Hematopoietic stem cell transplantation (HSCT) is a useful tool for the treatment of hematologic malignancies. However, transplantation-related complications are the main cause of non-relapse mortality. Previous reports suggest that endothelial damage is related to early complications after HSCT. Non-invasive reactive hyperemia peripheral arterial tonometry (RH-PAT) was performed to evaluate endothelial function as a predictive marker for these complications.The reactive hyperemia index (RHI) obtained from RH-PAT was evaluated before the conditioning regimen. The relationship between the RHI and the appearance of engraftment syndrome, thrombotic microangiopathy, and acute graft-versus-host disease (aGVHD) was assessed. Receiver operating characteristic curve analysis showed that an RHI value of 1.58 was the optimal cut-off for predicting transplantation-related complications.In total, 49 patients (22 acute myelogenous leukemia, 7 acute lymphocytic leukemia, 6 myelodysplastic syndrome, 6 adult T-cell leukemia, 6 non-Hodgkin lymphoma, and 2 others) were enrolled; 34 had a normal RHI (≥ 1.59), and 15 had an abnormally low RHI (≤ 1.58). Thrombotic microangiopathy (20.2% vs 0.0%, P = 0.025) and aGVHD (80.0% vs 41.2%, P = 0.015) were significantly more frequent in patients with a low RHI.Endothelial dysfunction assessed by RH-PAT before HSCT was able to predict transplantation-related complications.

A 3.3-kV diode-less (D-less) SiC power module adopting a “next high power density dual package” (named nHPD2) was developed. Configuring the module with SiC metal–oxide–semiconductor field-effect transistors (MOSFETs) only resulted in the ultrahigh power density. Its rated current of 800 A and power density of 37.7 kVA/cm2 are the highest value achieved among high power modules so far. As a countermeasure against bipolar degradation related to the body diode in the MOSFET structure, a high-throughput screening process was utilized for the first time. To clarify the advantageous features of this module, its characteristics an in comparison with those of a full-SiC module and a conventional silicon module in the same nHPD2 package. In addition, system-level impacts on actual traction inverter operations were simulated by using the evaluated characteristics. To accurately calculate the impacts, the temperature dependence of the SiC modules, which is a remarkable feature of high-voltage SiC devices, was taken into account. The superior performance of the developed D-less SiC power module, namely, low loss, low junction temperature, and high power density capability, was confirmed.

Acute graft-versus-host disease (aGVHD) is a major cause of treatment-related mortality after allogeneic hematopoietic stem cell transplantation. Endothelial cell damage may trigger the initiation of aGVHD.Endothelial damage and repair were evaluated by counting circulating endothelial cells (CECs) and endothelial progenitor cells (EPCs) in 17 allogeneic hematopoietic stem cell transplantation patients at pre-conditioning, day 0, day 7, day 14, day 30, and day 60 by multicolor flow cytometry. Von Willebrand factor activity was simultaneously measured.Eight patients developed aGVHD and were compared to non-aGVHD patients. Patients' characteristics were not different, except for previous treatment courses. There was no difference in von Willebrand factor activity between the two groups. Both CEC and EPC counts were decreased on day 7 and day 14 and then increased thereafter. The CEC count on day 7 was significantly lower in the aGVHD group than in the non-aGVHD group (p = .0401). Restoration of the EPC count on day 60 was significantly suppressed in the aGVHD group (p = .0464). The CEC count on day 7 could predict aGVHD development (AUC 0.8214, p = .0372).The present results showed that CEC count on day 7 could be a predictor of aGVHD.

Tyrosine kinase inhibitors (TKIs) have revolutionized the treatment of chronic myeloid leukemia (CML). However, TKI-related chronic renal toxicity has been reported, particularly in patients with hypertension. We assessed whether incidental use of specific types of antihypertensive drugs, including renin-aldosterone-angiotensin system inhibitors (RAASis), affects the change in estimated glomerular filtration rate (eGFR) during TKI treatment. We retrospectively analyzed all eGFR measurements during TKI treatment for 142 CML patients at Kyushu University Hospital, estimating the rate of eGFR change using a mixed-effects model. Overall, a significant interaction was found between the type of antihypertensive medication used and the yearly change in eGFR (P 0.01), with RAASi users exhibiting the most rapid decrease in eGFR (- 5.5%/year). The analysis by TKI used showed that the interaction was significant only in imatinib and bosutinib users (P 0.01 and P = 0.04, respectively). The yearly rate of eGFR decrease was the most notable in RAASi users, at - 5.7 (- 6.6, - 4.9) and - 10.1 (- 12.3, - 7.9) for imatinib and bosutinib users, respectively. Our findings indicate that eGFR should be carefully monitored in patients taking these TKIs.

The trade-off between low on-resistance and high short-circuit (SC) capability crucial for silicon carbide metal-oxide-semiconductor (MOS) field-effect transistors. To break this trade-off, we clarified the design parameters that are key in improving SC capability and confirmed that our trench-etched double-diffused MOS (TED-MOS) can be suitably designed using its unique parameters to achieve low on-resistance while maintaining high SC capability.

Behçet’s disease (BD) is a disorder characterized by systemic inflammation of multiple organs, including the intestines. Several studies have reported a relationship between myelodysplastic syndrome and BD, and trisomy 8 was frequently seen, especially in intestinal BD. However, the association of BD with primary myelofibrosis (PMF) has not been well documented. A 58-year-old Japanese female was diagnosed with PMF in 2014. The symptoms of PMF resolved with ruxolitinib. However, she developed fever and intestinal perforation due to multiple ulcers in the terminal ileum in 2017. Intestinal perforation recurred 1 month later, and the dose of ruxolitinib was tapered. After discontinuation of ruxolitinib, she presented with recurrent oral aphthous ulcers and uveitis. Subsequently, intestinal perforation recurred, and she was diagnosed with intestinal BD. Trisomy 8 was identified in her peripheral blood. She underwent steroid therapy, azathioprine, and infliximab. This case suggests relationships between PMF, trisomy 8, and BD.

This paper presents the analysis of a simplified model for the design of a module structure that avoids the risk of self-excited oscillation (SE-Osc). A necessary and sufficient simplified model that can extract the critical oscillation mode is proposed based on a comparison of several simplifying steps. The differential equation of the simplified model is solved, and the solution is plotted in the frequency domain to analyze the oscillatory conditions. The simplified model is verified via a time-domain full-model simulation modeled by 3-D electromagnetic simulation and solved by finite-element simulation. Measurements of test modules show consistent oscillatory conditions and frequency. SE-Osc modes are eliminated by reducing the inductance connected to the emitter and increasing the inductance connected to the collector. Increasing the ratio of $C_{\textsf {CE}}$ to $C_{\textsf {GC}}$ increases the risk of self-exciting oscillation. Suppressions of SE-Osc from a common package design with state-of-the-art Si-insulated gate bipolar transistors (IGBTs) presenting small feedback capacitance, SiC-MOS, and hybrids of state-of-art Si-IGBTs and SiC-Schottky barrier diodes are verified.

The perturbative path-integral gives a morphism of the (quantum) $A_{\infty }$ structure intrinsic to each quantum field theory, which we show explicitly on the basis of the homological perturbation. As is known, in the BV formalism, any effective action also solves the BV master equation, which implies that the path-integral can be understood as a morphism of the BV differential. Since each solution of the BV master equation is in one-to-one correspondence with a (quantum) $A_{\infty }$ structure, the path-integral preserves this intrinsic $A_{\infty }$ structure of quantum field theory, where $A_{\infty }$ reduces to $L_{\infty }$ whenever multiplications of space-time fields are graded commutative. Sinusitis is a serious infectious complication of allogeneic hematopoietic stem cell transplantation. Schizophyllum commune (S commune) is a common basidiomycete fungus that is rarely involved in human disease. We report herein a case of S commune sinusitis after allogeneic bone marrow transplantation. A 66-year-old man with myelodysplastic syndrome underwent allogeneic bone marrow transplantation and developed maxillary and ethmoid sinusitis. The sinusitis did not improve with liposomal amphotericin B after neutrophil engraftment, so we considered that surgical intervention was needed for the recovery of sinusitis. Endoscopic sinus surgery was performed. In the debridement tissue of paranasal mucosa, filamentous fungal elements were observed. Moreover, genetic analysis of the tissue revealed the presence of S commune. Schizophyllum commune should be recognized as a fungal pathogen that causes sinusitis after allogeneic hematopoietic stem cell transplantation. This case suggests the effectiveness of prompt surgical intervention with liposomal amphotericin B treatment for S commune sinusitis and the usefulness of genetic diagnosis for cases under antifungal treatment. We present a series of new gauge invariant quantities in Witten's open string field theory. They are defined for a given set of open string states which satisfy the physical state condition around a classical solution. For known classical solutions, we show that these gauge invariant quantities compute on shell tree-level scattering amplitudes around the correspondent D-brane configuration., 41 pages, 2 figures

The frequency of multiple primary malignant neoplasms (MPMN) is increasing due to population aging. Since consensus guidelines for the treatment of MPMN are lacking, treatment strategies are determined by disease status on a per-patient basis. In this report, we describe a case of MPMN with follicular lymphoma (FL) grade 1 that transformed to double-hit lymphoma during adjuvant chemotherapy for concurrent ovarian carcinoma. A 64-year-old woman was diagnosed with MPMN of FL and endometrioid carcinoma by staging laparotomy and lymph node biopsy. She received four cycles of adjuvant chemotherapy (carboplatin and paclitaxel) for endometrioid carcinoma, but during chemotherapy, the FL grade 1 transformed to double-hit lymphoma. We speculate that adjuvant chemotherapy for endometrioid carcinoma may have triggered the transformation of FL in the present case.

Upshaw-Schulman syndrome (USS) is an inherited type of thrombotic thrombocytopenic purpura (TTP) that is extremely rare, but often diagnosed during pregnancy. Reversible cerebral vasoconstriction syndrome (RCVS) is the transient stenosis of several cerebral arteries that is frequently diagnosed post-partum. We describe a 28-year-old woman with USS complicated by RCVS after delivery that was treated by plasma exchange with a good outcome. She was referred to our hospital with thunderclap headache, anemia and thrombocytopenia that occurred immediately postpartum. She was diagnosed with TTP and multiple cerebral infarctions. Plasma exchange promptly improved her symptoms on hospital day 3. Moreover, multiple stenoses of cerebral arteries indicating RCVS were resolved. Since her sister also had an episode of thrombocytopenia during pregnancy, inherited TTP was suspected and genetic analyses confirmed USS. Pregnancy is a risk for not only TTP, but also RCVS. Endothelial damage might be an underlining cause and vasospasm after delivery is a trigger of RCVS. Plasma exchange was effective against both TTP and RCVS.

We study profiles and gauge invariant observables of classical solutions corresponding to a constant magnetic field on a torus in open string field theory. We numerically find that the profile is not discontinuous on the torus, although the solution describes topologically nontrivial configurations in the context of low energy effective theory. We propose a method to design a module structure avoiding the risk of self-excited (SE) oscillation. By simplifying both the semiconductor device and lumped circuit model, oscillatory conditions can be extracted analytically. Results show good agreement with T-CAD simulation and measurement results of test modules. The method is applied to the design of next generation common package, which has realized very low system inductance. SE oscillation can be prevented for latest generation Si-IGBTs having very small feedback capacitance and SiC-MOS having high output capacitance mounted in the same common package design.

In Kumaishi Fishing Port, which locates at the coast of Sea of Japan in Hokkaido, troubles on cargo handling / mooring of fishing boats caused by harbor disturbance with rush of long-period waves became a problem in the harbor. Therefore, maintenance of long-period wave measure facilities by rubble-mound structures was planned. On this account, in this study, local wave observations were performed at the time of temporal construction of long-period wave measure facilities. Reflection factors of long-period waves in an actual scale were estimated in comparison with past local wave observation results and observed values to demonstrate long-period wave measure effects.

SiGe HBTs and their circuit technologies are suitable for future wireless communications. To achieve low 1/f-noise characteristics in RF applications, a SiGe HBT with a raised-emitter structure, fabricated by epitaxial growth of phosphorous-doped Si layers, was developed. Aimed at ultra-low power consumption in a wide range of microwave applications, a SiGe HBT fabricated by well-controlled SiGe/Si continuous epitaxial growth was developed. To improve the design process for Si-based RF-ICs, equivalent circuits for transmission lines under the slow-wave effect and for inductors under the peripheral ground effect were also demonstrated. Moreover, MMICs operating in the quasi-millimeter-wave region, namely, a 24-GHz LNA, a 27-GHz VCO, and a 24-GHz mixer were developed. In regards to these MMICs, new circuit technologies, namely, inductive biasing, merged transformer matching, and pseudo-stacked configuration, were developed.

A technique for predicting wafer temperature was developed, and a model for predicting critical dimension (CD) was devised. Using this technique and model in combination makes it possible to calculate wafer temperature during gate etching within an accuracy of 1 °C and to predict CD distribution after plasma etching. Etching at a temperature for uniform CD given by the CD prediction model reduces the CD variation (3σ) during gate etching from 2.3 to 1.5 nm. Applying this temperature prediction technique and CD prediction model together will contribute to improving etching apparatus design and process development.

胆管癌の術後再発は, 予後不良である. われわれは胆管癌切除後に異時性に胸壁転移と腹膜転移をきたし, 2度の切除後6年経過で無再発例を経験しえた. 症例は70歳, 女性. 主訴は発熱, 右季肋部痛. 平成10年当院に入院し, PTCD施行後胆汁細胞診の結果, 胆管癌と診断され, 平成11年2月膵頭十二指腸切除術を施行した. 高分化管状腺癌でリンパ節転移はなかった. 同12月PTCD挿入部の右胸壁に転移再発きたし, 胸壁腫瘍切除を行った. 病理結果は胸壁転移と診断された. 平成12年3月右腹部痛をきたし, 腹部CTで右下腹部に3カ所の腹膜転移を指摘された. 同4月右半結腸切除を行った. 病理結果は腹膜転移であった. 胆管癌の術後再発例でも切除にて本例のように予後良好なものがある.

Working environments at fishery ports and harbors during the winter in cold, windy regions are very severe for workers. Such environments may affect the worker's health, as well as lowering the work efficiency. To examine the necessity of facilities such as the wind and snow shelters, we should quantitatively evaluate in terms of human sensation. We conducted sensory experiments on subjects to examine the practical application of some thermal indices and “work efficiency” under the cold condition. There were no differences between sexes regarding the thermal responses. Of five thermal indices studied in this report, the WCI was the most useful index from a practical point of view. We also clarified influences of thermal environment and elapsed time (exposure time) on the work efficiency.

A modeling procedure to simulate stress-strain relations of sand subjected to cyclic loading is proposed. Results from drained plane strain compression, extension, and cyclic loading tests on Toyoura sand are analyzed. The monotonic loading behavior is simulated by the generalized hyperbolic equation to use as the skeleton curves in the simulation of cyclic behavior. To construct hysteretic stress-strain curves based on the skeleton curves, the Masing's rule is generalized to the proportional rule consisting of the internal and external rules. The drag rule is then introduced to simulate cyclic stress-strain behavior in which the stress amplitude increases at a decreasing rate during cyclic loading with a constant strain amplitude. It is assumed that any plastic shear strain increment taking place in a certain direction drags the whole skeleton curve for loading in the opposite direction towards the direction of the concerned shear strain increment. The measured cyclic stress-strain behavior is well simulated by the proposed method.

We consider a modified KBc algebra in bosonic open string field theory expanded around identity-based scalar solutions. By use of the algebra, classical solutions on the background are constructed and observables for them, including energy densities and gauge invariant overlaps, are calculable. A new architecture has been designed and demonstrated for a low-power SP10T-RF-Switch-IC using 0.18μm SOI-CMOS, implementing an RF-Switch, negative voltage generator, and MIPI in a chip. Clock frequency of the negative voltage generator is controlled to increase only in a switch transition and drop at other times in order to reduce power consumption. Results of an evaluation of a trial chip confirmed a 33% reduction in power consumption compared with conventional architecture while RF performance is maintained.
