Introduction:Ganglion cysts are the most common soft tissue masses seen on the wrist, which often cause pain or cosmetic complaints. The treatment of these masses includes intra-cystic injections or surgery. Recurrence rates are very high in surgical or non-surgical treatment. Inadequate excision for recurrence after surgery is blamed; however, the reasons for the recurrence still remain mysterious. Objectives:In this study, the effect of anesthesia selection and tourniquet use on the dorsal wrist ganglion cysts in open surgery was investigated. Materials and methods:Patients with dorsal wrist ganglion cysts, who were operated with open surgery between 2015 and 2018 and who had at least six months after the surgery, were examined. The patients were divided into two groups: patients who underwent surgery without tourniquet with local anesthesia and patients operated with tourniquet with general or regional anesthesia. Age, sex, cause of operation, visual analog scale (VAS) scores before and after surgery, limitation of movement, postoperative complications, and recurrence were compared. Results:There was no significant difference between the groups in terms of causes of surgery, recurrence rates, preoperative and postoperative limitations of movement, and complications. In terms of age, the group operated with local anesthesia and without tourniquet was significantly larger. There was also no significant difference between the groups in terms of preoperative pain. Postoperative pain was significantly less in the group operated by tourniquet with general-regional anesthesia. Conclusion:There is no significant difference in the recurrence and complications between patients operated under local anesthesia without tourniquets and patients operated with tourniquets under general or regional anesthesia during the open excision of the dorsal wrist ganglion cysts. However, it should be kept in mind that postoperative pain does not diminish in later ages, especially in cases of ganglion cysts, and other pathologies may also potentially cause pain in the wrist.

Objective: In this study, it was aimed to determine the incidence of the DDH inthe infants 0-9 months old. Materials and Methods: 620 hips of 301 infants with the age of 0-9 months were examined with ultrasonography by using the Graf method for DDH screening. Of 310 infants, 620 hips were evaluated according to the Graf's method. There were 137 boys and 173 girls of our patients. The relationships between physical signs or risk factors for DDH and hips which are dysplastic or dislocation, were assessed. The patients clinically or sonographically diagnosed as DDH in newborn period were reevaluated six weeks later. Results: As a results; according to the Graf method, of 620 hips, %6.93 were tip-Ia, %78.39 were tip-Ib, %9.52 were tip-IIa, %0.32 were tip-IIb, %1.45 were tip-IIc, %0.81 were tip-D, %2.1 were tip-III and %0.48 were tip-IV. In our study, the frequency of dysplasia was found %14.7 and the frequency of dislocation was found %3.4. Of %92.7, the new-borns had the Graf's type-II hip in newborn period was not brought to the control examination by their parents. Conclusion: Although parents were informed about DDH, most of them did not come for control USG examination was showed us that some belief still did not changed in our region and national health policy is inadequate. Ultrasonography is important for the early diagnosis and follow-up of DDH. To define the real frequency of the hip dysplasia, the control USG should be performed six weeks after the completion of hip development., Amaç: Bu çalışmada 0-9 aylık bebeklerde gelişimsel kalça displazisi (GKD) sıklığını belirlemek amaçlandı. Gereç ve Yöntemler: Kliniğimize başvuran 0-9 aylık 310 bebeğin 620 kalçası, Graf yöntemi kullanılarak ultrasonografik olarak incelendi. Olgularımızın 137'si (%44.2) erkek, 173'ü (%55.8) kızdı. Fizik muayene bulguları ve risk faktörleri ile displazik ve desentre kalçalar arasında ilişkiler değerlendirildi. Yenidoğan döneminde klinik veya sonografik olarak GKD tanısı konulan hastalar 6 hafta sonra tekrar değerlendirildi. Bulgular: İnceleme sonucunda, Graf metoduna göre sınıflandırıldığında; 620 kalçanın incelemesinde %6.93 tip Ia, % 78.39 tip Ib, %9.52 tip IIa, % 0.32 tip IIb, %1.45 tip IIc, %0.81 tip D, %2.1 tip III ve %0.48 tip IV kalça saptandı. Çalışmamızda displazi sıklığı %14.7, desentre kalça sıklığı ise %3.4 olarak bulundu. Yenidoğan döneminde Graf tip II kalça tespit edilenlerin %92.7'i ebeveynleri tarafından kontrol muayenesine getirilmedi. Sonuç: Gelişimsel kalça displazisi bölgemizde hala önemli bir sağlık sorunu olmasına rağmen GKD'li hastaların ebeveynleri hastaların takip ve tedavisi için yeterli ilgiyi göstermemektedirler. Gelişimsel kalça displazisinin erken tanı ve takibinde ultrasonografi önemlidir. Gerçek displazi sıklığını belirlemek için fizyolojik gelişimin tamamlandığı ilk altı haftadan sonra kontrol ultrasonografisinin yapılması gerekir.

Objective: Results of surgical treatment for unstable distal end radius fractures with articular involvement in adults as comparing with unlocked and locked anatomic plate by volar approach were evaluated. Materials and Methods: According to AO classification type C with 35 patient treated open reduction and two different volar plate fixation for distal end of the radius fractures retrospectively reviewed between January 2005 and May 2009. All type C fractures were evaluated 16 (% 46) C1, 9 (% 29) C 2, 10 (% 28) C3. The median follow-up period was 20 month (6-50). Volar locked plate treatment was used for 13 patient in Group 1 and volar unlocked anatomic plated was used 22 patient in Group 2. There were no statiscally differences in age, gender, fracture type among groups. Results: According to Gardland and Werley cilinically evaluation criters; excellent-good was % 86, fair was % 14 in group 1; excellent-good % 77, fair was % 23 in Group 2 respectively. Wrist range of motion and forearm rotation was similar statistically for 2 groups as were clinical scoring and grip strenght.According to radiological evaluation system of Steward et al. excellent-good were % 100; in Group 1 and % 91 in Group 2 respecti-vely. Althought there was no statistically differences among groups, but in locked plate group radiological improvement was better. Conclusion: Treatment of complex distal end fractures of radius involment articular with volar unlocked or locked plate provide good functional results and late period follow up there were no difference in terms of functional and radiologic results among two method. Locked anatomic plates provide good radiologic correction because of including multiple screws with low profile and allow to early joint movement because of rigidity., Amaç: Eklemi ilgilendiren instabil radius distal uç kırıklarının cerrahi tedavisinde volar girişimle uyguladığımız kilitsiz ve kilitli anatomik plakla tespit yöntemlerinin sonuçları değerlendirmek. Gereç ve Yöntem: Çalışmamızda Ocak 2005 ile Mayıs 2009 arasında AO sınıflandırmasına göre C grubunda yer alan radius distal uç kırığı nedeniyle açık redüksiyon ve iki değişik volar plak tespitiyle tedavi edilen erişkin 35 distal radius kırıklı hasta retrospektif olarak değerlendirildi. Hepsi Tip C olan kırıkların alt grupları 16 (% 46)'sı C1, 9 (% 26)'u C2, 10 (% 28)'u C3 şeklindeydi. Ortalama takip süresi 20 ay (6-50 ay) idi. Grup 1'deki 13 hastaya volar kilitsiz plak, Grup 2'deki 22 hastaya volar anatomik kilitli plak uygulandı. Gruplar arasında yaş, cins ve kırık tipi yönünden istatistik olarak anlamlı bir fark yoktu. Bulgular: Gartland ve Werley klinik değerlendirme kriterlerine göre Grup 1'de % 86 mükemmel-iyi, % 14 orta sonuç; Grup 2'de % 77 mükemmel-iyi, % 23 orta sonuç alındı. Her iki grup için eklem hareket açıları kavrama güçleri ve klinik değerlendirme sonuçları arasında istatistik olarak fark yoktu. Stewart radyolojik değerlendirmesine göre mükemmel-iyi sonuç Grup 1'de % 100, Grup 2'de % 91 olarak bulundu. Gruplar arasında istatistik olarak fark yoktu, ancak kilitli anatomik plak yapılan grupta radyolojik düzelme daha iyiydi. Sonuç: Radius distal uç kırıklarının eklemi ilgilendiren kompleks kırıklarının kilitsiz veya kilitli anatomik volar plaklarla tedavisinden iyi fonksiyonel sonuçlar alınmış, geç dönemde iki yöntem arasında fonksiyonel ve radyolojik sonuçlar yönünden fark bulunamamıştır. Ancak, kilitli anatomik plaklar distalde çok sayıda ve düşük profilli vida içerdiği için daha iyi radyolojik düzelme sağlamakta ve sıkılıkları nedeniyle erken harekete izin vermektedir.

Spinal epidural hematoma is a rare clinical disorder that might result in permanant neurological deficit and death when left untreated. We present a 59 year old male admitted to our emergency department with back pain falling to the walnut tree, and paraplegia, and being diagnosed with do not make the intra-channel compression in the T12 vertebral burst fractures and spinal epidural hematoma., Spinal epidural hematom nadir görülen, erken tanınıp, tedavi edilmediğinde kalıcı nörolojik defisit ve ölüm ile sonuçlanabilen bir acildir. Bu yazıda, acil servise ceviz ağacından düşme, sırt ağrısı ve parapleji ile başvuran, incelemeler sonrasında; nadir olarak görülen çökme ve kanal içi basısı yapmayan T12 vertebra Burst kırığı ile birlikte spinal epidural hematom tanısı alan 59 yaşındaki erkek hasta sunuldu.

Objective: We evaluated the results of arthroscopic reconstruction of anterior cruciate ligament ruptures using autogenous Hamstring tendon autograft. Materials and Methods: We evaluated 76 patients undergo arthroscopic reconstruction of ACL tears whom are treated surgically between years 2006 − 2008 in Fırat University Hospital Department of Orthopaedics Clinics. All patients were evaluated with Lysholm knee score, IKDC (International Knee Documentation Committee) score for functional evaluation, range of motion of the joint, stability tests, thigh atropy measurements and subjective satisfaction for clinical evaluation, at the end of the postoperative 20,5 (14 – 27) month. Results: The Lysholm scores were good and excellent for 46 patients (% 88,5) and the IKDC scores were grade A or B in 47 patients (% 90,4 ) and grade C in 7 patients (% 9,6). Radiographic examination showed mild (nine patients) or moderate (two patients) degenerative changes in the knee joint. Compared with the normal side, no flexion or extension losses occurred in the affected knees. Subjective satisfaction of the patients was noted as 7,5. Conclusion: We conclude that arthroscopic reconstructions of anterior cruciate ligament with use of a four strand Hamstring tendon autograft which revealed successful functional results and also provided high personal satisfaction, is a reliable technique in the surgical treatment of anterior cruciate ligament ruptures., Amaç: Bu çalışmada semptomatik kronik ön çapraz bağ yırtığının otojen Hamstring tendon grefti kullanılarak yapılan artrosko¬pik rekonstruksiyonunun sonuçları incelenmiştir. Gereç ve Yöntem: 2006 − 2008 tarihleri arasında Fırat Üniversitesi Tıp Fakültesi Hastanesi Ortopedi ve Travmatoloji Anabilim Dalında artroskopik olarak ön çapraz bağ yırtığı rekonstrüksiyonu yapılan 52 hasta incelendi. Tüm hastalar ortalama 20,5 (14 – 27) ay sonunda fonksiyonel olarak Lysholm diz skorlaması, IKDC (International Knee Documentation Committee) skorlaması ile klinik olarak diz eklem hareket açıklığı, stabilite testleri, uyluk çevresi ölçümü ve kişisel memnuniyet ile değerlendirildi. Bulgular: Lysholm skorlamasında 46 hasta (% 88,5) iyi ve mükemmel sonuç, IKDC skorlamasına göre 47 hasta (% 90,4) A veya B, 5 hasta (% 9,6) ise C olarak değerlendirildi. Radyografik değerlendirmede, 9 hastanın diz ekleminde hafif, iki hastada orta derecede dejeneratif değişiklikler gözlendi. Sağlam tarafla karşılaştırıldığında, hastalarda fleksiyon ve ekstansiyon kaybı gözlenmedi. Sonuç: Ön çapraz bağ yırtıklarının, dört katlı otojen Hamstring tendon grefti kullanılarak yapılan artroskopik rekonstrüksiyonlarında hem fonksiyonel olarak düzelme hem de hastalarda yüksek memnuniyet oranları tespit edilmiştir.

Objective: Mid-term results after treatment of talar osteochondral lesions with arthroscopic microfracture. Materials and Methods: This study includes 10 patients with osteochonral lesion of talus treated surgically with microfracture technique. Mean time between the initial complaint and surgery was 10.7 months. Previously, conservative treatment has been used in eight patients. Lesions were rigth ankle in six patients and left ankle in four patient in posteromedial side of talus. According to Berndt and Harty radiological classification, lesions were evaluated as stage II in 4, stage III in 5 and stage IV in 1 patient. Debridement and microfracture were applied in patients with stage II and stage III lesions, and curetage and microfracture were applied after fragman removed in patient with stage IV lesion. Ankle excercises were showed and weigt bearing not permitted for six weeks. Results: Clinical results were evaluated according to Berndt and Harty classification. Clinical results were good and excellent in all of four patients with stage II lesions. For five patients with stage III lesions, results were evaluated as good and excellent in three and moderate in two patients. One patient with stage IV, bad result was established. Time between the initial complaints and surgery were short in all patients with good results. Conclusion: If conservative treatment failed in patients with stage II and stage III osteochondral lesions of talus, early microfracture application is an efficient and successfull treatment options to cause lower morbidity rate because of minimal invazive surgery and to allow early return of daily activities., Amaç: Talusun osteokondral lezyonlarının artroskopik mikrokırık ile tedavisi sonrası orta dönem sonuçları değerlendirildi. Gereç ve Yöntemler: Talus osteokondral lezyonu tanısıyla mikrokırık tekniği kullanılarak cerrahi tedavi uygulanan 10 hasta çalışmaya alındı. Şikayetlerin başlangıcı ile ameliyat arasında geçen süre ortalama 10.7 ay idi. Sekiz hastaya önceden konservatif tedavi yapılmıştı. Lezyonlar altı hastanın sağ, dört hastanın sol ayak bileğinde talusun postero-medialinde yerleşmişti. Lezyonlar, Berndt ve Harty'nin radyolojik sınıflamasına göre 4 hastada evre II, 5 hastada evre III ve 1 hastada evre IV olarak değerlendirildi. Evre II ve evre III lezyonu olan hastalara debridman ile birlikte mikrokırık, evre IV lezyonu olan olguda ise fragmanın çıkarılması sonrası küretaj ve mikrokırık cerrahisi uygulandı. Mikrokırık cerrahisi uygulanan tüm hastalara ayak bileği egzersizleri gösterilerek altı hafta boyunca yük verdirilmedi. Bulgular: Klinik sonuçlar Berndt ve Harty'nin kriterlerine göre değerlendirildi. Evre II olan dört hastanın tümünde klinik sonuçlar iyi ve mükemmeldi. Evre III olan beş hastanın üçü iyi ve mükemmel, ikisi orta sonuç olarak değerlendirildi. Evre IV olan bir hastada ise kötü sonuç saptandı. İyi sonuçlanan hastaların hepsinde şikayetlerin başlama zamanı ve cerrahi zamanı arasındaki sürenin kısa olduğu görüldü. Sonuç: Konservatif tedavisi başarısızlıkla sonuçlanan Evre II ve III talus osteokondral lezyonlu hastalara erkenden yapılan mikrokırık yönteminin; minimal invaziv bir cerrahi olarak daha az morbiditeye yol açan, günlük aktivitelere erken geri dönüş sağlayan, etkili ve başarılı bir tedavi seçeneği olduğu sonucuna varıldı.
