in rivers. The study monitored 1,052 sampling sites along 258 rivers in 104 countries of all continents, thus representing the pharmaceutical fingerprint of 471.4 million people. We show that the presence of these contaminants in surface water poses a threat to environmental and/or human health in more than a quarter of the studied locations globally.

16. Acceptabilité, performances diagnostiques et faisabilité du dépistage néonatal de la drépanocytose à l'aide de deux tests de diagnostic rapide: 'Sicklescan' et 'Hemotypesc' au Mali

Abstract

BACKGROUND AND AIMS La drépanocytose est une maladie potentiellement mortelle dans les populations africaines, en particulier chez les enfants de moins de 5 ans. Dans des contextes à ressources limitées, l'amélioration de leur prise en charge passe par un diagnostic précoce et fiable. Nous avons évalué l'acceptabilité maternelle et des professionnels de santé et les performances diagnostiques de deux tests de diagnostic rapide (TDR) pour le dépistage systématique de la drépanocytose chez le nouveau-né au Mali. METHODS Un dépistage systématique des Syndromes Drépanocytaires Majeurs (SDM) (HbSS, HbSC ou HbS/B-thalassémie) a été implémenté à l'accouchement chez les nouveau-nés dans trois maternités à Bamako et Kayes. Les deux TDR (SickleScan® et HemotypeSC®) ont été réalisés à partir de sang de cordon et comparés à la technique de référence, la chromatographie liquide à haute performance (CLHP). Les nouveau-nés diagnostiqués SDM sont pris en charge. RESULTS Parmi les 4333 femmes ayant accouché entre avril et août 2021, 84,6% ont donné leur accord pour le dépistage néonatal. Au total, 3 648 nouveau-nés ont été diagnostiqués par HPLC dont 1,64% présentaient un SDM (0,63% HbSS, 0,85% HbSC et 0,16% HbSB+thalassémie), 78,21% une hémoglobine normale (HbAA), 19,84% étaient porteurs d'hémoglobine S, 0,22% étaient homozygotes pour l'HbC et 0,08% avaient une HbC/B+thalassémie. Comparé à l'HPLC, le SickleScan et l’HemotypeSC avaient respectivement une sensibilité de 81,67% et 78,33%, une spécificité de 99,75% et 99,67%, une valeur prédictive négative de 99,69% et 99,64% et une valeur prédictive positive de 84,48% et 79,66%. CONCLUSIONS Cette étude montre que le dépistage systématique des SDM à la naissance est bien accepté par les familles et professionnels de santé. Les deux TDR ont eu des performances de diagnostic remarquables par rapport à l'HPLC pour identifier les SDM, faisant de ces TDR un outil de choix pour un dépistage néonatal abordable, rapide et facile à utiliser.

21. Obstetric emergencies in a humanitarian context at the Somine Dolo hospital in Mopti, Mali

Abstract

Background: Conflicts are a barrier to physical and financial access to health centers. Our objective was to evaluate obstetric emergencies in a humanitarian context at the Sominé DOLO hospital in Mopti.Methods: This was a descriptive and prospective longitudinal study from January to September, 2020 in the obstetrical gynecology department of the Sominé DOLO hospital in Mopti. Our objective was to evaluate obstetric emergencies in the humanitarian context.Results: The frequency of patients evacuated was 30.61%. The average age of the patients was 27 years with extremes of 15 and 49 years. Hemorrhage during pregnancy was the main cause of reference with 23.83%. The ambulance was the most used means of transport with 51%. Delay in evacuation was observed in 31.33 cases. About 73.03% of our patients had given birth by caesarean section. We recorded 64 maternal deaths, a rate of 10.67%. Fetal mortality in our study was 23.28%. Attacks by armed groups had a negative impact on the referral/evacuation system.Conclusions: The health system had experienced severe challenges due to insecurity. Armed conflicts were the cause of evacuation delays. Haemorrhage during pregnancy was the main reference cause. Maternal and perinatal mortality was high.

33. APPORT DE LA TOMODENSITOMÉTRIE DANS LE BILAN LÉSIONNEL DES TUMEURS CARCINOÏDES BRONCHIQUES.

Abstract

Purpose: Report radiographic aspects and assess the contribution of computed tomography for the diagnosis and search for extension of bronchial carcinoid tumors. Material and methods: This retrospective study included 9 patients with a bronchial carcinoid tumor during a four years period. In all patients, the exploration included standard chest radiography, computed tomography (CT) and abdominal ultrasonography. Results: This series included three females and six males, mean age 25 years (age range 20-52 years). The average time between clinical symptoms and diagnosis was 24 months. The important signs were chest pain, dry cough and dyspnea in 7 cases, hemoptysis in 4 cases. Chest radiography has objectified a rounded opacity speculated in 4 cases, opacity systematized in 3 cases and an opaque lung in 2 cases. Computed tomography (CT) revealed an endobronchial process with a endobronchial budding in 5 cases, pneumonia systematized in 4 cases, collapse in 7 cases, a localized dilatation of bronchus in 2 cases, lymph node metastases in 4 cases. Bronchoscopy has the macroscopic diagnosis in all cases. All patients have surgical treatment, the lobectomy in 4 cases, pneumonectomy in 3 cases and bilobectomy in 2 cases. Conclusion: CT is indispensable for positive diagnosis, and topographic localization of extension of bronchial carcinoid tumors. The main contribution of CT compared with fibroscopy is to demonstrate exobronchial tumor development and upstream pulmonary complications. [ABSTRACT FROM AUTHOR]

37. Effect of compost and soil properties on the availability of compost phosphate for white clover ( Trifolium repens L.)

Abstract

Wide variation in results exists in the literature on the effectiveness of composts to sustain the phosphorus (P) nutrition of crops. The aim of this work was to assess the importance of some soil and composts properties on the utilization of compost-P by white clover (Trifolium repens L.). This study was carried out with samples collected from four composts made from solid kitchen and garden wastes, and with two soil samples taken from the A horizon of a P-rich sandy acidic Dystrochrept and of a P-limited clayey calcareous Eutrochrept. Changes in the amount of inorganic P (Pi) isotopically exchangeable within 1 min (E1min) were measured during 32 weeks in incubated soil-composts or soil-KH2PO4mixtures where P sources had been added at the rate of 50 mg P kg−1 soil. Uptake of compost-P or KH2PO4-P by white clover was measured on the same amended soils during 16 weeks. In both soils, the application of composts resulted after 32 weeks of incubation in E1min values ranging between those observed in the control without P and those observed in the KH2PO4treatment, i.e., in values ranging between 4.2 and 5.9 mg P kg−1 in the sandy acidic soil and between from 1.6 to 4.3 mg P kg−1 in the clayey calcareous soil. The total coefficient of utilization of compost-P (CU-P) by white clover reached values in both soils for the four composts ranging between 6.5% and 11.6% of the added P while in the presence of KH2PO4 the CU-P reached values ranging between 14.5% in the clayey calcareous soil and 18.5% in the sandy acidic soil. Results obtained in the sandy acidic soil suggest, that white clover initially used a fraction of the rapidly exchangeable compost P, while at a latter stage plant roots enhanced the mineralisation of compost organic P and took up a fraction of the mineralized P. 38. Forms and exchangeability of inorganic phosphate in composted solid organic wastes

Abstract

Switzerland yearly produces more than 260,000 Mg of compost, two thirds of which is recycled in agriculture and horticulture. This research was undertaken to examine the forms and availability of inorganic P (Pi) in Swiss composts made from solid kitchen and garden wastes using the isotopic exchange kinetic technique, a sequential Pi extraction and magic angle spinning (MAS) solid-state 31P nuclear magnetic resonance (NMR) spectroscopy. The different approaches described in this paper demonstrate the presence of a complex mixture of Pi species in the studied composts. Isotopic exchange experiments and sequential extraction showed that these composts contained relatively large concentrations of rapidly available Pi. Significant correlations were observed between the concentration of water-soluble Pi (Cp), and the total N, C and P content of composts suggesting that organic substances partly controlled the amount of rapidly available Pi. Significant correlations were observed in alkaline composts between the amount of Pi which can not be exchanged within 3 months and the total P and Ca content. In alkaline composts solid-state MAS 31P NMR results suggested the presence of a range of slightly soluble and poorly crystallized Ca-P compounds such as apatites or octacalcium phosphates and of organic P compounds. The slowly or non-exchangeable Pi present in these composts could therefore be bound to Ca in the form of apatites or octacalcium phosphates

Abstract

One of the current strategies to prevent malaria in pregnancy is intermittent preventive treatment with sulfadoxine-pyrimethamine (IPTp-SP). However, in order for pregnant women to receive an adequate number of SP doses, they should attend a health facility on a regular basis. In addition, SP resistance may decrease IPTp-SP efficacy. New or additional interventions for preventing malaria during pregnancy are therefore warranted. Because it is known that community health workers (CHWs) can diagnose and treat malaria in children, in this study screening and treatment of malaria in pregnancy by CHWs was evaluated as an addition to the regular IPTp-SP program. CHWs used rapid diagnostic tests (RDTs) for screening and artemether-lumefantrine was given in case of a positive RDT. Overall, CHWs were able to conduct RDTs with a sensitivity of 81.5% (95% confidence interval [CI] 67.9-90.2) and high specificity of 92.1% (95% CI 89.9-93.9) compared with microscopy. After a positive RDT, 79.1% of women received artemether-lumefantrine. When treatment was not given, this was largely due to the woman being already under treatment. Almost all treated women finished the full course of artemether-lumefantrine (96.4%). In conclusion, CHWs are capable of performing RDTs with high specificity and acceptable sensitivity, the latter being dependent on the limit of detection of RDTs. Furthermore, CHWs showed excellent adherence to test results and treatment guidelines, suggesting they can be deployed for screen and treat approaches of malaria in pregnancy.

Abstract

National audience; L’accident d’injection de réactivité («Reactivity Initiated Accident» (RIA)) est un accident dit detype grave, caractérisé par une insertion de réactivité dans le cœur d’un réacteur nucléaire. Il induit uneexcursion de puissance quasi-instantanée, avec un dépôt significatif d’énergie dans les crayons de combustibles, soumettant ainsi ceux-ci à une situation de stress extrême. Dans le cadre de l’étude d’accidentsde ce type, le centre CEA de Cadarache, à travers son Laboratoire de Préparation et de Réalisation desEssais (LPRE), exploite un réacteur de recherche de type piscine destiné à reproduire sur un crayon decombustible irradié, prélevé dans une centrale en activité, une situation de stress équivalente à celle d’unaccident de type RIA. Le crayon, constitué d’un ensemble de pastilles d’uranium empilées sur une hauteurd’un mètre environ, et placé dans une gaine en alliage de zirconium de 9.5 mm de diamètre et 0.57 mmd’épaisseur, est soumis à un transitoire de puissance pouvant atteindre jusqu’à 20 GW en 10 à 100 ms.

Please see later in the article for the Editors' Summary, Background A malaria vaccine could be an important addition to current control strategies. We report the safety and vaccine efficacy (VE) of the RTS,S/AS01 vaccine during 18 mo following vaccination at 11 African sites with varying malaria transmission. Methods and Findings 6,537 infants aged 6–12 wk and 8,923 children aged 5–17 mo were randomized to receive three doses of RTS,S/AS01 or comparator vaccine. VE against clinical malaria in children during the 18 mo after vaccine dose 3 (per protocol) was 46% (95% CI 42% to 50%) (range 40% to 77%; VE, p, Editors' Summary Background Every year, more than 200 million cases of malaria occur worldwide, and more than 600,000 people, mainly children living in sub-Saharan Africa, die from this parasitic disease. Malaria parasites are transmitted to people through the bites of infected night-flying mosquitoes and cause fever that needs to be treated promptly with anti-malarial drugs to prevent anemia (a reduction in red blood cell numbers) and life-threatening organ damage. Malaria transmission can be prevented by using long-lasting insecticides sprayed on the indoor walls of homes to kill the mosquitoes that spread the malaria parasite or by sleeping under insecticide-treated nets to avoid mosquito bites and further reduce mosquito numbers. Widespread use of these preventative measures, together with the introduction of artemisinin combination therapy (an effective anti-malarial treatment), has reduced the global burden of malaria by 45% in all age groups, and by 51% among young children, since 2000. Why Was This Study Done? Unfortunately, the emergence of insecticide and drug resistance is threatening this advance in malaria control. Moreover, additional interventions—specifically, effective malaria vaccines—will be needed to eliminate malaria in the large areas of Africa where malaria transmission remains high. Currently, there is no licensed malaria vaccine, but RTS,S/AS01, the most advanced malaria vaccine candidate, is undergoing phase 3 clinical trials (the last stage of testing before licensing) in infants and children in seven African countries. The RTS,S Clinical Trials Partnership reported encouraging results on the efficacy and safety of RTS,S/AS01 during 12 months of follow-up in 2011 and 2012. Here, researchers report on the 18-month efficacy and safety of RTS,S/AS01. Vaccine efficacy (VE) is the reduction in the incidence of a disease (the number of new cases that occur in a population in a given period) among trial participants who receive the vaccine compared to the incidence among participants who do not receive the vaccine. During 18 months of follow-up, there were 0.69 episodes of clinical malaria (a high temperature and parasites in the blood) per person-year among the children who received all the planned doses of RTS,S/AS01 (the “per protocol” population) and 1.17 episodes per person-year among the control children—a VE against clinical malaria in the per-protocol population of 46%. A similar VE was seen in an intention-to-treat analysis that included all the enrolled children, regardless of whether they received all of the planned vaccine doses; intention-to-treat analyses reflect the real-life situation—in which children sometimes miss vaccine doses—better than per-protocol analyses. In intention-to-treat analyses, the VE among children against severe malaria (fever, parasites in the blood, and symptoms such as anemia) and hospitalization for malaria was 34% and 41%, respectively. Among infants, the VE against clinical malaria was 27% in both per-protocol and intention-to-treat analyses; the vaccine showed no protection against severe malaria or hospitalization. In both infants and children, VE waned with time since vaccination. Across all the study sites, RTS,S/AS01 averted an average of 829 and 449 cases of clinical malaria per 1,000 children and infants vaccinated, respectively. Finally, the serious adverse event meningitis (inflammation of the tissues lining the brain and spinal cord) occurred more frequently in trial participants given RTS,S/AS01 than in those given the control vaccine, but the incidence of other serious adverse events was similar in both groups of participants. What Do These Findings Mean? These and other findings show that, during 18 months of follow-up, vaccination of children and young infants with RTS,S/AS01 prevented many cases of clinical and severe malaria and that the impact of vaccination was highest in regions with the highest incidence of malaria. They indicate, as in the earlier analysis, that the VE against clinical and severe malaria is higher in children than in young infants and suggest that protection wanes over time. Whether or not the vaccine played a causal role in the observed cases of meningitis cannot be determined from these results, and the occurrence of meningitis will be followed closely during the remainder of the trial. Other study limitations (for example, variations in the clinical characteristics of participants from one center to another) may also affect the accuracy of these findings and their interpretation. However, by showing that even a modest VE can avert a substantial number of malaria cases, these findings suggest that vaccination with RTS,S/AS01 could have a major public health impact in sub-Saharan Africa. Resume Par une etude prospective sur une annee, les auteurs ont montre que les complications des mutilations genitales feminines constituaient 7,3 % des consultations externes en gynecologie et que la majorite de ces complications etaient decouvertes entre 15 et 24 ans (36 cas sur 49). Les principaux motifs de consultation etaient la dyspareunie et l’impossibilite des rapports sexuels (30 cas sur 49). La vulvoplastie sous anesthesie locale avec un traitement en ambulatoire a ete tres efficace (plus de 90 % de succes) et beaucoup moins onereuse que la chirurgie sous anesthesie generale.
