5. Preparación y administración de medicación con chalecos identificativos: impacto en la seguridad del paciente

- Abstract
Background and rationale. Patients may suffer adverse events (AD) during healthcare. Among the most frequent AE, medication errors are found. One of the main reasons for these errors are interruptions during the preparation and administration of medication. Objectives. Evaluate the impact of wearing an informational vest to avoid interruptions during medication preparation and administration. Designs and Methods. Multicenter intervention study without control group. The intervention consisted of the use of a vest with the legend "Attention Do not disturb Preparing Medication". Wo measurements were made: pre-intervention (without vest) and post-intervention (with vest). Results. A total de 32 nurses prepared and administered 125 treatments with the vests on and 120 without them. With the use of the vests the interruptions decreased by 71% (p< 0,001). Preparation times were reduced by 23,1 % and administration times by 41,1%. Errors decreased by 80%. Conclusion. The use of the vests caused a reduction in the number of interruptions to nurses during the preparation and administration of the medication. There was also a significant decrease in errors that occur during these tasks., Justificación. Los pacientes pueden sufrir eventos adversos (EA) durante la asistencia sanitaria. Dentro de los EA, más frecuentes, se encuentran los errores de medicación. Uno de los principales motivos de estos errores son las interrupciones que soportan las enfermeras durante la preparación y administración de medicación. Objetivo. Evaluar el impacto del uso de un chaleco informativo para evitar interrupciones durante la preparación y administración de medicación. Metodología. Estudio multicéntrico de intervención sin grupo control. La intervención consistió en el uso de un chaleco con la leyenda “Atención No molestar Preparando medicación”. Se hicieron dos mediciones: preintervención (sin chaleco) y postintervención (con chaleco). Resultados. Un total de 32 enfermeras prepararon y administraron 125 tratamientos con los chalecos puestos y 120 sin ellos. Con el uso de los chalecos las interrupciones disminuyeron en un 71,3% (p< 0,001). Se rebajaron los tiempos de preparación en un 23,1% y de administración en un 41,1%. Los errores disminuyeron en un 80% (p>0,001). Conclusión. El uso de los chalecos originó una rebaja en el número de interrupciones a las enfermeras durante la preparación y administración de la medicación. También se produjo una importante disminución en los errores que acontecen durante estas tareas.

Actas del V Congreso Internacional de Jóvenes Investigadorxs con perspectiva de género (Getafe, 3 - 5 de junio de 2020) organizado por el Instituto Universitario de Estudios de Género de la Universidad Carlos III de Madrid. La detección automática de emociones a través de información fisiológica, física y biológica del usuario y su entorno para ayudar a combatir la violencia contra la mujer plantea una serie de retos, entre ellos, la dificultad para clasificar las emociones debido a su carácter subjetivo. Por ello, con este proyecto se plantea la posibilidad de utilizar la medida de neurotransmisores en sangre para poder clasificarlas de una manera más precisa. En este trabajo se presenta los primeros resultados del experimento piloto realizado con 5 mujeres sanas a las que se les sometió a diferentes estímulos audiovisuales mientras se recogían muestras de sangre para ver la evolución temporal de la concentración de neurotransmisores después de experimentar emociones de miedo o de otro tipo. Los resultados obtenidos abren la puerta a un estudio de mayor envergadura con el que poder obtener un modelo que relacione las emociones con las concentraciones de neurotransmisores en sangre. Este trabajo forma parte de la investigación del proyecto EMPATÍA-CM (Ref: Y2018/TCS-5046) del programa de proyectos sinérgicos de I+D en nuevas y emergentes áreas científicas en la frontera de la ciencia y de naturaleza interdisciplinar, cofinanciada con los Programas Operativos del Fondo Social Europeo y del Fondo Europeo de Desarrollo Regional, 2014-2020, de la Comunidad de Madrid; y del proyecto ARTEMISA-CM-UC3M (Ref: 2020/00048/001) de la convocatoria del programa de apoyo a la realización de Proyectos Interdisciplinares de I+D para jóvenes investigadores de la Universidad Carlos III de Madrid 2019-2022.

- Abstract
Justification. to understand the role played by nurses in Clinical Ethics Committees (CEC) is essential to identify the contribution of nursing to these committees. Purpose. To identify the role played by nurses in CEC. Methods. Descriptive cross-sectional study, carried out by means of a questionnaire completed by nurses from CEC from the Madrid Community. Results. The CEC were composed by an average of five nurses, with a seniority of six years. The presence of nurses in CEC serves to develop the profession and to highlight nursing care as a value element that can make a significant contribution to the humanization of health care. Conclusions. Nurses should expand their knowledge in bioethics in order to increase their participation in the deliberative process that takes place in CEC., Justificación. El conocimiento del rol que desempeñan las enfermeras en los Comités de Ética Asistencial (CEA) resulta esencial para identificar las aportaciones de la enfermería a estos comités. Objetivo. Conocer el rol que desempeñan las enfermeras en los CEA. Metodología. Estudio descriptivo transversal, realizado por medio de un cuestionario a enfermeras pertenecientes a los CEA de la Comunidad de Madrid. Resultados. Los CEA están compuestos por una media de cinco enfermeras, con una antigüedad en el comité de seis años. La presencia de las enfermeras en los CEAS sirve para desarrollar la profesión y poner en valor el cuidado enfermero como elemento fundamental para la humanización de la asistencia sanitaria. Conclusión. Las enfermeras deben profundizar en su formación en bioética para mejorar su participación en el proceso deliberativo que se da en los CEAS.

Background: In the last decades, there have been important developments in the scientific and technological areas of healthcare. On certain occasions this provokes conflict between the patients' rights and the values of healthcare professionals which brings about, within this clinical relationship, the problem of conscientious objection. Aims: To learn the opinions that the Nurses of the Madrid Autonomous Community have regarding conscientious objection. Research design: Cross-cutting descriptive study. Participants and research context: The nurses of 9 hospitals and 12 Health Centers in the Madrid Autonomous Community. The study was done by means of an auto completed anonymous questionnaire. The variables studied were social-demographical and their opinions about conscientious objections. Ethical considerations: The study was approved by the Ethical Community of Clinical Research of the University Hospital Príncipe de Asturias. Participants were assured of maximum confidentiality and anonymity. Findings: A total of 421 nurses answered the questionnaire. In total, 55.6% of the nurses confirmed they were religious believers, and 64.3% declared having poor knowledge regarding conscientious objection. The matters that caused the greatest objections were voluntary abortions, genetic embryo selection, refusal of blood transfusions, and therapy refusal. Discussion: Different authors state that the most significant cases of conscientious objections for health professionals are those regarding carrying out or assisting in abortions, euthanasia, the practice of assisted reproduction and, finally, the prescription and dispensing of the morning-after pill. In our study, the most significant cases in which the nurses would declare conscientious objections would be the refusal to accept treatment, the selection of embryos after genetic diagnosis preimplantation, the patient's refusal to receive blood transfusions due to religious reasons and pregnant women's request for voluntary abortions within the first 14 weeks. Conclusion: Nurses' religious beliefs influence their opinions regarding conscientious objection. The nurses who declare themselves as religious believers object in a higher percentage than those without religious beliefs. [ABSTRACT FROM AUTHOR]

Existe una controversia generada por el papel que juega la enfermera en la prestación de la ayuda para morir recogida en la Ley Orgánica 3/2021 de Regulación de la eutanasia. Objetivo. Explorar las actitudes de médicos y enfermeras de cuidados paliativos en relación con la participación enfermera en la prestación de ayuda para morir. Metodología. Estudio con metodología cualitativa, realizado desde el marco teórico de la fenomenología interpretativa. La recogida de los datos se hará mediante entrevistas individuales a los participantes, para posteriormente realizar un análisis de contenido de los datos recogidos. Resultados. A través del análisis de las entrevistas y la categorización de la información obtenida emergió la importancia de la participación de las enfermeras en todo el proceso de prestación de la ayuda para morir y en especial en el proceso deliberativo. Conclusiones. Todos los profesionales estuvieron de acuerdo qué en el procedimiento de la prestación de ayuda para morir, además de la participación del médico, es necesaria una mayor participación de otros profesionales sanitarios., There is a controversy generated by the role of the nurse in the provision of the aid in dying contained in the Organic Law 3/2021 on the Regulation of euthanasia. Objective. To explore the attitudes of palliative care physicians and nurses regarding nurse participation in the provision of aid in dying. Methodology. Study with qualitative methodology, carried out from the theoretical framework of interpretative phenomenology. The data will be collected through individual interviews with the participants, to later carry out a content analysis of the data collected. Results. Through the analysis of the interviews and the categorization, subcategorization of the information we obtained the main findings that expressly raise the importance of the participation of nurses in the provision of aid in dying. Conclusion. Objetivo: Determinar la satisfacción de pacientes con ictus y personas con afasia producida por una discapacidad intelectual al utilizar sistema alternativo y aumentativo de comunicación de alta tecnología (SAACAT) Metodología: Estudio descriptivo transversal realizado en pacientes con ictus y en padres de niños con afasia producida por discapacidad intelectual. Para conocer la satisfacción con el SAACAT se elaboro un cuestionario basado en la escala Stroke and Aphasia Quality of Life Scale -39 (SAQOL -39). Resultados: En la Unidad de Ictus solo se puedo reclutar a una persona que, a través de un familiar, mostró una satisfacción alta con el uso del SAACAT De los ocho padres de los niños con afasia por discapacidad intelectual, seis mostraron una satisfacción alta y dos una satisfacción media. Siete de ellos también señalaron el haber notado una mejora en la capacidad de sus hijos para hacerse entender por otras personas. Conclusiones: El uso de SAACAT en personas con afasias que han sufrido un ictus o presentan un trastorno cognitivo por discapacidad intelectual mejora la comunicación y el vocabulario, así como la relación familiar y social., Objective: To determine the satisfaction of patients with stroke and people with a intellectual disability when using an alternative high-tech communication system (AHTCS) Methodology: Cross-sectional descriptive study carried out in patients with stroke and in parents of children with aphasia caused by an intellectual disability. To determine satisfaction with the AHTCS, a questionnaire based on the Stroke and Aphasia Quality of Life Scale -39 (SAQOL -39) was developed. Results: In the Stroke Unit, it is only possible to recruit a person who, through a family member, showed high satisfaction with the use of the AHTCS. Of the eight parents of children with aphasia due to intellectual disability, six showed high satisfaction and two medium satisfaction. Seven of them also noted an improvement in their children's ability to make themselves understood by other people. 46 p., Antecedentes: En España el índice de otorgamiento de instrucciones previas es bajo, a fecha de abril de 2019 solo un 0,61% de la población española ha cumplimentado el documento de Instrucciones Previas (IIPP). Diversos estudios realizados por distintos autores reflejan la existencia de un nivel bajo de conocimientos sobre las instrucciones previas. Objetivo: Analizar los conocimientos y las actitudes de las enfermeras de Atención Primaria y Atención Especializada del Área Asistencial Este de la Comunidad de Madrid sobre la Planificación Anticipada de Decisiones (PAD) y las IIPP. Metodología: Estudio piloto observacional, descriptivo y transversal realizado mediante un cuestionario ad hoc sobre conocimientos y actitudes sobre las PA y las IIPP. Resultados: En total se cumplimentaron 44 cuestionarios. Los resultados reflejan un bajo conocimiento de la PAD (Mediana= 4; RI:2-6) y de las IIPP (Mediana= 5; RI: 4-7). No obstante, enfermeras consideran las IIPP una herramienta útil, en la toma de decisiones, para los profesionales y para las familias. El 72,8% de las enfermeras considera que sus pacientes crónicos no están bien informados sobre su enfermedad. Conclusiones: El conocimiento sobre la PAD y las IIPP de las enfermeras participantes es bajo, siendo necesario aumentar la formación sobre el tema. Sin embargo, se consideran herramientas muy útiles, tanto para familiares como profesionales. Además, las enfermeras consideran que sus pacientes crónicos no están bien informados de la evolución de la enfermedad, lo que se relaciona con la conspiración del silencio., Background: The Advance Directives (AD) document has been only completed by 0.61% of the Spanish population. Previous studies conducted by different health professionals reflect a general lack of knowledge on the subject. In order to provide patients with decision-making capacities in the context of their treatment and care, good communication with professionals is needed. Objective: To determinate the knowledge and attitudes of Primary and Specialized Care nurses of the East Care Area of the Community of Madrid about the Advance Care Planning (ACP) and AD. Methodology: an observational, descriptive and cross-sectional pilot study was conducted through an ad hoc questionnaire measuring knowledge and attitudes about ACP and AD. Results: In total, 44 questionnaires were completed, reflecting low levels of knowledge about both the ACP (Median=4) and the AD (Median=5). The AD is considered a useful tool both for professionals and families. On a percentage scale, 72.8% of the respondents considered that their chronic patients were not well informed about the evolution of their disease. Conclusions: The knowledge about the ACP and AD of the nurses of the Eastern Health Care Area of the Community of Madrid is considerably low. Although, they are considered very useful tools both for family members and professionals, implementing more training programs becomes imperative. Participants were assured of maximum confidentiality and anonymity., Findings: A total of 421 nurses answered the questionnaire. In total, 55.6% of the nurses confirmed they were religious believers, and 64.3% declared having poor knowledge regarding conscientious objection. The matters that caused the greatest objections were voluntary abortions, genetic embryo selection, refusal of blood transfusions, and therapy refusal., Discussion: Different authors state that the most significant cases of conscientious objections for health professionals are those regarding carrying out or assisting in abortions, euthanasia, the practice of assisted reproduction and, finally, the prescription and dispensing of the morning-after pill. In our study, the most significant cases in which the nurses would declare conscientious objections would be the refusal to accept treatment, the selection of embryos after genetic diagnosis preimplantation, the patient's refusal to receive blood transfusions due to religious reasons and pregnant women's request for voluntary abortions within the first 14 weeks., Conclusion: Nurses' religious beliefs influence their opinions regarding conscientious objection. The nurses who declare themselves as religious believers object in a higher percentage than those without religious beliefs.

Objectives: To describe and compare the knowledge and attitudes of the physicians and nurses towards the advance directives., Design: A descriptive, cross-sectional pilot study., Setting: East healthcare area of the Community of Madrid (Spain), Participants: Primary care and specialized care physicians and nurses., Main Measurements: Questionnaire about knowledge, use and attitudes from the healthcare professionals over the advance directives given, with dichotomous and Likert scale (0-10) variables., Results: Replies were received from a total of 192 physicians and nurses (response rate=83,4%), 72,4% were women and 27,6% were men. The mean age was 39,6 years (SD=10,86). For general knowledge on advance directives the median was 5 (RI=3-7). 60,1% were aware of the regulation by law, but only 22,8% had read the document., Conclusions: The knowledge of physicians and nurses on advance directives is low, so it is necessary to improve this knowledge. Physicians and nurses from both levels show positive attitude towards the use and usefulness and respect the contents of advance directive. The methodology proposed is efficient to implement in a larger study, but should improve the distribution and collection of questionnaires., (Copyright © 2012 Elsevier España, S.L. - Abstract
Human milk has been and it is an important means of survival for the human being. The history of the breastfeeding has been linked to the woman's social and cultural situation and it has gone by different vicissitudes. During a long period the breastfeeding was considered as an unsightly unworthy practice and characteristic of low classes, women that had resources nurses used. In the XIX century artificial nursing appears that will be developed significantly starting from half-filled of the following century relegating, again, to a second plane to the natural nursing. With the beginning of the XX century the first rights of the workers appear and among them it is regulated the right for the first time to the permission for nursing. With the advance of the century diverse norms enlarge their content. Arriving to the current moment in which the right enjoys a wide legal recognition inside the norms that regulate the family reconciliation.

- Abstract
Death is one of the few certainties in human beings. This reality generates uneasiness regarding how death will occur especially in circumstances of loneliness or suffering which can become agony. In this report, the author first analyzes the existence of the right to die with dignity as a human right which takes on the nature of being a subjective right. In continuation, the author describes the existing problematic in the application of this right to die in relationship to the so-called double effect mechanism and euthanasia. The author concludes this article by proposing previous instructions or anticipated desires as ideal measures to make the right to die with dignity valid.
