Abstract

Background: Metabolic syndrome (MS) is a powerful risk factor for cardiovascular and cerebrovascular diseases. Although lifestyle intervention reduces several of the symptoms of the syndrome and cardiovascular risks, the lifestyle intervention that yields the benefits is restrictive. Jinlida is a Chinese patent medicine that has shown activity in type 2 diabetes, which has been approved in China. Preclinical studies in Jinlida granules support an improved role of abnormal glucose and lipids metabolism as well as reducing weight. Here, we describe the protocol of an ongoing clinical trial investigating a new therapy for metabolic syndrome in patients with abnormal glucose metabolism.Methods: This study will enroll 880 subjects (aged 18–70 years) who have metabolic syndromes with abnormal glucose metabolism. All the participants in a double-blind, parallel, randomized, placebo-controlled trial, will receive Jinlida or placebo, orally, 9 g/time, three times daily for 2–4 years period on the basis of lifestyle intervention. The primary outcome measure (Incidence of type 2 diabetes) will be assessed during intervention cycles. Adverse events were monitored. All statistical tests will be performed using a two-sided test, and a p ≤ 0.05 (two-sided test) will be considered to be statistically significant results.Discussion: Results from this study will provide evidence on whether incorporating oral Jinlida granules treatment into lifestyle intervention can delay or inhibit the development of diabetes mellitus in metabolic syndrome subjects with abnormal glucose metabolism.Clinical trial registration: Registered at http://www.chictr.org.cn/enIndex.aspx. Abstract

Introduction Diabetic kidney disease (DKD) is one of the most common microvascular complications of diabetes mellitus and is the leading cause of end-stage renal disease. DKD seriously affects the quality of life of patients and brings heavy economic burden to the country. At present, the pathogenesis of DKD is not entirely clear, and clinical treatment is mainly to control blood glucose and lower blood pressure and urine protein. However, the clinical effect is not satisfactory. Case Presentation A female patient, aged 72 years, first visited our hospital endocrinology clinic, and was diagnosed with DKD. She suffered from fatigue, cold sensation in the lower extremities, convulsion of the lower limbs and frequent urination at night. She had a dark purple tongue with white and yellow fur and sublingual varices. Accessory examinations showed that her fasting blood glucose was 5.7 mmol/L, serum creatinine 159 μmol/L, blood urea nitrogen 13.6 μmol/L, blood uric acid 493 pmol/L, and blood pressure 138/65 mm Hg. The patient received Traditional Chinese Medicine treatment of the modified Shenzhuo formula [Huangqi (Radix Astragali Mongolici), Dahuang (Radix Et Rhizoma Rhei Palmati), Shuizhi (Hirudo) and Danshen (Radix Salviae Miltiorrhizae)] for 7 years and achieved good results. Conclusion This case provides a specific treatment plan and an effective reference for clinical application of Traditional Chinese Medicine for treating DKD. This may be an alternative treatment for DKD.

Abstract

OBJECTIVE To observe the efficacy and safety of different dosages of modified Zhibaidihuang decoction (MZBDD) on polycystic ovary syndrome (PCOS) hyperandrogenism (HA) patients. METHODS Ninety PCOS HA patients and 30 infertile patients due to tubal factor were selected. Ninety PCOS HA patients were randomly divided into three groups: low dosage group (LDG), medium dosage group (MDG) and high dosage group (HDG) and infertile patients were selected as normal control group (NCG). PCOS HA patients were treated with different dosage of MZBDD for 4 weeks. For HA patients, serum total testosterone (T), estrodial (E2), follicle stimulating hormone (FSH), luteinizing hormone (LH), prolatin (PRL), alanine aminotransferase (AST), aspartate amino transferase (AST) were determined before and after treatment, while acne scores (Rosenfield) were signed. Basal body temperatures (BBT) were asked to be measured every day. And for the else, T, E2, FSH, LH, PRL, AST, and AST were determined before treatment. RESULTS Totally 111 patients completed the clinical research. There were no differences among the four groups on serum T before treatment (P = 0.221). Serum T concentration of both MDG and HDG after treatment significantly were lower than that of before treatment (P = 0.039, P = 0.000), while there was no obvious difference in LDG (P = 0.829). Serum T concentration of both MDG and HDG were significantly lower than that of LDG after treatment (P = 0.048, P = 0.006). To compared with before treatment, there were no differences in Serum FSH, LH, E2, P and PRL among the three groups (LDG, MDG, HDG) (as for FSH, P = 0.136, P = 0.503, P = 0.062; as for LH, P = 0.473, P = 0.513, P = 0.096; as for E2, P = 0.206, P = 0.927, P = 0.076; as for PRL, P = 0.120, P = 0.903, P = 0.407, as for P, P = 0.308, P = 0.866, P = 0480). Acne scores of all the three groups were obviously lower than that of before treatment (P = 0.031; P = 0.033; P = 0.002). 39.5% of the patients had biphasic BBT, but there were no differences among the three groups (P = 0.510). There were no differences with ALT and AST between after and before treatment among the three groups (LDG, MDG, HDG) (as for ALT, P = 0.742, P = 0.383, P = 0.053; as for AST, P = 0.732, P = 0.519, P = 0.120). CONCLUSION Different dosage of MZBDD has dose-effect relationship in treating PCOS HA.

Abstract

Innate lymphoid cells (ILCs) are a novel family of innate immune cells that act as key coordinators of intestinal mucosal surface immune defense and are essential for maintaining intestinal homeostasis and barrier integrity by responding to locally produced effector cytokines or direct recognition of exogenous or endogenous danger patterns. ILCs are also involved in the pathogenesis of inflammatory bowel disease (IBD). Many studies have demonstrated the occurrence of crosstalk between ILCs and intestinal microbiota, and ILCs have recently been shown to be connected to the enteric nervous system (ENS). Thus, ILCs may act as a key link between the nervous system and microbiota in intestinal networks. In this review, we briefly summarize the role of the ILCs in the intestinal tract (particularly in the context of IBD) and discuss the relationship between ILCs and the microbiota/ENS.

Abstract

Introduction. In the present meta-analysis, we aimed to determine the effects of adjuvant treatment with Chinese herbal medicine (CHM) on antidiabetic agents having additional benefits in patients with type 2 diabetes. Methods. Randomized controlled trials were identified by searching the Cochrane Library, PUBMED, EMBASE, MEDLINE, the China National Knowledge Internet, Web of Science, Global Health, International Pharmaceutical Abstracts and the China biology medicine, Wanfang, and VIP databases. The intervention group received CHM as add-on treatment to antidiabetic agents therapy, and the control group received placebos in addition to antidiabetic agents or antidiabetic agents alone. We assessed pooled data, including weighted mean differences and 95% confidence intervals (CIs) using a random-effects model. Results. A total of 125 randomized controlled trials were included. 10 articles were included based on literature screening. All trials contrasted Chinese herbal medicines or Chinese herbal medicines + antidiabetic agents with placebo or antidiabetic agents + placebo and included a total of 2004 individuals with T2DM. All selected trials displayed evidence of high methodological quality and possessed a low risk of bias. Meta-analysis of the trials demonstrated that Chinese herbal medicines resulted in a more favorable blood glucose profile in contrast to placebo (P Conclusions. The results showed that in the intervention group, greater reductions were achieved for glucose control and body weight. The combined use of drugs improves the curative effect and has fewer adverse events and has additional benefits in patients with type 2 diabetes. This trial is registered with PROSPERO (CRD42018093867).

Abstract

ObjectiveTo observe the efficacy of a traditional Chinese medicine, Shenzhuo formula, on patients with diabetic kidney disease (DKD).MethodsEighty-eight outpatients with DKD were enrolled. Changes in estimated glomerular filtration rate (eGFR), creatinine clearance, serum creatinine, blood-urea-nitrogen, albuminuria, glycosylated hemoglobin (HbA1C), blood pressure, and lipid profile were measured and analyzed before and after intervention with Shenzhuo formula for 1, 3, 6, 9, 12, and 18 months.ResultsCompared with the baseline amounts, serum creatinine decreased, and eGFR and creatinine clearance increased, significantly after intervention for 1, 3, 6, 9, 12, and 18 months (all P < 0.05). Mean eGFR increased by 2.11 mL/min per 1.73 m2/y after 18-month treatment. Urinary protein at 24 h decreased significantly after 1, 3, 9, and 12 months (P < 0.05). HbA1C decreased significantly (P < 0.05) after 3, 6, 9, 12, and 18 months, and systolic blood pressure decreased significantly (P < 0.05) after 1, 3, and 6 months. Total cholesterol decreased significantly (P < 0.05) after 1, 3, 6, and 18 months. Triglyceride and low-density lipoprotein-cholesterol decreased significantly (P < 0.05) after 1 and 3 months.ConclusionShenzhuo formula can improve eGFR and possibly slow DKD progression. Shenzhuo formula can also lower HbA1C, lipid levels and blood pressure.

Abstract

Powder formulae are an indispensable part of prescription in Traditional Chinese Medicine. Powder formulae are characterized by good therapeutic efficacy and low dose used for their preparation. Analysis of the therapeutic application and material basis of pharmacological active substance in power formulae can enable the development of new powder formulae. This in turn can contribute to reduction of wastage of drug material, relief of shortage of herbal medicinal resources and sustainable development of Traditional Chinese Medicine.

The rapidly increasing diabetes mellitus (DM) is becoming a major public health issue globally; considerable progress has been made in the field of western hypoglycemic drug and insulin, but some shortages still exist. As one of the most important parts in complementary and alternative therapies, traditional Chinese medicine (TCM) performs a good clinical practice and is showing a bright future in the treatment of DM. TCM therapy has certain advantages of less toxicity and/or side effects, and Chinese herbal medicine which usually contains various active ingredients could provide multiple therapeutic effects. Huang Lian (Rhizoma coptidis, RC) is a herb frequently used in many traditional formulas for properties of “clearing damp-heat, quenching fire, and counteracting poison” in Asia for centuries. In this review, we summarize the application of RC in the treatment of DM from two aspects of contents. Firstly, theoretical principles are explained, including the properties and related records about RC in ancient references and modern pharmacological researches and pharmacokinetics on RC and its active components. Secondly, the clinical application of RC is mainly reviewed, such as applicable stage and syndrome, the reasonable dose range, the preparation formulations, and the toxicity and/or side effects and solutions to its adverse actions. This review provides scientific evidence about the effective components, pharmacological researches, and toxicity of RC, as well as introducing traditional Chinese medical theory and clinical experience, in order to guide clinician to use RC more suitably and reasonably in the clinical practice.

Abstract

Traditional Chinese medicine (TCM) performs a good clinical practice and is showing a bright future in the treatment of diabetes mellitus (DM). TCM treatment has certain advantages of less toxicity and/or side effects, and herbs could provide multiple therapeutic effects. Berberine (BBR) is a classical natural medicine. In this review, we summarize the application of BBR in the treatment of DM from two aspects. First, modern pharmacological effects of BBR on glucose metabolism are summarized, such as improving insulin resistance, promoting insulin secretion, inhibiting gluconeogenesis in liver, stimulating glycolysis in peripheral tissue cells, modulating gut microbiota, reducing intestinal absorption of glucose, and regulating lipid metabolism. BBR is used to treat diabetic nephropathy (DPN), diabetic neuropathy (DN), and diabetic cardiomyopathy due to its antioxidant and anti-inflammatory activities. Second, the clinical application of BBR is reviewed, such as listing some clinical trials on the effectiveness and safety of BBR, explaining applicable stage and syndrome, the reasonable dose and dose formulation, and the toxicity and/or side effects. This review provides scientific evidence about BBR, as well as introducing some traditional Chinese medical theory and clinical experience, in order to guide clinician to use BBR more suitably and reasonably.

Objective: To investigate the effect of Kaiyu Qingwei granule (KYQWG) on the insulin binding capacity of liver and skeletal muscular cell membrane and serum insulin-like growth factor-1 (IGF-1) in streptozotocin-induced diabetic rats.Methods: Rats in four experimental groups were investigated: the control group, the model group, the KYQWG group and the Metformin group.
