Background The hippocampus is a brain structure important for memory and cognitive function. Physical activity may help prevent hippocampal atrophy. However, few studies have measured sedentary behavior (SB) and intensity-specific physical activity using an accelerometer. This study aimed to examine the cross-sectional associations of objectively-determined SB, light-intensity physical activity (LPA), and moderate-to-vigorous physical activity (MVPA) measured by an accelerometer with hippocampal volume among community-dwelling older adults using compositional data analysis (CoDa) approach. Methods This cross-sectional study was part of the Neuron to Environmental Impact across Generations (NEIGE) study. A randomly recruited sample of 485 Japanese older adults (47% male; aged 65-84 years) wore tri-axial accelerometers (Omron Healthcare) for seven consecutive days in 2017. Hippocampal volume was measured with magnetic resonance imaging and the left and right hippocampal volumes were automatically segmented using FreeSurfer software. Associations of sedentary and physically active behaviors with hippocampal volume were examined with compositional linear regression analysis based on isometric log-ratio transformations of time use adjusted for potential confounding factors. Results The relative proportion of time spent in MVPA, compared to the other two activities, was significantly positively associated with right hippocampal volume (β: 57.1, p-value = 0.027).However, no association existed between higher proportions of MVPA and left hippocampal volume, or between proportions of SB or LPA with either left or right hippocampal volumes. Conclusions The proportion of time spent in MVPA, relative to the other two activities, was significantly positively associated with right hippocampal volume. MVPA may be beneficial for maintaining hippocampal volume.

Primary aldosteronism (PA) is a common disease. Especially in unilateral PA (UPA), the risk of cardiovascular disease is high and proper localization is important. Adrenal vein sampling (AVS) is commonly used to localize PA, but its availability is limited. Therefore, it is important to predict the unilateral or bilateral PA and to choose the appropriate cases for AVS or watchful observation.The purpose of this study is to develop a model using machine learning to predict bilateral or unilateral PA to extract cases for AVS or watchful observation.We retrospectively analyzed 154 patients diagnosed with PA and who underwent AVS at our hospital between January 2010 and June 2021. Based on machine learning, we determined predictors of PA subtypes diagnosis from the results of blood and loading tests.The accuracy of the machine learning was 88% and the top predictors of the UPA were plasma aldosterone concentration after the saline infusion test, aldosterone to renin ratio after the captopril challenge test, serum potassium and aldosterone-to-renin ratio. By using these factors, the accuracy, sensitivity, specificity and the area under the curve (AUC) were 91%, 70%, 99% and 0.91, respectively. Furthermore, we examined the surgical outcomes of UPA and found that the group diagnosed as unilateral by the predictors showed improvement in clinical findings, while the group diagnosed as bilateral by the predictors showed no improvement.Our predictive model based on machine learning can support to choose the performance of adrenal vein sampling or watchful observation.

Catechol O-methyltransferase (COMT) is known as an important drug-target protein in the field of Parkinson's disease. All clinically approved COMT inhibitors bring a 5-substituted-3-nitrocatechol ring as a pharmacophore, and they bind to COMT with S-adenosylmethionine (SAM) and an Mg2+ ion to form a quaternary complex (COMT/SAM/Mg2+/inhibitor). However, structural information about such quaternary complexes is only available for a few inhibitors. Here, a new crystal structure of COMT complexed with nitecapone (5), SAM and Mg2+ is revealed. Comparison of the structures of these complexes indicates that conformation of the catechol binding pocket is almost constant regardless of structure of the inhibitors. The only restriction of the side chain of inhibitors (i.e., the substituent at the 5-position of 3-nitrocatechol) seems to be that it does not make steric repulsion with COMT. However, recent crystallographic and biochemical studies suggest that COMT is a flexible protein, and its conformational flexibility seems crucial for its catalytic process. Based on this information, implications of these quaternary inhibitor complexes were investigated. Met 40 in the α2α3-loop makes atomic contacts with SAM or S-adenosylhomocysteine and the 3-position of the catechol inhibitor. This interaction seems to play a critical role in the affinity of the inhibitor and to stabilize the COMT/SAM/Mg2+/nitrocatechol inhibitor complex by fixing the flexible α2α3-loop.

Physical activity (PA) is a known modifiable risk factor for MetS and individual MetS components. However, the role of PA could differ between sub-populations due to differences in the variability of PA and other MetS risk factors. To examine these differences, multi-country studies with standardized outcome measurement methods across cohorts are needed. Methods Cross-sectional PA levels (total and domain specific) in healthy middle-aged (44–56 years) men in the Risk Factor Assessment among Japanese and U.S. Men in the Post-World War II Birth Cohort (ERA-JUMP) Study (n = 730; American: n = 417; Japanese: n = 313; from population-representative samples in Pittsburgh, Pennsylvania, USA, and Kusatsu, Shiga, Japan) were compared. The relationships between PA levels and MetS (overall and specific components) in/across the American and Japanese sub-cohorts (adjusting for age, smoking, and alcohol consumption) were also assessed using the same instruments (pedometer and validated questionnaire) to measure PA in both cohorts. Results A total of 510 individuals provided complete data on PA (American: n = 265; Japanese: n = 245). The American cohort had significantly lower mean ± SD steps/day (7878 ± 3399 steps/day) vs. the Japanese cohort (9055 ± 3797 steps/day) (p < 0.001) but had significantly higher self-reported moderate-vigorous leisure PA (American: 15.9 (7.4–30.3) metabolic task equivalent hours per week (MET-h/week) vs. Japanese: 4.0 (0–11.3) MET-h/week, p < 0.0001). In both sub-cohorts, each 1000 steps/day increase was associated with lower odds of having MetS (American: OR = 0.90, 95%CI: 0.83–0.98; Japanese: OR = 0.87, 95%CI: 0.79–0.95) and the individual MetS component of high waist circumference (American: OR = 0.86, 95%CI: 0.79–0.94; Japanese: OR = 0.87, 95%CI: 0.80–0.95). In the American cohort only, higher self-reported leisure PA (Met-h/week) was associated with lower odds of MetS and high waist circumference (OR = 0.98, 95%CI: 0.97–0.99 for MetS and waist circumference, respectively). Conclusion Higher total step counts/day had an important protective effect on MetS prevalence in both the Japanese and American cohorts, despite differences in PA levels and other MetS risk factors. The effect of steps/day (across all intensity levels) was much greater than domain-specific moderate-vigorous PA captured by questionnaire, suggesting the need for measurement tools that can best capture total movement when examining the effects of PA on MetS development., Graphical abstract Image, graphical abstract

Objectives Objectively measured sedentary behavior (SB) on weekdays and weekends has been mainly assessed in white‐collar workers, while data in blue‐collar workers are sparse. Therefore, this study presented the difference in accelerometer‐measured SB levels between weekdays and weekends, stratified by white‐ and blue‐collar occupations. Methods This study was a sub‐analysis of accelerometer data from 73 workers (31 blue‐collar and 42 white‐collar) at a Japanese manufacturing plant. SB was defined as ≤1.5 metabolic equivalents estimated using an accelerometer, and compared between weekdays and weekends by using mixed models adjusted for confounders. The proportion of workers who sat for ≤8 h/day on weekdays and weekends were compared using McNemar's test. Results In white‐collar workers, SB time on weekdays was significantly longer than that on weekends (598 vs 479 min/day, P .99). Conclusions White‐collar workers were exposed to significantly longer SB time on weekdays than on weekends, which was not the case in blue‐collar workers. It may be rather challenging for white‐collar workers to limit their SB time to the level recommended by the latest guidelines for better health, especially on weekdays.

We tried to clarify the trends in step-determined physical activity (PA) among Japanese adults from 1995 to 2016.Raw data from the National Health and Nutrition Surveys Japan (NHNS-J) performed between 1995 and 2016 were used, in compliance with the Statistics Act of Japan. NHNS-J was conducted annually by the Japanese government (the Ministry of Health, Labour, and Welfare) using a representative Japanese sample, with the same sampling method every year except in 2012 and 2016. A 1-d pedometer survey was conducted each year as a part of the NHNS-J, on a weekday in November specified by each individual. Because of the difference in age distribution of the samples, age-adjusted mean steps per day were calculated from 1995 to 2016 by sex. The time trends of step-determined PA levels among Japanese adults were described and examined using Joinpoint regression.The highest age-adjusted mean number of steps for men was 8235 steps per day in 2000, whereas it was 7667 steps per day in 2015. The highest age-adjusted mean numbers of steps for women were 7474 steps per day in 1998 and 6691 steps per day in 2015. The age-adjusted step-determined PA trend showed significantly decreasing trend from 1997 to 2008 in men (annual percentage change, -0.74; P0.001) and from 1998 to 2008 (annual percentage change, -1.30; P0.001) in women, by Joinpoint regression.The age-adjusted step-determined PA among Japanese adults between 1995 and 2016 decreased from around 1997 to 1998 until around 2008, but since then, the decreasing trend became unclear both in men and in women. Continuous monitoring of PA trends is essential to assess the effectiveness of policies.

Vaccination could be a key protective measure against coronavirus disease 2019 (COVID-19), and it is important to understand the acceptability of the COVID-19 vaccine among the general public. However, there is no study on the acceptance of a COVID-19 vaccine in Japan. Therefore, this study aimed to describe the COVID-19 vaccine acceptance and hesitancy situation in Japan and assess the factors associated with such issues. This was a cross-sectional study based on an internet survey completed by 2956 people. Participants were asked to indicate how likely they were to get vaccinated for COVID-19. In addition, the participants responded to questions regarding sociodemographic factors, attitudes, and beliefs regarding COVID-19 infection and vaccination. The proportion of participants with a high likelihood of getting a COVID-19 vaccine was 62.1%. Multiple logistic regression analysis showed that vaccine acceptance was lower among several sociodemographic groups, such as women, adults aged 20–49 years, and those with a low-income level. Several psychological factors, especially the perceived effectiveness of the COVID-19 vaccine, and willingness to protect others by getting oneself vaccinated, were associated with vaccine acceptance. Our results indicate that the perceived effectiveness of the vaccine and willingness to protect others may play an important role in the acceptance of the COVID-19 vaccine.

We aimed to clarify the status of hand hygiene practices among ordinary citizens during the COVID-19 pandemic in Japan, as well as the frequency of daily hand hygiene as an indicator of education and evaluation. This cross-sectional study was based on an internet survey completed by 2,149 participants (age range: 20-79 years, men: 51.0%, response rate: 89.5%), selected from June 23 to 28, 2020. The participants responded regarding the frequency of implementing hand hygiene at 5 moments (after returning from a public place, after using the toilet, after touching something outside, before eating food, and after blowing the nose, coughing, or sneezing). Additionally, the participants responded to the number of daily hand hygiene events. The cutoff value of the total number of daily hand hygiene events to determine whether hand hygiene was performed at all 5 time points was determined using receiver operating characteristic analysis. The mean number of hand hygiene events was 10.2 times/day. The prevalence of implementing hand hygiene at each moment ranged from 30.2% to 76.4%; only 21.1% of respondents practiced hand hygiene at all times. Both Youden Index and specificity were high when the cut-off value was 11 times/day. Therefore, the criterion of hand hygiene (≥11 times/day) may be useful in education and evaluation.

Objectives To compare physical activity (PA) and sedentary behavior (SB) levels during work time between those who work from home (WFH) and at workplaces (no WFH), and by WFH subgroups. Methods This cross‐sectional internet‐based survey included 1239 workers (mean age [standard deviation], 44.7 [13.7] years; 59.2% men) living in the Tokyo Metropolitan Area. Time spent sitting (SB), standing (light‐intensity PA; LPA), walking, and engaging in heavy labor (moderate‐to‐vigorous PA; MVPA) during work time was measured using the Work‐related Physical Activity Questionnaire. Workers reported weekly WFH percentages (eg, 0% implies no WFH and 100% implies full WFH), and WFH percentages were categorized into no WFH (0% WFH) and WFH (1%‐100% WFH) groups. The WFH group was further subcategorized into 1%‐25%, 26%‐50%, 51%‐75%, and 76%‐100% subgroups. Results Overall, 494 workers (39.9%) worked from home. During working hours, SB time was longer in the WFH group than in the no WFH group (mean minutes [% working‐time SB]: 335.7 vs 224.7 min [74% vs 50%]). Objectives The purpose of this study was to examine the relationship between social participation (type/pattern) and mortality according to company size of the longest‐held job among older men in Japan who have worked in the company. Methods Longitudinal data from the Japan Gerontological Evaluation Study were used in this study. Functionally independent individuals aged 65 years and older in Japan were surveyed. Work and community organizations (local community, hobbies, and sports) were used as social participation. A Cox proportional hazards model was used to calculate mortality hazard ratios. Results Analysis was carried out on 19 260 participants. A total of 2870 deaths occurred during the 6‐year follow‐up period. Those in companies with 49 or fewer employees had the highest prevalence of work participation and the lowest participation in any community organization. Regardless of company size, the mortality risk was significantly lower for participants in any social participation (eg, the hazard ratio for participation in a hobby organization among those with a company size of 49 employees or fewer was 0.74, 95% CI: 0.65‐0.85) compared to nonparticipants whose company size was 49 or fewer employees. Conclusions In Japan, although older men who have worked for small companies may have fewer benefits, their social participation may reduce their mortality risks. To avoid increasing health inequalities, it is necessary to create an environment in which they are more likely to participate in social activities.

Background The coronavirus disease 2019 (COVID‐19) has a tremendous influence in general public's behaviors; however, changes in the status of regularly scheduled outpatient visits in Japan during COVID‐19 pandemic are still unknown. Methods This cross‐sectional study was conducted in May 2020. Participants were recruited by an Internet‐based survey company. A total of 659 patients (54% male, average age 60 ± 14 years) who had regularly scheduled outpatient visits prior to the onset of COVID‐19 were enrolled. Participants answered four questions (“decrease in medical visit frequency,” “inability to take regular medication,” “deterioration of a chronic disease,” and “utilization of telephone/online medical care”) and stated whether they had a fear of acquiring infection at a medical facility. The associations between answers, fear of infection, and socio‐demographic factors were examined. Results Among the participants, 37.8% had decreased their medical visits, 6.8% were unable to take regular medications, 5.6% experienced a deterioration of chronic disease, and 9.1% utilized telephone/online medical care. Fear of being infected by COVID‐19 at medical facilities was strongly associated with a reduced frequency of medical visits and lack of regular medications even after adjusting for socio‐demographic factors and current medical histories. Conclusions During the first wave of COVID‐19, approximately 40% of participants reduced their frequency of medical visits. It is important to continue implementing thorough infection control measures at facilities and educating the public the importance of keeping chronic diseases in good condition, as well as promoting telephone/online medical care., We conducted an Internet survey of 2,400 Japanese men and women in May 2020 and analyzed changes in the medical visit behavior/status of 659 people who had regularly visited to medical institutions before the coronavirus disease 2019 pandemic. Since the emergence of the COVID-19 pandemic, the use of face masks by healthy individuals for prevention has been attracting public attention. However, efficacy depends on proper usage. We set out to determine the prevalence of wearing masks to prevent COVID-19 and compliance with appropriate measures for the correct use of face masks among the general public in Japan where wearing medical masks is a &ldquo, cultural&rdquo, normality. This cross-sectional study was based on an internet-based survey completed by 2141 people (50.8% men, aged 20&ndash, 79 years) who were selected among registrants of an Internet research company between 1 April and 6 April 2020. Participants were asked to indicate how often they wore masks for prevention and to what extent they practiced appropriate measures suggested by the World Health Organization. The prevalence of wearing masks was 80.9% and compliance rates with appropriate measures ranged from 38.3% to 83.5%. Only 23.1% complied with all recommendations. Compliance rates were overall low in men and persons with low household incomes. Our results, hence show that many citizens implement inaccurate measures when using face masks. Therefore, providing guidance on correct usage is essential when encouraging the use of face masks to prevent COVID-19.

Background Self-isolation is an important personal protective measure in inhibiting the transmission of coronavirus disease 2019 (COVID-19) as people carry out economic and social activities amid its spread. Yet few studies have clarified the actual implementation status of self-isolation during an outbreak. This study aimed to reveal the actual implementation of self-isolation among Japanese workers during the COVID-19 outbreak and the factors inhibiting this measure. Methods This was a cross-sectional study based on an internet survey completed by 1,226 workers (60.0% men) living in 7 prefectures (i.e., Tokyo, Kanagawa, Saitama, Chiba, Ibaraki, Tochigi, and Gunma) who were selected among registrants of an Internet research company, between May 12 and 17, 2020. Participants were asked whether they had experienced fever or other cold symptoms between February 17, 2020 and the date of the survey. Those who responded affirmatively were asked where they had visited (e.g., hospital, work, and shopping for groceries or necessities) to clarify whether they had left the house within 7 days after symptom onset. We performed multivariate logistic regression analysis to clarify the relationship between going to work within 7 days after symptom onset and both sociodemographic factors and employment-related constraints. Results Of the survey participants, 82 had experienced fever or other cold symptoms (6.7%). Among these participants, 51 (62.2%) went to work within 7 days after symptom onset. A mere 17.1% practiced strict self-isolation. Multivariate logistic regression analysis revealed that those living outside the metropolitan area (i.e., Ibaraki, Tochigi, and Gunma), working as a company employee, and being unable to work from home were associated with going to work within 7 days after symptom onset. Conclusions The prevalence of strict self-isolation among participants who experienced cold-like symptoms during the COVID-19 outbreak was extremely low, and 62.2% of these participants went to work within 7 days after symptom onset. This study highlights the need for further public awareness regarding self-isolation and countermeasures against factors that obstruct it.

Methods This longitudinal, internet-based survey included 2,141 people (50.8% men; 20-79 years). The baseline and follow-up surveys were conducted from February 25-27, 2020 and April 1-6, 2020, respectively. Participants were asked how often they implemented the 5 personal protective measures recommended by the World Health Organization (hand hygiene, social distancing, avoiding touching the eyes, nose and mouth, respiratory etiquette, and self-isolation) in the baseline and follow-up surveys. Results The prevalence of 3 of the 5 personal protective measures significantly improved in the community transmission phase compared to the early phase. Social distancing measures showed significant improvement, from 67.4% to 82.2%. However, the prevalence of avoiding touching the eyes, nose and mouth, which had the lowest prevalence in the early phase, showed no significant improvement (approximately 60%). Multivariate logistic regression analysis revealed, men and persons of low-income households made fewer improvements than women and persons of high-income households. Conclusions The prevalence of personal protective measures by ordinary citizens is improving, however there is potential for improvement, especially in regard to avoiding touching eyes, nose and mouth.

Methods This was a cross-sectional study based on internet-based survey. A total of 2400 people (50% male: 20–79 years) were selected between February 25 and 27, 2020, from registrants of an Internet research company, to complete a questionnaire. Participants were asked to indicate how often they implemented the following five personal protective measures recommended by the World Health Organization (hand hygiene, social distancing measures, avoiding touching the eyes, nose and mouth, respiratory etiquette, and self-isolation). In addition, the participants responded to questions regarding the daily frequency of hand hygiene events. Results The prevalence of the five personal protective measures ranged from 59.8% to 83.8%, with the lowest being avoiding touching the eyes, nose, and mouth. In total, 34.7% implemented all personal protective measures. The median daily hand hygiene events were 5 per day (25th percentile, 75th percentile: 3,8). Conclusions The protective measures implemented by ordinary citizens are insufficient and further public awareness activities are required.

Objectives Very few longitudinal studies have investigated the question of whether differences in company size may give rise to health inequalities. The aim of this study was to examine the relationship between company size of the longest‐held job and mortality in older Japanese adults. Methods This study used longitudinal data from the Japan Gerontological Evaluation Study. Surveys were sent to functionally independent individuals aged 65 or older who were randomly sampled from 13 municipalities in Japan. Respondents were followed for a maximum of 6.6 years. The Cox proportional hazards model was used to calculate mortality hazard ratios (HRs) for men and for women. Analysis was carried out on 35 418 participants (197 514 person‐years). Results A total of 3935 deaths occurred during the 6‐year follow‐up period. Among men, in Model 1 that adjusted for age, educational attainment, type of longest‐held job, and municipalities, mortality HRs decreased significantly with increasing size of company (P for trend = .002). Compared to companies with 1‐9 employees, the mortality HR (0.78, 95% confidence interval: 0.68‐0.90) was significantly lower for companies with 10 000 or more employees. However, there were no significant differences among women (P for trend = .41). Conclusions In men, mortality in old age may decrease with increasing size of company of the longest‐held job. To reduce health inequalities in old age due to differences in size of company, studies should be conducted to determine the underlying mechanisms and moderating factors and those findings should be reflected in labor policies and occupational health systems.

Background An increasing number of studies have demonstrated that light-intensity physical activity (LPA) confers health benefits after adjustment for moderate-to-vigorous physical activity (MVPA). The purpose of this systematic review was to summarize existing epidemiological evidence on associations of objectively measured LPA with health outcomes in adults. Methods This review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. We searched on PubMed, Web of Science, CINAL, and Cochrane Library for articles analyzing the association between objectively determined LPA and health outcomes that were published up to January 2017. Data were extracted regarding authors, publication year, country of survey, study setting, number of participants, study design, physical activity (PA) assessment (type of accelerometer and intensity), health outcomes, confounders, and results (summary measures and association). A coding system was used to summarize the results. Results Of the 3254 studies identified, 24 cross-sectional and 6 longitudinal studies were included in this review. Most of the studies targeted the Western population. LPA was inversely associated with all-cause mortality risk and associated favorably with some cardiometabolic risk factors including waist circumference, triglyceride levels, insulin, and presence of metabolic syndrome. Only a small amount of data were available on mental health and cognitive function. Conclusions LPA appears to be beneficially associated with important health outcomes after adjustment for MVPA in the adult population. Although current global PA guidelines recommend only MVPA, promoting LPA may confer additional health benefits. Genus Dendrobium (Orchidaceae) contains numerous species. Phylogenetic analyses based on morphological characteristics and DNA sequences indicated that this genus is divided into two major groups: Asian and Australasian clades. On the other hand, little is known about the phytochemical differences and similarities among the species in each clade. In this study, we selected 18 Dendrobium species (11 from the Asian clade and 7 from the Australasian clade) and constructed HPLC profiles, arrays composed of relative intensity of the chromatographic peaks. Next, orthogonal partial least square discriminant analysis (OPLS-DA) was applied to the profile matrix to classify Dendrobium species into the Asian and Australasian clades in order to identify the peaks that significantly contribute to the class separation. In the end, two phenanthrenes, 4,9-dimethoxyphenanthrene-2,5-diol 1 and 1,5-dimethoxyphenanthrene-2,7-diol 2, which contributed to the class separation, were isolated from the HPLC peaks. The existence of 2 was limited to the genetically related Australasian species.

To explore the structure-activity relationships of flavanonols on the inhibition of nitric oxide (NO) production in RAW 264.7 cells, we have prepared a series of synthetic flavanonols. In our previous study, the 2',3'-dihydroxyphenyl substructure was found to be the most potent B ring substructure among the flavanonols having 3,5,7-trihydroxychroman-4-one as the A/C ring. In this study, we examined the effect of diverse substitutions on the A ring of the 2-(2,3-dihydroxyphenyl)-3-hydroxychroman-4-one scaffold, i.e., by fixing the B ring to the 2',3'-dihydroxyphenyl substructure. Eighteen stereoisomers and 4 racemic mixtures were prepared, and their inhibitory potency on NO production in RAW 264.7 cells was tested. We observed higher inhibitory activity in the (2R,3R) stereoisomers than in the (2S,3S) stereoisomers. The presence of a hydroxy or a methoxy group at the 7-postiion enhanced the inhibitory potency, and the additional substitutions at the 6- or 8-position in the A ring increased potency and stereospecificity. AimSocial participation provides health benefits for older adults. However, there is the need to identify whether higher social participation is associated with older adults being more physically active and less sedentary (sitting time). We examined the associations of social participation with physical activity, and sedentary time, in a population-based sample of older Japanese adults.

Cycling is a way for older adults to be physically active and remain healthy. This descriptive epidemiological study investigated the role of sociodemographic, biological, behavioral, and psychological correlates associated with cycling among community-dwelling older adults in Japan. This cross-sectional study, conducted in 2010, included 1,938 community-dwelling older adults aged 65 to 74 years, from three cities in Japan: Bunkyo City (urban area) and Fuchu City (suburban area) in Tokyo, and Oyama City in Shizuoka (rural area). Participants were randomly selected from resident registries and a questionnaire was mailed to each one (response rate of 75.7%). All data was self-reported, including cycling time. Multivariable logistic regression analysis was used to calculate the adjusted odds ratios (ORs) and 95% confidence intervals (CIs) for cycling. The proportion of men and women who cycled was 30.1% and 28.3%, respectively. Cycling was strongly associated with residential area: in men, Fuchu (suburban) (ORs = 3.15, 95% CIs = 2.20-4.52), Oyama (rural) (0.14, 0.08-0.25); in women, Fuchu (suburban) (4.15, 2.80-6.16), Oyama (rural) (0.21, 0.11-0.40), compared to Bunkyo (urban). Moreover, cycling was inversely associated with being underweight (0.26, 0.08-0.80) in men; for women, it was inversely associated with living alone (0.51, 0.29-0.90) and with not working (0.56, 0.38-0.84), and positively associated with not driving often (2.81, 1.47-5.40), and with not possessing a driver’s license (2.16, 1.23-3.79). The prevalence of cycling among older adults is relatively high in Japan. However, it varies greatly depending on residential area. Differences in residential areas may be taken into account to promote effective strategies for cycling.

Objective We developed the Work-related Physical Activity Questionnaire (WPAQ) to measure occupational physical activity, including sedentary behavior and sedentary breaks (interruptions of prolonged sedentary behavior), during work. This study aimed to examine the WPAQ's reliability and criterion validity using an accelerometer. Methods To examine criterion validity, 97 full-time factory workers (male: 89.7%) wore a triaxial accelerometer while working and completed the WPAQ. The questionnaire inquired about participants' normal work activities and the proportion of sitting, standing, walking, and heavy labor engaged in during work. In calculating time spent in each behavior, the proportion of each occupational activity was multiplied by the total minutes of work. Duration of uninterrupted sitting time was also measured. For reliability, the same questionnaire was administered twice (median test-retest interval: 9 days) to another convenient sample of 54 participants. Spearman's ρ was used to assess validity and intraclass correlation coefficients (ICCs) and Cohen's kappa with quadratic weighing were used to assess reliability. Results The criterion validity of occupational sitting time measured by the WPAQ was moderate (Spearman's ρ for sedentary behavior: 0.69) based on comparison with accelerometer data. Significant positive correlations were found for standing (ρ = 0.66) and walking (ρ = 0.39) between the WPAQ and accelerometer data, though not for heavy labor. A moderate but significant correlation (ρ = 0.27) was found for sedentary breaks. Test-retest reliability for all items was adequate (ICC = 0.59-0.79 for occupational sedentary behavior and physical activities, and Cohen's kappa with quadratic weighting = 0.84 for sedentary breaks). Conclusions The WPAQ has acceptable properties for measuring workers' activities, including sedentary breaks. Reduced physical activity, increased time spent in sedentary behaviors, and fewer sedentary breaks during working time are all associated with adverse health outcomes. Though some occupational exposure to these activities might be unavoidable, the WPAQ may be a practical tool for assessing them.

We aimed to clarify the patterns of moderate to vigorous physical activity (MVPA) in the Japanese adult population, and the proportion of people meeting the recommendations of the Physical Activity Guidelines (PAG) for Americans, second edition (2nd PAG, &ge, 150 min/week of total MVPA including bouts of any length) and those meeting the previously recommended PAG (2008-PAG, of &ge, 150 min/week of total MVPA lasting 10 min or longer [long-bout MVPA]). A total of 204 adults (aged 18 to 64 years) from two workplaces were asked to wear an accelerometer. MVPA was classified by bout length, and the proportion of long-bout MVPA was clarified. The proportion of participants adhering to the 2008-PAG and the 2nd PAG recommendations was calculated. Valid data was obtained from 184 adults. Long-bout MVPA accounted for 13.4% of total MVPA. Our results showed that 12.5% of individuals performed MVPA as recommended by the 2008-PAG whereas 92.4% performed MVPA as recommended by the 2nd PAG. Our results, hence, showed that long-bout MVPA comprised only a small proportion of total MVPA, and the proportion of individuals who satisfied the criteria stated in the guidelines (&ge, 150 min/week) significantly changed by whether or not bout length of MVPA was taken into account.
