Background: Chen's Peiyuan Tang (CSPYT) is a compound herbal formula that has shown the potential to enhance ovarian function and reduce autophagy in ovarian granulosa cells, which plays a crucial role in follicular development and maturation. The application of Chinese herbal medicine offers a promising alternative to traditional hormone replacement therapy (HRT). Methods: This study explores CSPYT's therapeutic mechanisms in treating POF, focusing on its modulation of autophagy through network pharmacology and transcriptomics-based analysis, predicting potential interactions and pathways. KGN cell line and rat ovarian granulosa cells were used for in vitro experiment. 4-Hydroperoxy cyclophosphamide(4-HC) stimulation was carried out for establishing the POF cell model. Q-PCR, Western Blot, Transmission electron microscopy to detect the results. Results: According to the drug and disease database, the common targets of Chen's Peiyuan Tang and premature ovarian failure were screened, combined with autophagy gene targets and transcriptome analysis, and finally 8 intersection targets were obtained, namely CDKN1B, MAPK3, PRKCD, CDKN1A, MAPK1, RAF1, BIRC5, CTSB. Enrichment analysis of 8 genes found that they were closely related to the animal autophagy pathway. Construct PPI network diagram. CytoScape 3.9.1 builds CSPYT Drug Target-POF Disease Target-Autophagy Gene Network Diagram. Based on the PPI network diagram and CytoScape 3.9.1 analysis results, it is estimated that MAPK1 and MAPK3 are the key targets of CSPYT in the treatment of POF. The eight final intersection targets were docked with the corresponding active pharmaceutical ingredients. The one that docked most closely with the MAPK family was naringenin. In cell experiment verification, it was confirmed that Chen's Peiyuan Tang can inhibit the MAPK signaling pathway, significantly reduce the number of autophagosomes, and reduce autophagy damage in ovarian granulosa cells. Discussion: CSPYT can inhibit the MAPK signaling pathway, prevent autophagy overexpression and restore ovarian granulosa cell function, effectively alleviating the disease pressure of POF. Context: Plant microRNAs (miRNAs) present in Yishen Tongluo formula (YSTL, a traditional Chinese herbal medicine formula) are considered as potential therapeutic drugs for reducing the sperm DNA fragmentation index (DFI). Objective: To study the effectiveness of plant miRNAs in YSTL for repairing mouse sperm DNA damage caused by benzo(a)pyrene (BaP). Methods and materials: Twenty-four male SPF ICR (CD1) mice were divided into control, BaP and YSTL groups. A BaP-induced (100 mg/kg) sperm DNA damage model was established in the BaP and YSTL groups, and the mice in the YSTL group were treated with YSTL (23.78 g/kg) for 8 weeks. Sperm DFI was determined via a sperm chromatin structure assay (SCSA). MicroRNAs in the testes of the mice were analysed via RNA-seq, and the top four plant miRNAs were screened, identified and overexpressed in GC cells. The effects of plant miRNAs on the viability and DNA integrity of GC cells exposed to benzo(a)pyrene diol epoxide (BPDE) (1 μM) were tested using CCK8 and comet assays. Results: Compared with that of the BaP group, the DFI of the YSTL group decreased (9.57% vs. 18.54%, F = 18.645, p = 0.0236). miR166-y, miR894-x, miR822-x and miR396-x were screened. The CCK8 and comet assays revealed that the DFI of the mimic group was significantly lower than that of the BPDE (IC50 = 1.006 μM) group, with the most significant difference in the miR396-x group. Context: Shenxiang Suhe pill (SXSH), a traditional Chinese medicine, is clinically effective against coronary heart disease, but the mechanism of cardiac-protective function is unclear. Objective: We investigated the cardiac-protective mechanism of SXSH via modulating gut microbiota and metabolite profiles. Materials and methods: Sprague-Dawley (SD) male rats were randomly divided into 6 groups (n = 8): Sham, Model, SXSH (Low, 0.063 g/kg; Medium, 0.126 g/kg; High, 0.252 g/kg), and Ato (atorvastatin, 20 mg/kg). Besides the Sham group, rats were modelled with acute myocardial infarction (AMI) by ligating the anterior descending branch of the left coronary artery (LAD). After 3, 7, 14 days' administration, ultrasound, H&E staining, serum enzymic assay, 16S rRNA sequencing were conducted to investigate the SXSH efficacy. Afterwards, five groups of rats: Sham, Model, Model-ABX (AMI with antibiotics-feeding), SXSH (0.126 g/kg), SXSH-ABX were administrated for 14 days to evaluate the gut microbiota-dependent SXSH efficacy, and serum untargeted metabolomics test was performed. Results: 0.126 g/kg of SXSH intervention for 14 days increased ejection fraction (EF, 78.22%), fractional shortening (FS, 109.07%), and aortic valve flow velocities (AV, 21.62%), reduced lesion area, and decreased serum LDH (8.49%) and CK-MB (10.79%). Meanwhile, SXSH upregulated the abundance of Muribaculaceae (199.71%), Allobaculum (1744.09%), and downregulated Lactobacillus (65.51%). The cardiac-protective effect of SXSH was disrupted by antibiotics administration. SXSH altered serum metabolites levels, such as downregulation of 2-n-tetrahydrothiophenecarboxylic acid (THTC, 1.73%), and lysophosphatidylcholine (lysoPC, 4.61%). The pharmacological activity and clinical efficacy of traditional Chinese medicine (TCM) are significantly influenced by the geographical origin. Therefore, the effective and reliable identification of TCM producing regions is crucial for the quality control and clinical application of TCM. Ophiopogonis Radix (Chinese name: Maidong (MD)) is a well-known TCM with various properties, such as anti-oxidation, anti-inflammation, anticancer, and cardiovascular protection. In this study, the feasibility of Raman spectroscopy combined with partial least squares discriminant analysis (PLS–DA) to distinguish Sichuan Ophiopogonis Radix (CMD) from Zhejiang Ophiopogonis Radix (ZMD) was investigated. Extensive experimental results demonstrated that the Raman peaks at 363, 465, 1345, 1458, 2893 and 2933 cm−1 corresponding to polysaccharides, homoisoflavonoids, and steroidal saponins could be used as identification chemical markers for CMD and ZMD. Additionally, the PLS–DA-based scheme proposed in this study could identify the geographical origins of CMD from ZMD with an accuracy of 92.7%. Unlike conventional morphological and microscopic identification and physicochemical analysis, the proposed method is nondestructive, involves simple sample preparation, requires a very low mass of the sample, and exhibits a high classification accuracy to discriminate between CMD and ZMD. Therefore, despite analyzing a limited number of samples, this study proposes a method by the combination of Raman spectroscopy and PLS–DA to rapidly, sensitively, and nondestructively discriminate Ophiopogonis Radix samples with different geographical origins. This study confirms that Raman spectroscopy exhibits high potential to facilitate the authentication, quality control, and supervision of herbal medicines and processed products. Osteoarthritis (OA) is a chronic degenerative articular disease that leads to physical disability and reduced quality of life. The key pathological events in OA are cartilage degradation and synovial inflammation. Conventional therapies often lead to adverse effects that some patients are unwilling to endure. Traditional Chinese Medicines (TCMs) have long been known for their efficacy in treating OA with minimal side effects. The wingless-type (Wnt) signaling pathway is believed to play a role in OA progression, but there is still a lack of comprehensive understanding on how TCM may treat OA via the Wnt signaling pathway. This study aims to fill this gap by reviewing relevant research on the association between the Wnt signaling pathway and cartilage degradation and synovial inflammation in OA. Meanwhile, we also summarized and categorized TCMs and their active components, such as alkaloids, polysaccharides, flavonoids, sesquiterpene lactones, etc., which have shown varying efficacy in treating OA through modulation of the Wnt/ β -catenin signaling pathway. This work underscores the pivotal role of the Wnt signaling pathway in OA pathogenesis and progression, suggesting that targeting this pathway holds promise as a prospective therapeutic strategy for OA management in the future. TCMs and their active components have the potential to alleviate OA by modulating the Wnt signaling cascade. Harnessing TCMs and their active components to regulate the Wnt signaling pathway presents an encouraging avenue for delivering substantial therapeutic benefits to individuals with OA. Purpose of review: This review aims to describe the association of integrating traditional Chinese medicine (TCM) herbs into conventional medicine (CM) in preventing breast cancer and improving survival rates among breast cancer patients of Taiwan. Recent findings: Of 7 relevant studies, spanning 2014–2023, 4 investigated breast cancer risk in women with menopausal symptoms and other comorbidities. All 4 reported that TCM herbal use was associated with lower risks of developing breast cancer. Three studies investigated survival in newly-diagnosed breast cancer patients receiving CM. All reported that adjunctive TCM users had lower mortality rates than CM-only patients. However, the heterogeneity of study designs, populations, and interventions may limit the generalizability and robustness of the findings. Summary: TCM herbs may promote breast cancer prevention and survival when used alongside CM. As an effective formula of traditional Chinese medicine, Yang-Gan-Jiang-Mei (YGJM) formula exhibited a unique advantage in ameliorating liver injury and hepatic steatosis of non-alcoholic steatohepatitis (NASH). Nevertheless, the related pharmacological mechanism needs to be elucidated. This study aimed to explore the molecular mechanism of YGJM formula on mitophagy mediated by PINK1/parkin signaling pathway and NOD-like receptor protein 3 (NLRP3) inflammasome in NASH. High-fat-diet rats and HepG2 cells induced by free fatty acid were used as NASH models in vivo and in vitro. Liver pathology and serum indicator embodying liver function (aspartate transferase, alanine transferase, triglyceride, and total cholesterol) were applied to evaluate the extent of hepatic damage and lipid accumulation. Besides, transmission electron microscopy, JC-1 and 2',7'-dichlorofluorescein diacetate were utilized to observe hepatic mitochondrial morphology, as well as cellular mitochondrial membrane potential and reactive oxygen species level. Additionally, expression of PINK1/parkin-mediated mitophagy and NLRP3 inflammasome was detected to elucidate the underlying mechanism of YGJM formula by immunohistochemistry, immunofluorescence, RT-PCR (reverse transcription-polymerase chain reaction) and Western blot. The manifestations of pathology and biochemical detection confirmed the efficacy of YGJM formula in relieving hepatic damage and lipid deposition. Simultaneously, YGJM formula could obviously improve mitochondrial function. In addition, YGJM formula exhibited the promotion of PINK1/parkin-mediated mitophagy, which could perturb NLRP3 inflammasome activation, and as a result, the hepatocyte inflammation was also suppressed both in vitro and in vivo. Our preliminary results indicate that YGJM formula can ameliorate NASH mechanistically by interfering with PINK1/parkin-mediated mitophagy and NLRP3 inflammasome to exert anti-inflammation ability and promote mitochondrial function restoration. This study introduces magnetically powered microrobots utilizing Traditional Chinese Medicine (TCM) Spora Lygodii (SL) encapsulated with Doxorubicin (DOX) and Fe3O4 nanoparticles (Fe/DOX@SL) for targeted therapy.Methods: Fe3O4 nanoparticles were synthesized via co-precipitation and combined with SL spores and DOX through dip-coating to form Fe/DOX@SL microrobots. Their propulsion was controlled by a rotating magnetic field (RMF) for precise delivery. The microrobots’ mobility and adherence were assessed in various biological media. Therapeutic efficacy was evaluated using an orthotopic bladder cancer model in mice treated intravesically with Fe/DOX@SL under RMF guidance, compared to controls.Results: Fe/DOX@SL microrobots demonstrated efficient movement and stable navigation in biological environments. In vivo experiments showed superior retention in the bladder, prolonged adherence to the mucosa, and significantly enhanced tumor suppression in the RMF-guided group. Bioluminescence imaging confirmed reduced tumor growth, and histological analysis revealed substantial tumor regression compared to other treatments.Discussion and Conclusion: This study highlights the potential of integrating TCM with advanced microrobotics. The biocompatible Fe/DOX@SL microrobots leverage SL’s therapeutic properties and fuel-free magnetic control to overcome challenges in bladder cancer treatment, such as poor drug retention and off-target toxicity. Abstract Background Squama Manis is a valuable traditional Chinese medicine with a long history of medicinal use in the treatment of breast-related diseases. However, owing to the excessive exploitation and utilization of the resources, Squama Manis has been included in the list of rare and endangered wild animals. The conservation of the resources of Squama Manis and continuing its clinical application has become an urgent problem, and the search for small-molecule substitutes for Squama Manis is an effective way to achieve this goal. Previous studies have identified PA3264 as a possible active ingredient in Squama Manis. In this study, we systematically investigated the pharmacological effects and mechanisms of PA3264 in the treatment of triple-negative breast cancer (TNBC), a representative breast-related disease. Methods Cell viability and colony formation assays were performed after treatment with the target dipeptide PA3264 in vitro. Next, 4T1 orthotopic tumors and humanized PBMC-CDX mouse models were generated to examine the antitumor effect of PA3264 in vivo. Transcriptome sequencing and molecular docking experiments were performed to predict pathways to function. Western blotting and quantitative real-time PCR were used to validate the molecular mechanisms underlying the anticancer effects of PA3264. Results PA3264 significantly inhibited cell viability and migration of breast cancer cells in vitro. Furthermore, PA3264 suppressed the tumor size and reduced the tumor weight in vivo. Finally, it was verified that PA3264 prevented the progression of breast cancer by inhibiting the PI3K/AKT/NF-κB pathway, causing cell cycle arrest, and promoting apoptosis. Conclusions This study elucidated that PA3264 derived from rare and endangered Squama Manis was a novel bioactive peptide for treating triple-negative breast cancer from a scientific research perspective.

Abstract Introduction Chronic inflammation is the major pathological feature of Atherosclerosis(As). Inflammation may accelerate plaque to develop, which is a key factor resulting in the thinning of the fibrous cap and the vulnerable rupture of plaque. Presently, clinical treatments are still lacking. It is necessary to find a safe and effective treatment for As inflammation. Simiaoyongan Decoction (SMYA) has potential anti-inflammatory and plaque protection effects. This protocol aims to evaluate the efficacy, safety, and mechanism of SMYA for patients with carotid atherosclerotic plaque. Methods/design The assessment of SMYA clinical trial is designed as a randomized, double-blind, placebo-controlled study. The sample size is 86 cases in total, with 43 participants in the intervention group and the control group respectively. The intervention group takes SMYA, while the control group takes SMYA placebo. The medication lasts for 14 days every 10 weeks, with a total of 50 weeks. We will use carotid artery high resolution magnetic resonance imaging (HR-MRI) to measure plaque. The plaque minimum fiber cap thickness (PMFCT) is adopted as the primary outcome. The secondary outcomes include plaque fiber cap volume, volume percentage of fiber cap, lipid-rich necrotic core (LRNC) volume, volume percentage of LRNC, internal bleeding volume of plaque, internal bleeding volume percentage of plaque, plaque calcification volume, volume percentage of plaque calcification, lumen stenosis rate, average and a maximum of vessel wall thickness, vessel wall volume, total vessel wall load, carotid atherosclerosis score, hs-CRP, IL-1β and IL-6, the level of lipid profiles and blood glucose, blood pressure, and body weight. Discussion We anticipate that patients with As plaque will be improved from SMYA by inhibiting inflammation to enhance plaque stability. This study analyzes plaque by using HR-MRI to evaluate the clinical efficacy and safety of SMYA. Moreover, we conduct transcriptome analysis, proteomic analysis, and metagenomic analysis of blood and stool of participants to study the mechanism of SMYA against As plaque. This is the first prospective TCM trial to observe and treat As plaque by inhibiting inflammatory reaction directly. If successful, the finding will be valuable in the treatment of As plaque and drug development, especially in the “statin era”. Background: Body constitution is the foundation of Traditional Chinese Medicine (TCM), and model workers consist of a special group of workers within China. This study aims to research the relationship between the physical body constitutions based on TCM and the mental health of model workers. Methods: We recruited 314 model workers from Beijing Rehabilitation Hospital to conduct the questionnaires such as SCL-90 and CCMQ to investigate if there is an association between mental health status and TCM body constitutions. We performed a Partial Least Squares Path Modeling (PLS path modeling). Results: Our path model results revealed associations between different TCM constitution types and SCL scores, which serve as indicators of psychological well-being. Our research findings demonstrate a strong correlation between the Balanced constitution and elevated levels of psychological well-being, with a path coefficient of -0.503. In contrast, the other eight constitutional types exhibit path coefficients exceeding 0.3, indicating a tendency toward lower levels of psychological well-being. We also investigated the intricate connections between various TCM constitutional types and both mild and severe psychological well-being. Conclusion: In conclusion, the Balanced constitution continues to be closely associated with higher levels of psychological well-being, while the remaining eight body constitution types are consistently linked to lower levels of psychological well-being. The skin barrier is essential for maintaining the body's internal homeostasis, protecting against harmful external substances, and regulating water and electrolyte balance. Traditional Chinese Medicine (TCM) offers notable advantages in restoring skin barrier function due to its diverse components, targets, and pathways. Recent studies have demonstrated that active ingredients in TCM can safely and effectively repair damaged skin barriers, reinstating their proper functions. This review article provides a comprehensive overview of the mechanisms underlying skin barrier damage and explores how the bioactive constituents of TCM contribute to skin barrier repair, thereby offering a theoretical framework to inform clinical practices. Herbal medicines, particularly traditional Chinese medicines (TCMs), are a rich source of natural products with significant therapeutic potential. However, understanding their mechanisms of action is challenging due to the complexity of their multi-ingredient compositions. We introduced Herb-CMap, a multimodal fusion framework leveraging protein–protein interactions and herb-perturbed gene expression signatures. Utilizing a network-based heat diffusion algorithm, Herb-CMap creates a connectivity map linking herb perturbations to their therapeutic targets, thereby facilitating the prioritization of active ingredients. As a case study, we applied Herb-CMap to Suhuang antitussive capsule (Suhuang), a TCM formula used for treating cough variant asthma (CVA). Using in vivo rat models, our analysis established the transcriptomic signatures of Suhuang and identified its key compounds, such as quercetin and luteolin, and their target genes, including IL17A, PIK3CB , PIK3CD , AKT1 , and TNF. These drug–target interactions inhibit the IL-17 signaling pathway and deactivate PI3K , AKT , and NF-κB , effectively reducing lung inflammation and alleviating CVA. Gegen Qinlian Decoction (GGQLT) is a traditional Chinese herbal medicine that has been reported to have a significant therapeutic effect in the management of type II diabetes mellitus (T2DM). In this study, we constructed a T2DM rat model by feeding a high-fat diet and injecting streptozotocin (STZ) and tested the effects of feeding GGQLT and fecal transplantation on the physiological indices, microbiota, and metabolism of rats. The results showed that the administration of GGQLT can significantly improve the growth performance of rats and has a remarkable antihyperlipidemic effect. In addition, GGQLT altered the composition of gut microbiota by increasing beneficial bacteria such as Coprococcus, Bifidobacterium, Blautia, and Akkermansia. In addition, GGQLT elevated levels of specific bile acids by metabolomic analysis, potentially contributing to improvements in lipid metabolism. These findings suggest that GGQLT may have beneficial effects on T2DM by influencing lipid metabolism and gut microbiota. Chinese herbs are a huge treasure trove of natural products and an important source of many active molecules. The theory of traditional Chinese medicine compatibility (TCMC) is widely applied in clinical practice, but its mechanism is still ambiguous. This study aims to open a new window for this predicament by studying the interaction between the main active ingredients from a drug pair. Carrier-free assembly of natural products improves the shortcomings of traditional nanodelivery systems and opens a new path for the development of new nanomaterials. The drug pair "Pueraria and Hedyotis diffusa" has been commonly used in clinical practice, with a predominant therapeutic effect. This study is devoted to the study of the binary small molecule co-assembly of the main active molecules from the drug pair. In this study, we introduce a carrier-free composite gel, formed by the co-assembly of puerarin (PUE) and deacetylasperulosidic acid (DAA) via non-covalent bonds including π–π packing, intermolecular hydrogen bonding, and C=O π interactions. With a strain point 7-fold higher than that of P gel, the P − D gel exhibited favorable rheological properties. The survival rate of SW1990 cells in the P − D group was only 21.39% when the concentration of administration reached 200 μM. It thus demonstrated activity in inhibiting SW1990 cells' survival, suggesting potential in combating pancreatic cancer. Furthermore, this research offers a valuable concept for enhancing the mechanical properties and bioactivity of hydrogel materials through the utilization of a multi-component natural small molecule co-assembly approach. Background: Squama Manis is a valuable traditional Chinese medicine with a long history of medicinal use in the treatment of breast-related diseases. However, owing to the excessive exploitation and utilization of the resources, Squama Manis has been included in the list of rare and endangered wild animals. The conservation of the resources of Squama Manis and continuing its clinical application has become an urgent problem, and the search for small-molecule substitutes for Squama Manis is an effective way to achieve this goal. Previous studies have identified PA3264 as a possible active ingredient in Squama Manis. In this study, we systematically investigated the pharmacological effects and mechanisms of PA3264 in the treatment of triple-negative breast cancer (TNBC), a representative breast-related disease. Methods: Cell viability and colony formation assays were performed after treatment with the target dipeptide PA3264 in vitro. Next, 4T1 orthotopic tumors and humanized PBMC-CDX mouse models were generated to examine the antitumor effect of PA3264 in vivo. Transcriptome sequencing and molecular docking experiments were performed to predict pathways to function. Western blotting and quantitative real-time PCR were used to validate the molecular mechanisms underlying the anticancer effects of PA3264. Results: PA3264 significantly inhibited cell viability and migration of breast cancer cells in vitro. Furthermore, PA3264 suppressed the tumor size and reduced the tumor weight in vivo. Finally, it was verified that PA3264 prevented the progression of breast cancer by inhibiting the PI3K/AKT/NF-κB pathway, causing cell cycle arrest, and promoting apoptosis. Conclusions: This study elucidated that PA3264 derived from rare and endangered Squama Manis was a novel bioactive peptide for treating triple-negative breast cancer from a scientific research perspective. The study aimed to evaluate the impact of combining silibinin capsules with Jiangzhi Paizhuo Decoction (JZPZ) versus silibinin capsules alone in patients with MAFLD. The research was carried out at the Xi'an Hospital of Traditional Chinese Medicine using a case-control design following to STROBE guideline. Eligible participants meeting the inclusion criteria were randomly allocated into two groups. The participants in control group and intervention group were assigned to receive oral administration of 70 mg (3 times daily) silibinin capsules or 70 mg silibinin capsules (3 times daily) plus colon dialysis with 150 ml JZPZ decoction for 8 weeks. The primary outcome and secondary outcome on the effects of JZPZ decoction in MAFLD were detected. We found that liver function significantly improved in both groups after treatment (p < 0.05) compared with the baseline. Importantly, JZPZ decoction was associated with significant decrease in hepatic steatosis (CAP changes: −18.91 ± 11.50 vs −26.86 ± 16.62, P = 0.0305). The JZPZ decoction reduced significantly the TCM syndromes score compared to control (−5.74 ± 0.95 versus −7.17 ± 1.23, P < 0.0001). Meanwhile, the weight and BMI in JZPZ group increased significantly more than control group (P = 0.0003, P < 0.0001). In conclusion, the JZPZ decoction combined with silibinin capsules in the treatment of MAFLD patients with liver depression and heat syndrome has more advantages in decreasing hepatic steatosis and TCM syndromes scores. Introduction: Chronic inflammation is the major pathological feature of Atherosclerosis(As). Inflammation may accelerate plaque to develop, which is a key factor resulting in the thinning of the fibrous cap and the vulnerable rupture of plaque. Presently, clinical treatments are still lacking. It is necessary to find a safe and effective treatment for As inflammation. Simiaoyongan Decoction (SMYA) has potential anti-inflammatory and plaque protection effects. This protocol aims to evaluate the efficacy, safety, and mechanism of SMYA for patients with carotid atherosclerotic plaque. Methods/design: The assessment of SMYA clinical trial is designed as a randomized, double-blind, placebo-controlled study. The sample size is 86 cases in total, with 43 participants in the intervention group and the control group respectively. The intervention group takes SMYA, while the control group takes SMYA placebo. The medication lasts for 14 days every 10 weeks, with a total of 50 weeks. We will use carotid artery high resolution magnetic resonance imaging (HR-MRI) to measure plaque. The plaque minimum fiber cap thickness (PMFCT) is adopted as the primary outcome. The secondary outcomes include plaque fiber cap volume, volume percentage of fiber cap, lipid-rich necrotic core (LRNC) volume, volume percentage of LRNC, internal bleeding volume of plaque, internal bleeding volume percentage of plaque, plaque calcification volume, volume percentage of plaque calcification, lumen stenosis rate, average and a maximum of vessel wall thickness, vessel wall volume, total vessel wall load, carotid atherosclerosis score, hs-CRP, IL-1β and IL-6, the level of lipid profiles and blood glucose, blood pressure, and body weight. Discussion: We anticipate that patients with As plaque will be improved from SMYA by inhibiting inflammation to enhance plaque stability. This study analyzes plaque by using HR-MRI to evaluate the clinical efficacy and safety of SMYA. Moreover, we conduct transcriptome analysis, proteomic analysis, and metagenomic analysis of blood and stool of participants to study the mechanism of SMYA against As plaque. This is the first prospective TCM trial to observe and treat As plaque by inhibiting inflammatory reaction directly. If successful, the finding will be valuable in the treatment of As plaque and drug development, especially in the "statin era". The organic anion transporters OAT1 (SLC22A6) and OAT3 (SLC22A8) are drug transporters that are expressed in the kidney, with well-established roles in the in vivo transport of drugs and endogenous metabolites. A comparatively unexplored potential function of these drug transporters is their contribution to the in vivo regulation of natural products (NPs) and their effects on endogenous metabolism. This is important for the evaluation of potential NP interactions with other compounds at the transporter site. Here, we have analyzed the NPs present in several well-established databases from Asian (Chinese, Indian Ayurvedic) and other traditions. Loss of OAT1 and OAT3 in murine knockouts caused serum alterations of many NPs, including flavonoids, vitamins, and indoles. OAT1- and OAT3-dependent NPs were largely separable based on a multivariate analysis of chemical properties. Direct binding to the transporter was confirmed using in vitro transport assays and protein binding assays. Our in vivo and in vitro results, considered in the context of previous data, demonstrate that OAT1 and OAT3 play a pivotal role in the handling of non-synthetic small molecule natural products, NP-derived antioxidants, phytochemicals, and nutrients (e.g., pantothenic acid, thiamine). As described by remote sensing and signaling theory, drug transporters help regulate redox states by meditating the movement of endogenous antioxidants and nutrients between organs and organisms. Naoxintong Capsule (NXT), a renowned traditional Chinese medicine (TCM) formulation, has been broadly applied in China for more than 30 years. Over decades, accumulating evidences have proven satisfactory efficacy and safety of NXT in treating cardiovascular and cerebrovascular diseases (CCVD). Studies have been conducted unceasingly, while this growing latest knowledge of NXT has not yet been interpreted properly and summarized comprehensively. Hence, we systematically review the advancements in NXT research, from its chemical constituents, quality control, pharmacokinetics, to its profound pharmacological activities as well as its clinical applications in CCVD. Moreover, we further propose specific challenges for its future perspectives: 1) to precisely clarify bioactivities of single compound in complicated mixtures; 2) to evaluate the pharmacokinetic behaviors of NXT feature components in clinical studies, especially drug-drug interactions in CCVD patients; 3) to explore and validate its multi-target mechanisms by integrating multi-omics technologies; 4) to re-evaluate the safety and efficacy of NXT by carrying out large-scale, multicenter randomized controlled trials. In brief, this review aims to straighten out a paradigm for TCM modernization, which help to contribute NXT as a piece of Chinese Wisdom into the advanced intervention strategy for CCVD therapy. • Analysis of the effects of TCM exercises therapy interventions on cognitive function in elderly. • Analysis the seven moderating variables affecting the relationship between TCM exercise therapy and the cognitive function of the elderly. • Analysis and proposal of the best exercise prescription for the TCM exercise therapy intervention to delay cognitive decline in elderly. There exists a deficiency in a distinct understanding of the intervention effects of Traditional Chinese Medicine (TCM) exercise therapies (Tai Chi, Yi Jin Jing, Ba Duan Jin, Liu Zi Jue, Qigong, Wu Qin Xi etc.) on cognitive function and its moderating variables in the elderly. This study aims to systematically evaluate the effects of TCM exercise therapies on the cognitive function of the elderly and further propose the best exercise intervention programme to delay the cognitive decline of the elderly. PubMed, EBSCO host, Web of Science, EMbase, China National Knowledge Infrastructure and Wan Fang databases were searched for the effects of TCM exercise therapies on the cognitive function in older adults until July 2022. A meta-analysis of the included literature was performed using Stata 12.0 software, with a subgroup analysis of seven moderating variables: subject type, intervention content, intervention duration, intervention frequency, intervention period, study type and sample size. A random effects model was used to combine the overall effect sizes and to test for heterogeneity and publication bias across studies. A total of 20 publications with 1975 subjects were included. The TCM exercise therapies delayed cognitive decline in older adults (d = 0.83; 95 % CI [0.62–1.04]; P < 0.001). Subgroup analysis found that intervention content, intervention duration, intervention frequency, and intervention period were significant moderating variables influencing the effectiveness of the intervention. Among them, the Ba Duan Jin intervention (d = 0.85; 95 % CI [0.65–1.06]; P < 0.001), the duration of each exercise session of 60 min or more (d = 0.86; 95 % CI [0.71–1.00]; P < 0.001), the frequency of exercise of more than 5 times per week (d = 0.80; 95 % CI [0.64–0.96]; P < 0.001) and exercise cycles of 6–9 months (d = 0.96; 95 % CI [0.80–1.12]; P < 0.001) produced the largest effect sizes. TCM exercise therapies can effectively improve the cognitive function of the elderly. The best effect on the cognitive function of the elderly was achieved by choosing Ba Duan Jin and exercising at least five times a week for at least 60 min each time for a total of 6–9 months. The effect size of the TCM exercise therapy interventions on the cognitive function in older adults may be overestimated because of publication bias. Hypertension is a common disease; however, it is more prevalent in older adults, and its prevalence is increasing in younger populations. Numerous studies have revealed that hypertension and the composition and functionality of the intestinal flora are closely correlated. The balance of the intestinal flora, intestinal barrier integrity, and metabolite content of the intestinal flora play significant roles in the occurrence and progression of hypertension. Therefore, we performed a comprehensive review of Traditional Chinese medicine (TCM) for hypertension, focusing on the role of the intestinal flora to understand the mechanism by which TCM regulates hypertension through its effects on the intestinal flora. We analyzed the findings using the terms “traditional Chinese medicine,” “hypertension,” “high blood pressure,” “blood pressure,” “intestinal flora,” “intestinal barrier function,” “intestinal flora metabolites,” and other keywords from the China National Knowledge Infrastructure, VIP Chinese Science and Technology, Wanfang Data, PubMed, and ScienceDirect databases. We found that TCM treats hypertension by regulating the balance of the intestinal microbiota, increasing the abundance of beneficial bacteria, reducing the abundance of harmful bacteria, improving intestinal barrier function, increasing compact proteins, reducing intestinal permeability, and regulating the content of intestinal flora metabolites. The use of TCM to treat hypertension by regulating the intestinal flora is a promising therapeutic strategy. However, most studies are limited by small sample sizes and there is a lack of large-scale randomized controlled trials. In the future, multi-center controlled clinical trials are needed to verify the efficacy and safety of TCM, optimize therapeutic protocols, and establish a foundation for the standardized and personalized application of TCM in hypertension management.

Introduction: Methicillin-resistant Staphylococcus aureus (MRSA) poses a significant challenge to global health due to its resistance to many conventional antibiotics. Emerging resistance increasingly compromises the efficacy of vancomycin, a glycopeptide antibiotic, which remains a critical treatment option for MRSA infections. Traditional Chinese Medicine (TCM), with its rich history and diverse pharmacopoeia, offers potential complementary therapies. This review explores the synergistic effects of combining vancomycin with TCM to enhance antibacterial efficacy against MRSA. Method: We conducted a comprehensive literature review using databases such as PubMed, Scopus, and Web of Science. Studies included were those that evaluated the combined effects of vancomycin and Traditional Chinese Medicine (TCM) on MRSA in vitro, in vivo, and in clinical settings. Our selection criteria focused on peer-reviewed articles published in the last two decades. We included studies that provided clear methodologies for assessing bacterial inhibition rates, synergy assessments (using methods such as the checkerboard assay and time-kill curves), and mechanisms of action. Additionally, studies were required to detail the specific TCM components used, dosages, and outcomes. We excluded studies that did not specifically assess the combination of vancomycin with TCM, lacked a control group, or did not provide sufficient methodological details for assessing the synergy between the treatments. Data extraction was standardized to include study design, TCM components used, dosages, and outcomes. Results: The review identified several TCM herbs and formulations that exhibited synergistic effects when combined with vancomycin. Notable examples include Huang Lian (Coptis chinensis), which demonstrated enhanced bacterial inhibition through disruption of bacterial cell walls and biofilm reduction. The combination therapy not only improved bacterial clearance but also reduced the required dosage of vancomycin, potentially mitigating adverse effects. Mechanistic studies showed that TCM compounds could make bacterial cell membranes more permeable and stop efflux pumps from working, which raised the concentration of vancomycin inside cells. Conclusion: The synergistic use of vancomycin and TCM offers a promising strategy to combat MRSA infections, potentially addressing the limitations of vancomycin monotherapy. This review highlights the need for further clinical trials to validate these findings and optimise combination protocols. Integrating TCM with conventional antibiotics could lead to more effective, lower-dose treatments, reducing the risk of resistance development and improving patient outcomes. Future research should focus on elucidating the precise mechanisms of synergy and exploring the clinical applicability of these combinations.

This study analyzes 134 invalidated traditional Chinese medicine (TCM) patents from 1984 to 2021 to explore common weaknesses and track quality progression. The findings highlight that TCM patents applied for between 2001 and 2005 face higher invalidation rates, with disputes often arising 4–7 years post-application. Disputes are concentrated in patents related to cardiovascular and cerebrovascular diseases, exacerbated by widespread geographical distribution and technology saturation. Combination patents are more prone to invalidation than single-component ones, and patents using preparation methods to describe products carry hidden risks. Additionally, citing more references positively impacts patent stability. Despite progress, TCM patents still encounter significant challenges in quality and consistency.

BackgroundChen’s Peiyuan Tang (CSPYT) is a compound herbal formula that has shown the potential to enhance ovarian function and reduce autophagy in ovarian granulosa cells, which plays a crucial role in follicular development and maturation. The application of Chinese herbal medicine offers a promising alternative to traditional hormone replacement therapy (HRT).MethodsThis study explores CSPYT’s therapeutic mechanisms in treating POF, focusing on its modulation of autophagy through network pharmacology and transcriptomics-based analysis, predicting potential interactions and pathways. KGN cell line and rat ovarian granulosa cells were used for in vitro experiment. 4-Hydroperoxy cyclophosphamide(4-HC) stimulation was carried out for establishing the POF cell model. Q-PCR, Western Blot, Transmission electron microscopy to detect the results.ResultsAccording to the drug and disease database, the common targets of Chen’s Peiyuan Tang and premature ovarian failure were screened, combined with autophagy gene targets and transcriptome analysis, and finally 8 intersection targets were obtained, namely CDKN1B, MAPK3, PRKCD, CDKN1A, MAPK1, RAF1, BIRC5, CTSB. Enrichment analysis of 8 genes found that they were closely related to the animal autophagy pathway. Construct PPI network diagram. CytoScape 3.9.1 builds CSPYT Drug Target-POF Disease Target-Autophagy Gene Network Diagram. Based on the PPI network diagram and CytoScape 3.9.1 analysis results, it is estimated that MAPK1 and MAPK3 are the key targets of CSPYT in the treatment of POF. The eight final intersection targets were docked with the corresponding active pharmaceutical ingredients. The one that docked most closely with the MAPK family was naringenin. In cell experiment verification, it was confirmed that Chen’s Peiyuan Tang can inhibit the MAPK signaling pathway, significantly reduce the number of autophagosomes, and reduce autophagy damage in ovarian granulosa cells.DiscussionCSPYT can inhibit the MAPK signaling pathway, prevent autophagy overexpression and restore ovarian granulosa cell function, effectively alleviating the disease pressure of POF.

Background: Clinical depression is a serious public health issue that affects 4.7 % of the world's population and can lead to suicide tendencies. Although drug medications are available, only 60 % of the depressed patients respond positively to the treatments, while the rest experience side effects that resulted in the discontinuation of their medication. Thus, there is an urgent need for developing a new anti-depressant with a distinct mode of action and manageable side effects. One of the options is using medicinal plants or plant-based traditional medicine as alternative therapies for psychiatric disorders. Objectives: Therefore, the objective of this review was twofold; to identify and critically evaluate anti-depressant properties of medicinal plants or those incorporated in traditional medicine; and to discuss their possible mechanism of action as well as challenges and way forward for this alternative treatment approach. Methods: Relevant research articles were retrieved from various databases, including Scopus, PubMed, and Web of Science, for the period from 2018 to 2020, and the search was updated in September 2024. The inclusion criterion was relevance to antidepressants, while the exclusion criteria included duplicates, lack of full-text availability, and non-English publications. Results: Through an extensive literature review, more than 40 medicinal plant species with antidepressant effects were identified, some of which are part of traditional medicine. The list of the said plant species included Albizia zygia (DC.) J.F.Macbr., Calculus bovis Sativus, Celastrus paniculatus Willd., Cinnamomum sp., Erythrina velutina Willd., Ficus platyphylla Delile, Garcinia mangostana Linn., Hyptis martiusii Benth, and Polygonum multiflorum Thunb. Anti-depressant mechanisms associated with those plants were further characterised based on their modes of action such as anti-oxidation system, anti-inflammation action, modulation of various neurotransmitters, neuroprotective effect, the regulation of hypothalamic-pituitary-adrenal (HPA) axis and anti-depressant mechanism. The challenges and future outlook of this alternative and complementary medicine are also explored and discussed. Conclusion: This pool of identified plant species is hoped to offer health care professionals the best possible alternatives of anti-depressants from natural phytocompounds that are efficacious, safe and affordable for applications in future clinical settings.

Abstract Objective Sarcopenia is a geriatric syndrome that occurs with age and is characterized by a gradual decline in muscle mass, power, and functionality. It serves as a prominent contributor to frailty, disability, and mortality among older individuals. Currently, no standardized global guidelines exist for the diagnosis of sarcopenia. This study aimed to establish the correlation between sarcopenia and the constitutions of traditional Chinese medicine (TCM), considering the connection between physical functioning and sarcopenia. Methods A total of 1441 participants in this study were diagnosed with sarcopenia. The Asian Working Group for Sarcopenia (AWGS) proposed a sarcopenia definition algorithm. To determine the constitution of each participant, classification and determination standards were used in traditional Chinese medicine. This study evaluated the demographics, lifestyles, and self‐reported medical history of individuals diagnosed with sarcopenia through a self‐administered questionnaire. The constitution of the participants was determined using TCM classification and determination standards. Subsequently, we analyzed the results of univariate analysis and multivariate regression and constructed a receiver operating characteristic (ROC) curve. This paper examines the supply and utilization of traditional Chinese medicine (TCM) in Hong Kong during the influenza epidemics of the 1950s and 1960s. Existing narratives of TCM in Hong Kong have predominantly framed with within the dichotomy of Western medicine “Xiyi” and Chinese medicine “Zhongyi,” portraying TCM as marginalized and nearly wiped out by colonial power. Departing from this binary opposition, this study views TCM as an autonomous space that had never been subjugated by the colonial power which opted for minimal interventionist approach toward TCM. By adopting diachronic and synchronic perspectives on Hong Kong's unique environment shaped by its colonial history and the geopolitics of the Cold War in East Asia, particularly its relationships with “China,” this research seeks to reassess the role and status of TCM in post-World War II Hong Kong.In Hong Kong, along with other countries in East Asia, traditional medicine has ceded its position as mainstream medicine to Western medicine. Faced with the crisis of “extinction,” Chinese medical professionals, including medical practitioners and merchant groups, persistently sought solidarity and “self-renewal.” In the 1950s and 1960s, the colonial authorities heavily relied on private entities, including charity hospitals and clinics; furthermore, there was a lack of provision of public healthcare and official prevention measures against the epidemic influenza. As such, it is not surprising that the Chinese utilized TCM, along with Western medicine, to contain the epidemics which brought about an explosive surge in the number of patients from novel influenza viruses. TCM was significantly consumed during these explosive outbreaks of influenza in 1957 and 1968.In making this argument, this paper firstly provides an overview of the associations of Chinese medical practitioners and merchants who were crucial to the development of TCM in Hong Kong. Secondly, it analyzes one level of active provision and consumption of Chinese medicine during the two flu epidemics, focusing on the medical practices of TCM practitioners in the 1957 epidemic. While recognizing the etiologic agent or agents of the disease as influenza viruses, the group of Chinese medical practitioners of the Chinese Medical Society in Hong Kong adopted the basic principles of traditional medicine regarding influenza, such as Shanghanlun and Wenbingxue, to distinguish the disease status among patients and prescribe medicine according to correct diagnoses, which were effective. Thirdly, this paper examines the level of folk culture among the people, who utilized famous prescriptions of Chinese herbal medicine and alimentotherapy, in addition to Chinese patent medicines imported from mainland China. In the context of regional commercial network, this section also demonstrates how Hong Kong served as a sole exporting port of medicinal materials (e.g., Chinese herbs) and Chinese patent medicines from the People’s Republic of China to capitalist markets, including Hong Kong, under the socialist planned or controlled economy in the 1950s and 1960s.It was not only the efficacy of TCM in restoring immunity and alleviating symptoms of the human body, but also the voluntary efforts of these Chinese medical practitioners who sought to defend national medicine “Guoyi,” positioning it as complementary and alternative medicine to scientific medicine. Additionally, merchants who imported and distributed Chinese medicinal materials and national “Guochan” Chinese patent medicine played a crucial role, as did the people who utilized Chinese medicine, all of which contributed to making TCM thrive in colonial Hong Kong.

Objective To explore the differential expression and mechanisms of bone formation-related genes in osteoporosis (OP) leveraging bioinformatics and machine learning methodologies, and to predict the active ingredients of targeted traditional Chinese medicine (TCM) herbs. Methods The Gene Expression Omnibus (GEO) and GeneCards databases were employed to conduct a comprehensive screening of genes and disease-associated loci pertinent to the pathogenesis of OP. The R package was utilized as the analytical tool for the identification of differentially expressed genes. Least absolute shrinkage and selection operator (LASSO) logistic regression analysis and support vector machine-recursive feature elimination (SVM-RFE) algorithm were employed in defining the genetic signature specific to OP. Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analyses for the selected pivotal genes were conducted. The cell-type identification by estimating relative subsets of RNA transcripts (CIBERSORT) algorithm was leveraged to examine the infiltration patterns of immune cells, with Spearman's rank correlation analysis utilized to assess the relationship between the expression levels of the genes and the presence of immune cells. Coremine Medical Database was used to screen out potential TCM herbs for the treatment of OP. Comparative Toxicogenomics Database (CTD) was employed for forecasting the TCM active ingredients targeting the key genes. AutoDock Vina 1.2.2 and GROMACS 2020 softwares were employed to conclude analysis results, facilitating the exploration of binding affinity and conformational dynamics between the TCM active ingredients and their biological targets. Results Ten genes were identified by intersecting the results from the GEO and GeneCards databases. Through the application of LASSO regression and SVM-RFE algorithm, four pivotal genes were selected: coat protein (CP), kallikrein 3 (KLK3), polymerase γ (POLG), and transient receptor potential vanilloid 4 (TRPV4). GO and KEGG pathway enrichment analyses revealed that these trait genes were predominantly engaged in the regulation of defense response activation, maintenance of cellular metal ion balance, and the production of chemokine ligand 5. These genes were notably associated with signaling pathways such as ferroptosis, porphyrin metabolism, and base excision repair. Immune infiltration analysis showed that key genes were highly correlated with immune cells. Macrophage M0, M1, M2, and resting dendritic cell were significantly different between groups, and there were significant differences between different groups (P < 0.05). The interaction counts of resveratrol, curcumin, and quercetin with KLK3 were 7, 3, and 2, respectively. It shows that the interactions of resveratrol, curcumin, and quercetin with KLK3 were substantial. Molecular docking and molecular dynamics simulations further confirmed the robust binding affinity of these bioactive compounds to the target genes. Conclusion Pivotal genes including CP, KLK3, POLG, and TRPV4, exhibited commendable significant prognostic value, and played a crucial role in the diagnostic assessment of OP. Resveratrol, curcumin, and quercetin, natural compounds found in TCM, showed promise in their potential to effectively modulate the bone-forming gene KLK3. Objective Clinical medical record data associated with hepatitis B-related acute-on-chronic liver failure (HBV-ACLF) generally have small sample sizes and a class imbalance. However, most machine learning models are designed based on balanced data and lack interpretability. This study aimed to propose a traditional Chinese medicine (TCM) diagnostic model for HBV-ACLF based on the TCM syndrome differentiation and treatment theory, which is clinically interpretable and highly accurate. Methods We collected medical records from 261 patients diagnosed with HBV-ACLF, including three syndromes: Yang jaundice (214 cases), Yang-Yin jaundice (41 cases), and Yin jaundice (6 cases). To avoid overfitting of the machine learning model, we excluded the cases of Yin jaundice. After data standardization and cleaning, we obtained 255 relevant medical records of Yang jaundice and Yang-Yin jaundice. To address the class imbalance issue, we employed the oversampling method and five machine learning methods, including logistic regression (LR), support vector machine (SVM), decision tree (DT), random forest (RF), and extreme gradient boosting (XGBoost) to construct the syndrome diagnosis models. This study used precision, F1 score, the area under the receiver operating characteristic (ROC) curve (AUC), and accuracy as model evaluation metrics. The model with the best classification performance was selected to extract the diagnostic rule, and its clinical significance was thoroughly analyzed. Furthermore, we proposed a novel multiple-round stable rule extraction (MRSRE) method to obtain a stable rule set of features that can exhibit the model's clinical interpretability. Results The precision of the five machine learning models built using oversampled balanced data exceeded 0.90. Among these models, the accuracy of RF classification of syndrome types was 0.92, and the mean F1 scores of the two categories of Yang jaundice and Yang-Yin jaundice were 0.93 and 0.94, respectively. Additionally, the AUC was 0.98. The extraction rules of the RF syndrome differentiation model based on the MRSRE method revealed that the common features of Yang jaundice and Yang-Yin jaundice were wiry pulse, yellowing of the urine, skin, and eyes, normal tongue body, healthy sublingual vessel, nausea, oil loathing, and poor appetite. The main features of Yang jaundice were a red tongue body and thickened sublingual vessels, whereas those of Yang-Yin jaundice were a dark tongue body, pale white tongue body, white tongue coating, lack of strength, slippery pulse, light red tongue body, slimy tongue coating, and abdominal distension. This is aligned with the classifications made by TCM experts based on TCM syndrome differentiation and treatment theory. Conclusion Our model can be utilized for differentiating HBV-ACLF syndromes, which has the potential to be applied to generate other clinically interpretable models with high accuracy on clinical data characterized by small sample sizes and a class imbalance. Objective To build a dataset encompassing a large number of stained tongue coating images and process it using deep learning to automatically recognize stained tongue coating images. Methods A total of 1 001 images of stained tongue coating from healthy students at Hunan University of Chinese Medicine and 1 007 images of pathological (non-stained) tongue coating from hospitalized patients at The First Hospital of Hunan University of Chinese Medicine with lung cancer, diabetes, and hypertension were collected. The tongue images were randomized into the training, validation, and testing datasets in a 7: 2: 1 ratio. A deep learning model was constructed using the ResNet50 for recognizing stained tongue coating in the training and validation datasets. The training period was 90 epochs. The model's performance was evaluated by its accuracy, loss curve, recall, F1 score, confusion matrix, receiver operating characteristic (ROC) curve, and precision-recall (PR) curve in the tasks of predicting stained tongue coating images in the testing dataset. The accuracy of the deep learning model was compared with that of attending physicians of traditional Chinese medicine (TCM). Results The training results showed that after 90 epochs, the model presented an excellent classification performance. The loss curve and accuracy were stable, showing no signs of overfitting. The model achieved an accuracy, recall, and F1 score of 92%, 91%, and 92%, respectively. The confusion matrix revealed an accuracy of 92% for the model and 69% for TCM practitioners. The areas under the ROC and PR curves were 0.97 and 0.95, respectively. Conclusion The deep learning model constructed using ResNet50 can effectively recognize stained coating images with greater accuracy than visual inspection of TCM practitioners. Objective To construct a precise model for identifying traditional Chinese medicine (TCM) constitutions, thereby offering optimized guidance for clinical diagnosis and treatment planning, and ultimately enhancing medical efficiency and treatment outcomes. Methods First, TCM full-body inspection data acquisition equipment was employed to collect full-body standing images of healthy people, from which the constitutions were labelled and defined in accordance with the Constitution in Chinese Medicine Questionnaire (CCMQ), and a dataset encompassing labelled constitutions was constructed. Second, heat-suppression valve (HSV) color space and improved local binary patterns (LBP) algorithm were leveraged for the extraction of features such as facial complexion and body shape. In addition, a dual-branch deep network was employed to collect deep features from the full-body standing images. Last, the random forest (RF) algorithm was utilized to learn the extracted multifeatures, which were subsequently employed to establish a TCM constitution identification model. Accuracy, precision, and F1 score were the three measures selected to assess the performance of the model. Results It was found that the accuracy, precision, and F1 score of the proposed model based on multifeatures for identifying TCM constitutions were 0.842, 0.868, and 0.790, respectively. In comparison with the identification models that encompass a single feature, either a single facial complexion feature, a body shape feature, or deep features, the accuracy of the model that incorporating all the aforementioned features was elevated by 0.105, 0.105, and 0.079, the precision increased by 0.164, 0.164, and 0.211, and the F1 score rose by 0.071, 0.071, and 0.084, respectively. Conclusion The research findings affirmed the viability of the proposed model, which incorporated multifeatures, including the facial complexion feature, the body shape feature, and the deep feature. Eye diagnosis is a method for inspecting systemic diseases and syndromes by observing the eyes. With the development of intelligent diagnosis in traditional Chinese medicine (TCM), artificial intelligence (AI) can improve the accuracy and efficiency of eye diagnosis. However, the research on intelligent eye diagnosis still faces many challenges, including the lack of standardized and precisely labeled data, multi-modal information analysis, and artificial intelligence models for syndrome differentiation. The widespread application of AI models in medicine provides new insights and opportunities for the research of eye diagnosis intelligence. This study elaborates on the three key technologies of AI models in the intelligent application of TCM eye diagnosis, and explores the implications for the research of eye diagnosis intelligence. First, a database concerning eye diagnosis was established based on self-supervised learning so as to solve the issues related to the lack of standardized and precisely labeled data. Next, the cross-modal understanding and generation of deep neural network models to address the problem of lacking multi-modal information analysis. Traditional Chinese medicine (TCM) has relied on pulse diagnosis as a cornerstone of healthcare assessment for thousands of years. Despite its long history and widespread use, TCM pulse diagnosis has faced challenges in terms of diagnostic accuracy and consistency due to its dependence on subjective interpretation and theoretical analysis. This study introduces an approach to enhance the accuracy of TCM pulse diagnosis for diabetes by leveraging the power of deep learning algorithms, specifically LeNet and ResNet models, for pulse waveform analysis. LeNet and ResNet models were applied to analyze TCM pulse waveforms using a diverse dataset comprising both healthy individuals and patients with diabetes. The integration of these advanced algorithms with modern TCM pulse measurement instruments shows great promise in reducing practitioner-dependent variability and improving the reliability of diagnoses. This research bridges the gap between ancient wisdom and cutting-edge technology in healthcare. LeNet-F, incorporating special feature extraction of a pulse based on TMC, showed improved training and test accuracies (73% and 67%, respectively, compared with LeNet's 70% and 65%). Moreover, ResNet models consistently outperformed LeNet, with ResNet18-F achieving the highest accuracy (82%) in training and 74% in testing. The advanced preprocessing techniques and additional features contribute significantly to ResNet18-F's superior performance, indicating the importance of feature engineering strategies. Furthermore, the study identifies potential avenues for future research, including optimizing preprocessing techniques to handle pulse waveform variations and noise levels, integrating additional time–frequency domain features, developing domain-specific feature selection algorithms, and expanding the scope to other diseases. Background: Traditional Chinese Medicine (TCM) has been used in the treatment of anthracycline (ANTs)-induced cardiotoxicity. This meta-analysis aims to evaluate systematically the efficacy and safety of TCM combined with dexrazoxane (DRZ) on cardiotoxicity caused by ANTs. Materials and Methods: Retrieval of the China National Knowledge Infrastructure (CNKI), WAN FANG DATA, Chinese Biomedical Database (CBM), Cochrane Library, and PubMed was conducted through various methods. The meta-analysis was registered on PROSPERO (Registration ID: CRD42023435324). A randomized controlled trial (RCT) of TCM in combination with DRZ to avert cardiotoxicity caused by ANTs was compiled. The efficacy and safety of TCM Combined with DRZ, the cardiotoxicity induced by ANTs was synthesized and discussed, along with the outcomes of cardiac troponin T (cTnT), cardiac troponin I (cTnI), brain natriuretic peptide (BNP), creatine kinase isoenzymes (CK-MB), creatine kinase (CK), left ventricular ejection fraction (LVEF), electrocardiogram (ECG) abnormalities, and the incidence of adverse reactions. RevMan 5.3 was used for meta-analysis. Results: A total of 10 RCTs with 746 patients were included. The pooled results showed that TCM combined with DRZ treatment decreased significantly the incidence of ECG abnormalities (RR = 0.47, 95% CI [0.32,0.70], p = 0.0001), the level of cTnT (MD = −7.16, 95% CI [−8.31, −6.02], p< 0.001), CK (MD = −14.94, 95% CI [−22.91, −6.97], p< 0.001), cTnI (MD = −0.10, 95% CI [−0.14, −0.06], p< 0.001), BNP (MD = −33.66, 95% CI [−36.95, −30.37], p< 0.001), CK-MB (MD = −5.93, 95% CI [−8.16, −3.70], p< 0.001). However, there was no significant change in LVEF in the experimental group compared with the control group (MD = 4.81, 95% CI [−02.99, 12.21], p = 0.23). Moreover, adverse events have no significant difference between the two groups as well (RR = 0.60, 95% CI [0.31, 1.15], p = 0.13). Conclusion: This meta-analysis revealed TCM combined with DRZ appears to be able to improve the efficacy of cardiotoxicity caused by ANTs, but the included studies were of poor quality, and high-quality RCTs would be preferred for future studies. Hepatitis B, caused by the hepatitis B Virus (HBV), is an inflammatory liver disease that poses a significant threat to public health. HBV, a hepatotropic DNA virus, can cause chronic and acute Hepatitis B and persistent infection may cause serious complications like cirrhosis, liver cancer, and liver failure. Though nucleos(t)ide analogues (NAs) is the first-line drug for chronic hepatitis B (CHB), they still have several drawbacks such as drug resistance, and low-level viremia. Recent studies suggest that traditional Chinese medicine (TCM) and its active ingredients can inhibit HBV activity and replication, indicating their potentials for CHB treatment. This article focuses on the anti HBV activity and mechanism of TCM, aiming to help develop new anti-HBV drugs and improve the cure rate of CHB. Objective: Sarcopenia is a geriatric syndrome that occurs with age and is characterized by a gradual decline in muscle mass, power, and functionality. It serves as a prominent contributor to frailty, disability, and mortality among older individuals. Currently, no standardized global guidelines exist for the diagnosis of sarcopenia. This study aimed to establish the correlation between sarcopenia and the constitutions of traditional Chinese medicine (TCM), considering the connection between physical functioning and sarcopenia. Methods: A total of 1441 participants in this study were diagnosed with sarcopenia. The Asian Working Group for Sarcopenia (AWGS) proposed a sarcopenia definition algorithm. To determine the constitution of each participant, classification and determination standards were used in traditional Chinese medicine. This study evaluated the demographics, lifestyles, and self‐reported medical history of individuals diagnosed with sarcopenia through a self‐administered questionnaire. The constitution of the participants was determined using TCM classification and determination standards. Subsequently, we analyzed the results of univariate analysis and multivariate regression and constructed a receiver operating characteristic (ROC) curve. Results: Participants who were diagnosed with sarcopenia had substantially lower original Neutral constitution scores (P < 0.050). In comparison to those without sarcopenia, individuals with sarcopenia exhibited notably elevated original Qi‐deficiency, Yang‐deficiency, Yin‐deficiency, Blood‐stagnation, and Qi‐stagnation scores in contrast to those in the healthy group (P < 0.050). The identified risk factors associated with sarcopenia included the following: Neutral (OR = 0.903), Qi‐deficiency (in males, OR = 1.126), Yang‐deficiency (OR = 1.062), Phlegm‐dampness (in males, OR = 0.833), and Blood‐stagnation (in females, OR = 1.089). The highest area under the curve (AUC) was observed for the original neutral constitution score, followed by the Yang‐deficiency and blood‐stagnation scores (0.644, 0.613, and 0.611, respectively). Additionally, the AUC for the combined original scores of all nine constitutions among males reached 0.778. Conclusions: In this cross‐sectional study of older people with higher original Qi‐deficiency, Yin deficiency, Yang‐deficiency, Blood‐stagnation, and Qi‐stagnation were associated with sarcopenia. Notably, various TCM constitutions are significantly linked to sarcopenia. There was a significant occurrence of various body constitution types among individuals diagnosed with sarcopenia. Heart failure is a leading cause of death in the elderly. Traditional Chinese medicine, a verified alternative therapeutic regimen, has been used to treat heart failure, which is less expensive and has fewer adverse effects. In this study, a total of 15 active ingredients of Astragalus membranaceus (Huangqi, HQ) were obtained; among them, Isorhamnetin, Quercetin, Calycosin, Formononetin, and Kaempferol were found to be linked to heart failure. Ang II significantly enlarged the cell size of cardiomyocytes, which could be partially reduced by Quercetin, Isorhamnetin, Calycosin, Kaempferol, or Formononetin. Ang II significantly up‐regulated ANP, BNP, β‐MHC, and CTGF expressions, whereas Quercetin, Isorhamnetin, Calycosin, Kaempferol or Formononetin treatment partially downregulated ANP, BNP, β‐MHC and CTGF expressions. Five active ingredients of HQ attenuated inflammation in Ang II‐induced cardiomyocytes by inhibiting the levels of TNF‐α, IL‐1β, IL‐18 and IL‐6. Molecular docking shows Isorhamnetin, Quercetin, Calycosin, Formononetin and Kaempferol can bind with its target protein ESR1 in a good bond by intermolecular force. Quercetin, Calycosin, Kaempferol or Formononetin treatment promoted the expression levels of ESR1 and phosphorylated ESR1 in Ang II‐stimulated cardiomyocytes; however, Isorhamnetin treatment had no effect on ESR1 and phosphorylated ESR1 expression levels. In conclusion, our results comprehensively illustrated the bioactives, potential targets, and molecular mechanism of HQ against heart failure. Isorhamnetin, Quercetin, Calycosin, Formononetin and Kaempferol might be the primary active ingredients of HQ, dominating its cardioprotective effects against heart failure through regulating ESR1 expression, which provided a basis for the clinical application of HQ to regulate cardiac hypertrophy and heart failure. Due to the high risks of postoperative complications brought on by gastric cancer, traditional Chinese medicine (TCM) as a commonly used therapy, has exerted its vital role in postoperative recovery care. In this sense, this meta‐analysis was conducted to explore the related documents about TCM's impact on gastric cancer postoperative recovery. During the research, we explored a total of 1549 results from databases PubMed, China National Knowledge Infrastructure (CNKI), Embase, Cochrane Library and Web of Science (WoS). Thirty‐two clinical randomized trials (RCTs) were then selected and analysed for this meta‐analysis by using the software RevMan 5.4 (under PRISMA 2020 regulations), with a population of 3178 patients. Data prove that TCM therapy reduced the risks for postoperative complications exposure by an estimated average of 19% (95% CI). Among the complications, TCM therapy suppressed the risks of wound infection and incisional infections by 53% and 48% respectively. Meanwhile, the patient's wound healing duration exhibited a significant reduction compared to those without TCM treatment, with a difference at around 0.74 days (95% CI). TCM also exerted its potential to strengthen the patient's immune and health conditions, leading to a significantly promoted gastrointestinal function in the patients with a shorter duration to release first exhaustion and defecation compared to those with no TCM therapy. In addition, similar promoted phenomena also exist in those patients with TCM therapy in terms of their immunity and nutritional conditions. Introduction: Within the framework of Traditional Chinese Medicine (TCM), this study investigates the vast field of Chinese herbal medicine's potential as a powerful weapon in the fight against cervical cancer. Methods: A thorough search was made through Scopus, PubMed, ResearchGate, and Elsevier Journals for future studies on the healing and cervical cancer-fighting properties of Chinese plants and herbal formulations. Result: Scutellaria baicalensis, Curcuma longa, Ganoderma lucidum, Camptotheca acuminata, Antrodia cinnamomea, Terminalia catappa, Chelidonium majus, Lycopodium clavatum, and Myrica cerifera possess cancer-fighting bioactive compounds. Baicalein in Scutellaria baicalensis suppresses cervical cancer growth and alters Notch signaling, while curcumin in Curcuma longa inhibits cell migration, invasion, and HPV oncogenes, also reducing inflammation and protecting cells. Ganoderma lucidum lowers inflammation, boosts the immune system, and blocks JAK/STAT5 signaling, preventing EMT in cervical cancer cells. Camptothecin in Camptotheca acuminata inhibits DNA topoisomerase I, reducing cancer cell growth, with Antrodia cinnamomea controlling apoptosis. Terminalia catappa uses MMP-9 and MAPK to inhibit spread, while Chelidonium majus, Lycopodium clavatum, and Myrica cerifera induce apoptosis and cell cycle arrest. Zhidai Decoction and Guizhi-Fuling-decoction suggest traditional Chinese medicine can treat HPV and cervical cancer. Discussion: Scutellaria baicalensis, Curcuma longa, Ganoderma lucidum, Camptotheca acuminata, Antrodia cinnamomea, Terminalia catappa, Chelidonium majus, Lycopodium clavatum, and Myrica cerifera possess bioactive substances effective against cancer. They work through mechanisms like inhibiting cell growth, altering signaling pathways, and inducing apoptosis. Traditional Chinese medicine formulations like Zhidai Decoction and Guizhi-Fuling-decoction show potential in treating HPV and cervical cancer.

Objective: To clarify the distribution of traditional Chinese medicine (TCM) pattern and its associated risk factors after percutaneous coronary intervention (PCI), and evaluate the reporting quality of existing studies to guide future research standardization. Methods: English databases including PubMed, Cochrane Library, and Web of Science, as well as Chinese databases including China National Knowledge Infrastructure (CNKI), China Scientific Journal Database (VIP), and Wanfang Database were searched to retrieve papers about PCI. The time span for the paper retrieval was set from the foundation of the databases to October 1, 2023. Statistical analyses were performed using Stata 12 and Python (V 3.9). The Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) statement was used to assess the reporting quality of included studies. Results: Overall, 1 356 articles were selected, and 40 cross-sectional studies were included with 10 270 participants. The most common TCM patterns before, one to two weeks after, and six months to one year after PCI was Qi stagnation and blood stasis (n = 261, 36.45%), intertwined phlegm and blood stasis (n = 109, 27.18%), and Qi deficiency and blood stasis (n = 645, 37.03%), respectively. Smoking [odds ratio (OR) = 1.15, 95% confidence interval (CI) (0.83 – 1.47), I2= 24.7%, P = 0.257], pattern of congealing cold and Qi stagnation [OR = 4.62, 95% CI (1.37 – 7.86), I2 = 61.6%, P = 0.074], and low-density lipoprotein (LDL) [OR = 1.38, 95% CI (0.92 – 1.85), I2= 12.2%, P = 0.286] were risk factors for restenosis. Hypertension [OR = 7.26, 95% CI (3.54 – 14.88), I2= 91.6%, P = 0.001], and overweight [i.e., body mass index (BMI) > 23] [OR = 1.20, 95% CI (1.07 – 1.35), I2= 85.3%, P = 0.009] were significant risk factors of concomitant anxiety. Conclusion: This systematic review and meta-analysis revealed that patients with different TCM pattern types have distinct characteristics and risk factors after PCI. More high-quality studies are warranted to provide supportive evidence for future research and clinical practice.

Objective: To investigate the effects of Tuina targeting different body parts on the behaviors and gut microflora of rat models with valproic acid (VPA)-induced autistic spectrum disorder (ASD). Methods: Twenty female Sprague Dawley (SD) rats with 12.5 d of pregnancy were randomly divided into VPA model group [intraperitoneal injection of VPA (600 mg/kg), n = 15] and saline group (intraperitoneal injection of equal amount of normal saline, n = 5). The offspring male rats injected with saline were secleted as control group. The offspring male rats injected with VPA were randomly divided into VPA, dorsal, and abdominal groups (n = 7 in each group). On the 21st day after birth, three-chamber social test, open field test, and marble-burying test were carried out to observe the social abilities, anxiety behaviors, and stereotypical behaviors of rats in the four groups. Rats in dorsal and abdominal groups underwent Tuina for 14 d, twice a day. On the 35th day, behavioral tests were conducted again, and intestinal contents were taken for species composition and structural analysis, as well as marker and differential species analysis. Results: (i) According to behavioral observations, compared with VPA group, the social and movement time in the central open field of rats in dorsal group increased significantly (P < 0.05), and the number of buried marbles decreased markedly (P < 0.01), indicating improvement on their social abilities, anxiety behaviors, and stereotypical behaviors as consequences of dorsal Tuina; and the number of buried marbles was reduced as well in abdominal group when compared with VPA group (P < 0.05), suggesting the improvement on their stereotypical behaviors following abdominal Tuina. In the marble-burying test, the number of marbles buried in dorsal group was less than in abdominal group, and the stereotypical behaviors were improved more significantly (P < 0.05), and there were no significant differences in the three-chamber social and open field tests between the two groups (P >0.05). (ii) In accordance with intestinal microflora detection results, compared with VPA group, both dorsal and abdominal groups showed increased richness (P < 0.05) and elevated diversity (P < 0.05 in dorsal group and P < 0.01 in abdominal group) in intestinal microflora. The results of differential analysis indicated that at the phylum level, compared with VPA group, the relative abundance of Firmicutes in rats in abdominal group showed a significant reduction trend (P < 0.05); at the genus level, compared with VPA group, the relative abundance of Lactobacillus in rats in dorsal and abdominal groups decreased significantly (P < 0.05). Dorsal group also showed significant increase in the genus Blautia in the analysis of marker species compared with VPA group (P < 0.05). Conclusion: Tuina impacted the behavior and gut microflora structure of ASD model rats. Dorsal intervention had a significant effect on social abilities, anxiety behaviors, and stereotypical behaviors of ASD model rats, while abdominal intervention only had an obvious effect on stereotypical behaviors. Both dorsal and abdominal interventions increased the richness and diversity of gut microflora of ASD model rats, with abdominal intervention improving the intestinal microbial diversity more significantly and resulting in a more uniform species distribution.

Objective: In the theories of pulse disgnosis in traditional Chinese medicine (TCM), it is emphasized that pulse manifestations at the radial artery within the wrist (called Cunkou) signify the physiological and pathological conditions of different internal organs in the human body. However, different opinions exist among researchers about the objectiveness of the pulse diagnosis technique. Some researchers mentioned that no significant differences were observed in pulse manifestations at various Cunkou areas, hence there might be some difficulty in evaluating the status of different organs through checking pulse manifestations at Cunkou. This research aims to analyze the pulse response at Cunkou from the aspect of the characteristics of tactile sensing, thus to give a preliminary explanation to the above question. Methods: This research utilized the Weber-Fechner law to model the tactile sensing as a dynamic low-pass signal filter with varying bandwidths under different compression levels during pulse diagnosis. The model was applied to analyzing the clinical data collected previously by our group. The arterial pressures measured invasively with equipment from 14 patients with aorta coarctation were processed to simulate different pulse manifestations at Cun, Guan, and Chi positions of Cunkou when different compression levels were applied. Results: Due to the characteristics of tactile sensing, significant variations were observed in pulse manifestations at different pulse-depths under the application of changing compression levels; while no such changes in pulse manifestations were observed from the employment of transducer only, without tactile sensing involved. The results explained why different understandings on pulse manifestations were formed between direct pulse-taking technique in TCM and modern sphygmography using transducers. The features of pulse manifestations at Cunkou, using direct pulse-taking technique but at different depths, superficial, middle, and deep positions, respectively, predicted by the developed tactile sensing model were in line with those described in TCM pulse theories. Conclusion: Based on the developed tactile sensing model, this study preliminarily explains the phenomenon that pulse manifestation at Cunkou changes in response to the compression force applied during TCM pulse-taking. Integrating the tactile sensing model with the study of TCM pulse diagnosis opens a new chapter for visualizing and quantitatively interpreting pulse manifestations. This not only expands the scope of pulse diagnosis study effectively, but also provide a scientific basis for further technical upgrading and optimization of existing pulse diagnosis equipment.

Objective: To construct a traditional Chinese medicine (TCM) knowledge base using knowledge graph based on deep learning methods, and to explore the application of joint models in intelligent question answering systems for TCM. Methods: Textbooks Prescriptions of Chinese Materia Medica and Chinese Materia Medica were applied to construct a comprehensive knowledge graph serving as the foundation for the intelligent question answering system. In the study, a BERT+Slot-Gated (BSG) deep learning model was applied for the identification of TCM entities and question intentions presented by users in their questions. Answers retrieved from the knowledge graph based on the identified entities and intentions were then returned to the user. The Flask framework and BSG model were utilized to develop the intelligent question answering system of TCM. Results: A TCM knowledge map encompassing 3 149 entities and 6 891 relational triples based on the prescriptions and Chinese materia medica was drawn. In the question answering test assisted by a question corpus, the F1 value for recognizing entities when answering 20 types of TCM questions was 0.996 9, and the accuracy rate for identifying intentions was 99.75%. This indicates that the system is both feasible and practical. Users can interact with the system through the WeChat Official Account platform. Conclusion: The BSG model proposed in this paper achieved good results in experiments by increasing the vector dimension, indicating the effectiveness of the joint model method and providing new research ideas for the implementation of intelligent question answering systems in TCM.

Objective Osteonecrosis of the femoral head (ONFH) is a common orthopedic disease with a high disability rate. The clinical effect of BuShenHuoXue decoction (BSHX) for ONFH is satisfactory. We aimed to elucidate the potential angiogenic mechanisms of BSHX in a rat femoral osteonecrosis model and bone marrow mesenchymal stem cells (BMSCs). Methods With in vivo experiments, we established the steroid‐induced osteonecrosis of the femoral head (SONFH) model using Sprague–Dawley (SD) rats (8‐week‐old). The rats were randomly divided into five group of 12 rats each and given the corresponding interventions: control, model (gavaged with 0.9% saline), BSHX low‐, medium‐ and high‐dose groups (0.132 3, 0.264 6, and 0.529 2 g/mL BSHX solution by gavage). After 12 weeks, haematoxylin and eosin (H&E) staining was preformed to evaluate rat osteonecrosis. the expression of angiogenic factors (CD31, VEGFA, KDR, VWF) in rat femoral head was detected by immunohistochemistry, qPCR and western blotting. In cell experiment, BMSCs were isolated and cultured in the femoral bone marrow cavity of 4‐week‐old SD rats. BMSCs were randomly divided into eight groups and intervened with different doses of BSHX‐containing serum and glucocorticoids: control group (CG); BSHX low‐, medium‐, and high‐dose groups (CG + 0.661 5, 1.323, and 2.646 g/kg BSHX gavage rat serum); dexamethasone (Dex) group; and Dex + BSHX low‐, medium‐, and high‐dose groups (Dex + 0.661 5, 1.323, and 2.646 g/kg BSHX gavaged rat serum), the effects of BSHX‐containing serum on the angiogenic capacity of BMSCs were examined by qPCR and Western blotting. A co‐culture system of rat aortic endothelial cells (RAOECs) and BMSCs was then established. Migration and angiogenesis of RAOECs were observed using angiogenesis and transwell assay. Cerebral ischemia/reperfusion injury (CIRI) is a major contributor to poor prognosis of ischemic stroke. Flavonoids are a broad family of plant polyphenols which are abundant in traditional Chinese medicine (TCM) and have beneficial effects on several diseases including ischemic stroke. Accumulating studies have indicated that flavonoids derived from herbal TCM are effective in alleviating CIRI after ischemic stroke in vitro or in vivo, and exhibit favourable therapeutical potential. Herein, we systematically review the classification, metabolic absorption, neuroprotective efficacy, and mechanisms of TCM flavonoids against CIRI. The literature suggest that flavonoids exert potential medicinal functions including suppressing excitotoxicity, Ca2+ overloading, oxidative stress, inflammation, thrombin's cellular toxicity, different types of programmed cell deaths, and protecting the blood-brain barrier, as well as promoting neurogenesis in the recovery stage following ischemic stroke. Furthermore, we identified certain matters that should be taken into account in future research, as well as proposed difficulties and opportunities in transforming TCM-derived flavonoids into medications or functional foods for the treatment or prevention of CIRI. Background: Common symptoms of Chronic Non-atrophic Gastritis (CNAG) include nausea, stomach distension, and abdominal pain. The Houtou Jianweiling Tablet (HTJWT) is a chinese patent medicine (CN1368229A) and it has been used clinically for more than 20 years with proven clinical efficacy in treating CNAG, prompted us to establish the clinical efficacy and safety of HTJWT on patients with mild to moderate CNAG symptoms in Pakistani population. Methods: This phase II, double-blind, randomized, parallel-controlled trial was conducted in a single center between November 2022 and February 2023 in Pakistan. In a ratio of 1:1, total 240 CNAG patients with erosion identified by pathological biopsy and gastroscopy were randomly assigned to control (Omeprazole) group (n = 120) and the treatment (HTJWT) group (n = 120). Patients in the treatment group received orally four HTJWT (0.38g/tablet), three times a day and one placebo of Omeprazole enteric-coated tablet prior to breakfast, daily. On the other hand, patients in the control group received one Omeprazole enteric-coated tablet (20 mg/tablet) prior to breakfast and four placebo of HTJWT, thrice a day. The patients consumed the investigated drugs (i.e., treatment and control) treatment regimen was followed for a duration of 28 days. The safety of the patients were evaluated through adverse events, serious adverse events and laboratory tests such as blood biochemistry, urine analysis, liver and renal function tests. Vital signs like; blood pressure, pulse rate, body temperature, respiratory rate for all the patients were recorded. The cardiac status of the patients were assessed through electrocardiogram (ECG). The primary efficacy indicators were the improvement rate of gastric distention and gastralgia as the main clinical symptoms. Secondary indicators were visual analogue score (VAS); improvement rate of secondary clinical symptoms and signs; improvement rate of total clinical signs and symptoms; the disappearance/ remission rate of Gastric pain and, remission/disappearance time of gastric distension; and the negative conversion rate of Helicobacter pylori (H. pylori). The outcomes among each group were compared using the chi-square test. Results: Patients in both groups had good drug compliance (80%-120%), and there was no statistically significant difference in the patients' baseline characteristics. The clinical improvement rate was found to be 91.1% in the treatment group and 91.0% in the control group with negligible variation among the two groups (p = 0.9824; 95% confidence interval: -0.0781-0.0798). Similarly, hardly no difference was found in the negative conversion rate of H. pylori between the treatment group and the control group (i.e., 70.1% and 71.8% respectively, p = 0.8125). There were no significant differences in respiratory rate, vital signs, blood pressure, laboratory results for blood biochemistry, urine analysis, liver and renal function tests between the two groups. The ECG assessment carried out for the treatment and control group revealed no considerable difference. Margin variation in the disappearance time of gastric pain (p = 0.1860) and remission rate (p = 0.5784) between the two groups were observed. The control group exhibited a faster remission period for gastrointestinal discomfort indications as compared to treatment group (p = 0.0430). Only one patient in the control group experienced mild to moderate adverse events, namely; epigastric pain and dyspepsia. The results were consistent with the intention-to-treat and per-protocol analysis that included patients who were 100% compliant to the assigned therapy. Conclusion: The lower limit of confidence interval (CI, 95%) for the differences in the effective rate between the treatment and the control groups was found to be -0.0781 which is greater than -0.15, hence the treatment group is non-inferior to the control group. The therapeutic dosage used in the trial and treatment period did not cause any significant adverse event, and there were no obvious changes in the ECG profile, vital signs and biochemistry of the patients. Based on the clinical efficacy evaluation and reported adverse events, it can be concluded that the HTJWT is a safe and effective traditional chinese medicine for the treatment of patients suffering from chronic non-atrophic gastritis with mild to moderate symptoms.
