5. Distinct Driver Pathway Enrichments and a High Prevalence of TSC2 Mutations in Right Colon Cancer in Chile: A Preliminary Comparative Analysis.

Colorectal cancer (CRC) is the second leading cause of cancer deaths globally. While ethnic differences in driver gene mutations have been documented, the South American population remains understudied at the genomic level, despite facing a rising burden of CRC. We analyzed tumors of 40 Chilean CRC patients (Chp) using next-generation sequencing and compared them to data from mainly Caucasian cohorts (TCGA and MSK-IMPACT). We identified 388 mutations in 96 out of 135 genes, with TP53 (45%), KRAS (30%), PIK3CA (22.5%), ATM (20%), and POLE (20%) being the most frequently mutated. TSC2 mutations were associated with right colon cancer (44.44% in RCRC vs. 6.45% in LCRC, p-value = 0.016), and overall frequency was higher compared to TCGA (p-value = 1.847 × 10
−5 ) and MSK-IMPACT cohorts (p-value = 3.062 × 10−2 ). Limited sample size restricts definitive conclusions, but our data suggest potential differences in driver mutations for Chilean patients, being that the RTK-RAS oncogenic pathway is less affected and the PI3K pathway is more altered in Chp compared to TCGA (45% vs. 25.56%, respectively). The prevalence of actionable pathways and driver mutations can guide therapeutic choices, but can also impact treatment effectiveness. Thus, these findings warrant further investigation in larger Chilean cohorts to confirm these initial observations. 11. Diversity, Equity, and Inclusion in Independent Schools: How Silence and Privilege Hinder the Work

Most private or independent schools originated as schools of privilege, serving the offspring of a culturally and socially elite group of white American families (Cookson, 2013; Flewelling, 2013; Slaughter-Defoe & Johnson, 1988). Since the 1960s these institutions have worked to change their predominantly White, elitist image by broadening their access and striving to become diverse and inclusive institutions that prepare all students to “thrive in a global, multicultural community” (Quanti, 2013, p. 13). Although independent schools have made some progress toward diversity, equity, and inclusion, more work remains (Bisgaard, 2005). Independent schools continue to serve low numbers of historically underrepresented groups, such as African American and Latino students. These students often feel disconnected from their school community (Arrington, Hall, & Stevenson, 2003; National Association of Independent Schools [NAIS], 2015; White & Boyd, 2015). Despite increases in numerical diversity, some schools continue to lack the appropriate systems to support students of color, particularly through faculty professional development, curricula, and school programming (Stevenson, 2014). Research stresses the importance of schools engaging in strategic planning for diversity and inclusion along with continual monitoring and assessment to identify any “gaps between [a] school’s vision and its current reality and direction” (Arrington et al., 2003; Harris, 2013, p. 44). This qualitative multi-site case study investigated how three Southern California member schools of the National Association of Independent Schools and the California Association of Independent Schools take their espoused commitments related to diversity, equity, and inclusion and make them the lived experience of the school. The study examined how these institutions carry out, monitor, and measure their stated goals related to these topics, particularly through recruitment practices, curricula, and professional development. Finally, this study investigated the barriers schools face in this work.The findings from this study demonstrated that independent schools continue to need programs and policies that support diversity, equity, and inclusion, as well as tools to evaluate the outcomes of such initiatives. Across the three schools the barriers of silence, discomfort, and a lack of engagement around diversity, equity, and inclusion hindered further progress in this work. These reactions to diversity, equity, and inclusion relate to White privilege and White fragility and result in slow cultural and organizational change within historically White independent schools (DiAngelo, 2011; Hossain, 2015).

Background: The potential role of the gut microbiome as a predictor of immune-mediated HIV-1 control in the absence of antiretroviral therapy (ART) is still unknown. In the BCN02 clinical trial, which combined the MVA.HIVconsv immunogen with the latency-reversing agent romidepsin in early-ART treated HIV-1 infected individuals, 23% (3/13) of participants showed sustained low-levels of plasma viremia during 32 weeks of a monitored ART pause (MAP). Here, we present a multi-omics analysis to identify compositional and functional gut microbiome patterns associated with HIV-1 control in the BCN02 trial. Results: Viremic controllers during the MAP (controllers) exhibited higher Bacteroidales/Clostridiales ratio and lower microbial gene richness before vaccination and throughout the study intervention when compared to non-controllers. Longitudinal assessment indicated that the gut microbiome of controllers was enriched in pro-inflammatory bacteria and depleted in butyrate-producing bacteria and methanogenic archaea. Functional profiling also showed that metabolic pathways related to fatty acid and lipid biosynthesis were significantly increased in controllers. Fecal metaproteome analyses confirmed that baseline functional differences were mainly driven by Clostridiales. Participants with high baseline Bacteroidales/Clostridiales ratio had increased pre-existing immune activation-related transcripts. The Bacteroidales/Clostridiales ratio as well as host immune-activation signatures inversely correlated with HIV-1 reservoir size. Conclusions: The present proof-of-concept study suggests the Bacteroidales/Clostridiales ratio as a novel gut microbiome signature associated with HIV-1 reservoir size and immune-mediated viral control after ART interruption. La sobredentadura retenida por medio de dos implantes se ha convertido en una buena opción para la rehabilitación del edentulismo total inferior, debido a que es un procedimiento relativamente simple y poco invasivo; presenta una baja pérdida del nivel óseo peri-implantar, buen comportamiento protésico y una alta satisfacción por parte de los pacientes, pero no se ha estudiado a profundidad su efecto sobre el reborde alveolar posterior. Objetivo: Determinar los cambios en altura del reborde alveolar posterior mandibular después de 7 años en pacientes que usan sobredentadura mandibular sobre dos implantes conectados por ajustes en bola. Metodología: Estudio observacional descriptivo comparativo de serie de casos. Muestra por conveniencia de 17 pacientes. Cada paciente recibió una sobredentadura mandibular retenida por dos implantes no ferulizados cargados de forma inmediata y unidos mediante ajustes en bola. De cada participante, se obtuvieron dos radiografías panorámicas (una tomada inmediatamente antes de la inserción de la sobredentadura [T0] y la otra tomada 7 años después de la inserción [T7]), el método consistió en mediciones proporcionales de área de la mandíbula posterior, las mediciones fueron basadas y validadas en estudios previos. Resultados: Se encontró que el cálculo de los cambios aproximados en la altura del reborde alveolar residual posterior resultó en una pérdida estimada de altura de 1,93 mm a los 7 años de seguimiento, lo que corresponde a una reabsorción aproximada de 0,27 mm por año. Conclusión: En el presente estudio, la reabsorción del reborde alveolar posterior a los 7 años estuvo dentro de los parámetros considerados normales por la literatura científica, lo que parece indicar que no existe ningún efecto nocivo de la sobredentadura sobre el reborde alveolar. The retained overdenture withheld through two implants has become a good option for the rehabilitation of lower total edentulism, because it is a relatively simple and minimally invasive procedure; presents a low loss of peri-implant bone level, good prosthetic performance and high patient satisfaction, although its effect on the posterior alveolar ridge has not been studied in depth. Objective: To determine the changes in height of the mandibular posterior alveolar ridge after 7 years in patients using a mandibular overdenture on two implants connected by ball adjustments. Methodology: Comparative descriptive observational study of a series of cases. Convenience sample of 17 patients. Each patient received a mandibular overdenture retained by two non-splinted implants that were immediately loaded and joined by ball adjustments. Two panoramic radiographs were obtained from each participant (one taken immediately before insertion of the overdenture [T0] and the other taken 7 years after insertion [T7]), the method consisted of proportional measurements of posterior mandible area. Results: It was found that the calculation of the approximate changes in the height of the posterior residual alveolar ridge resulting in an estimated loss of height of 1.93 mm at 7 years of follow-up, which corresponds to an approximate reabsorption of 0.27 mm by year. Conclusion: In the present study, the resorption of the alveolar ridge after 7 years was within the parameters considered normal by the scientific literature, which seems to indicate that there is no harmful effect of the overdenture on the alveolar ridge.

- Abstract
Background The banning of mass-gathering indoor events to prevent SARS-CoV-2 spread has had an important effect on local economies. Despite growing evidence on the suitability of antigen-detecting rapid diagnostic tests (Ag-RDT) for mass screening at the event entry, this strategy has not been assessed under controlled conditions. We aimed to assess the effectiveness of a prevention strategy during a live indoor concert. Methods We designed a randomised controlled open-label trial to assess the effectiveness of a comprehensive preventive intervention for a mass-gathering indoor event (a live concert) based on systematic same-day screening of attendees with Ag-RDTs, use of facial masks, and adequate air ventilation. The event took place in the Sala Apolo, Barcelona, Spain. Adults aged 18–59 years with a negative result in an Ag-RDT from a nasopharyngeal swab collected immediately before entering the event were randomised 1:1 (block randomisation stratified by age and gender) to either attend the indoor event for 5 hours or go home. Nasopharyngeal specimens used for Ag-RDT screening were analysed by real-time reverse-transcriptase PCR (RT-PCR) and cell culture (Vero E6 cells). 8 days after the event, a nasopharyngeal swab was collected and analysed by Ag-RDT, RT-PCR, and a transcription-mediated amplification test (TMA). The primary outcome was the difference in incidence of RT-PCR-confirmed SARS-CoV-2 infection at 8 days between the control and the intervention groups, assessed in all participants who were randomly assigned, attended the event, and had a valid result for the SARS-CoV-2 test done at follow-up. The trial is registered at ClinicalTrials.gov, NCT04668625. Findings Participant enrollment took place during the morning of the day of the concert, Dec 12, 2020. Of the 1140 people who responded to the call and were deemed eligible, 1047 were randomly assigned to either enter the music event (experimental group) or continue with normal life (control group). The dynamic properties of this cytoskeleton were explored by time-lapse confocal and video microscopy. Cytoskeleton assembly was investigated in zygotes pulse-labeled with microinjected fluorophore-tagged or biotin-tagged dimeric tubulin and G-actin. Cytoskeleton disassembly was studied by comparing the linear dimensions of the cytoskeleton at different time points during late interphase. The relative distributions of F- and-G-actin were determined after microinjection of rhodamine-labeled actin and fluorescein-labeled DNase I. Results showed that labeled precursors were readily incorporated into a network of microtubules or actin filaments. Bipolar translocation of the rings and meridional bands was accompanied by the rapid assembly and disassembly of microtubules and actin filaments. Because labeled microtubules and microfilaments gradually decreased, the rate of cytoskeleton disassembly was greater than the rate of cytoskeleton assembly. Hence, ooplasmic segregation was accompanied by the rapid turnover of cytoskeletal components. Co-distribution of F- and-G-actin during mid and late interphase may favor polymer-monomer interchange. We conclude that cytoskeleton reorganization during foundation of cytoplasmic domains can be conveniently studied in the live leech zygote after microinjection of labeled precursors. - Abstract
About 4% to 7% of the non–small-cell lung cancer patients have anaplastic lymphoma kinase (ALK) rearrangements, and specific targeted therapies improve patients' outcomes significantly. ALKgene fusions are detected by immunohistochemistry or fluorescent in situhybridization as gold standards in South America. Next-generation sequencing–based assays are a reliable alternative, able to perform simultaneous detection of multiple events from a single sample. We analyzed 4240 non–small-cell lung cancer samples collected in 37 hospitals from Chile, Brazil, and Peru, where ALK rearrangements were determined as part of their standard of care (SofC) using either immunohistochemistry or fluorescent in situhybridization. A subset of 1450 samples was sequenced with the Oncomine Focus Assay (OFA), and the concordance with the SofC tests was measured. An orthogonal analysis was performed using a real-time quantitative PCR echinoderm microtubule-associated protein-like 4-ALK fusion detection kit. ALK fusion prevalence is similar for Chile (3.67%; N = 2142), Brazil (4.05%; N= 1013), and Peru (4.59%; N= 675). Although a comparison between OFA and SofC assays showed similar sensitivity, OFA had significantly higher specificity and higher positive predictive value, which opens new opportunities for a more specific determination of ALKgene rearrangements.

Kick&kill strategies combining drugs aiming to reactivate the viral reservoir with therapeutic vaccines to induce effective cytotoxic immune responses hold potential to achieve a functional cure for HIV-1 infection. Here, we report on an open-label, single-arm, phase I clinical trial, enrolling 15 early-treated HIV-1-infected individuals, testing the combination of the histone deacetylase inhibitor romidepsin as a latency-reversing agent and the MVA.HIVconsv vaccine. Romidepsin treatment resulted in increased histone acetylation, cell-associated HIV-1 RNA, and T-cell activation, which were associated with a marginally significant reduction of the viral reservoir. Vaccinations boosted robust and broad HIVconsv-specific T cells, which were strongly refocused toward conserved regions of the HIV-1 proteome. During a monitored ART interruption phase using plasma viral load over 2,000 copies/ml as a criterium for ART resumption, 23% of individuals showed sustained suppression of viremia up to 32 weeks without evidence for reseeding the viral reservoir. Results from this pilot study show that the combined kick&kill intervention was safe and suggest a role for this strategy in achieving an immune-driven durable viremic control. [ABSTRACT FROM AUTHOR]

Salmon esh spoilage can be greatly reduced through the use of preservation methods, using natural products combined with low temperature and packaging. Microbiological and physicochemical characteristics of fresh salmon llets (Salmo salar), sprayed with an aromatic vinegar and stored in modi ed atmosphere packaging, were investigated. Fillets were kept at 4°C and sampled after 2 h and 3, 7 and 10 days. An untreated control group was used as well. Fish samples were analysed for microbiological (total viable count, Enterobacteriaceae, psychrotrophic microbial count, Pseudomonas spp.) and physicochemical (pH, colour, total volatile basic nitrogen, and biogenic amines) properties. Aromatic vinegar was found to bene cially contribute to the hygienic quality of the salmon, reducing microbial growth during storage and exerting a positive effect, mainly on psychrotrophic loads and Pseudomonas spp. The treatment had a positive effect on biogenic amine levels, showing lower values for histamine, putrescine, cadaverine, and tyramine. - Abstract
The objective of this research is to relate the benchmark interest rate, the prices of oil prices and the nominal exchange rate; to determine to what extent each of these exogenous variables a"ect the exchange rate. The methodology used for this purpose is to wield the theory raised by Mauricio Cardenas (Introduction to the Colombian Economy, 2013) and the implementation of Mundell - Fleming. The analysis is done by implementing regression models using ordinary least squares, GARCH, EGARCH and VAR. The study warns that the exchange rate was a"ected by elements such as: the lowest price of WTI, the collapse of the major stock exchanges of the world, the decision of the US central bank to keep its benchmark interest rate and the devaluation of the Chinese currency Yuan. The study concludes that oil prices and the exchange rate are negatively related, so a devaluation in oil prices leads to a depreciation of the peso. Similarly, the reference interest rate is inversely related to the rate of change, allowing accept referrals of Mundell - Fleming., O objetivo desta pesquisa é relacionar a referência taxa de juro, os preços do petróleo e da taxa de câmbio nominal; para determinar em que medida cada uma dessas variáveis exógenas afetam a taxa de câmbio. A metodologia utilizada para esta &nalidade é de manejar a teoria levantada por Mauricio Cárdenas (Introdução à Economia colombiana, 2013) ea implementação de Mundell - Fleming. A análise é feita através da aplicação de modelos de regressão utilizando mínimos quadrados ordinários, GARCH, EGARCH e VAR. O estudo adverte que a taxa de câmbio foi in+uenciada por elementos, tais como: o menor preço do WTI, o colapso das principais bolsas de valores do mundo, a decisão do banco central dos EUA para manter a sua taxa de juro de referência e a desvalorização da moeda chinesa Yuan. O estudo conclui que o preço do petróleo e da taxa de câmbio está negativamente relacionado, de modo à desvalorização dos preços do petróleo leva a uma desvalorização do peso. Da mesma forma, a taxa de juro de referência está inversamente relacionada com a taxa de mudança, permitindo aceitar referências de Mundell - Fleming., El objetivo de esta investigación reside en relacionar la tasa de interés de referencia, las cotizaciones del precio del petróleo y la tasa de cambio nominal, para determinar en qué medida cada una de estas variables exógenas afectan la tasa de cambio. La metodología utilizada para tal propósito consiste en esgrimir la teoría planteada por Mauricio Cárdenas (Introducción a la Economía Colombiana, 2013) y la implementación del modelo Mundell-Fleming. El análisis se hace mediante la implementación de modelos de regresión por Mínimos Cuadrados Ordinarios, GARCH, EGARCH y VAR. El estudio advierte que la tasa de cambio se vio afectada por elementos como: la cotización más baja del WTI, el desplome de las principales bolsas de valores del mundo, la decisión del Banco Central de Estados Unidos de mantener su tasa de interés de referencia y la devaluación de la moneda china, Yuan. El estudio concluye que los precios del crudo y la tasa de cambio están relacionados de manera negativa (tal y como lo sugieren AUTORES), por lo que una desvalorización en los precios del petróleo conduce a una depreciación del peso. De manera análoga, la tasa de interés de referencia está relacionada de manera inversa con la tasa de cambio, permitiendo aceptar las derivaciones del modelo Mundell – Fleming.

Hoy en día ha ido aumentando la necesidad de encontrar procesos que permitan obtener agua limpia, debido a actividades de carácter antrópico que poco a poco han afectado la calidad del recurso tal como menciona Greenpeace Colombia en su página web (Greenpeace, 2016) y como lo menciona la Secretaria Distrital de Ambiente y el Acueducto en un documento estructurado por la Pontificia Universidad javeriana denominado como “Calidad del sistema hídrico de Bogota”,donde resalta que a pesar de la importancia que tiene este recurso para el hombre tan solo en la última década se ha empezado a generar una preocupación, debido a la escasez que se ha presentado en los últimos periodos. Según una evaluación realizada para un proyecto por la GIWA1 ([GIWA], 2001) indicó que la presión constante de las actividades humanas viene deteriorando drásticamente los ecosistemas acuáticos y como resultado se tiene que en promedio para el año 2020 no será nada fácil la accesibilidad al agua potable, adicional a ello se suman los problemas de salud. Dentro de los problemas de salud identificados por problemas de aguas se encuentra su relación con el aluminio, cromo y coliformes como se resalta a continuación: El aluminio forma parte del 8 % (aproximadamente) de la corteza terrestre, por lo tanto el ser humano se encuentra expuesto a este metal por la presencia que hay en el medio ambiente, aunque este no es el único medio de exposición debido a que se ha industrializado, hace parte de muchos productos, tales como: los envases, aditivos para alimentos, cosméticos, utensilios de cocina, textiles, colorantes, desodorantes, medicamentos entre otros. Aunque aún no se tenga un estimado de lo perjudicial que puede ser para la salud de los seres vivos, se prevee que en el futuro pueda causar severas consecuencias. 37. Liolaemus lonquimayensis (Squamata: Liolaemidae), a new lizard species for Chile without precloacal pores

- Abstract
This study describes a new species of lizard of the genus Liolaemus (L. lonquimayensis), without precloacal pores, associated with the lava flows of the Lonquimay volcano in the Región de la Araucanía, Chile. This species belongs to the elongatus clade, and is a sister lineage of L. elongatus. L. lonquimayensis differs from its sister species, L. elongatus, in the total absence of precloacal pores, smaller maximum SVL (69.7 mm in L. lonquimayensis and 91.0 mm in L. elongatus), greater number of midline scales (88 in L. lonquimayensis and 77 in L. elongatus), body color light grey to black in L. lonquimayensis and light to dark brown in L. elongates. L. lonquimayensis has an black occipital band formed by small black lines arranged horizontally flanked by two lighter dorsal longitudinal bands, more separated towards the tail, forming incomplete rings. Finally, a combination of genetic, meristic and morphological characters distinguishes L. lonquimayensis from the other species of the elongatus clade. En el presente estudio se describe una nueva especie de lagartija para el género Liolaemus (Liolaemus lonquimayensis), carente de poros precloacales y vinculada a los flujos de lava del Volcán Lonquimay, Región de la Araucanía, Chile. Esta especie se adscribe al clado elongatus y constituye un linaje hermano de L. elongatus. L. lonquimayensis difiere de su especie hermana L. elongatus por la total ausencia de poros precloacales, su menor LHC maxima (69.7 mm en L. lonquimayensis y 91.0 mm en L. elongatus), mayor número de escamas al medio del cuerpo (88 en L. lonquimayensis y 77 en L. elongatus), color del cuerpo gris claro a negro en L. lonquimayensis y café claro a oscuro en L. elongates. El diseño dorsal de L. lonquimayensis está compuesto por una banda occipital negra formada por pequeñas líneas negras dispuestas horizontalmente flanqueadas por dos bandas longitudinales claras. Estas líneas se van espaciando más hacia la cola, formando anillos incompletos. Finalmente una combinación de caracteres genéticos, merísticos y morfológicos distinguen a L. lonquimayensis de las otras especies del clado elongatus.

- Abstract
Extensively pretreated subjects with virological failure (VF) may receive salvage regimens containing NRTIs with only residual or no activity. Once virological suppression is achieved, their contribution remains elusive. This was a multicentre, randomized, prospective study. Subjects with at least one prior VF, HIV-1 RNA < 50 copies/mL for a parts per thousand yen6 months and receiving a regimen with at least two active drugs (one of them a boosted PI) were randomized 1:1 to stop (experimental arm) or maintain (control arm) NRTIs. EudraCT: 2012-000198-21. Ninety subjects were randomized (experimental, naEuroS=aEuroS45; and control, naEuroS=aEuroS45). The mean age was 50 years, 80% were male, the mean CD4+ cell count was 542 cells/mm(3) and the median number of prior VFs was 3. Seventy-four subjects (82%) harboured the mutation M184V/I and the median number of thymidine-associated mutations was 3 (IQR: 0-4). In the experimental arm, thirty-two (71%) subjects removed one NRTI and 13 (29%) subjects removed two. Twenty-two of 45 (49%) discontinued tenofovir disoproxil fumarate. Forty-one of 45 (91.1%, experimental arm) and 44 of 45 (97.8%, control arm) had HIV-1 RNA < 50 copies/mL at 48 weeks (difference: -6.7%; 95% CI: -17.4, 4.1). In a post-hoc analysis allowing NRTI reintroduction, efficacy rates were 95.6% and 97.8%, respectively (difference: -2.2%; 95% CI: -7.2, 2.7). Rates of discontinuation at 48 weeks were 2% in both arms. One subject developed a late VF with resistance selection. In patients receiving a successful multidrug salvage regimen with at least two active drugs (one a boosted PI), the withdrawal of inactive NRTIs was safe, rates of VF were low and drug resistance was uncommon at 48 weeks in this small study. This strategy could potentially prevent long-term toxicities, reduce the number of drugs and reduce costs if non-inferiority was met in a fully powered trial., Peer Reviewed, Postprint (published version)

Background: Mental retardation or intellectual disability affects 2% ofthe general population, but in 60% to 70% of cases the real cause ofthis retardation is not known. An early etiologic diagnosis of intellectual disability can lead to opportunities for improved educational interventions, reinforcing weak aáreas and providing a genetic counseling to the family Aim: To search genetic diseases underíying intellectual disabilities of children attending a special education school. Material and methods: A clinical geneticist performed the history and physical examination in one hundred and three students aged between 5 and 24 years (51 males). A blood sample was obtained in 92 of them for a genetic screening that included a standard karyotype, fragile X molecular genetic testing and search for inborn errors of metabolism by tándem mass spectrometry. Results: This approach yielded an etiological diagnosis in as much as 29 patients. Three percent of them had a fragile X syndrome. Inborn errors of metabolism were not detected. Conclusions: This type of screening should be done always in children with intellectual disability to establish an etiological diagnosis.
