BackgroundPhase 1/2 dose-escalation and expansion study evaluating varlilumab, a fully human agonist anti-CD27 mAb, with nivolumab in anti-PD-1/L1 naïve, refractory solid tumors.MethodsPhase 1 evaluated the safety of varlilumab (0.1–10 mg/kg) with nivolumab (3 mg/kg) administered once every 2 weeks. Phase 2 evaluated varlilumab regimens (3 mg/kg once every 2 weeks, 3 mg/kg once every 12 weeks, and 0.3 mg/kg once every 4 weeks) with nivolumab 240 mg once every 2 weeks in tumor-specific cohorts. Primary objective was safety; key clinical endpoints included objective response rate (ORR) and overall survival rate at 12 months (OS12) (glioblastoma (GBM) only). Exploratory objectives included determination of effects on peripheral blood and intratumoral immune signatures.Results175 patients were enrolled (36 in phase 1 and 139 in phase 2). Phase 1 dose-escalation proceeded to the highest varlilumab dose level without determining a maximum tolerated dose. In phase 2, ORR were ovarian 12.5%, squamous cell carcinoma of the head and neck 12.5%, colorectal cancer 5%, and renal cell carcinoma 0%; GBM OS12 was 40.9%. Increased tumor PD-L1 and intratumoral T cell infiltration were observed in ovarian cancer patients, with increases of ≥5% associated with better progression-free survival. The most common treatment related adverse events were fatigue (18%), pruritus (16%), and rash (15%).ConclusionVarlilumab and nivolumab were well tolerated, without significant toxicity beyond that expected for each agent alone. Clinical activity was observed in patients that are typically refractory to anti-PD-1 therapy, however, overall was not greater than expected for nivolumab monotherapy. Treatment was associated with proinflammatory changes in the tumor microenvironment, particularly in ovarian cancer where the changes were associated with better clinical outcomes.Trial registration numberNCT02335918.

CD27, a costimulatory molecule on T cells, induces intracellular signals mediating cellular activation, proliferation, effector function, and cell survival on binding to its ligand, CD70. Varlilumab, a novel, first-in-class, agonist immunoglobulin G1 anti-CD27 antibody, mediates antitumor immunity and direct killing of CD27+ tumor cells in animal models. This first-in-human, dose-escalation, and expansion study evaluated varlilumab in patients with hematologic malignancies. Primary objectives were to assess safety and the maximum tolerated and optimal biologic doses of varlilumab. Secondary objectives were to evaluate pharmacokinetics, pharmacodynamics, immunogenicity, and antitumor activity. In a 3 + 3 dose-escalation design, 30 patients with B-cell (n = 25) or T-cell (n = 5) malignancies received varlilumab (0.1, 0.3, 1, 3, or 10 mg/kg IV) as a single dose with a 28-day observation period, followed by weekly dosing (4 doses per cycle, up to 5 cycles, depending on tumor response). In an expansion cohort, 4 additional patients with Hodgkin lymphoma received varlilumab at 0.3 mg/kg every 3 weeks (4 doses per cycle, up to 5 cycles). No dose-limiting toxicities were observed. Treatment-related adverse events, generally grade 1 to 2, included fatigue, decreased appetite, anemia, diarrhea, and headache. Exposure was linear and dose-proportional across dose groups and resulted in increases in proinflammatory cytokines and soluble CD27. One patient with stage IV Hodgkin lymphoma experienced a complete response and remained in remission at >33 months with no further anticancer therapy. These data support further investigation of varlilumab for hematologic malignancies, particularly in combination approaches targeting nonredundant immune regulating pathways. Background CDX-1140 is an agonist anti-CD40 mAb selected to optimize systemic exposure and hence tumor microenvironment (TME) ingress. CDX-1140 activity may be enhanced by combining with CDX-301 (recombinant Flt3L), a dendritic cell growth factor, or with pembrolizumab, an anti-PD-1 mAb. Methods Patients with advanced solid or hematologic (Part 1 only) tumors are enrolled. Part 1 dose-escalation results have been presented (SITC 2019). In Part 2, CDX-1140 dose-escalation (0.09–1.5 mg/kg q4w) is in combination with CDX-301 (75 mcg/kg sc QD x 5 for 2 cycles). In Part 3, CDX-1140 dose-escalation (0.72–1.5 mg/kg q3w) is in combination with pembrolizumab 200 mg q3w. Part 1 and 2 expansion cohorts are dosed at the CDX-1140 MTD, 1.5 mg/kg q4w. Part 3 expansion cohorts are planned. Peripheral blood and tumor biomarkers analysis are ongoing. Results 92 patients have been treated (Part 1 n=57, Part 2 n=31, Part 3 n=4). Part 1 expansion cohorts in SCCHN (n=7) and RCC (n=5) are fully enrolled. Part 2 dose-escalation completed to the highest CDX-1140 dose and a SCCHN expansion cohort is ongoing. Part 3 dose-escalation recently initiated. Safety data is available for 23 and 10 patients at the MTD in Part 1 and 2, respectively. In general, the safety profiles were similar, with arthralgia (52% vs. 50%), pyrexia (44% vs 50%), fatigue (30% vs. 50%), chills (39% vs. 40%), vomiting (30% vs. 20%), nausea (26% vs 40%), myalgia (22% vs. 30%), increased ALT (22% vs. 20%), and increased AST (22% vs. 30%) being the most common drug related AEs at the MTD in Part 1 and 2, respectively. Most AEs were low grade. Across all cohorts, cytokine release syndrome (CRS) (G2 n=4, G3 n=2) occurred in 6 (Part 1 n=2; Part 2 n=4) and pneumonitis (G3) occurred in 5 (Part 1 n=4; Part 2 n=1) patients. Immune activation in the TME consistent with CD40 agonism and increases serum inflammatory cytokines were observed. Evidence of anti-tumor activity/clinical benefit include SD (n=13), tumor cavitation (n=2) and a uPR in solid tumors. A patient with follicular lymphoma has an ongoing durable complete metabolic response. Conclusions The CDX-1140 MTD dose of 1.5 mg/kg, a dose level expected to provide good systemic exposure and TME penetration, is generally well tolerated alone and with CDX-301. Transaminitis and CRS have generally been low grade and infrequent. A cohort combining CDX-1140 with chemotherapy will be initiated in patients with previously untreated metastatic pancreatic adenocarcinoma. Background CD27 ligation and PD-1 blockade elicit complementary signals mediating T cell activation and effector function. CD27 is constitutively expressed on most mature T cells and the interaction with its ligand, CD70, plays key roles in T cell costimulation leading to activation, proliferation, enhanced survival, maturation of effector capacity, and memory. The PD-1/PD-L1 pathway plays key roles in inhibiting T cell responses. Pre-clinical studies demonstrate synergy in T cell activation and anti-tumor activity when combining a CD27 agonist antibody with PD-(L)1 blockade, and clinical studies have confirmed the feasibility of this combination by demonstrating safety and biological and clinical activity. CDX-527 is a novel human bispecific antibody containing a neutralizing, high affinity IgG1k PD-L1 mAb (9H9) and the single chain Fv fragment (scFv) of an agonist anti-CD27 mAb (2B3) genetically attached to the C-terminus of each heavy chain, thereby making CDX-527 bivalent for each target. Pre-clinical studies have demonstrated enhanced T cell activation by CDX-527 and anti-tumor activity of a surrogate bispecific compared to individual mAb combinations, and together with the IND-enabling studies support the advancement of CDX-527 into the clinic. Methods A Phase 1 first-in-human, open-label, non-randomized, multi-center, dose-escalation and expansion study evaluating safety, pharmacokinetics (PK), pharmacodynamics (PD), and clinical activity of CDX-527 is ongoing. Eligible patients have advanced solid tumor malignancies and have progressed on standard-of-care therapy. Patients must have no more than one prior anti-PD-1/L1 for tumor types which have anti-PD-1/L1 approved for that indication and no prior anti-PD-1/L1 for tumor types that do not have anti-PD-1/L1 approved for that indication. CDX-527 is administered intravenously once every two weeks with doses ranging from 0.03 mg/kg up to 10.0 mg/kg or until the maximum tolerated dose. The dose-escalation phase initiates with a single patient enrolled in cohort 1. In the absence of a dose limiting toxicity or any ≥ grade 2 treatment related AE, cohort 2 will enroll in a similar manner as cohort 1. Subsequent dose-escalation cohorts will be conducted in 3+3 manner. 2585 Background: CDX-527 is a bispecific antibody (BsAb) targeting PD-L1 and CD27 that is designed to block immune checkpoint PD-L1/PD-1 interactions while providing immune costimulation through CD27 signaling. CD27 is a key immunostimulatory molecule that enhances T cell activation, effector function, and survival. Combining anti-PD-L1 and anti-CD27 mAbs synergize in preclinical studies, activating complementary cytotoxic and proliferative gene expression profiles, respectively. Clinical studies demonstrated the safety and biological activity of combining varlilumab, an agonist anti-CD27 mAb, with nivolumab or atezolizumab, along with modest clinical activity of the combinations. Biomarker assessments include characterizing the effects on peripheral blood immune cells and cytokines, and for the expansion cohorts, the impact of CDX-527 on the tumor microenvironment in paired tumor biopsies. Results: To date, 8 patients have received CDX-527 in doses ranging from 0.03 mg/kg to 1 mg/kg and 3 are still on treatment. There has been no drug related SAEs, DLTs or discontinuations due to an AE. Most common treatment related AEs were influenza-like illness, fatigue, and arthralgia (all at 25%). All drug related AEs have been grade 1 or 2. Conclusions: Preliminary results indicate that the novel anti-PD-L1xCD27 bispecific antibody CDX-527 up to and including the 1 mg/kg dose level has been well tolerated. CDX-1140 is an agonist CD40 mAb intended to promote priming of CD8+ effector T cells through activation of dendritic cells (DC), enhance macrophage tumoricidal activity, and mediate direct killing of CD40-expressing tumor cells. CDX-1140 is a human IgG2 mAb with Fc receptor independent activity and is synergistic with sCD40L, suggesting a potential to enhance CD40L-mediated immune activation. CDX-1140 has a broad dose-dependent activity that may achieve maximal agonist activity at doses that provide significant tumor penetration with minimal toxicities from systemic CD40 activation. CDX-301 (rhFLT3L) is a DC growth factor that markedly increases CD141+ DCs, including in the tumor microenvironment (TME). CD141+ DCs are critical for initiating anti-tumor immune responses and are often depleted within the TME. Preclinical studies have shown that treatment with CDX-301 synergizes with CD40 signaling to enhance anti-tumor immune responses, suggesting that combining CDX-1140 and CDX-301 could result in enhanced clinical benefit. CDX1140-01(NCT03329950) is a Phase I dose-escalation study of CDX-1140 as monotherapy or in combination with CDX-301 evaluating safety, PK, PD, and preliminary clinical activity in patients (pts) with refractory solid tumors and non-Hodgkin lymphoma. CDX-1140 is given iv q 4 weeks at doses from 0.01 to 3 mg/kg utilizing a 3+3 design (first 2 cohorts are single-pts). In combination cohorts (solid tumors only), CDX-301 at 75 μg/kg daily x 5 is administered sc prior to cycles 1 and 2 of escalating doses of CDX-1140 starting at 0.09 mg/kg. Tumor-specific expansion cohorts are planned to further explore the activity of monotherapy or combination treatment. To date, 18 pts across 10 tumor types have been dosed in CDX-1140 monotherapy cohorts with doses ranging from 0.01 to 0.72 mg/kg and without identification of an MTD. There has been one DLT (0.18 mg/kg): grade 3 pneumonitis and hypoxia. One pt (0.72 mg/kg) experienced grade 3 fatigue requiring hospitalization 48 hrs after the first infusion. There have been 3 additional grade 3 treatment-related adverse events: dyspnea (0.18 mg/kg), nausea (0.36 mg/kg), and vomiting (0.36 mg/kg). The first combination cohort (n=5) has been completed. In monotherapy and combination cohorts there have been no significant drug-related changes in liver function tests to date. CDX-1140 PK is quantifiable at doses ≥ 0.09 mg/kg and exposure appears dose proportional. Expected dose-dependent PD effects have shown activation of peripheral blood lymphocytes and increases in pro-inflammatory cytokines and chemokines. Combination with CDX-301 has shown enhanced cytokine responses. The data suggest that CDX-1140 has the expected biologic and safety profile predicted from preclinical studies and may achieve dose levels optimal for systemic exposure. Preclinical studies have demonstrated synergistic antitumor activity when combining PD-1 blockade and varlilumab, a CD27 agonist antibody. The improved efficacy of the combination treatment was correlated with increases in the number of effector T cells, and the ratio of effector T cells to regulatory T cells (Teff:Treg) within the tumor. Based on these data, a Phase I trial using the approved monotherapy dose of nivolumab (3 mg/kg every 2 weeks) and escalating doses of varlilumab (0.1, 1, or 10 mg/kg every 2 weeks) in patients with advanced cancer was initiated. The primary objective was to assess the safety and tolerability of the combination. Extensive biomarker analysis was implemented to help in selection of the varlilumab dose for the upcoming Phase II portion of the trial, in which preliminary antitumor activity, as measured by ORR, of the combination treatment will be assessed. Varlilumab dose escalation has been completed and a total of 33 patients have been dosed (18 colorectal cancer, 8 ovarian cancer, 4 melanoma and 3 head and neck squamous cell carcinoma). The maximum dose level of 10 mg/kg was reached with no maximum tolerated dose identified. One patient with ovarian cancer in the 10 mg/kg cohort had a dose limiting toxicity of hepatitis (grade 4) and acute kidney injury (grade 3). Another patient with colorectal cancer in the 10 mg/kg cohort had paresthesias (grade 2) and muscle weakness (grade 2), no additional drug related SAEs or DLTs were reported. Toxicity has been consistent with the safety profile of each agent individually and no unexpected toxicities have been seen with the combination. Additional treatment-related AEs have included infusion reactions, lymphopenia, fatigue, nausea, vomiting, rash, and elevated pancreatic enzymes, with the majority of AEs being low grade. Preliminary biomarker analysis suggests a strong immunologic signature in patients across different dose cohorts. In particular, we observed consistent increases in the levels of serum pro-inflammatory cytokines/chemokines including CXCL9, CXCL10, MIP-1b, and MCP that were generally transient and peaking within several hours of dosing.
