Abstract

A pan-viral DNA microarray, the Virochip (University of California, San Francisco), was used to detect human parainfluenzavirus 4 (HPIV-4) infection in an immunocompetent adult presenting with a life-threatening acute respiratory illness. The virus was identified in an endotracheal aspirate specimen, and the microarray results were confirmed by specific polymerase chain reaction and serological analysis for HPIV-4. Conventional clinical laboratory testing using an extensive panel of microbiological tests failed to yield a diagnosis. This case suggests that the potential severity of disease caused by HPIV-4 in adults may be greater than previously appreciated and illustrates the clinical utility of a microarray for broad-based viral pathogen screening.

20. Getting to zero: Impact of a device to reduce blood culture contamination and false-positive central-line–associated bloodstream infections

Abstract

AbstractObjective:To assess the impact of initial specimen diversion device (ISDD) on inpatient and emergency department blood culture contamination (BCC), central-line–associated bloodstream infection (CLABSI) standardized infection ratios (SIRs), and antibiotic administration.Design:Single-center quasi-experimental prospective cohort study wherein phlebotomists used traditional venipuncture with or without the ISDD while registered nurses (RNs) used traditional venipuncture.Method:BCC events among phlebotomists and RNs were observed and compared from March 17, 2019, through January 21, 2020, defined by contaminant detection in 1 of 4 bottles for matched sets or 1 of 2 bottles in both subsets for coagulase negative staphylococci. CLABSIs throughout this period were recorded and SIRs were calculated. Enhanced oversight took place through July 21, 2019, with chart review assessing antibiotic use for patients with possible BCC.Results:Overall, 24% of blood cultures obtained were from patients in intensive care. Phlebotomists using traditional venipuncture (n = 4,759) had a 2.3% BCC rate; phlebotomists using the ISDD (n = 11,202) had a 0% BCC rate. RNs drew 7,411 BCs with a 0.8% BCC rate. The CLABSI SIR was decreased from 1.103 in 2017 and 0.658 in 2018 to 0.439 in 2019. The CLABSI incidence was 33%–64% of predicted value for each 2019 quarter. This range fell to 18%–37% after the exclusion of likely false-positive results. Among 42 patients with possible BCC under enhanced oversight, 2 patients were treated with prolonged antibiotic courses.Conclusions:ISDD use by phlebotomists was associated with BCC reduction and reduced false-positive CLABSI results. This patient-care quality improvement could constitute sustainable antibiotic stewardship expansion.

AbstractWe used a strand-specific RT-qPCR to evaluate viral replication as a surrogate for infectiousness among 242 asymptomatic inpatients with a positive severe acute respiratory coronavirus virus 2 (SARS-CoV-2) admission test. Only 21 patients (9%) had detectable SARS-CoV-2 minus-strand RNA. Because most patients were found to be noninfectious, our findings support the suspension of asymptomatic admission testing.

AbstractSevere acute respiratory coronavirus virus 2 (SARS-CoV-2) real-time reverse-transcription polymerase chain reaction (rRT-PCR) strand-specific assay can be used to identify active SARS-CoV-2 viral replication. We describe the characteristics of 337 hospitalized patients with at least 1 minus-strand SARS-CoV-2 assay performed >20 days after illness onset. This test is a novel tool to identify high-risk hospitalized patients with prolonged SARS-CoV-2 replication.

Abstract

Molecular biology techniques can be used to identify different strains of microorganisms responsible for infectious diseases outbreaks. These methods can also be used to diagnose patients suffering from different infectious diseases. Plasmid profile analysis is a technique used to examine plasmids, or small circular pieces of genetic material in the cytoplasm of bacterial cells. Plasmids can serve as identifying markers for different strains of disease-causing bacteria. Genomic fingerprinting can be used to study bacteria, viruses and protozoa that do not have plasmids. Southern hybridization fingerprinting is used to examine fragments of DNA, or genetic material. Polymerase chain reaction (PCR) amplification is a technique used to study small amounts of viral or bacterial DNA that may be present in tissue samples.

Abstract

Drinking water supplied to homes, businesses and hospitals could harbor the bacterium that causes Legionnaires' disease. Of 20 patients diagnosed with Legionnaires' disease, eight were found to be infected with the same type of Legionella pneumophila as was found in samples of their drinking water. Three patients were living at home prior to their infection, two were in a nursing home, one was in a hospital and one was working in an industrial plant. Two of the patients living at home lived in an apartment complex. Drinking water may be a source of sporadic cases of Legionnaires' disease.

Abstract

Legionella infections (caused by the bacteria associated with Legionnaire's disease, a type of pneumonia) transmitted in hospital settings rarely cause infections in sites other than the lungs. Three cases of infection of the surgical wound made near the breastbone (sternum) to perform cardiac surgery are identified and described. The cases occurred within a larger group of patients who developed infections due to Legionella over a seven-year period; this investigation revealed the route of transmission. The cardiac patients' conditions necessitated an unusual amount of sternal disruption (due to need for cardiac massage, chest compression, and related treatments), and within about seven days signs of infection began to appear. Two of the three patients died. Three control patients underwent cardiovascular surgery close to the time of the patients' surgeries without subsequent infection; data from their medical records were compared with data from the infected patients (cases) to learn more concerning risk factors for Legionella. Cases were more likely than controls to have two or more previous operations in which the sternum had been cut; they also had more changes of the wound dressings during the first postsurgical week. A survey of nursing personnel (102 people) revealed that 97 percent bathed patients in tap water after surgery, using the water to wash away the colored liquid with which the patients were cleaned prior to surgery. Many of the nurses washed the area near the wound and chest tubes (inserted to drain the chest) with tap water. Tests showed that the edges and undersurface of the dressing became wet after washing. Analysis of water samples from 61 locations in the hospital revealed the presence of the strains Legionella dumoffii and Legionella pneumophila only in water from the intensive care unit where the patients had been housed. On only one occasion was Legionella dumoffii isolated from the hospital tap water, but the organisms are known to persist in plumbing, even though concentrations may fluctuate. Abstract

Bacillary angiomatosis is a disease that afflicts HIV-infected people (people infected with human immunodeficiency virus, associated with AIDS), causing new blood vessels to grow in the skin and lymph nodes. Although cat scratch disease is thought to be caused by the same organism, the infectious agent had not been definitively identified prior to this report. A method is presented whereby the agent was identified in tissue from three patients with bacillary angiomatosis. The method used the polymerase chain reaction (PCR), which can amplify (copy) unknown segments of DNA if the sequences on either side of the unknown region are known. When PCR was applied to DNA extracted from the patients' infected tissues, the gene that encodes the 16S rRNA sequence (a rRNA subunit found in all cells) of the (unknown) bacterium was amplified. Control tissue from patients who were not infected with this disease was analyzed in the same way. Comparisons of results for a patient and several control subjects indicated that a 200-base-pair segment was present in the patient, but not the controls. This segment was cloned and found to be similar to other bacterial 16S rRNA, but not identical to any known sequence. It was designated strain BA-TF. Comparisons with tissue from the other patients indicated the presence of similar sequences. Most bacteria with structures similar to that of BA-TF are rickettsiae. The technique allows identification of infectious organisms even when they cannot be grown in tissue culture. An evaluation of the approach is presented. Opportunistic infection with the causative agent of cat scratch disease maY be responsible for an unusual vascular proliferative lesion, referred to as bacillary epithelioid angiomatosis, previously described only in human immunodeficiency virus (HIV)-infected patients. We present a case of an HIV-infected patient with bacillary epithenoid angiomatosis involving the liver and bone marrow causing progressive hepatic failure. We also report a case of a cardiac transplant recipient with hepatic and splenic bacillary epithelioid angiomatosis manifesting as a fever of unknown origin, a previously unreported event in a non-HIV-infected patient. These cases represent the first documentation of bacillary epithelioid angiomatosis with visualization of cat scratch-like organisms involving internal organs., Bacillary epithelioid angiomatosis (BEA) is a disorder of the skin blood vessels and may occur in patients infected with human immunodeficiency virus (HIV). Although it was first thought to be a cancer-like process, the lesions, characterized by overgrowth of blood vessels, were later identified as a reaction to infection with a cat scratch bacillus or similar microorganism. No lesions other than those affecting the bone and skin have been reported previously. Two cases are described of patients infected with cat scratch-like bacilli, who had BEA involving internal organs, including the liver, bone marrow, and spleen. The first case is of a patient with HIV infection and BEA involving the liver and bone marrow. This patient developed rapidly progressing liver failure, brain disease, disorders of blood coagulation, anemia, and decreased numbers of platelets, cells involved in blood clotting. The second case is a heart transplant recipient, who developed a fever due to BEA involving the liver and spleen. This second case shows that BEA can affect persons not infected with HIV. These two cases are the first reports of BEA involving internal organs and associated with infection by cat scratch-like microorganisms.
