9. Exploring experiences and needs of spousal carers of people with Behavioural variant Frontotemporal Dementia (BvFTD) including those with Genetic variant FTD (GvFTD) : a qualitative study

Introduction: Frontotemporal dementia (FTD) is one of the most common types of dementia in persons under the age of 65. Diagnosis is often delayed due to slow gradual decline and misinterpretation of ‘non-typical’ dementia symptoms (linguistic, behavioural and personality changes). Given the younger age of onset and the challenging presentation of symptoms, particularly in those with behavioural variant type FTD (BvFTD), informal carers of people with FTD experience greater challenges than carers of people with more general forms of dementia. The aim of the present study was therefore to explore experiences of spousal carers of people with BvFTD, including those with familial or genetic links to the disease (GvFTD). Method: Fourteen qualitative interviews were analysed using an inductive approach to Thematic Analysis (Braun & Clarke, 2006) to understand the experiences of spousal carers of people with BvFTD and GvFTD with a particular focus on understanding how they get their needs met, and to identify any potential differences across these two groups. Results: Five main themes were identified that illustrate the experiences of spousal carers, including their needs, and the facilitators and barriers of getting their needs met. These included: a) The “Constant Battle” – A journey toward an FTD diagnosis, b) Shock, Relief and Fear – Challenges persist post diagnosis, c) The “Life Altering” impact – The loss of the spousal relationship and shifting roles, d) Adapting, Managing Symptoms and Receiving Carer Support, e) Lack of General Knowledge – Barriers to support. Conclusions: Findings highlight the need for healthcare professionals to be educated on the initial symptoms and presentations of FTD, and to enable carers and families to receive timely diagnosis and appropriate support. Future research should investigate the impact of GvFTD on carers and the family system, to explore positive or meaningful experiences in caring, as well as theory-driven research to identify helpful coping strategies for carers of people with FTD.

Power loading from plasma in the scrape-off layer limits the lifetime of plasma-facing components in tokamak-based power plants. The Mega Ampere Spherical Tokamak Upgrade [W. Morris et al., IEEE Trans. Plasma Sci. 46(5), 1217–1226 (2018)] (MAST-U) features four divertor strike points (SP) owing to its up-down symmetry. This paper introduces the five-camera infrared thermography system, covering all four SPs and their mode of operation on MAST-U. The system employs both medium and long wavelength cameras, offering high frame rates up to 4 kHz and spatial resolutions of down to 3 mm. Postprocessing involves artifact elimination and heat flux calculation using in-house analysis software that incorporates the inverse heat transfer code, THEODOR (THermal Energy Onto DivertOR). Within the 9th European Framework programme, since 2021 EUROfusion is operating five tokamaks under the auspices of a single Task Force called ‘Tokamak Exploitation’. The goal is to benefit from the complementary capabilities of each machine in a coordinated way and help in developing a scientific output scalable to future largre machines. The programme of this Task Force ensures that ASDEX Upgrade, MAST-U, TCV, WEST and JET (since 2022) work together to achieve the objectives of Missions 1 and 2 of the EUROfusion Roadmap: i) demonstrate plasma scenarios that increase the success margin of ITER and satisfy the requirements of DEMO and, ii) demonstrate an integrated approach that can handle the large power leaving ITER and DEMO plasmas. The Tokamak Exploitation task force has therefore organized experiments on these two missions with the goal to strengthen the physics and operational basis for the ITER baseline scenario and for exploiting the recent plasma exhaust enhancements in all four devices (PEX: Plasma EXhaust) for exploring the solution for handling heat and particle exhaust in ITER and develop the conceptual solutions for DEMO. The ITER Baseline scenario has been developed in a similar way in ASDEX Upgrade, TCV and JET. Key risks for ITER such as disruptions and run-aways have been also investigated in TCV, ASDEX Upgrade and JET. Experiments have explored successfully different divertor configurations (standard, super-X, snowflakes) in MAST-U and TCV and studied tungsten melting in WEST and ASDEX Upgrade. The input from the smaller devices to JET has also been proven successful to set-up novel control schemes on disruption avoidance and detachment.

In 2021 JET exploited its unique capabilities to operate with T and D–T fuel with an ITER-like Be/W wall (JET-ILW). This second major JET D–T campaign (DTE2), after DTE1 in 1997, represented the culmination of a series of JET enhancements—new fusion diagnostics, new T injection capabilities, refurbishment of the T plant, increased auxiliary heating, in-vessel calibration of 14 MeV neutron yield monitors—as well as significant advances in plasma theory and modelling in the fusion community. DTE2 was complemented by a sequence of isotope physics campaigns encompassing operation in pure tritium at high T-NBI power. Carefully conducted for safe operation with tritium, the new T and D–T experiments used 1 kg of T (vs 100 g in DTE1), yielding the most fusion reactor relevant D–T plasmas to date and expanding our understanding of isotopes and D–T mixture physics. Furthermore, since the JET T and DTE2 campaigns occurred almost 25 years after the last major D–T tokamak experiment, it was also a strategic goal of the European fusion programme to refresh operational experience of a nuclear tokamak to prepare staff for ITER operation. The key physics results of the JET T and DTE2 experiments, carried out within the EUROfusion JET1 work package, are reported in this paper. Progress in the technological exploitation of JET D–T operations, development and validation of nuclear codes, neutronic tools and techniques for ITER operations carried out by EUROfusion (started within the Horizon 2020 Framework Programme and continuing under the Horizon Europe FP) are reported in (Litaudon et al Nucl. Fusion accepted), while JET experience on T and D–T operations is presented in (King et al Nucl. Fusion submitted).

JET, the world’s largest operating tokamak with unique Be/W wall and tritium handling capability, completed a Deuterium-Tritium (D-T) campaign in 2021 (Maggi et al 29th Fusion Energy Conf. ) following a decade of preparatory experiments, dedicated enhancements, technical rehearsals and training (Horton et al 2016 Fusion Eng. Des. 109–111 925). Operation with tritium raises significant technical, safety and scientific challenges not encountered in standard protium or deuterium operation. This contribution describes the tritium operational requirements, pulses and technical preparations, new operating procedures, lessons learned and details on the achieved operational availability and performance. The preparation and execution of the recent JET tritium experiments benefitted from the previous experience in 1991 (Preliminary Tritium Experiment), 1997 (DTE1 campaign) and 2003 (Trace Tritium Campaigns) and consisted of the following five phases: technical rehearsals and scenario preparation, tritium commissioning, 100% tritium campaign, D-T campaign (DTE2), tritium clean-up. Following the clean-up JET resumed normal operation and is currently undertaking a further D-T campaign (DTE3).

This paper reports the first experiment carried out in deuterium–tritium addressing the integration of a radiative divertor for heat-load control with good confinement. Neon seeding was carried out for the first time in a D–T plasma as part of the second D–T campaign of JET with its Be/W wall environment. The technical difficulties linked to the re-ionisation heat load are reported in T and D–T. This paper compares the impact of neon seeding on D–T plasmas and their D counterpart on the divertor detachment, localisation of the radiation, scrape-off profiles, pedestal structure, edge localised modes and global confinement.

Abstract Introduction Carers of people with frontotemporal dementia (FTD) experience greater challenges than carers of people with other dementias due to the younger age of onset and the challenging presentation of symptoms. The aim of the present study was to explore experiences of spousal carers of people with bvFTD, including those with the familial form of the disease (fFTD). Method Fourteen qualitative interviews were analysed using an inductive approach to Thematic Analysis to understand experiences of spousal carers of people with bvFTD including those with fFTD. Results Five main themes were identified including: a) The “Constant Battle” – A journey toward an FTD diagnosis, b) Shock, Relief and Fear – Challenges persist post diagnosis, c) The “Life Altering” impact – The loss of the spousal relationship and shifting roles, d) Adapting, Managing Symptoms and Receiving Carer Support, e) Lack of General Knowledge – Barriers to support. Conclusions Healthcare professionals should be educated on the initial presentations of FTD, to enable carers and families receive timely diagnosis and appropriate support. Future research should investigate the impact of fFTD on carers and families, to explore positive or meaningful experiences in caring, as well as theory-driven research to identify helpful coping strategies for carers of people with FTD.

Abstract Introduction Supervised consumption services (SCS), intended to reduce morbidity and mortality among people who inject drugs, have been implemented in a variety of delivery models. We describe and compare access to and uptake of co-located and external services among clients accessing harm reduction-embedded (HR-embedded) and community health center-embedded (CHC-embedded) SCS models. Methods Cross-sectional baseline data were collected between November 2018 and March 2020 as part of a cohort of people who inject drugs in Toronto, Canada designed to evaluate one HR-embedded and two CHC-embedded SCS. This analysis was restricted to clients who reported accessing these SCS more than once in the previous 6 months. Participants were classified as HR-embedded or CHC-embedded SCS clients based on self-reported usage patterns. Client characteristics, as well as access to onsite services and referral and uptake of external services, were compared by SCS model. Results Among 469 SCS clients, 305 (65.0%) primarily used HR-embedded SCS and 164 (35.0%) primarily used CHC-embedded SCS. BackgroundWe compared Kaposi sarcoma (KS) risk in adults who started antiretroviral therapy (ART) across the Asia-Pacific, South Africa, Europe, Latin, and North America.MethodsWe included cohort data of human immunodeficiency virus (HIV)-positive adults who started ART after 1995 within the framework of 2 large collaborations of observational HIV cohorts. We present incidence rates and adjusted hazard ratios (aHRs).ResultsWe included 208140 patients from 57 countries. Over a period of 1066572 person-years, 2046 KS cases were diagnosed. KS incidence rates per 100000 person-years were 52 in the Asia-Pacific and ranged between 180 and 280 in the other regions. KS risk was 5 times higher in South African women (aHR, 4.56; 95% confidence intervals [CI], 2.73-7.62) than in their European counterparts, and 2 times higher in South African men (2.21; 1.34-3.63). The construction industry’s inherent fragmentation fuels Construction Supply Chain (CSC) complexity. Logistics form an essential part of the CSC in terms of costs and project management. In turn, transportation accounts for more than half the logistics costs due to construction materials being low-cost/high-volume, and most other logistics processes being business processes and not physical ones. Construction transport is almost entirely road bound, one of the least sustainable modes with externalities far out of proportion to its numbers. Ergo, its optimisation presents substantial greening (de-carbonisation) opportunity, with potentially far-reaching sustainability impacts. The ASI (Avoid-Shift-Improve) concept considers activity, modal structure, and energy intensity/efficiency as transport carbon components, providing a fundamental optimisation framework. 93% of New Zealand’s freight (a third of it construction related) is road bound, with 99% fossil-fuel (diesel) dependence. Government adoption of ASI elements viz Supply Chain (SC) efficiency, transport optimisation, data/information sharing, and collaboration for transport decarbonisation aligns with Industry 5.0, but seeks further research for concrete implementation pathways. This paper aims to bridge the gap by proposing a research framework for greening of construction transport as a sustainability enabler for New Zealand.

36. Help-seeking for potential symptoms of gynaecological cancer amongst obese women : a mixed methods study

Obesity is associated with an increased risk of developing some gynaecological cancers, and delayed help-seeking. The thesis aims were to identify factors predictive of delayed help-seeking for women seeking help for potential symptoms of gynaecological cancers, particularly looking at the role of obesity on time to help-seeking. Qualitative and exploratory research further investigated health beliefs and help-seeking attitudes of women experiencing gynaecological cancer symptoms and built a framework for understanding women’s complex journeys toward medical help-seeking. Additionally, the research assessed for differences in time to help-seeking and cancer awareness across groups with varying body mass indexes (BMIs) and different ethnic groups. Findings suggested that women have a limited awareness of gynaecological cancers, nevertheless, low awareness of gynaecological cancers was not associated with help-seeking delay. Thesis studies showed that obese women were not more likely to delay help-seeking when compared to non-obese women, and that attendance at preventative screenings was a predictor of delayed help-seeking. In conclusion the thesis provided an understanding of the often-complex transition through the menopause and challenged assumptions in the literature about the association between obesity, help-seeking and preventative screenings. Future research should further explore the complex relationship between BMI and help-seeking for gynaecological cancer symptoms.

There are certain factors within and outside organisations that can influence organisational performance. The presence or absence of those factors impacts organisations' ability to gain a competitive advantage over their rivals. As a risk-prone business sector, construction organisations need to identify those determinants that could ensure their superior performance. To date, global studies have identified many determinants that govern the performance of construction organisations. However, few of these are known in New Zealand, and their weightings could be useful in forming business strategies. Therefore, this paper measures the significance of identified determinants of construction organisational performance. A total of 100 professionals participated in a questionnaire survey administered using a snowball sampling technique. The Relative Importance Index approach was used to rank the determinants according to their importance to organisational performance. Five main determinants were significant and ranked highly, with more than 80% relative importance index scores. These determinants are resources and capabilities, competitive strategies, organisational characteristics, environmental factors, and customers relationship management. This finding provides a useful benchmark for future research on the significance of some determinants that could explain the performance differentials experienced in the construction domain in New Zealand.

Divertor power load is a major challenge towards a burning plasma in a next-step tokamak. Here, the first results of a divertor power load characterisation in tritium plasmas in type-I ELMy H-mode, obtained in the JET deuterium-tritium campaign (DTE2) performed in 2021, are presented. It is demonstrated that both, transient loads due to type-I ELMs as well as the power fall-off length, do not exhibit an explicit ion mass dependence, with remarkably similar values in the tritium plasmas and in the deuterium references. This gives an improved credence to published scaling law predictions, solely based on deuterium plasma experiments. Moreover, the type-I ELM impact on the inner divertor target is studied in deuterium discharges. A slightly increased parallel energy fluence on the inner target with a factor of 1.08 compared to the outer target is observed. This is explained by the smaller major radius of the inner target.

Despite the multidisciplinary networks involved in refurbishment of complex building projects, the lack of BIM adoption signifies lack of real BIM benefits towards acceptance of BIM within Architecture, Engineering and Construction (AEC).To this end, this study empirically examines the potential real benefits between traditional network and BIM network for a real-time refurbishment case study project, through agent-based simulation modelling. A social network analysis theory is adapted to model the project interaction networks and a BIM prototype network. An assessment of the main stakeholders for BIM perception is carried out. We offered three prototype interaction networks for comparison of real BIM benefit.An agent-based Bayesian network model is used to simulate the propagation of design error within the project networks. The result of the analysis show that BIM project diffuses error efficiently, while stakeholders recovers faster and nearly at the same time than traditional network. The optimised network shows better performance to the traditional network, when there is early involvement of subcontractors. The main contribution of this study is providing a novel approach to compare real benefits for traditional method to BIM method for refurbishment project and to provide avenue for project stakeholders to optimise their interaction through adoption of BIM.
