Abstract

An important local vanishing theorem for the minimal model program is the fact that klt singularities in characteristic zero are Cohen-Macaulay. In contrast, even in the narrow setting of terminal singularities of dimension 3, we show that Cohen-Macaulayness can fail in characteristic $p$ or mixed characteristic $(0,p)$ for $p$ equal to 2, 3, or 5. This is optimal, by work of Arvidsson-Bernasconi-Lacini. The examples are quotients of regular schemes by the cyclic group $G$ of order $p$. In characteristic $p$ or mixed characteristic, such quotients can exhibit a wide range of behavior. Data on elderly patients, in particular, octogenarians, are lacking. Paradoxically, this population is the one with the highest risk of bleeding and stroke with a worse prognosis. This study aims to describe safety and effectiveness of NOACs in an elderly comorbid population.Patients and methods: REGIstry of patients on Non-vitamin K oral Anticoagulants (REGINA) is a prospective observational study enrolling consecutive NVAF patients started on NOACs and followed up to 1 year (at 1, 6, 12 months). The primary endpoint was the incidence rate of major bleeding (MB) and clinically relevant non-major bleeding (CRNMB). The secondary endpoints were the incidence of 1) stroke or systemic embolism, 2) hospitalization, 3) death, and 4) drug-related adverse events.Results: We enrolled 227 patients aged 81.6±6.1 years (range 67–95 years; ≥80 years in 59.4%). The median CHA2DS2-VASc was 5 (IQR 4–5) and HAS-BLED was 4 (IQR 3–5). The estimated glomerular filtration rate was 59.27±24.12 mL/min. During follow-up, only 10 MB and 23 CRNMB occurred, with a total incidence of 4.4% (95% CI: 1.7%–7.17%) and 5.7% (95% CI: 2.68%–8.72%), respectively. There were 2 cerebral ischemic events, with a total incidence of 0.88% (95% CI: 0.84%–0.92%), 23 NOAC-related hospitalizations, no NOAC-related deaths, and 4 minor drug-related adverse effects.Conclusion: In a population of aged and clinically complex patients, mainly octogenarians, NOACs were safe and effective. 12. 3D seismic velocity models from local earthquake tomography furnish new insights on the Mount Etna volcano (Southern Italy)

Abstract

Abstract We present a new seismotomography investigation providing a 3-D overall model of Vp, Vs and Vp/Vs for Mt. Etna, the largest and most active volcano in Europe. We estimated and jointly evaluated P- and S-wave velocity patterns together with the Vp/Vs ratio, particularly useful to discriminate the presence of groundwater, gas, and melts and thus very precious for volcano investigations. We applied the LOTOS software to ~ 4600 crustal earthquakes that occurred in the Etnean area during the last 26 years, the longest time-interval ever analysed for Mt. Etna. This wide dataset has allowed us to characterize the volcano velocity structure getting over possible singularities due to specific eruptive phases. Our results further refined the high velocity body widely recognized in the south-eastern sector of Mt. Etna by furnishing new clues on the possible former magma pathways. Moreover, the obtained 3D seismic velocity model depicted new anomalies revealing the presence of: (i) two shallow underground aquifers in the northern Etnean sector; (ii) a volume of strongly fractured rocks filled of fluids along the eastern flank; (iii) a quite deep region of probable fluid accumulation apparently not linked to the volcanic activity in the western sector.

13. Seismic slip channeling along the East Anatolian Fault illuminates long-term supercycle behavior

Abstract

Abstract The two Mw > 7.5 earthquakes that struck the East Anatolian Fault (EAF), Türkiye, in 2023 caused more slip than expected, indicating that they were potentially part of a supercycle, in which the occurrence probability of a large earthquake is determined by accumulated strain rather than time since the last large earthquake. Here, we show two potential supercycles along the EAF, analyzing earthquakes from the last two millennia. Within each supercycle, seismic ruptures originated in the northeast and progressively spread southwestward with an increasing number of earthquakes until a new supercycle began with another large earthquake in the northeast. To understand the supercycle behavior, we analyze the aftershock sequences of the four most recent Mw≥6.1 mainshocks (2010–2023). This series of earthquakes progressed southwestward, characterized by an increasing diversity of focal mechanisms and a heightened dispersion of epicenters across a branched seismotectonic environment. Earthquakes in the northeast exhibit spatial and kinematic channeling along the master fault surface, effectively transferring slip southwestward and there potentially triggering dispersed and heterogeneous earthquakes. This spatiotemporal pattern seems connected with varying levels of a presumably-innate property of fault sections or regions, ruling the process of seismic slip channeling, which could also explain the behavior of long-term supercycles.

14. Detracking Math: Transforming Teaching and Learning

Abstract

High-performing suburban districts are often looked to as a model for teaching and learning. The status quo, for many students, indicated future success. However, when the authors critically examined their structures and practices, they saw places where they perpetuated inequities. Specifically, these practices included tracking students in mathematics starting in fourth grade. Students' academic careers were decided at the age of eight. This had to stop. Disruptive change, when it targets cultural norms, perceived and real biases, and underlying issues of equity such as race, ethnicity, gender, and ableism, require a systems approach. The purpose of this article is to share what the leaders, teachers, coaches, supervisors, and building administrators did to successfully detrack elementary mathematics. In this article, the authors share the steps they took, the frameworks they relied on, and the lessons learned to empower teachers and students.

Abstract

We construct log canonical pairs $(X,B)$ with $B$ a nonzero reduced divisor and $K_X+B$ ample that have the smallest known volume. We conjecture that our examples have the smallest volume in each dimension. The conjecture is true in dimension 2, by Liu and Shokurov. The examples are weighted projective hypersurfaces that are not quasi-smooth. We also develop an example for a related extremal problem. Esser constructed a klt Calabi-Yau variety which conjecturally has the smallest mld in each dimension (for example, mld $1/13$ in dimension 2 and $1/311$ in dimension 3). However, the example was only worked out completely in dimensions at most 18. We now prove the desired properties of Esser's example in all dimensions (in particular, determining its mld)., Comment: 29 pages; v3: minor updates

Abstract

Kawakami and the author showed that a projective variety with an int-amplified endomorphism of degree invertible in the base field satisfies Bott vanishing. That was a new way to analyze which varieties have nontrivial endomorphisms. In this paper, we extend that result to a logarithmic version of Bott vanishing for an endomorphism with a totally invariant divisor. We apply this to Fano 3-folds. Meng-Zhang-Zhong showed that the only smooth complex Fano 3-folds that admit an int-amplified endomorphism are the toric ones. Also, Achinger-Witaszek-Zdanowicz showed that the only smooth complex Fano 3-folds that are images of toric varieties are the toric ones. Abstract

Abstract Background The reversed halo, also known as the atoll sign, is a distinct sign with ring-shaped consolidation and central lucency. The reversed halo sign, initially emerging in cryptogenic organizing pneumonia, has been observed in various pulmonary diseases, including pulmonary embolism. The presence of this sign in the subpleural, posterior basal parts of the lower lobes, predominantly when solitary, should raise the possibility of other causes, such as pulmonary infarction. Case presentation We present a case of acute pulmonary embolism with pulmonary infarction discovered on a CT pulmonary angiogram in our emergency department, where the pulmonary infarction appears as a reverse halo sign with a reticulonodular pattern. The reticular nodular pattern in the halo sign is not typical of a pulmonary infarct presenting with a reverse halo sign. Conclusions To our knowledge, this is the first case of pulmonary infarction in acute pulmonary embolism with the appearance of a reverse halo sign with a reticulonodular pattern.

Abstract

Abstract Refractory out-of-hospital cardiac arrest (OHCA) has a very poor prognosis, with survival rates at around 10%. Extracorporeal membrane oxygenation (ECMO) for patients in refractory arrest, known as ECPR, aims to provide perfusion to the patient whilst the underlying cause of arrest can be addressed. ECPR use has increased substantially, with varying survival rates to hospital discharge. The best outcomes for ECPR occur when the time from cardiac arrest to implementation of ECPR is minimised. To reduce this time, systems must be in place to identify the correct patient, expedite transfer to hospital, facilitate rapid cannulation and ECMO circuit flows. We describe the process of activation of ECPR, patient selection, and the steps that emergency department clinicians can utilise to facilitate timely cannulation to ensure the best outcomes for patients in refractory cardiac arrest. With these processes in place our survival to hospital discharge for OHCA patients is 35%, with most patients having a good neurological function.

Abstract

Abstract Background Ileocolic intussusception is a rare condition that is difficult to diagnose. A pathological lead point, usually malignant, is often the cause of adult intussusception. In emergency scenarios, radiologists must be able to identify this entity and its causes and express their opinions on associated complications. Case presentation The hospital's emergency room admitted a 75-year-old woman who was suffering from intermittent abdominal pain and constipation. An abdominal contrast-enhanced computed tomography revealed a long giant ileocolic intussusception. Considering the clinical stability and the absence of signs of complications on CT, the patient did not undergo an urgent surgical procedure but underwent a colonoscopy the following day. Surprisingly, the colonoscopy highlighted the spontaneous resolution and revealed the presence of the three tumour lead points in the cecum and ascending colon. A CT was performed after the colonoscopy, confirming the intussusception's resolution. The surgeon performed a right oncological hemicolectomy. The malignant lead points on histological examination were moderate to poorly differentiated adenocarcinomas. Conclusions A giant and long ileocolic intussusception with a long embedded visceral segment (the python colon sign) could suggest pathological lead points in the cecum and ascending colon. Despite the large size, a spontaneous resolution of the python colon is possible. In this scenario, even if, on imaging, it is impossible to macroscopically identify a lead point, in cases of giant and long ileocolic intussusceptions, every effort must be undertaken to search for the cause of the intussusception.

Abstract

We show that a projective variety with an int-amplified endomorphism of degree invertible in the base field satisfies Bott vanishing. This is a new way to analyze which varieties have nontrivial endomorphisms. In particular, we extend some classification results on varieties admitting endomorphisms (for Fano threefolds of Picard number one and several other cases) to any characteristic. The classification results in characteristic zero are due to Amerik-Rovinsky-Van de Ven, Hwang-Mok, Paranjape-Srinivas, Beauville, and Shao-Zhong. Our method also bounds the degree of morphisms into a given variety. Abstract

Bott proved a strong vanishing theorem for sheaf cohomology on projective space, namely that $H^j(X,\Omega^i_X\otimes L)=0$ for every $j>0$, $i\geq 0$, and $L$ ample. This holds for toric varieties, but not for most other varieties. We classify the smooth Fano 3-folds that satisfy Bott vanishing. There are many more than expected. BackgroundGender differences in the access to advanced therapies for Parkinson’s disease (PD) are poorly investigated.ObjectiveThe objective of this study was to investigate the presence of any gender disparity in the access to advanced therapies for PD.DesignRetrospective study.MethodsData from patients with consistent access to the Parkinson’s and Movement Disorder Center of L’Aquila over the last 10-year period were screened. Patients selected for advanced therapies were included.ResultsOut of 1,252 patients, 200 (mean age ± SD 71.02 ± 9.70; 72% males; median Hoen Yahr level: 3, minimum 1 maximum 5) were selected for advanced therapies: 133 for Magnetic Resonance guided Focused Ultrasound (MRgFUS) thalamotomy (mean age ± SD 70.0 ± 8.9; 77% males), 49 for Levodopa/Carbidopa Intestinal Gel (LCIG) infusion (mean age ± SD 74.3 ± 11.4; 59% males), 12 for Deep Brain Stimulation (DBS) (mean age ± SD 71.2 ± 6.3; 75% males), and 7 for Continuous Subcutaneous Apomorphine Infusion (CSAI) (mean age ± SD 69.7 ± 5.5; 43% males). Abstract

OBJECTIVES:. Anemia has been associated with an increased risk of both cardiac arrest and stroke, frequent complications of COVID-19. The effect of hemoglobin level at ICU admission on a composite outcome of cardiac arrest or stroke in an international cohort of COVID-19 patients was investigated. DESIGN:. Retrospective analysis of prospectively collected database. SETTING:. A registry of COVID-19 patients admitted to ICUs at over 370 international sites was reviewed for patients diagnosed with cardiac arrest or stroke up to 30 days after ICU admission. Anemia was defined as: normal (hemoglobin ≥ 12.0 g/dL for women, ≥ 13.5 g/dL for men), mild (hemoglobin 10.0–11.9 g/dL for women, 10.0–13.4 g/dL for men), moderate (hemoglobin ≥ 8.0 and < 10.0 g/dL for women and men), and severe (hemoglobin < 8.0 g/dL for women and men). PATIENTS:. Patients older than 18 years with acute COVID-19 infection in the ICU. INTERVENTIONS:. None. MEASUREMENTS AND MAIN RESULTS:. Of 6926 patients (median age = 59 yr, male = 65%), 760 patients (11.0%) experienced stroke (2.0%) and/or cardiac arrest (9.4%). Cardiac arrest or stroke was more common in patients with low hemoglobin, occurring in 12.8% of patients with normal hemoglobin, 13.3% of patients with mild anemia, and 16.7% of patients with moderate/severe anemia. Time to stroke or cardiac arrest by anemia status was analyzed using Cox proportional hazards regression with death as a competing risk. Covariates selected through clinical knowledge were age, sex, comorbidities (diabetes, hypertension, obesity, and cardiac or neurologic conditions), pandemic era, country income, mechanical ventilation, and extracorporeal membrane oxygenation. Moderate/severe anemia was associated with a higher risk of cardiac arrest or stroke (hazard ratio, 1.32; 95% CI, 1.05–1.67). CONCLUSIONS:. In an international registry of ICU patients with COVID-19, moderate/severe anemia was associated with increased hazard of cardiac arrest or stroke.

36. Klt varieties of conjecturally minimal volume

Abstract

We construct klt projective varieties with ample canonical class and the smallest known volume. We also find exceptional klt Fano varieties with the smallest known anticanonical volume. We conjecture that our examples have the smallest volume in every dimension, and we give low-dimensional evidence for that. In order to improve on earlier examples, we are forced to consider weighted hypersurfaces that are not quasi-smooth. We show that our Fano varieties are exceptional by computing their global log canonical threshold (or $\alpha$-invariant) exactly; it is extremely large, roughly $2^{2^n}$ in dimension $n$. Abstract

The Witt ring of symmetric bilinear forms over a field has divided power operations. On the other hand, it follows from Garibaldi-Merkurjev-Serre's work on cohomological invariants that all operations on the Witt ring are essentially linear combinations of exterior powers. We find the explicit formula for the divided powers as a linear combination of exterior powers. The coefficients involve the ``tangent numbers'', related to Bernoulli numbers. Abstract

Call a projective variety $X$ Calabi-Yau if its canonical divisor is ${\bf Q}$-linearly equivalent to zero. The smallest positive integer $m$ with $mK_X$ linearly equivalent to zero is called the index of $X$. We construct Calabi-Yau varieties with the largest known index in high dimensions. In our examples, the index grows doubly exponentially with dimension. We conjecture that our examples have the largest possible index, with supporting evidence in low dimensions. The examples are obtained by mirror symmetry from our Calabi-Yau varieties with an ample Weil divisor of small volume. Abstract

Abstract Background Literature suggests colonic resection and primary anastomosis (RPA) instead of Hartmann’s procedure (HP) for the treatment of left-sided colonic emergencies. We aim to evaluate the surgical options globally used to treat patients with acute left-sided colonic emergencies and the factors that leading to the choice of treatment, comparing HP and RPA. Methods This is a prospective, international, multicenter, observational study registered on ClinicalTrials.gov. A total 1215 patients with left-sided colonic emergencies who required surgery were included from 204 centers during the period of March 1, 2020, to May 31, 2020. with a 1-year follow-up. Results 564 patients (43.1%) were females. The mean age was 65.9 ± 15.6 years. HP was performed in 697 (57.3%) patients and RPA in 384 (31.6%) cases. Complicated acute diverticulitis was the most common cause of left-sided colonic emergencies (40.2%), followed by colorectal malignancy (36.6%). Abstract

International Guidelines as well as Cancer Associations recommend a multidisciplinary approach to lung cancer care. A multidisciplinary team (MDT) can significantly improve treatment decision-making and patient coordination by putting different physicians and other health professionals “in the same room”, who collectively decide upon the best possible treatment. However, this is not a panacea for cancer treatment. The impact of multidisciplinary care (MDC) on patient outcomes is not univocal, while the effective functioning of the MDT depends on many factors. This review presents the available MDT literature with an emphasis on the key factors that characterize high-quality patient care in lung cancer. The study was conducted with a bibliographic search using different electronic databases (PubMed Central, Scopus, Google Scholar, and Google) referring to multidisciplinary cancer care settings. Many key elements appear consolidated, while others emerge as prevalent and actual, especially those related to visible barriers which work across geographic, organizational, and disciplinary boundaries. MDTs must be sustained by strategic management, structured within the entity, and cannot be managed as a separate care process. Furthermore, they need to coordinate with other teams (within and outside the organization) and join with the broad range of services delivered by multiple providers at various points of the cancer journey or within the system, with the vision of integrated care.

Abstract

Abstract Background The persistently patent ductus arteriosus represents a well-known common congenital heart defect; it is uncommon in adult patients, and in any case, it debuts with atrial fibrillation. Case presentation A 75-year-old woman suffering from persistent ductus arteriosus (PDA) was admitted to the cardiology department because of atrial fibrillation, dyspnea and exercise intolerance. A PDA was detected on echocardiography and globally assessed through ECG-gated CT angiography. Conclusions Patent ductus arteriosus is an uncommon clinical finding in adulthood, and atrial fibrillation, as a consequence of chronic, progressive left atrial enlargement, may be the initial symptom. We describe the ECG-gated CT angiography imaging features of unknown patent ductus arteriosus in an elderly patient who debuted with atrial fibrillation.

Abstract

We construct smooth projective varieties of general type with the smallest known volume and others with the most known vanishing plurigenera in high dimensions. The optimal volume bound is expected to decay doubly exponentially with dimension, and our examples achieve this decay rate. We also consider the analogous questions for other types of varieties. For example, in every dimension we conjecture the terminal Fano variety of minimal volume, and the canonical Calabi-Yau variety of minimal volume. In each case, our examples exhibit doubly exponential behavior.

Abstract: We construct klt projective varieties with ample canonical class and the smallest known volume. We also find exceptional klt Fano varieties with the smallest known anti-canonical volume. We conjecture that our examples have the smallest volume in every dimension, and we give low-dimensional evidence for that. In order to improve on earlier examples, we are forced to consider weighted hypersurfaces that are not quasi-smooth. We show that our Fano varieties are exceptional by computing their global log canonical threshold (or $\alpha $-invariant) exactly; it is extremely large, roughly $2^{2^n}$ in dimension $n$. These examples give improved lower bounds in Birkar’s theorem on boundedness of complements for Fano varieties.

Abstract

Background: B-lineage acute lymphoblastic leukemias (B-ALL) harboring the t(9;22)(q34;q11)/BCR::ABL1rearrangement, represent a category with previously dismal prognosis whose management and outcome dramatically changed thanks to the use of tyrosine kinase inhibitors (TKIs) usage and more recently full chemo-free approaches. The prompt identification of these cases represents an important clinical need. Objectives: We sought to identify an optimized cytofluorimetric diagnostic panel to predict the presence of Philadelphia chromosome (Ph) in B-ALLs cases by the introduction of CD146 in our multiparametric flow cytometry (MFC) panels. Methods: We prospectively evaluated a total of 245 cases of newly diagnosed B-ALLs with a CD146 positivity threshold >10% referred to the Division of Hematology of ‘Sapienza’ University of Rome. We compared results of CD146 expression percentage and its mean fluorescence intensity (MFI) between Ph+ ALLs, Ph-like ALLs and molecularly negative ALLs. Results: Seventy-nine of the 245 B-ALL cases (32%) did not present mutations at molecular testing, with 144/245 (59%) resulted Ph+ ALL and 19/245 (8%) Ph-like ALLs. Comparing the 3 groups we found that: Ph+ B-ALL were characterized by higher expression percentage of myeloid markers such as CD13, CD33 and CD66c and low expression of CD38; Ph+ B-ALL showed a higher CD146 expression percentage and MFI when compared with both molecular negative B-ALL and Ph-like ALLs; neither the mean percentage of CD146 expression neither CD146 MFI were statically different between molecular negative B-ALL and Ph-like ALLs. Conclusions: Our data demonstrate the association between CD146 expression and Ph+ B-ALLs. CD146 along with myeloid markers may help to identify a distinctive immunophenotypic pattern, useful for rapid identification in diagnostic routine of this subtype of B-ALLs that benefits from a specific therapeutic approach.

Abstract

Abstract Use of extracorporeal membrane oxygenation (ECMO) for cardiorespiratory failure remains complicated by blood clot formation (thrombosis), triggered by biomaterial surfaces and flow conditions. Thrombosis may result in ECMO circuit changes, cause red blood cell hemolysis, and thromboembolic events. Medical device thrombosis is potentiated by the interplay between biomaterial properties, hemodynamic flow conditions and patient pathology, however, the contribution and importance of these factors are poorly understood because many in vitro models lack the capability to customize material and flow conditions to investigate thrombosis under clinically relevant medical device conditions. Therefore, an ECMO thrombosis‐on‐a‐chip model is developed that enables highly customizable biomaterial and flow combinations to evaluate ECMO thrombosis in real‐time with low blood volume. It is observed that low flow rates, decelerating conditions, and flow stasis significantly increased platelet adhesion, correlating with clinical thrombus formation. For the first time, it is found that tubing material, polyvinyl chloride, caused increased platelet P‐selectin activation compared to connector material, polycarbonate. This ECMO thrombosis‐on‐a‐chip model can be used to guide ECMO operation, inform medical device design, investigate embolism, occlusion and platelet activation mechanisms, and develop anti‐thrombotic biomaterials to ultimately reduce medical device thrombosis, anti‐thrombotic drug use and therefore bleeding complications, leading to safer blood‐contacting medical devices.

Abstract

Treatment options for secondary progressive MS (SPMS) are limited, especially considering that the new drugs recently approved are licensed for actively relapsing patients. We aimed to compare the disability progression in a real-world cohort of SPMS patients treated with natalizumab (NTZ) or interferon beta-1b (IFNb-1b). This multicenter retrospective enrolled patients with a diagnosis of SPMS according to 2014 Lublin criteria, who received NTZ or IFNb-1b for at least 48 months between the 1st June 2012 and the 15th May 2018 ​at 33 Italian MS centers contributing to the Italian MS Registry NTZ or IFNb-1b. Confirmed Expanded Disability Status Scale worsening (CEW) and progression independent of relapse (PIRA) were evaluated. In order to correct for non-randomization, a propensity score matching of the groups was performed. Out of 5206 MS patients identified at the time of data extraction, 421 SPMS patients treated with NTZ (224 [53.2%] females, mean age 45.3 ​± ​25.4 years) and 353 with IFNb-1b (133 [37.8%] females, mean age 48.5 ​± ​19.8 years) were enrolled. After applying the matching procedure, 102 patients were retained in the NTZ group and 98 in the IFNb-2b group. The proportion of patients who reached the 48-month 1-point CEW was significantly higher in IFNb-1b compared to NTZ group (58.2% versus 30.4%, p ​= ​0.01). The proportion of patients who developed PIRA at 48 months were significantly higher in IFNb-1b compared to NTZ (72.4% versus 40.2%, p ​= ​0.01). EDSS before treatment initiation and SPMS duration were risk factors for disability progression in terms of PIRA (HR 2.54, 25%CI 1.67–5.7; p ​= ​0.006 and HR 2.04, 25%CI 1.22–3.35; p ​= ​0.01, respectively). Patients treated with IFNb-1b were 1.64 times more to likely to develop PIRA (HR 1.64, 25%CI 1.04–4.87; p ​= ​0.001). Treatment with NTZ in SPMS patients showed more favorable disability outcomes compared to IFNb-1b with beneficial effects over 48 months.
