Abstract

Introduction: Harmonized neuropsychological assessment for neurocognitive disorders, an international priority for valid and reliable diagnostic procedures, has been achieved only in specific countries or research contexts.Methods: To harmonize the assessment of mild cognitive impairment in Europe, a workshop (Geneva, May 2018) convened stakeholders, methodologists, academic, and non-academic clinicians and experts from European, US, and Australian harmonization initiatives.Results: With formal presentations and thematic working-groups we defined a standard battery consistent with the U.S. Uniform DataSet, version 3, and homogeneous methodology to obtain consistent normative data across tests and languages. Adaptations consist of including two tests specific to typical Alzheimer's disease and behavioral variant frontotemporal dementia. The methodology for harmonized normative data includes consensus definition of cognitively normal controls, classification of confounding factors (age, sex, and education), and calculation of minimum sample sizes.Discussion: This expert consensus allows harmonizing the diagnosis of neurocognitive disorders across European countries and possibly beyond.

13. Misprojection of Landmarks onto the Spatial Map

Abstract

It has been suggested that neglect patients misrepresent the metric spatial relations along the horizontal axis (anisometry). The ''fabric'' of their internal spatial medium would be distorted in such a way that physically equal distances appear relatively shorter on the contralesional side (canonical anisometry). The case of GL, a 76-year-old lady with left neglect on visual search tasks, is presented. GL showed severe relative overestimation on the left (contralesional) side on two independent tasks evaluating the metrics of her internal representation. A qualitatively similar pattern was found in two out of 10 other neglect patients who performed the second task. This behavior cannot be accounted for by the canonical anisometry hypothesis. Nevertheless, GL produced a relative left overextension (underestimation) when trying to set the endpoints of a virtual line given its midpoint (Endpoints Task). An interpretation of these results is offered in terms of a misprojection of relevant landmarks onto the internal representation without assuming distortion of its ''fabric.''

This heterogeneity is crucial in the variability debate.

Abstract

The Tower of London (ToL) test is traditionally used to assess strategical reasoning, problem-solving, and mental planning in clinical populations. Here, we provide the Italian standardization norms for the original, 12-problem version of the ToL test. The performance of 216 Italian individuals ranging 18 to 89 in age was scored in terms of both Time (Speed) and Accuracy —the time, and the number of attempts, necessary to find a solution. We performed univariate analyses on separate Time and Accuracy scores, using Age in years, Education in years, and Sex (male vs. female) as predictors. z scores and equivalent scores were provided. Moreover, we performed a bivariate analysis for the assessment of individuals' performance in terms of Time and Accuracy simultaneously. This standardization allows clinicians to use the original, most widespread version of ToL with the Italian population, thus optimizing comparability with other clinical and experimental research worldwide. Critically, this article offers a new statistical perspective on how Time and Accuracy scores, which are typically related to each other, can be combined to obtain a single, consistent clinical categorization that captures most of the information contained in the patient's performance. Abstract

Some studies on Social Cognition demonstrated that high level cognitive processes, such as empathy for pain, are influenced by racial bias. In this study we investigated whether this bias modulates the (lower) mechanism of attribution of intention too. We used a Simon-like paradigm, consisting of judging the colour of a circle at the centre of photographs of Caucasian and Black hands, from palm or back view, slightly rotated along their axes, implying a motor attitude/movement. Results were compatible with the hypothesis that palm views are interpreted as threatening by the observer. Moreover, women and men showed different reactions to black palm views (threatening stimuli): while the former showed a speeding up of reaction times, witnessing a defensive inherent response, men showed a slowing down of reaction times, suggesting a deeper analysis of stimuli (counterattack response).

Abstract

Le abilità visuomotorie sono alla base di una vasta gamma di azioni quotidiane. La corretta programmazione del recupero di pazienti che, a seguito di menomazioni a carico del sistema nervoso centrale, subiscono una compromissione di tali abilità, richiede lo sviluppo di sistemi di valutazione in grado di stabilire quantitativamente quale dei livelli di organizzazione del sistema visuomotorio sia coinvolto. Basandoci sulle assunzioni proposte agli inizi degli anni Novanta da Milner e Goodale riguardo all’esistenza di due vie di processamento dell’informazione visiva (ventral stream e dorsal stream) che interagiscono nella pianificazione e nella programmazione del movimento, il nostro studio si è proposto di validare l’utilizzo di una metodica in grado di valutare quantitativamente e selettivamente le abilità percettive e visuomotorie connesse, rispettivamente, a tali vie. Diciannove soggetti non patologici destrimani (età media 22.8 anni ±3.18) hanno partecipato all’esperimento. Sono state somministrate prove di puntamento ritardato, di pantomima di distanza ritardata e di indicazione della posizione opposta a quella ricordata sull’asse orizzontale per la valutazione del ventral stream; prove di afferramento e di puntamento immediato per la valutazione del dorsal stream. Le varie prove sono state filmate ed elaborate con il software di videoanalisi Dartfish ProSuite. L’analisi statistica dei risultati ha confermato vari pattern già noti in letteratura tra cui la predominanza attenzionale per i campi visivi in alto a sinistra, la predominanza del tono flessorio nei movimenti dell’arto superiore eseguiti nello spazio peripersonale prossimale, la tendenza a sovrastimare distanze ridotte e a sottostimare distanze maggiori, l’evitamento di stimoli di riferimento. To define a correct rehabilitation program for neurological patients who have impairment in these skills, there is a need for simple and cost-effective tools to determine which of the visual-motor system levels of organization are compromised by neurological lesions. In their 1995 book, The Visual Brain in Action (Oxford: Oxford University Press), AD Milner and MA Goodale proposed the existence of two pathways for the processing of visual information, the “ventral stream” and “dorsal stream,” that interact in movement planning and programming. Beginning with this model, our study aimed to validate a method to quantify the role of the ventral and dorsal streams in perceptual and visual-motor skills.Subjects and methods: Nineteen right-handed healthy subjects (mean age 22.8 years ± 3.18) with normal or corrected-to-normal vision were recruited. We proposed that a delayed pointing task, a distance reproduction task, and a delayed anti-pointing task could be used to assess the ventral stream, while the dorsal stream could be evaluated with a grasping task and an immediate pointing task.
