Since the International Research Institute for Nuclear Decommissioning (IRID) was established as a technology research association in August 2013, it has been engaged in research and development (R&D) for decommissioning the Fukushima Daiichi nuclear power plant, which is currently an urgent issue, to strengthen the platform for decommissioning technology for the future. The work of IRID R&D is classified into three main pillars: removal of spent nuclear fuel from the pool; retrieval of fuel debris; and technological development for treatment and disposal of solid radioactive waste. This article describes an overview of R&D as of the first half of the fiscal year 2020, mainly focusing on investigation inside primary containment vessels and retrieval of fuel debris.

We developed a new interactive web-based teaching material to improve lung auscultation skills. Our objective was to investigate the effectiveness of the web-based teaching material on nurses with less than one-year work experience, using a prospective, open-label, stratified block randomized controlled trial. Of the 69 participants, 23, 22, and 24 participants were assigned to the web-based, paper-based, and control (with no intervention) groups, respectively. Using a simulator, a discrimination test on seven lung sounds, such as "normal," "wheeze," "rhonchi," "coarse crackles," "fine crackles," "left lung diminish," and "right lung absent," was conducted. Next, a post-test was conducted after one-week of training. Answers with formal names were considered "correct"; those with common names, misspellings, and without left and right parts were considered "insufficient"; and wrong answers were considered "incorrect." The control group showed no significant difference between the pre-test and post-test for any lung sounds. The paper-based group showed significant improvement in performance for "wheeze" (

Japanese patients often seek hospital services without a primary provider’s referral. A triage nurse who is the initial point of contact for a patient is challenged with the task of expertly evaluating the urgency of the condition and selecting the appropriate specialty service for every patient’s needs. A triage nurse must also recognize any conditions requiring emergency medical services instead of a specialty outpatient service. A modified Delphi method was used to establish expert consensus regarding triage nursing competencies for secondary and higher-level health care facilities in Japan. The initial Delphi round was completed using a questionnaire of 80 competencies that were evaluated by 85 Japanese nurse experts with in-depth knowledge of triage and/or the current Japanese hospital system. Four additional competency items were added based on the experts’ suggestions for a total of 84 items. The experts rated these items on a 7-point Likert scale based on importance. Minimal attrition rate yielded consistent and rich results. The results were analyzed to identify items rated as very important by the majority. Twenty-two items were included in the final list of competencies. The authors then refined the language and reorganized the items into four proposed domains. The proposed domains and the refined list of competencies provide a foundation for the development of training programs for outpatient triage nurses in the current Japanese health care system.

Purpose The study purpose was to investigate whether years of clinical experience and experience of encountering heart failure affect perceptions of the need for physical assessment, and the nature of the association between perceptions and years of clinical experience. Methods This study was survey-based. Of hospitals in Japan with ≥200 beds, 298 hospitals were randomly selected as candidate research sites. Ultimately, 44 hospitals agreed to participate in this study. A self-administered questionnaire measuring 48 physical assessment skills was developed and distributed to nurses. The questionnaire rated the perceived need for 48 skills assessing acute progression of heart failure. Results Of 1,113 nurses sampled, 373 (33.5%) responded. Of these, 347 completed questionnaires (response rate: 93.0%). Nurses' perceived need for skills involving systematic or cardiovascular system assessment decreased with clinical experience. Nurses' perceived need for skills involving cardiovascular symptoms and circulatory failure assessment increased with clinical experience. We found four patterns of change for nurses' perceptions of physical assessment skills according to clinical experience. Conclusions The findings showed that differences in nurses' perceived need for physical assessment skills were associated with differences in clinical experience. Nurses with more experience selected more suitable skills for detecting acute progression of heart failure. Nurses' perceptions of the need for specific skills were proportional to years of clinical experience, and there may be a threshold value of years of experience.

This study aimed to develop a reliable and valid measure of functional health literacy in a Japanese clinical setting. Test development consisted of three phases: generation of an item pool, consultation with experts to assess content validity, and comparison with external criteria (the Japanese Health Knowledge Test) to assess criterion validity. A trial version of the test was administered to 535 Japanese outpatients. Internal consistency reliability, calculated by Cronbach's alpha, was 0.81, and concurrent validity was moderate. Receiver Operating Characteristics and Item Response Theory were used to classify patients as having adequate, marginal, or inadequate functional health literacy. Both inadequate and marginal functional health literacy were associated with older age, lower income, lower educational attainment, and poor health knowledge. The time required to complete the test was 10-15 min. This test should enable health workers to better identify patients with inadequate health literacy.

Objective: This study aimed to investigate the effect of an educational program on physical assessment skills for registered nurses focusing on respiratory and cardiovascular systems.Methods: Design: Survey research using a self-administered questionnaire was used. Methods: The study was conducted in the physical assessment education program and the clinical settings. The study involved 104 registered nurses who completed a self-administered questionnaire, distributed immediately and 2 months after the physical assessment education program. The data were analyzed using the Wilcoxon signed-rank test.Results: The usage frequencies of 19 physical assessment skills increased after the educational program. The most frequently cited barrier to using these skills changed from a lack of knowledge to a lack of confidence and insufficient time. Before the program, the hospital nurses used their physical assessment skills more frequently than the home-visit nurses, but the reverse became true afterward. Nurses who recognized that they needed physical assessment skills that were learned through education showed a tendency to use these skills in nursing practice.Conclusions: These results suggest that the educational program on physical assessment skills affected the use of these skills in clinical practice for this study cohort for this study cohort.

Wire Mesh Tomography (WMT), which is system to measure two-phase flow, has been developed in our laboratory. Measurement principle of WMT is detecting conductivity difference between gas and liquid. WMT measures void fraction as raw date, and calculates gas velocity and bubble volume etc. In this paper, this measurement technique was applied to vertical circular pipe of 50mm diameter and about 7m heights. New Wire Mesh Sensor (WMS), which is measurement part of WMT, for circular pipe, have been made. When experiment was performed, superficial gas and water velocity. The effect of each flow parameter was found for void fraction, true gas velocity and bubble volume and the results was in good agreement with the past research, qualitatively.

An acid sialidase [EC 3.2.1.18], partially purified from human placenta by Con A-Sepharose adsorption and p-aminophenyl thio-beta-D-galactoside-CH-Sepharose (PATG-Sepharose) affinity chromatographies, was activated by incubation at 37 degrees C. This activation showed both time and temperature dependencies, with the most effective activation observed at 37 degrees C in the pH range between 4.3 and 5.2. The influence of various protease inhibitors on its activation was investigated. Among the protease inhibitors tested, amastatin, an inhibitor of aminopeptidase A, significantly inhibited activation. The partially purified enzyme preparation contained aminopeptidase activity, which was inhibited by amastatin. Zinc ions inhibited either the activation of sialidase or the aminopeptidase activity in the enzyme preparation. These results suggest the possibility of participation of aminopeptidase function in the activation process of sialidase.

Prosaposin, a 66 kDa glycoprotein, was identified initially as the precursor of the sphingolipid activator proteins, saposins A-D, which are required for the enzymatic hydrolysis of certain sphingolipids by lysosomal hydrolases. While mature saposins are distributed to lysosomes, prosaposin exists in secretory body fluids and plasma membranes. In addition to its role as the precursor, prosaposin shows a variety of neurotrophic and myelinotrophic activities through a receptor-mediated mechanism. In studies in vivo, prosaposin was demonstrated to exert a variety of neuro-efficacies capable of preventing neuro-degeneration following neuro-injury and promoting the amelioration of allodynia and hyperalgesia in pain models. Collective findings indicate that prosaposin is not a simple house-keeping precursor protein; instead, it is a protein essentially required for the development and maintenance of the central and peripheral nervous systems. Accumulating evidence over the last decade has attracted interests in exploring and developing new therapeutic approaches using prosaposin for human disorders associated with neuro-degeneration. In this review we detail the structure characteristics, cell biological feature, in vivo efficacy, and neuro-therapeutic potential of prosaposin, thereby providing future prospective in clinical application of this multifunctional protein.

In Japan, more people require care activities at home. In particular, patients with an invasive mechanical ventilator in the home require extensive care by family caregivers. Collaboration between their nurses and paid caregivers in the provision of care activities, especially tracheal suctioning, is necessary. This three-round Delphi study identifies the essential items required by nurses to instruct the paid caregivers in how to carry out tracheal suctioning on patients with an invasive mechanical ventilator in the home. By the final round, three competencies were found to be important by the home-visit nurses and paid caregivers: the anticipation of risk, the manner of handling an emergency situation, and the observation of a patient's breathing. The Delphi technique was used to obtain consensus between the home-visit nurses and the paid caregivers regarding the essential knowledge, skills, and attitudes required to carry out tracheal suctioning for patients with an invasive mechanical ventilator in the home.

The purpose of this study was to establish the minimum essential Japanese nursing education health assessment competency levels, with particular emphasis on the respiratory system, from the viewpoint of clinical nurses and nurse educators. The design of this study was a quantitative descriptive study using the Delphi technique. The participants consisted of two groups, including 210 clinical nurses and 76 nurse educators. Questionnaires were sent three times to each group, along with summaries of each previous result. The respondents selected 29 competency items in the following categories: five structure and function items; six interview items; eight inspection items; one palpation item; six auscultation items; and three diagnostic test items. By releasing the results of this survey to nurse educators, it might be possible to improve the nursing curriculum. For clinical nurses, knowing these results will help them to bridge the gap between essential nursing education and clinical practice.

The objective of this experimental study is to clarify the heat transfer characteristics of the Passive Containment Cooling System (PCCS) with vertical heat transfer tubes for investigating the influence of non-condensable gas on condensation. Furthermore, hence we obtained new experimental correlation formula to calculate the transients in system temperature and pressure using the simulation program of the PCCS. The research was carried out using a forced circulation experimental loop, which simulates atmosphere inside PCCS with vertical heat transfer tubes if a loss of coolant accident (LOCA) occurs. The experimental facility consists of cooling water supply systems, an orifice flowmeter, and a tank equipped with the heat transfer pipe inside. Cooling water at a constant temperature is injected to the test part of heat transfer pipe vertically installed in the tank by forced circulation. At that time, the temperature of the cooling water between inlet and outlet of the pipe was measured to calculate the overall heat transfer coefficient between the cooling water and atmosphere in the tank. Thus, the heat transfer coefficient (hc ) between heat transfer surface and the atmosphere in the tank considering the influence of the non-condensable gas was clarified. An important finding of this study is that the amount of condensation in the steamy atmosphere including non-condensable gas depends on the cooling water Reynolds number, especially the concentration of non-condensable gas that has great influence on the amount of condensation.Copyright © 2004 by ASME

Small-to-medium-sized (300–600MWe) reactors are required for the electric power market in the near future (2010–2030). The main theme in the development of small-to-medium-sized reactor is how to realize competitive cost against other energy sources. As measures to this disadvantage, greatly simplified and downsized design is needed. From such point of view, Integrated Modular Water Reactor (IMR), which electric output power is 350 MWe, adopts integrated and high temperature two-phase natural circulation system for the primary system. In this design, main coolant pipes, a pressurizer, and reactor coolant pumps are not needed, and the sizes of a reactor vessel and steam generators are minimized. Additionally, to enhance the economy of the whole plant, fluid system, and Instrumentation & Control system of IMR have also been reviewed to make them simplest and smallest taking the advantage of the IMR concept and the state of the art technologies. For example, the integrated primary system and the stand-alone direct heat removal system make the safety system very simple, i.e., no injection, no containment spray, no emergency AC power, etc. The chemical and volume control system is also simplified by eliminating the boron control system and the seal water system of reactor coolant pumps. In this paper, the status of the IMR development and the outline of the IMR design efforts to achieve the simplest and smallest plant are presented.

Prosaposin, the precursor of saposins or saps, is an injury-repair protein that acts on both neurons and glia. Previous studies identified the prosaposin gene as one of differentially expressed genes following nerve injury. In the present study, we investigated expression of prosaposin mRNA in injured brain utilizing rat models of focal cerebral ischemia and cortical stab wound in order to explore the significance of prosaposin in nerve injury. In ischemic brain, the level of prosaposin mRNA was elevated greater than 400% over controls within 5 days after ischemic insults. Importantly, this induction was accompanied by a 9-base splicing consistent with the alternative Exon-8 splicing of human prosaposin mRNA. In normal brain, two prosaposin mRNA species with and without the 9-base insertion were expressed at a ratio of 85:15; however, this equilibrium reverted to 5:95 following ischemic injury. Similar inductions were observed in stab wound brains. Immunohistochemical staining and in situ hybridization demonstrated an enhanced signal distribution of prosaposin mRNA and injury-induced prosaposin protein around the lesion. The data suggest the expression and processing of prosaposin mRNA may be crucially regulated not only for cerebral homeostasis but also during nerve regenerative and degenerative processes.

In Japan, each person is obligated to enroll in a national health insurance plan. Based on egalitarianism, the system is managed by the government and it finances almost the entire healthcare needs. However low the total healthcare expenditure in Japan is compared with other major countries, the statistics cause concern for the Japanese population for the future growth of its health care. Even though the Japanese healthcare system faces many problems at present, it provides everyone with a sense of security. The key to its success is the sense of and art of balance in healthcare policy making.

We report a new Japanese family of sialidosis type I. The sialidase activity was deficient in the lymphocytes of 2 patients (6.8% (sister) and 12.5% (brother) of control mean). However, surprisingly, using the transformed lymphocytes by EB virus, this activity was activated to 51.7% (sister) and 49.5% (brother) of control mean, respectively. Although the mechanism for this activation was not known, we discussed the possible mechanisms for this phenomenon.

Two members of a consanguineous Japanese family with a clinical picture of aspartylglycosaminuria (AGU) are described. Both patients exhibited mental retardation, coarse facial features, angiokeratoma and myoclonic seizures. Biochemical studies showed elevated excretion of urinary sialyloligosaccharides and decreased activity of aspartylglycosaminidase in lymphoblasts. Morphologic studies of skin biopsy specimens showed many clear vacuoles mainly in the vascular endothelial cells and secretory cells of the sweat glands. Osmiophilic lamellar cytoplasmic inclusions were also noted in the ganglion cells in rectal biopsy. The ethnic distribution of AGU may be more widespread than previously suspected and appears not to be restricted to Finnish populations. Ours are the first Japanese patients diagnosed as AGU. We conclude that AGU should also be included in the differential diagnosis of mentally retarded patients in Asian countries.

Two species of cDNAs for human α-N-acetylgalactosaminidase were isolated from a human fibroblast cDNA library. The two species differ each other by a 70 bp insertion in the coding region. Transient expression study in COS cells demonstrated that only the cDNA without the 70 bp insertion expressed α-N-acetylgalactosaminidase activity. Analysis of mRNA species utilizing polymerase chain reaction revealed that the majority of the mRNA does not contain the 70 bp insertion, and the mRNA containing the 70 bp insertion is present only in a minor amount in human brain.

Stereospecific synthetic routes to methyl (R)-7-hydroxy-(2E,4E)-octadienoate (9) and its (2E,4Z)-isomer (16), the former which can be a useful chemical precursor to the 16-membered-ring aglycon of carbomycin B (carbonolide B), are discussed. Compound 9, corresponding to its C-9–C-16 segment, was synthesized starting with methyl 2-deoxy-α-D-arabion-hexopyranoside (2). The deoxysugar 2 was converted to the masked 2,4,6-trideoxy-D-threo-hexopyranose (6) via the 4,6-dichloro-2,4,6-trideoxy-α-D-lyxo-hexopyranoside derivative. Treatment of 6 with p-toluenesulfonyl chloride and triethylamine afforded 3-O-acetyl-1,5-anhydro-2,4,6-trideoxy-D-threo-hex-1-enitol, which by reaction with mercury(II) acetate was converted to (R)-5-hydroxy-(2E)-hexenal (8). Reaction of 8 with Wittig reagent gave the (2E,4E)-isomer 9. Compound 16 was prepared by the Wittig reaction with the 2,3-unsaturated deoxysugar, 2,3,4,6-tetradeoxy-D-glycero-hex-2-enopyranose, which was derived from methyl 2,3-anhydro-4,6-dideoxy-α-D-lyxo-hexopyrano...

In the process of molecular cloning of cDNA for proteins associated with a purified human placental sialidase fraction, we discovered one of the proteins with apparent molecular weight of 46 kDa is in reality alpha-N-acetylgalactosaminidase. The full length cDNA, pcD-HS1204, codes for 358 amino acids with the first 17 residues representing a putative signal peptide. The predicted amino acid sequence shows striking homology with human alpha-galactosidase A and yeast alpha-galactosidase. The substrate specificities as well as the behavior of the 46 kDa protein on hydroxylapatite chromatography confirmed that the 46 kDa protein is in reality alpha-N-acetylgalactosaminidase.
