4. Safety analysis on surgical treatment of elderly patients with primary retroperitoneal soft tissue sarcoma: a retrospective cohort study

Abstract

Objective: To analyze the risk factors for postoperative complications of the elderly patients with primary retroperitoneal sarcoma(RPS). Methods: A retrospective cohort study was performed for elderly patients with primary RPS treated by soft tissue sarcoma multidisciplinary team Zhongshan Hospital from July 2009 to December 2020. Logistic regression analysis was used to find risk factors for postoperative complications. Results: A total of 98 cases were enrolled including male 54 cases and female 44 cases with 26 cases (26.5%) of age 70 years or older. Five-year overall survival following surgery was 61.2% (60 cases with 95% confidence interval, 49.1-73.7). Seventy-eight cases (79.6%) had tumor burden greater than 10 cm, and 87 cases (88.8%) with single lesion. There were 35 cases (35.7%) with well-differentiated liposarcoma, 31 cases (31.6%) with dedifferentiated liposarcoma, and 15 cases (15.3%) with leiomyosarcoma. A total of 96 cases(98.0%) underwent radical resection and 54 cases (55.1%) had more than one organ resected. The postoperative complications were found 55 cases (56.1%) and severe complications 16 cases (16.3%). Four cases (4.1%) died postoperatively. It was shown by multivariate analysis that both tumor burden greater than 10 cm (OR=4.597, P=0.012) and more than one organ resected (OR=4.858, P=0.003) were independent risk factors for postoperative complications in elderly patients with RPS. Conclusions: Operative treatment remained the preferred type for elderly cases with primary RPS and the surgery should be completed in high-flow centers of RPS.

Abstract

Near-infrared II fluorescence imaging holds great promise for in vivo imaging and imaging-guided surgery with deep penetration and high spatiotemporal resolution. However, most NIR-II aromatic luminophores suffer from the notorious aggregation-caused quenching (ACQ) effect in the aqueous solution, which largely hinders their biomedical application in vivo. In this study, the first NIR-II organic aggregation-induced emission (AIE) fluorophore (HLZ-BTED), encapsulated as nanoparticles (HLZ-BTED dots) for in vivo biomedical imaging, was designed and synthesized. The NIR-II AIE HLZ-BTED dots showed high temporal resolution, high photostability, outstanding water-solubility and biocompatibility in vitro and in vivo. The HLZ-BTED dots were further used for long-term breast tumor imaging and visualizing tumor-feeding blood vessels, long-term hind limb vasculature and incomplete hind limb ischemia. More importantly, as a proof-of-concept, this is the first time that non-invasive and real-time NIR-II imaging of the gastrointestinal tract in health and disease has been performed, making the AIE dots a promising tool for gastrointestinal (GI) tract research, such as understanding the healthy status of GI peristalsis, diagnosing and evaluating intestinal motility dysfunction, and assessing drug effects on intestinal obstruction.

They both received tumor resection surgery and treatment with ALK inhibitors crizotinib followed by alectinib, and upon receiving each of the drugs, showed a brief response, then experienced recurrence or progression of the disease. During the treatment, whole exome sequencing (WES) and RNA sequencing (RNA-Seq) were applied to monitor potential drug-induced gene mutation and expression changes. A novel, secondary mutation in ALK exon 23 (L1196Q) was identified in patient 1 after alectinib resistance developed. Guided by this result, a newer ALK inhibitor, ceritinib was prescribed. The patient was able to achieve a partial response (PR) and is in good condition as of the manuscript date. Abstract

The rarity and heterogeneity of liposarcomas (LPS) pose significant challenges in their diagnosis and management. In this work, a series of metal–organic frameworks (MOFs) engineering is designed and implemented. Through comprehensive characterization and performance evaluations, such as stability, thermal‐driven desorption efficiency, as well as energy‐ and charge‐transfer capacity, the engineering of group IV bimetallic MOFs emerges as particularly noteworthy. This is especially true for their derivative products, which exhibit superior performance across a range of laser desorption/ionization mass spectrometry (LDI MS) performance tests, including those involving practical sample assessments. The top‐performing product is utilized to enable high‐throughput recording of LPS metabolic fingerprints (PMFs) within seconds using LDI MS. With machine learning on PMFs, both the LPSrecognizer and LPSclassifier are developed, achieving accurate recognition and classification of LPS with area under the curves (AUCs) of 0.900–1.000. Simplified versions are also developed of the LPSrecognizer and LPSclassifier by screening metabolic biomarker panels, achieving considerable predictive performance, and conducting basic pathway exploration. The work highlights the MOFs engineering for the matrix design and their potential application in developing metabolic analysis and screening tools for rare diseases in clinical settings.
