The epidemiology of patients with traumatic brain injury (TBI) has changed dramatically over recent decades as a result of rapid advances in aging societies. We assessed the influence of age on outcomes of patients with TBI and sought to identify prognostic factors for in-hospital mortality of TBI among elderly patients.Using a nationwide database, we analyzed data from 5651 patients with TBI. Univariate analysis was conducted to compare patient demographics, neurologic status on admission, radiologic findings, systemic complication rates, length of hospital stay, in-hospital mortality, and home discharge rates between elderly and nonelderly groups. Multivariable analysis was conducted to determine prognostic factors for in-hospital mortality among elderly patients.Overall in-hospital mortality was significantly higher in elderly patients (12.8% vs. 19.3%; P0.001). In-hospital mortality of elderly patients with mild TBI increased significantly at7 days after admission, whereas that of elderly patients with moderate or severe TBI was significantly higher immediately after admission. Age (odds ratio [OR], 1.62; P = 0.024), male sex (OR, 1.30; P = 0.004), Japan Coma Scale score on admission (OR, 5.95, P0.001), and incidence of acute subdural hematoma (OR, 1.89; P0.001) were associated with in-hospital mortality in elderly patients with TBI.Elderly patients with TBI showed significantly higher in-hospital mortality. Delayed increases in in-hospital mortality were observed among elderly patients with mild TBI. Level of consciousness on admission was the strongest predictor of in-hospital mortality among elderly patients.

A 71-year-old man was admitted to our hospital with a diagnosis of subarachnoid hemorrhage (SAH). Angiographies revealed neither aneurysms nor vascular anomalies. However, these images elucidated the occlusion of the left cervical internal carotid artery as well as developed leptomeningeal anastomoses through the ipsilateral posterior cerebral artery, which resulted in blood perfusing the ipsilateral middle and anterior cerebral artery territories. Because the localization of SAH coincided with the developed leptomeningeal anastomosis, we speculated that the rupture of the developed leptomeningeal anastomosis in the basal cistern was the cause of SAH. We performed superficial temporal and middle cerebral artery bypass surgery to prevent rebleeding and ischemic stroke. In patients with occlusion of the internal carotid artery, SAH induced by the rupture of aneurysm formed by hemodynamic stress was recognized. However, rupture of developed leptomeningeal anastomosis should be considered as a possible cause of SAH of unknown origin. Background It is said atheromatous plaque is located very focally, but there have been few reports regarding this matter. Various aspects of the pathogenesis of the development of atheromatous plaque at the carotid bifurcation have previously been discussed. We have noted the correlation of plaque localization with characteristics of the cervical carotid artery wall. Methods Morphological and histopathologic changes in the carotid bifurcation were examined in 72 cadaver cases with or without atheromatous plaque. We determined the level at which the wall structure changed to muscular artery from elastic artery and analyzed its influence on the development of atheromatous plaque. Result Atheromatous plaques at the distal site of the ICA extended within 0 to 37 mm from the carotid bifurcation. The proximal side of the CCA more than 5 mm away from the bifurcation was elastic artery, whereas the distal side of the ICA more than 15 mm from the bifurcation was muscular artery. The area of the carotid bifurcation between elastic artery and muscular artery was a transitional zone. Approximately 80% of them were located within 15 mm, and these areas were coincident with the transitional zone. Conclusion Most atheromatous plaque was located in the transitional zone. The arterial wall structure is related to the development of atheromatous plaque at the cervical carotid bifurcation.

The intracranial internal carotid artery (ICA) is well known to be a muscular artery that lacks an external elastic lamina. Elastic fibers are concentrated inside the internal elastic lamina of the wall of the intracranial ICA. We examined a portion where the external elastic lamina disappeared in the course of the ICA. We extirpated 32 intracranial ICA specimens from 50 cadavers. We made running specimens of the ICA every 3 mm and stained them using the Elastica van Gieson method and then investigated a portion where the external elastic lamina disappeared in the wall of the ICA. We also examined the intimal thickness of various portions of the ICA. We identified both the internal elastic lamina and the external elastic lamina in the petrous portion of the ICA in all cases. In the intradural portion of the ICA, the external elastic lamina was not recognized and only the internal elastic lamina was seen in all cases. In 31 of the 32 specimens (98%), the external elastic lamina disappeared in the horizontal portion of the cavernous portion of the ICA. The intimal thickness of the ICA was recognized in 23/32 specimens. The starting point of the intimal thickness approximated the portion where the external elastic lamina disappeared. The external elastic lamina of the ICA disappeared at the horizontal portion of the cavernous sinus, which was a site where intimal thickness was frequently observed. Changes in the elasticity of the wall of the intracranial ICA may therefore cause atherosclerotic changes in the ICA.

It has been proved that development of the atheromatous plaque at the carotid bifurcation is related with its variability. We examined a correlation of plaque localization with characteristics of the cervical carotid artery wall. We studied the thickness and length of the atheromatous plaque of the carotid bifurcation and wall characteristics of the carotid artery. Our results suggest the atheromatous plaque localized at the carotid bifurcation, restricted elastic artery and transitional zone that changed to muscular artery.

Based on our clinical experience of 240 cases, we present the noticeable anatomical structures and our principles of procedures during carotid endarterectomy (CEA). We also present a newly developed self-retaining retractor and spring hook for CEA. The noticeable anatomical structures are as follows: the internal jugular and facial veins, the hypogrossal nerve and ansa cervicals, vagus nerve, superoir laryngeal nerve, and branches of the external carotid artery. Our principles of exposure of the carotid arteries during CEA are the meticulous anatomical dissection and identification of the cervical structures and the intentional circumferential dissection of the carotid complex. Our perioperative cerebral complication rate (mortality and morbidity) is 2.4%, and the incidence of the basic technical complications such as cranial nerve palsy and wound hematoma are 0.4% and 1.3%, respectively. The meticulous anatomical dissection and the intentional circumferential dissection of the carotid complex are essential to minimize postoperative complications.

We report a pediatric case of acute cerebellitis with hydrocephalus requiring emergency placement of external ventricular drainage. A 7-year-old boy presented with acute development of headache, nausea and vomiting. Magnetic resonance (MR) examination revealed obstructive hydrocephalus and marked bilateral cerebellar swelling on T2-weighted imaging. After the placement of external ventricular drainage, symptoms of intracranial hypertension promptly improved, and other clinical and radiological abnormalities gradually resolved following intravenous injection of corticosteroids. Surgical treatment and timing in the treatment of acute cerebellitis are discussed.

The intracranial internal carotid artery (ICA) is a muscular artery and lacks external elastic lamina (EEL). Stenosis of the intracranial ICA is relatively uncommon, but the most common site is the cavernous portion. The characteristics of the arterial wall structures were examined using serial 3-mm sections of 32 intracranial ICAs obtained from 50 cadavers to identify where the EEL disappeared. The portions of the ICA where the intima exhibited thickening were also determined. Both the internal elastic lamina (IEL) and EEL were observed in all 32 specimens of the petrous portion of the ICA. Only the IEL was observed in all 32 specimens of the intradural portion of the ICA. The EEL had disappeared in 31 of the 32 specimens of the horizontal segment of the cavernous portion of the ICA. Intimal thickening of the ICA was observed in 23 of 32 ICA specimens, and frequently appeared in the horizontal segment of the cavernous portion of the ICA. The EEL disappeared in the cavernous portion of the ICA, which is the most common site of stenosis of the intracranial ICA. Change in the elasticity of the arterial wall in the cavernous portion may be an important factor in the process of atherosclerosis in the intracranial ICA.

In elderly patients with aneurysmal subarachnoid hemorrhage (SAH), complications including vasosopasm, subdural effusion, and late hydrocephalus, are liable to occur even after aneurysmal surgery. We examined prospectively the efficacy of arachnoid plasty using fibrin glue membrane during surgery of ruptured aneurysms in the elderly patients for preventing complications. The effects on the modified Rankin scale (mRS) and the Glasgow outcome scale (GOS) 3 months after SAH were noted.Total of 31 patients aged more than 70 years selected from a consecutive series of patients with aneurysmal SAH, were divided into two groups alternately, a group with arachnoid plasty (n = 16) and a control group without arachnoid plasty (n = 15). Statistical analyses were performed to assess relationships among various clinical and neuroradiological variables, especially between arachnoid plasty and occurrence of symptomatic vasospasm, subdural effusion, late hydrocephalus, or outcome such as mRS and GOS 3 months after onset.Statistical analyses revealed that arachnoid plasty were associated with late hydrocephalus and subdural effusion negatively, but with better mRS at 3 months after SAH. A tendency to be associated with less frequent symptomatic vasospasm was also noted.Arachnoid plasty using fibrin glue is suggested to be effective in preventing complications associated with SAH and aneurysmal surgery. A better outcome in the elderly patients can be achieved.

Objective: The surgical anatomy of structures surrounding the anterior clinoid process (ACP) was examined in 39 cadavers to enable safe performance of selective extradural anterior clinoidectomy. Methods: The lateral portion of the optic canal (OC) at the end of the orbit was defined as point A, and the lateral margin of the dural insertion into the superior orbital fissure (SOF) as B. A vertical line from A to the lateral margin of the ACP was assumed to define a crossing, defined as C. Distances between A and B, C and B, and C and A were measured. The length and width of OC and the optic strut were also measured. Results: The mean distances between A and B, C and B, and C and A were 11.8, 8.9 and 6.3 mm, respectively. The mean length and width of OC were 8.9 and 5.7 mm, and those of the optic strut 5.4 and 2.4 mm, respectively. Conclusion: We propose a modified, efficient procedure for selective extradural anterior clinoidectomy, as follows. Drilling is started from the point approximately 9 mm posterior to the lateral margin of the dural insertion into the SOP, and pointed medially in the direction with a right angle to the lateral margin of the ACP. After drilling about 6 mm to reach the lateral border of the OC, unroofing of the OC is carried out to remove the ACP en bloc by fracturing of the optic strut. Using the present procedure, the distance of drilling of the lesser wing of the sphenoid bone is minimized.

✓ The authors present the case of an elderly patient with a quadrigeminal arachnoid cyst who was successfully treated with endoscopic fenestration through the posterior wall of the third ventricle via the anterior horn of the lateral ventricle. This 71-year-old man suffered from progressive gait instability and disorientation. Radiological examination revealed hydrocephalus caused by a quadrigeminal arachnoid cyst. The patient underwent endoscopic fenestration of the quadrigeminal cistern arachnoid cyst and third ventriculostomy via one burr hole placed at the coronal suture. This method is less invasive and is effective for quadrigeminal cistern arachnoid cyst and accompanying hydrocephalus.
