4. The challenge of return to work in workers with cancer: employer priorities despite variation in social policies related to work and health

Abstract

Purpose: This study explored employer’s perspectives on (1) their experience of good practice related to workers diagnosed with cancer and their return to work (RTW), and (2) their perceived needs necessary to achieve good practice as reported by employers from nine separate countries. Methods: Twenty-five semi-structured interviews were held in eight European countries and Israel with two to three employers typically including HR managers or line managers from both profit and non-profit organisations of different sizes and sectors. Interviews were recorded and transcribed verbatim. A grounded theory/thematic analysis approach was completed. Results: Employers’ experience with RTW assistance for workers with cancer appears to be a dynamic process. Results indicate that good practice includes six phases: (1) reacting to disclosure, (2) collecting information, (3) decision-making related to initial actions, (4) remaining in touch, (5) decision-making on RTW, and (6) follow-up. The exact details of the process are shaped by country, employer type, and worker characteristics; however, there was consistency related to the need for (1) structured procedures, (2) collaboration, (3) communication skills training, (4) information on cancer, and (5) financial resources for realizing RTW support measures. Conclusions: Notwithstanding variations at country, employer, and worker levels, the employers from all nine countries reported that good practice regarding RTW assistance in workers with a history of cancer consists of the six phases above. Employers indicate that they would benefit from shared collaboration and resources that support good practice for this human resource matter. Implications for cancer survivors: Further research and development based on the six phases of employer support as a framework for a tool or strategy to support workers with a history of cancer across countries and organisations is warranted.

Abstract

Aims: Methylation profiling (MP) is increasingly incorporated in the diagnostic process of central nervous system (CNS) tumours at our centres in The Netherlands and Scandinavia. We aimed to identify the benefits and challenges of MP as a support tool for CNS tumour diagnostics. Methods: About 502 CNS tumour samples were analysed using (850 k) MP. Profiles were matched with the DKFZ/Heidelberg CNS Tumour Classifier. For each case, the final pathological diagnosis was compared to the diagnosis before MP. Results: In 54.4% (273/502) of all analysed cases, the suggested methylation class (calibrated score ≥0.9) corresponded with the initial pathological diagnosis. The diagnosis of 24.5% of these cases (67/273) was more refined after incorporation of the MP result. In 9.8% of cases (49/502), the MP result led to a new diagnosis, resulting in an altered WHO grade in 71.4% of these cases (35/49). In 1% of cases (5/502), the suggested class based on MP was initially disregarded/interpreted as misleading, but in retrospect, the MP result predicted the right diagnosis for three of these cases. In six cases, the suggested class was interpreted as ‘discrepant but noncontributory’. The remaining 33.7% of cases (169/502) had a calibrated score <0.9, including 7.8% (39/502) for which no class indication was given at all (calibrated score <0.3). Conclusions: MP is a powerful tool to confirm and fine-tune the pathological diagnosis of CNS tumours, and to avoid misdiagnoses. However, it is crucial to interpret the results in the context of clinical, radiological, histopathological and other molecular information.

Moen (b) Keywords: Health and safety management; Work environment; Musculoskeletal; Prospective Abstract: According to Norway's Internal Control Regulation, all companies are required to have an occupational health and safety (H&S) management system. This study investigated the effects of implementing or improving occupational H&S management on the work environment, H&S-related behaviour and musculoskeletal health of workers in small and medium-sized companies. A one-year prospective cohort study, using self-administered questionnaires, was performed among the managers and blue-collar workers in 226 motor vehicle repair garages. Out of 1559 workers that responded at baseline 721 workers could be identified at follow-up. These 721 workers were included in the study. The workers in companies with improved H&S management from baseline to follow-up reported increased satisfaction with the H&S activities at the garage; improved support from management and colleagues; improved health-related support and control; and increased participation in H&S activities. 33. Annual emissions of CH4 and N2O, and ecosystem respiration, from eight organic soils in Western Denmark managed by agriculture

Abstract

The use of organic soils by agriculture involves drainage and tillage, and the resulting increase in C and N turnover can significantly affect their greenhouse gas balance. This study estimated annual fluxes of CH4 and N2O, and ecosystem respiration (Reco), from eight organic soils managed by agriculture. The sites were located in three regions representing different landscape types and climatic conditions, and three land use categories were covered (arable crops, AR, grass in rotation, RG, and permanent grass, PG). The normal management at each site was followed, except that no N inputs occurred during the monitoring period from August 2008 to October 2009. The stratified sampling strategy further included six sampling points in three blocks at each site. Environmental variables (precipitation, PAR, air and soil temperature, soil moisture, groundwater level) were monitored continuously and during sampling campaigns, where also groundwater samples were taken for analysis. Gaseous fluxes were monitored on a three-weekly basis, giving 51, 49 and 38 field campaigns for land use categories AR, PG and RG, respectively. Climatic conditions in each region during monitoring were representative as compared to 20-yr averages. Peat layers were shallow, typically 0.5 to 1 m, and with a pH of 4 to 5. At six sites annual emissions of N2O were in the range 3 to 24 kg N2O-N ha−1, but at two arable sites (spring barley, potato) net emissions of 38 and 61 kg N2O-N ha−1 were recorded. The two high-emitting sites were characterized by fluctuating groundwater, low soil pH and elevated groundwater SO42− concentrations. Annual fluxes of CH4 were generally small, as expected, ranging from 2 to 4 kg CH4 ha−1. However, two permanent grasslands had tussocks of Juncus effusus L. (soft rush) in sampling points that were consistent sources of CH4 throughout the year. Emission factors for organic soils in rotation and with permanent grass, respectively, were estimated to be 0.011 and 0.47 g m−2 for CH4, and 2.5 and 0.5 g m−2 for N2O. This first documentation of CH4 and N2O emissions from managed organic soils in Denmark confirms the levels and wide ranges of emissions previously reported for the Nordic countries. However, the stratified experimental design also identified links between gaseous emissions and site-specific conditions with respect to soil, groundwater and vegetation which point to areas of future research that may account for part of the variability and hence lead to improved emission factors or models.
