We aimed to determine if pediatric farm injuries were associated with longer EMS response and transport times compared to pediatric non-farm injuries in rural communities.The 2021 National EMS Information System (NEMSIS) database was used to identify rural EMS activations where injured pediatric patients who were transported to a hospital. Median transport times for farm and non-farm injuries, as well as other components of prehospital time and use of air EMS transport, were compared between injuries on farms and injuries in non-farm rural settings.The analytic sample included 22,248 rural EMS activations for pediatric injuries, of which 156 (1%) were for pediatric farm injuries. For non-farm and farm injuries, the median transport times were 20 minutes and 28 minutes, respectively. Median total prehospital time was 50 minutes compared to 62 minutes, and 9.8% of patients with non-farm injuries versus 20.5% of those with farm injuries were transported to a hospital by air EMS units. After multivariable adjustment, farm vs. non-farm injury location was associated with a 4 minute increase in EMS transport time, but no difference in initial EMS response time, EMS time on scene, or use of air EMS units.Among children sustaining an injury that resulted in rural EMS activation, farm injuries were associated with prolonged transport time compared to non-farm injuries, which may contribute to worse in-hospital outcomes described to pediatric farm injuries in prior research. Abstract A comprehensive emergency medical service (EMS) system significantly enhances a city’s capacity to prevent and mitigate disasters. Using Huangshi as a case study, this research evaluated the service radium coverage rate of the current EMS system by examining its transport capacity, population density, and prevalence rate, finding it to be only 61.49% with an inefficient spatial layout. To address this, we proposed transforming urban brownfields into EMS parks. By selecting the most suitable brownfields based on capacity and service radius, we increased the coverage rate to 90.21%. We introduced a new "consultation-referral" model, where existing EMS facilities serve as pre-diagnosis and triage centers, and the urban brownfield EMS parks function as isolation and centralized treatment centers. GIS network analysis confirmed the feasibility, showing all transit times to be under 30 min. The methodology outlined in this study—comprising "demand assessment, supply optimization, and feasibility verification"—not only strengthens the city’s EMS system but also facilitates the renewal of urban brownfields. This approach can serve as a valuable reference for enhancing EMS systems in other cities.

The rise in the global mean sea level (MSL) is a significant consequence of climate change, attributed to both natural and anthropogenic forces. This phenomenon directly affects the dynamic equilibrium of Earth's oceanic and estuarine ecosystems, particularly impacting the Amazon estuary. In this study, a numerical model was employed to investigate the long-term impacts of MSL fluctuations on key hydrodynamic parameters crucial to regional environmental dynamics. Our investigation was based on scenarios derived from Representative Concentration Pathways (RCPs) and Coupled Model Intercomparison Project Phase 5 (CMIP5) projections, incorporating MSL variations ranging from 30 to 150 cm above the current mean level. Following careful calibration and validation procedures, which utilized observational and in situ data, notably from field expeditions conducted in 2019, our simulations unveiled significant impacts on certain hydrodynamic parameters. Specifically, we observed a pronounced increase in diurnal tidal amplitude (p < 0.05) within the upstream sections of the North and South channels. Additionally, discernible alterations in water renewal rates throughout the estuary were noted, persisting for approximately 2 days during the dry season (p < 0.05). These findings provide valuable insights into the vulnerability of key parameters to hydrologic instability within the Amazonian coastal region. In conclusion, this study represents a pivotal scientific endeavor aimed at enhancing the preservation of aquatic ecosystems and advancing the environmental knowledge of the Lower Amazon River, with the goal of proactively informing measures to safeguard the current and future sustainability of these vital ecosystems. Kinetic models of solvent transport behaviors are widely used in rubber–solvent systems, and some key points are still worthy of attention. In this work, the Korsmeyer–Peppas and Peppas–Sahlin models were chosen to fit the transport behaviors of three aromatic solvents, benzene, toluene and p-xylene, in the hydrogenated nitrile rubber (HNBR)/ethylene propylene diene monomer (EPDM)-based vulcanizates. The different effects of the various selected transport times (ti) used for fitting on the results of the mathematical models were compared. Moreover, a method to obtain the n parameter for the Korsmeyer–Peppas model and the m parameter for the Peppas–Sahlin model at ti = 0 was discussed. It was found that the differences in values of ti greatly influenced the impact on the fitting results of all the parameters for the two models. In addition, the n parameter for the Korsmeyer–Peppas model along with the m parameter for the Peppas–Sahlin model, which can characterize the transport mechanism, showed differing applicability. But the n and m parameters at ti = 0 obtained by linear fitting showed similar rules with some differences in values. Previous rural vascular trauma research has focused on case series dating back two decades. The current research aims to measure clinical decline in comparison to time to care in rural vascular trauma. This single-center retrospective cohort study included adult trauma patients with vascular injury who were admitted to a level II trauma center. Multivariable logistic regression assessed the effect of clinical decline based on arrival within the golden hour. 149 patients were included. For every 1 unit increase in the shock index ratio, there was 99.9% reduction in odds that the patient would arrive to the trauma center within the golden hour. This study is the first of its kind within the last two decades to comprehensively review rural vascular trauma. Our research showed clinical decline in SIR associated with prolonged time to care and will allow us to optimize pre-hospital care and transport in regions with prolonged arrival times. Introduction: Increased out-of-hospital time is associated with worse outcomes in trauma. Sparse literature exists comparing prehospital scene and transport time management intervals between adult and pediatric trauma patients. National Emergency Medical Services guidelines recommend that trauma scene time be less than 10 minutes. The objective of this study was to examine prehospital time intervals in adult and pediatric trauma patients.Methods: We performed a retrospective cohort study of blunt and penetrating trauma patients in a five-county region in North Carolina using prehospital records. We included patients who were transported emergency traffic directly from the scene by ground ambulance to a Level I or Level II trauma center between 2013-2018. We defined pediatric patients as those less than 16 years old. Urbanicity was controlled for using the Centers for Medicare and Medicaid’s Ambulance Fee Schedule. We performed descriptive statistics and linear mixed-effects regression modeling.Results: A total of 2179 records met the study criteria, of which 2077 were used in the analysis. Mean scene time was 14.2 minutes (95% confidence interval [CI], 13.9-14.5) and 35.3% (n = 733) of encounters had a scene time of 10 minutes or less. Objective The purpose of this study is to determine the optimal time of transportation of floor-feed and scatter-feed broilers. Methods Eighty healthy Arbor Acres (AA) broilers (21-day-old, 624.4 g, male, standard error = 6.65) were selected and randomly divided into two experimental groups (floor-feed and scatter-feed), then fed for three weeks. The experiment comprised a 2×4 factorial design with 2 feed patterns (floor-feed and scatter-feed) and 4 transport periods (2, 3, 4, and 5 h), and 4 replicates of 5 broilers (54-day-old, 2243 g, standard error = 46.65) was used to compare weight loss, meat quality and behavior index of different groups. Results It appeared that drip loss, meat color and resting behavior of experimental broilers changed as length of transportation (p0.05). Compared with floor-feed group, broilers in scatter-feed group had lower pH at 24 hours (3 h) and different behavioral indicators (p0.05). Conclusion Comprehensive analysis showed that the maximum transport duration of floor-feed and scatter-feed broilers should not exceed 3 h, and scatter-feed broilers were more likely prone to fear.

IntroductionPotential nutrient losses and mycotoxin accumulation caused by abnormal fermentation during transportation from cropland to dairy farms leads to the diseases incidence and threatens the health of dairy cows, then further causes financial losses. The aim of this study was to investigate the effects of different transportation times on the nutritional composition, mycotoxins, and microbial communities in whole-plant corn silage (WPCS).MethodsThree groups were subjected to different transport times: DY, short (600 min). WPCS were collected from the same field, and nutrient composition and microbial composition before and after transportation were analyzed.Results and discussionOur results showed that the temperature of WPCS was higher in the ZY and CY groups than in the DY group (P < 0.01). There were no significant differences in dry matter (DM), crude protein (CP), neutral detergent fiber (NDF), acid detergent fiber (ADF), ether extract (EE) and starch contents after different transportation times (P > 0.05), whereas the starch and water-soluble carbohydrates (WSC) contents in the CY group was significantly decreased after transport (P < 0.05). Similarly, the concentration of vomitoxin in the DY and CY groups declined markedly (P < 0.05) and the zearalenone content in the DY group also significantly decreased after transportation (P < 0.05). Regarding the analysis of microorganisms in WPCS, UniFrac-distance matrices and Shannon indices showed differences in the ZY group (P < 0.05), but fungal diversities were not influenced by the transport time (P > 0.05). In the ZY group, the relative abundance of Lactiplantibacillus decreased significantly after transportation (P > 0.05), but the relative abundances of unidentified_Chloroplast, Pantoea, Gluconobacter, unidentified Acetobacter and Acinetobacter increased markedly (P < 0.05). In addition, the relative abundances of Acetobacter and Gluconobacter in the CY group increased after transport (P < 0.05). Among fungal communities, a total of three, nine, and ten different fungal flora were observed in the DY, ZY, and CY groups, respectively, although no difference was found in fungal diversity. In conclusion, increased temperature, loss of starch, and mycotoxin variation were found with increased transport time. This might be the result of competition between bacteria and fungi, and novel technologies will need to be utilized for further exploration of the mechanism.

The aim of this study was to discuss the differences in pre-hospital time intervals between rural and urban communities regarding emergency medical services (EMS). A systematic search was conducted through various relevant databases, together with a manual search to find relevant articles that compared rural and urban communities in terms of response time, on-scene time, and transport time. A total of 37 articles were ultimately included in this review. The sample sizes of the included studies was also remarkably variable, ranging between 137 and 239,464,121. Twenty-nine (78.4%) reported a difference in response time between rural and urban areas. Among these studies, the reported response times for patients were remarkably variable. However, most of them (number (n) = 27, 93.1%) indicate that response times are significantly longer in rural areas than in urban areas. Regarding transport time, 14 studies (37.8%) compared this outcome between rural and urban populations. All of these studies indicate the superiority of EMS in urban over rural communities. In another context, 10 studies (27%) reported on-scene time. Most of these studies (n = 8, 80%) reported that the mean on-scene time for their populations is significantly longer in rural areas than in urban areas. On the other hand, two studies (5.4%) reported that on-scene time is similar in urban and rural communities. Finally, only eight studies (21.6%) reported pre-hospital times for rural and urban populations. All studies reported a significantly shorter pre-hospital time in urban communities compared to rural communities. Objective: To analyze the response time and transport time taken by the emergency medical services (EMS), considering their urban or rural location, to attend traffic accident casualties that occurred in the different geographical areas of Extremadura (Spain) from 2012 to 2015. Methods: This was a cross-sectional study of the data recorded by the Emergency Response Coordination Center 112 (ERCC-112) from traffic accidents attended by EMS. Response time was defined as the time elapsed from the request-for-care receipt until arrival of the EMS at the accident scene, and transport time as that from leaving the scene until arrival to the referral hospital. Rural EMS were those based in locations where there is no hospital, and urban EMS those located in towns or cities with a hospital. Results: During the 4-year period studied, 5,572 traffic accidents requested assistance through the ERCC-112. From the 2,875 accidents (51.9%) in which EMS were mobilized, 55.4% occurred in urban roads and the remaining in interurban ones. A total of 113 people (mean age 48.4 ± 19.0 years, range 15–84 years) died at the accident scene or before arrival to the hospital, 88.5% of them in interurban accidents. The average response time of urban and rural EMS was 10.7 ± 7.3 and 18.0 ± 12.6 min (p < 0.001), respectively, and the average transport time was 13.2 ± 11.7 and 45.2 ± 25.0 min (p = 0.009). Response time was longer than the 30-min optimum only in the most peripheral areas of Extremadura, while transport time exceeded the optimum of 90 min in the eastern regions of two health areas (Cáceres and Don Benito-Villanueva). 19.1% of the victims attended by rural EMS were classified as having a serious prognosis or as having died, as compared with 11.2% (p = 0.048) of those attended by urban EMS. Conclusions: The geographical location of EMS in Extremadura (Spain) guarantees adequate response times in traffic accidents, both in rural and urban areas. However, recommended transport times were occasionally exceeded in the most peripheral areas, due to hospital location.

The coronavirus disease 2019 (COVID-19) pandemic has prompted many changes. Revised cardiopulmonary resuscitation (CPR) recommendations were issued including increased requirement for personal protective equipment (PPE) during CPR and isolation rooms. We hypothesized that these changes might have affected transport times and distance. Accordingly, we investigated any differences in transport time and distance and their effect on patient neurologic outcomes at hospital discharge. This retrospective study was conducted among patients who experienced cardiopulmonary arrest and were admitted to an emergency department during specific periods — pre-COVID-19 (January 1 to December 31, 2019) and COVID-19 (March 1, 2020, to February 28, 2021). The mean transport distance was 3.5 ± 2.1 km and 3.7 ± 2.3 km during the pre-COVID-19 and COVID-19 periods, respectively (p = 0.664). The mean total transport time was 30.3 ± 6.9 min and 35.6 ± 9.3 min during the pre-COVID-19 and COVID-19 periods, respectively (p < 0.001). The mean activation time was 1.5 ± 2.2 min and 2.9 ± 4.5 min during the pre-COVID-19 and COVID-19 periods, respectively (p = 0.003). The mean transport time was 9.3 ± 3.5 min and 11.5 ± 6 min during the pre-COVID-19 and COVID-19 periods, respectively (p = 0.001). Total transport time, including activation time for out-of-hospital cardiac arrest patients, increased owing to increased PPE requirements. Abstract The article is devoted to the study of the mechanism of piezoelectric scattering of charge carriers in an InGaN/AlGaN/ GaN heterostructures with one filled quantum-confinement subband. The aim of the article is to create a mathematical model that can be used to estimate the kinetic parameters of a two-dimensional electron gas (2DEG) in the structure under study. The results of the calculation of the matrix scattering potentials demonstrated the reliability and consistency on the used model of industrial LED heterostructures with InGaN/GaN quantum wells. It is shown that mechanical stresses in the InGaN/GaN layers lead to an uneven distribution of the total electric field along the active layer. It has been established that the lifetime limiting the mobility of a two-dimensional electron gas in piezoelectric scattering is of the order of 10–9 s.

The aspirating smoke detector (ASD) is one of the most critical pieces of equipment for detecting smoke in a protected area when a fire occurs. It has more advantages than a conventional smoke detector because it can be used in extreme conditions, such as cold storage facilities or hot aisle containment areas. ASD uses a fan to draw air from the protected area into the pipe network system via pipe holes. The sucked air is transported into the sensing chamber to detect smoke. If the obscuration in the sensing chamber is greater than the setpoint, the ASD will sound an alarm so that people realize there is a fire. For this reason, investigating the effect of the pipe hole network on obscuration in the ASD is critical. In this study, a Pipe Hole Network Program was developed to consider the pipe flow parameter. A numerical study based on the program and an experimental study was performed. The results showed that the numerical results had the same trend as the experimental study. The further the location of the fire source was, the lower the obscuration was. In addition, the correlation between the obscuration parameter and the fire source distance was also derived. Background: Patients presenting with a diagnosis of ruptured abdominal aortic aneurysm (RAAA) to community hospitals must be transported to tertiary care centers, where necessary resources are available. Unfortunately, guidelines for treatment of RAAA lack high-level evidence on the optimal resuscitation of RAAA patients during transport. We hypothesized that transfusion of packed red blood cells (PRBCs) during transport would not delay transport times in patients with RAAA. Methods: We performed a retrospective analysis of a prospective registry including prehospital data of patients with RAAA presenting to a single academic hospital in Western Pennsylvania between 2001 and 2019. Our primary outcomes were prehospital transport times: "transport interval" and "total interval." "Transport interval" is the duration from patient pickup at the outside hospital (OSH) to arrival at the receiving facility. "Total interval" is the duration from dispatch of the air medical transport to arrival of the patient to the receiving facility. We then compared two groups of patients, stratified by the receipt of PRBCs in transit, by reporting mean difference (95% confidence interval: CI) for continuous variables and percent difference (95% CI) for categorical variables. We performed two multivariate linear regression models to test if there was any effect of the receipt of PRBCs in transit on transport times. Results: We included 271 patients with RAAA transported by our air ambulance system who underwent an operation at the receiving facility, 99 (37%) of whom received PRBCs in transit. Mean ± standard deviation (SD) of the total intervals were 67 ± 28 and 71 ± 42 minutes, among patients who received or did not receive PRBCs in transit respectively, with no significant difference (p = 0.437). Following adjusted analysis, the receipt of PRBCs during transport was not associated with increased transport times, after accounting for age, hypotension, endovascular aneurysm repair (EVAR), and PRBC transfusion at the OSH. Air ambulance has a huge advantage over the ground transport in providing rapid medical access to remote areas, shortening transport time, and improving survival rate. However, one of the hurdles to expanding air service coverage is the limited heliports for landing. This study aims to quantitatively assess the impact of adding school playgrounds as sub-heliports to reduce air medical transport time. To achieve this goal, the existing air medical transport infrastructure and traffic crash injury data were collected and analyzed to establish the relationship between transport time and injury severities. Then, air medical transport time with and without the augmentation of school playgrounds were estimated, respectively. Additionally, air medical service coverage maps were created to show the impact of fatal injury reduction and the estimated transport times. The findings have shown promising results of using school playgrounds to reduce air medical transport time, which is an encouraging sign for converting school playgrounds to potential heliports. Our research provides the quantitative evidence to the policy makers on expanding air medical capacity with existing infrastructure to improve the emergency medical services in South Korea. Introduction: Mobilization of trauma resources has the potential to cause ripple effects throughout hospital operations. One major factor affecting efficient utilization of trauma resources is a discrepancy between the prehospital estimated time of arrival (ETA) as communicated by emergency medical services (EMS) personnel and their actual time of arrival (TOA). The current study aimed to assess the accuracy of the perceived prehospital estimated arrival time by EMS personnel in comparison to their actual arrival time at a Level II trauma center in San Bernardino County, California.Methods: This retrospective study included traumas classified as alerts or activations that were transported to Arrowhead Regional Medical Center in 2013. We obtained estimated arrival time and actual arrival time for each transport from the Surgery Department Trauma Registry. The difference between the median of ETA and actual TOA by EMS crews to the trauma center was calculated for these transports. Additional variables assessed included time of day and month during which the transport took place.Results: A total of 2,454 patients classified as traumas were identified in the Surgery Department Trauma Registry. After exclusion of trauma consults, walk-ins, handoffs between agencies, downgraded traumas, traumas missing information, and traumas transported by agencies other than American Medical Response, Ontario Fire, Rialto Fire or San Bernardino County Fire, we included a final sample size of 555 alert and activation classified traumas in the final analysis. Response time was longer than the 30-min optimum only in the most peripheral areas of Extremadura, while transport time exceeded the optimum of 90 min in the eastern regions of two health areas (Cáceres and Don Benito-Villanueva). 19.1% of the victims attended by rural EMS were classified as having a serious prognosis or as having died, as compared with 11.2% (p = 0.048) of those attended by urban EMS. Conclusions: The geographical location of EMS in Extremadura (Spain) guarantees adequate response times in traffic accidents, both in rural and urban areas. Background: China's medical system has not yet issued quality control indicators related to the transfer of critically ill children, and when transport teams receive a transfer request for these children, due to various reasons, the time to arrive at the bedside varies. The aim of this study was to investigate the effect of the time taken by the pediatric intensive care transport team to reach the bedside of children after receiving a transport request on the prognosis of these children., Methods: Clinical data of 298 critically ill children admitted to Anhui Children's Hospital through long-distance transport from March 2020 to February 2022 were retrospectively analyzed. Pediatric patients were divided into three groups according to the time taken by the transport team to reach the bedside after receipt of a transport request: the ≤60, >60 to ≤180, and >180 min groups. The 30-day mortality of children after admission (0= no, 1= yes) was used as the dependent variable for multivariate logistic regression analysis, with the odds ratio (OR) and 95% confidence interval (CI) indicating the relation between the time taken by the transport team to reach the bedside and the 30-day mortality rate after admission. P<0.05 indicated a statistically significant difference., Results: During the study period, there were 298 children for whom transports were requested, 50 (16.8%) of whom died within 30 days after admission. The aim of this study was to discuss the differences in pre-hospital time intervals between rural and urban communities regarding emergency medical services (EMS). A systematic search was conducted through various relevant databases, together with a manual search to find relevant articles that compared rural and urban communities in terms of response time, on-scene time, and transport time. A total of 37 articles were ultimately included in this review. The sample sizes of the included studies was also remarkably variable, ranging between 137 and 239,464,121. Twenty-nine (78.4%) reported a difference in response time between rural and urban areas. Among these studies, the reported response times for patients were remarkably variable. However, most of them (number (n) = 27, 93.1%) indicate that response times are significantly longer in rural areas than in urban areas. Regarding transport time, 14 studies (37.8%) compared this outcome between rural and urban populations. All of these studies indicate the superiority of EMS in urban over rural communities. In another context, 10 studies (27%) reported on-scene time. Most of these studies (n = 8, 80%) reported that the mean on-scene time for their populations is significantly longer in rural areas than in urban areas. On the other hand, two studies (5.4%) reported that on-scene time is similar in urban and rural communities. Finally, only eight studies (21.6%) reported pre-hospital times for rural and urban populations. All studies reported a significantly shorter pre-hospital time in urban communities compared to rural communities. Conclusions: Even with the recently added data, short pre-hospital time intervals are still superior in urban over rural communities.

The article is devoted to the study of the mechanism of piezoelectric scattering of charge carriers in InGaN/AlGaN/GaN heterostructures with one filled size quantization subband. A mathematical model is created that can be used to estimate the kinetic parameters of a two-dimensional electron gas (2DEG) in the structure under study. The results of calculation of the matrix scattering potentials demonstrated the reliability and consistency on the model of industrial LED heterostructures with InGaN/GaN quantum wells. It is shown that mechanical stresses in the InGaN/GaN layers lead to an uneven distribution of the total electric field along the active layer. It has been established that the relaxation time limiting the mobility of a two-dimensional electron gas under piezoelectric scattering is of the order of 10–9 s. [ABSTRACT FROM AUTHOR]

Background: This study aimed to evaluate whether out-of-hospital cardiac arrest (OHCA) patients with initial shockable rhythm without prehospital return of spontaneous circulation (ROSC) who are directly transported to Heart Centers in appropriate time will have better post-cardiac arrest four months survival and neurological outcomes at discharge. Methods: This retrospective study assessed the data of 1,588 OHCA patients with shockable rhythm and without prehospital ROSC collected from the registry database of Taoyuan City between January 2014 and June 2018. The relationships of transport time to Heart Centers with survival at discharge and with neurological outcomes were investigated for survival analysis. Results: Among the 1,588 OHCA patients with initial shockable rhythm and without prehospital ROSC, 1,222 (77.0%) and 366 (23.0%) were transported to Heart Centers and non-Heart Centers, respectively. However, the transport to Heart Centers was associated with an increased survival at discharge (adjusted odds ratio [aOR] 2.00, 95% confidence interval [CI], 1.42-2.81) and good neurological outcomes (cerebral performance category [CPC] 1 and 2) (aOR 3.14, 95% CI, 1.88-5.23), regardless of the transport time. The overall mortality reduction for Heart Centers was 39% (hazard ratio [HR] = 0.61; 95% CI 0.47-0.78), compared to that for non-Heart Centers. At 120 days of follow-up, the results showed a higher survival rate for patients who were transported to Heart Centers within a short time. The percentages of good CPC showed a better distribution for non-Heart Centers versus those for Heart Centers. Conclusions: Adult OHCA patients with initial shockable rhythm and without prehospital ROSC who were transported to Heart Centers directly had better post-cardiac arrest survival and good neurologic outcomes, regardless of the transport time. In this study, we developed a system to collect and analyze log data related to truck travel times in underground mines using Bluetooth beacons and tablet computers. When a signal from beacons installed at a major underground mine is received by a truck-mounted tablet computer, the beacon information is collected and uploaded to a cloud server. A data processing program integrates the uploaded log data files into a single file, calculating the statistical values for each section of the transport route. The developed system was applied to a limestone underground mine located in Jeongseon, Korea, to diagnose and analyze the transport routes in the study area. As a result of this analysis, it was possible to select sections in which the truck transport time was stable and sections in which it was unstable. Consequently, the transport route could be classified into four types based on the distribution and fluctuations in the truck transport time data. Moreover, it was possible to analyze the causes of the stable and unstable sections through production logs and field staff interviews. This work presents multi-objective optimization of an aspirating smoke detection system which uses the pipeline to transport air sample from the sampling points to the analysing module. On the basis of 3D computational fluid dynamics simulation it has been shown that the smoke transport will not always take place in the centre of the pipe and that one dimensional analysis is not able to provide information in which layer smoke transport will take place. Consequently, velocity from the layer at the distance of 0.95R from the pipe centreline was taken as an input for calculation of the transport time to bind the calculation on the safe side. By employing the multi-objective optimization approach, balancing of specific transport time and volume flow at the sampling points' location was achieved through pipeline configuration variation with respect to pipe diameters and the position of branches along the main pipe. Objective function was assembled from the flow rate function and transport time function using the weighted sum method. Results for five different values of weight factor have been discussed. After reaching weight factor value of 0.75, configuration reached requested sensitivity, and further increase of weight factor enabled more uniform flow across the network without deterioration of transport time. Abstract The incidence of PSE (pale, soft and exudative) pork meat is directly influenced by pre-slaughter handling of animals, compromising meat quality and resulting in economic losses for industry. Considering management, the main extrinsic stress factors are: the pre-slaughter rest time in lairage, the total fasting time and at farm and the transport time. To evaluate the effects of these factors on PSE meat incidence, pH of 1038 carcasses was measured at 45 minutes post mortem (pH45) in a pig slaughterhouse in Paraná, Brazil. Carcasses with pH45≤5.8 were classified as PSE, and those with pH45>5.8 as normal. In order to verify if the factors influence PSE meat appearance, it was divided into categories according to it duration, and according to the number of animals evaluated in each category, the PSE meat frequency was estimated, and which time intervals obtained the lower incidence of the change. The total incidence of PSE carcasses was 19.17%. The periods of rest in lairage, total fasting, fasting at farm and transport that minimized the occurrence of PSE meat were from 04h01 to seven hours, from 14h01 to 17h00, from 03h01 to five hours, and from 02h01 to three hours, respectively.

The aspirating smoke detector (ASD) is one of the most critical pieces of equipment for detecting smoke in a protected area when a fire occurs. It has more advantages than a conventional smoke detector because it can be used in extreme conditions, such as cold storage facilities or hot aisle containment areas. ASD uses a fan to draw air from the protected area into the pipe network system via pipe holes. The sucked air is transported into the sensing chamber to detect smoke. If the obscuration in the sensing chamber is greater than the setpoint, the ASD will sound an alarm so that people realize there is a fire. For this reason, investigating the effect of the pipe hole network on obscuration in the ASD is critical. In this study, a Pipe Hole Network Program was developed to consider the pipe flow parameter. A numerical study based on the program and an experimental study was performed. The results showed that the numerical results had the same trend as the experimental study. The further the location of the fire source was, the lower the obscuration was. In addition, the correlation between the obscuration parameter and the fire source distance was also derived. It could be used to predict the fire source location in the aspirating smoke detection system.

Background: In the prehospital setting, the use of ambulance lights and sirens (L&S) has been found to result in minor decreases in transport times, but has not been studied in interfacility transportation.Objective: The objective of this study was to evaluate the indications for L&S and the impact of L&S on transport times in interfacility critical care transport.Methods: We performed a retrospective analysis using administrative data from a large, urban critical care transportation organization. The indications for L&S were assessed and the transport times with and without L&S were compared using distance matching for common transport routes. Median times were compared for temporal subgroups.Results: L&S were used in 7.3% of transports and were most strongly associated with transport directly to the operating room (odds ratio 15.8; 95% confidence interval 6.32-39.50; p < 0.001). The timing of the transport was not associated with L&S use. For all transports, there was a significant decrease in the transport time using L&S, with a median of 8 min saved, corresponding to 19.5% of the overall transportation time without L&S (33 vs. 41 min; p < 0.001). The reduction in transport times was consistent across all temporal subgroups, with a greater time reduction during rush hour transports.Conclusions: The use of L&S during interfacility critical care transport was associated with a statistically significant time reduction in this urban, single-system retrospective analysis. Significant disparities in access to prompt helicopter transport exist among rural trauma populations. We evaluated the impact of an additional helicopter base on transport time and mortality in a rural adult trauma population. We performed a retrospective cohort study of adult patients with trauma transported by helicopter from scene to a level one trauma center between 2014 and 2018. A new rural helicopter base added to the trauma center's catchment area in 2016 served as the transition time for an interrupted time series analysis. Patients injured in this base's county and adjoining counties were analyzed. Baseline characteristics were compared with a Student's t -test and Pearson's chi-squared test. Cox and linear regression models evaluated the new base's effect on mortality and transport time, respectively. A total of 332 patients were analyzed: 120 (36.1%) transported before the addition of the new helicopter base and 212 (63.9%) transported after. Patients transported after the addition of the base had higher injury severity score (13.7 versus 10.1, P < 0.001) and were more likely to receive blood en route (19.3% versus 6.7%, P = 0.005). After the addition of the base, there was a decreased hazard ratio for mortality (hazard ratio 0.26, 95% confidence interval: 0.11-0.65, P = 0.004) with no significant change in transport time (−36.7 min, P = 0.071) for the area. Local helicopter transport units may confer improved survival for the injured patient. This study demonstrates the important role of helicopter transport within a regional trauma system and the impact that expanded access to rapid air transport can have on mortality. Abstract Background In combat operations, patients with traumatic injuries require expeditious evacuation to improve survival. Studies have shown that long transport times are associated with increased morbidity and mortality. Limited data exist on the influence of transport time on patient outcomes with specific injury types. The objective of this study was to determine the impact of the duration of time from the initial request for medical evacuation to arrival at a medical treatment facility on morbidity and mortality in casualties with traumatic extremity amputation and non-compressible torso injury (NCTI). Methods We completed a retrospective review of MEDEVAC patient care records for United States military personnel who sustained traumatic amputations and NCTI during Operation Enduring Freedom between January 2011 and March 2014. We grouped patients as traumatic amputation and NCTI (AMP+NCTI), traumatic amputation only (AMP), and neither AMP nor NCTI (Non-AMP/NCTI). Analysis was performed using chi-squared tests, Fisher’s exact tests, Cochran-Armitage Trend tests, Shapiro-Wilks tests, Wilcoxon and Kruskal-Wallis techniques and Cox proportional hazards regression modeling. Results We reviewed 1267 records, of which 669 had an injury severity score (ISS) of 10 or greater and were included in the analysis. In the study population, 15.5% sustained only amputation injuries (n=104, AMP only), 10.8% sustained amputation and NCTI (n=72, AMP+NCTI), and 73.7% did not sustain either an amputation or an NCTI (n=493, Non-AMP/NCTI). AMP+NCTI had the highest mortality (16.7%) with transport time greater than 60 min. While the AMP+NCTI group had decreasing survival with longer transport times, AMP and Non-AMP/NCTI did not exhibit the same trend. Conclusions A decreased transport time from the point of injury to a medical treatment facility was associated with decreased mortality in patients who suffered a combination of amputation injury and NCTI. No significant association between transport time and outcomes was found in patients who did not sustain NCTI. Priority for rapid evacuation of combat casualties should be given to those with NCTI.

The mechanism of charge carrier scattering on ionized impurity atoms in an InAs/AlSb heterostructure with two filled size quantization subbands is studied. The purpose of the article is to develop a theory that fully describes quantum-mechanical processes in the structure under study. The calculation performed within the framework of the proposed transport time algorithm showed that the developed theory describes with high accuracy the mechanism of scattering of two-dimensional carriers by donor impurity ions in an active InAs layer. It was established that the lifetime limiting the mobility of two-dimensional carriers upon scattering by impurity ions is of the order of 10–12 s. [ABSTRACT FROM AUTHOR]

Background: The impact of distance between donor and recipient hospitals on outcomes in cadaveric kidney transplantations is unknown. We investigated the association between inter-hospital distance and outcomes in cadaveric kidney transplantations in Japan. Methods: We retrospectively analyzed 363 cadaveric kidney transplantations between 2002 and 2017 in Japan. Inter-hospital distance, graft transport time, total ischemic time (TIT), and graft survival were compared between our hospital and national transplantation cohort in Japan. Estimated glomerular filtration rate (eGFR) 1 month and 1 year after transplantation was compared between cadaveric and living-donor kidney transplantations in our hospitals. Additionally, inter-hospital distances among the seven geographical regions in Japan were assessed. Results: There were 12 and 351 cadaveric kidney transplantations at our hospital and in Japan, respectively. Mean inter-hospital distance at our hospital (217 ± 121 km) was significantly longer than that of the national cohort (53 ± 80 km; P < 0.001). Mean TIT and graft survival for our hospital and national cohort were 539 ± 200 min and 91% and 491 ± 193 min and 81%, respectively. Mean eGFRs 1 year after cadaveric and living-donor transplantations at our hospitals were comparable (47 ± 16 vs. 47 ± 15 mL/min/1.73 m2). The comparison among seven regions in Japan indicated a regional difference in inter-hospital distance with an association between area (km2) and inter-hospital distance (km). Conclusions: Despite the longer inter-hospital distance at our hospital, TIT and transplant outcomes were acceptable in our cases. This study aims at analysing the latitudinal variability of both the yearly and seasonal pattern of 7Be surface activity concentrations, at addressing the impact of tropopause height (TPH) on 7Be distribution and at evaluating the time lag between TPH and 7Be at European level. With this aim, weekly 7Be and daily TPH data at 17 sampling stations during 10 yr (2001–2010) are analysed. 7Be shows a clear increasing tendency in the period and generally tends to increase with decreasing latitude. The seasonal pattern generally shows maxima during the warm period and minima during the cold one. The seasonal variogram analysis points out a good spatial correlation for TPH data while a weaker one is observed for 7Be, having TPH a larger influence on 7Be during summer. The influence of TPH on 7Be exhibits a large spatial variability, with a clear gap between south and north in the area of the polar front jet. The results identify the presence of two main groups, in particular separating between stations located in northern Europe (50 °N and higher) and stations in southern Europe (south of 50 °N). A similar behaviour for stations located in the same geographical area is also observed when looking at the day of maximum impact of TPH on 7Be concentrations. The results suggest that 7Be concentrations respond in different time ranges to changes in the TPH, observing seasonal differences in each group. These results represent the first European approach to the understanding of the TPH impact on 7Be concentrations at surface levels.
