OBJECTIVE: To know the variability in the depth of the sigmoid sinus within the mastoid cavity in temporal bones according to the classifications of Sun et al, Ichigo et al and Melo et al. MATERIALS AND METHODS: Descriptive, non experimental, retrospective, quantitative study was done, assessing computed tomographies of Mexican patients older than 18 years of Regional Hospital General Ignacio Zaragoza (ISSSTE), Mexico City, who attended to external consultation of Otorhinolaryngology from June 2022 to March 2023. Prevalence of each classification and the distribution of demographic variables were calculated, the degree of relationship among the data sets of each sigmoid sinus classification was measured by correlation coefficient analysis. RESULTS: A total of 105 cases of patients ranging in age from 18 to 84 years were analyzed; the study showed variability in the depth of the sigmoid sinus within the mastoid cavity in the temporal bones among patients in position and shape according to sex and side of the ear, it was observed that the most frequent classification of sigmoid sinus in the right ear was lateral position, type 3 and protrusive shape, in the left ear it was lateral position, type 2 and saucer shape. The degree of relationship of the right ear was statistically significant between the proposal of Sun et al and Ichigo et al. CONCLUSIONS: The right ear showed the most protrusive sigmoid sinus and it is more frequent that in women the position of the sigmoid sinus is deeper than in men. [ABSTRACT FROM AUTHOR]

OBJECTIVE: To determine the diagnostic performance of computed tomography (CT) as part of the study protocol of patients with unexplained weight loss. MATERIALS AND METHODS: Cohort, ambispective study was made in the department of Internal Medicine, Specialty Hospital Antonio Fraga Mouret, CMN La Raza, Mexico City, in patients with unexplained weight loss recorded in the database during 9 years. A descriptive analysis with frequency distribution of variables was performed and sensitivity, specificity and predictive values were calculated, as well as χ² for the evaluation and comparison of the diagnostic performance of CT. RESULTS: The CT has a sensitivity and specificity greater than 80%, so it is considered useful to guide a firm diagnosis; mainly in the group of patients with high suspicion of malignancy. If the cause is organic, non-malignant, it is considered a very specific (100%) study enabling us to rule out disease in those subjects with any finding. CONCLUSIONS: The advantages of computed tomography allow it to be validated as the imaging study in unexplained weight loss that provides the most data to the final diagnosis. [ABSTRACT FROM AUTHOR]

Laryngocele is a pathological dilation of the saccule or appendage of the Morgagni's ventricle. It is estimated that the incidence of laryngocele is 1 per 2.5 million inhabitants per year. The causes of laryngoceles are not well understood yet. In adults, an increase in intraluminal pressure in the laryngeal saccule is frequently associated, such as playing wind instruments, sing, lift weights, or glass blowers. CLINICAL CASE: A 25-year-old female patient, referred to our service, with failure in her previous approach, diagnosis and treatment. We diagnosed external laryngocele and it was successfully treated by external approach. CONCLUSIONS: External laryngocele, although it is a rare condition, should be considered among the differential diagnoses of cervical tumors of chemical consistency, taking into account a detailed clinical examination, with the support of direct laryngoscopy and the characterization of the lesion and its limits by an imaging study such as computed tomography. Computer tomography image quality control assessment take part of Cuban national legislation, which could be difficult to accomplish for many reasons. This work is part of the set-up verifications of "in house tool" (EvaCT) designed to calculate parameters of quality control of CT images (CCCT) in various manufacturer's phantoms, "at distance". For this, the behavior of the established parameters was studied in 17 studies of the stablished CCCT routine with the Mediso PET/SPECT/CT Anyscan Trio equipment, comparing the results using manufacture´s reference values, and/ or the international recommended parameter´s tolerances. The results of the system were compared numerically with those obtained by the manufacturer, for each available related parameter, in the corresponding protocol, and did not show significant differences (p≥0.05). In general, the system showed a satisfactory behavior, taking as reference the values and tolerances of the manufacturer, CECMED and IAEA. The correlation between quality descriptors established with different methodologies was analyzed. The tool showed correspondence with the manufacturer's results, making available a greater number of parameters and more operational conditions to be evaluated. The mean values ±SD characterize these parameters in the equipment and its phantom, and can be used for periodic, independent and remote CCCT verification. [ABSTRACT FROM AUTHOR]

Introducción: El estudio imagenológico de las características morfométricas de la articulación de la cadera en una muestra de casos asintomáticos, reviste importancia para comprender variantes de normalidad. Material y método: Estudio retrospectivo sobre un archivo de tomografías computadas solicitadas por patologías abdomino-pélvicas, de ambos sexos con edades entre 18 y 45 años, ortopédicamente sanos. Se realizó una medición digital de la versión acetabular. Las variables consideradas y analizadas con estadística descriptiva fueron edad, sexo y versión acetabular. Resultados: Se incluyeron 165 pelvis, 102 masculinas (61,9%) y 63 femeninas (38,1%). La edad media de la cohorte fue de 36,4 años. Fused renal crossover ectopia is a rare entity within the spectrum of congenital anomalies of urinary tract development, with an estimated incidence of approximately one in 1000 live births and a male predilection in a two to one ratio when compared to the female population. This entity is usually asymptomatic in most patients. The presence of symptoms is usually linked to a spectrum of complications, including hydronephrosis, infections, bile-ureteral reflux and nephrolithiasis. Recognizing the low incidence of this anomaly and the importance of its identification, in this article we report a case from our institution, observed as an incidental finding in a contrasted abdominopelvic tomography for non-specific abdominal pain. Introduction: Neoadjuvant therapy is an emerging treatment for selected cases of colon cancer. The decision is based on the analysis of preoperative images taken by computed tomography. However, its ability to discriminate those who could benefit from this therapy is unknown. Aim: Our objective is to determine the diagnostic accuracy of computed tomography to evaluate the colon cancer stage and its correlation among expert radiologists. Materials and Method: Diagnostic accuracy study. After sample size calculation, we randomly selected 47 patients who underwent a colonic adenocarcinoma resection with curative intent and had available images up to 60 days after surgery. The depth of tumor and lymph nodal invasion were evaluated by two expert radiologists, blinded to each other. The differences between radiologists were audited by a third one. The results were correlated with biopsy as the gold standard. We calculated sensitivity, specificity, negative predictive value, positive predictive value, and diagnostic accuracy of computed tomography. The agreement between radiologists was calculated using the kappa index. Results: The accuracy to differentiate T3-T4 tumors from T1-T2 was 89.4%. In contrast, ability to differentiate T4 tumors from T3 tumors was 65.9%. The accuracy to determine an N (+) was 66%. Agreement between radiologists was 0.929 to identify T1-2, 0.602 for T3, 0.584 for T4 tumors. To determine an N (+), the concordance was 0.521. Conclusion: Diagnostic computed tomography accuracy in colon cancer is adequate to differentiate advanced tumors. However, its ability to discriminate between T3 and T4, N (+) and the low agreement between radiologists suggests the need for complementary imaging techniques. [ABSTRACT FROM AUTHOR]

BACKGROUND: The frontal recess is the drainage pathway of the frontal sinus. It is a space surrounded by frontoethmoidal cells, that have great variability, which makes its understanding difficult. The International Frontal Sinus Anatomy Classification (IFAC) is a precise system that contributes to the presurgical planning for the frontal sinus endoscopic approach. OBJECTIVE: To classify the frontoethmoidal cells using IFAC in CT scans of paranasal sinuses done in Mexican patients from a third level hospital. MATERIALS AND METHODS: Cross-sectional, observational and retrolective study in which CT scans were analyzed, from patients who attended the ISSSTE Hospital Regional General Ignacio Zaragoza in Mexico, from January to December, 2018. Radiological presence of frontoethmoidal variants was evaluated, according to the IFAC. Descriptive statistics were performed to report the results, as well as Cohen's kappa index for the interobservers concordance. RESULTS: There were assessed 165 CT scans. The agger nasi cell was found with the highest prevalence (94.5%), followed by the suprabullar (58.7%) and in third place the supra agger cell (40.6%). The frontal septal cell was the least prevalent (5.4%). The inter-rater reliability showed values of the ? index of 0.745. CONCLUSIONS: The IFAC is a reliable system for classifying the frontoethmoidal region. In the Mexican population, the cells agger nasi, suprabullar and supra agger predominate. Introduction: Total anomalous pulmonary venous anomalous connection (TAPVC) is a relatively rare defect, with a prevalence of 0.8 per 10 000 live births, while the partial form, has a higher prevalence, ranging from 0.4 to 0.7% of the population. In Panama, TAPVC accounts for 9% of births with cardiac defects. The aim of this study was to evaluate the computed tomography characteristics of children with this diagnosis who were attended between 2010 and 2020 at the Hospital del Niño Dr. José Renán Esquivel. Methods: A descriptive, retrospective, cross-sectional, retrospective study was conducted in the population undergoing computed tomography for congenital heart disease at the Hospital del Niño Dr. José Renán Esquivel in Panama City, Panama, between January 2010 and December 2020. A total of 615 CT scans were performed for cardiac malformation. Of these, 51 had a diagnosis of anomalous pulmonary venous connection (APVC). Results: A predominance of this pathology was found in the male sex (57.5%). Some 76.2% of the patients had another associated cardiac malformation, with atrial septal defect (ASD) being the most frequent (54.5%). The most frequent types were supracardiac (50.0%) and infracardiac (28.5%). Conclusion: Computed tomography with intravenous contrast is a useful method that allows detailed evaluation of cardiac and great vessels anatomy in patients with cardiac malformations and provides valuable information on other associated pathologies. Introducción: Las neoplasias mediastinales son tumores poco frecuentes, pueden aparecer a cualquier edad y por lo general se descubren incidentalmente en una radiografía de tórax de rutina en pacientes asintomáticos. Objetivo: Caracterizar a los pacientes operados por tumores de mediastino según variables clínico- epidemiológicas y diagnósticas seleccionadas. Métodos: Se realizó un estudio observacional, descriptivo, de tipo serie de casos con 37 enfermos ingresados y operados con diagnóstico de tumor mediastinal, en el Servicio de Cirugía General del Hospital Provincial Docente "Saturnino Lora" de Santiago de Cuba, durante los años 2010 a 2017. Resultados: Predominó el sexo femenino con 23 pacientes y el intervalo de edad entre 31 y 40 años (11 casos). Los síntomas más comunes fueron dolor torácico, tos y disnea. El compartimiento posterior fue el mayormente afectado (11 pacientes), así como los tumores con tamaños entre 7 y 9 cm (17 enfermos). Los medios diagnósticos más utilizados fueron la tomografía computarizada, la broncoscopia y la mediastinoscopia. Primaron los tumores malignos (54,1 %) en los que se identificó una amplia gama de variedades histológicas. Conclusiones: El diagnóstico definitivo generalmente se establece mediante el estudio histopatológico posquirúrgico, aunque la tomografía computarizada asociada o no a la biopsia percutánea es la regla de oro para el diagnóstico preoperatorio. Las variedades histológicas de los tumores malignos son atribuibles a las características del órgano afectado.

Introduction: The imaging study of the morphometric characteristics of the hip joint, in a sample of asymptomatic cases, is important to understand variations from normality. Materials and method: This is a retrospective study on CT scans requested because of abdominal-pelvic pathologies, in both genders, aged between 18 and 45 years, orthopedically healthy. A digital measurement of the acetabular version was performed. Variables considered and analyzed with descriptive statistics were age, sex and acetabular version. Results: 165 pelvises were included, 102 male (61.9%) and 63 female (38.1%). The mean age of the cohort was 36.4 years. The acetabular version was positive (anteversion) in 163 cases, with a mean male of 17.1° ± 2.7° (p <0.03) and female of 19.3° ± 3.9° (p <0.01). Two male pelvises presented retroversion (1.2%). The difference in the contralateral version mean was 1.15° ± 2.2°. Conclusion: The female acetabulum has a higher average anteversion than the male. Different contralateral acetabular anteversion is common. The prevalence of retroversion in an asymptomatic young adult sample is low. [ABSTRACT FROM AUTHOR]

Los germinomas son los tumores intracraneales de células germinales más frecuentes. Los germinornas comprometen el 65% de este tipo de tumores y representan una forma menos maligna. Ésta neoplasia constituye aproximadamente el 0.1% al 3.4% de todas las neoplasias intracraneales. Su origen embriológico continúa sin conocerse con exactitud. El noventa y cinco por ciento se originan en la región del tercer ventrículo a lo largo de un eje, desde la cisterna supraselar en un 48%, hacia la región pineal en un 37%, el compromiso en ambas regiones, secuencialmente 6 simultáneamente es raro, solamente en un 6%. Las manifestaciones clínicas dependen de la localización del tumor y podrían comprometer la función endócrina, hipotalámica, visual y cognoscitiva. Presentamos dos casos de germinoma intracrancal con lesiones simultáneas en las regiones supraselar y pineal, y revisamos aspectos clínicos, métodos de diagnóstico, tratamiento y pronóstico.

El desarrollo de nuevos métodos de diagnóstico, actualmente nos permite evaluar las lesiones metastásicas que se presentan en la columna vertebral y la médula espinal. Presentamos la revisión de casos clínicos de los anos 1996 y 1997, de pacientes que asisten a nuestro Instituto, resaltando parámetros importantes como la edad, localización regional más frecuente, entre otros aspectos. Si bien la radiografía convencional y la mielografía nos ayudan a identificar el aspecto y compromiso que presentan las metástasis, estos estudios actualmente han sido sustituidos en gran parte por la tomografía computada e imagen por resonancia magnética. La tomografía computada nos proporciona datos específicos del componente óseo como son las erosiones óseas corticales, fracturas patológicas, depósitos calcificados, desorganización del hueso trabecular. La mayor sensibilidad en estudios de imagen corresponde a imagen por resonancia magnética, la cual nos permite valorar la infiltración metastásica de la médula ósea, compresión medular, además, permite obtener cortes axiales, sagitales y coronales. El uso de medios de contraste pararnagnéticos ayudan a identificar otras características de éstas lesiones.

Los lipomas intracraneales son lesiones benignas infrecuentes y raramente sintomáticos. Ocupan el 0.06 al 0.46% de las lesiones intracraneales. El diagnóstico de éstas lesiones, fue limitado a hallazgos de autopsia. La tomografía computada de alta resolución y la imagen por resonancia magnética han hecho posible el descubrimiento de éstas lesiones benignas "ante-mortem". Los lipomas intracraneales localizados en la región dorsal del mesencéfalo ocupan el segundo lugar en frecuencia de presentación y usualmente son asintomáticos. Las manifestaciones clínicas son relacionadas a la compresión de la lámina cuadrigeminal ó hidrocefalia hipertensiva. Presentamos dos casos de lipomas localizados en la región dorsal del mesencéfalo y revisamos la literatura con énfasis en la patogénesis y tratamiento de éstas lesiones benignas.

Introduction: An important complication of colorectal surgery is anastomotic dehiscence (AD). Nutritional status is one of the important factors in AD. An objective way to evaluate the patients' nutritional status is to measure sarcopenia, which is the reduction of skeletal muscle mass. It is possible to standardize Sarcopenia using the analysis of the Hounsfield Units (HU) and the muscular area (MA) which consider Computed Tomography of Abdomen and Pelvis (CTAP). Aim: To evaluate whether there is a relationship between AD and the presence of sarcopenia detected by the measurement of HU and MA using CTAP. The situation considers patients undergoing colectomy for cancer. Materials and Method: Cases and controls were studied with analytical statistics. 21 patients with AD and 40 without AD were chosen randomly. They include > 18 years, with colectomy for cancer and primary anastomosis. Ostomized patients, who previous the surgery do not have CTAP or if it was not available to measure HU and MA, were excluded. The imaging evaluation was performed by an expert radiologist. Results: The comparison between groups shows that they are homogeneous with respect the sex (predominant men), age (average 60 years) and location. There are signs of imaging which suggest the presence of sarcopenia in the AD group. This is explained because there is an important statistical decrease in the HU values and a tendency to present lower MA values. Conclusions: The presence of sarcopenia due to alteration of HU in a radiological study is correlated with AD, and could be a predictor of risk. OBJECTIVE: To identify a constant in the emergence of the anterior ethmoidal artery in conventional tomographic studies. MATERIAL AND METHOD: A descriptive and longitudinal study selecting simple computed tomographies of facial mass was done from January to December 2018. Patients older than 15 years were included with complete sequences of the study. We excluded studies with a history of acute facial trauma, tumor and inflammatory disease and studies in which the artery could not be located. In the coronal sections, the length was obtained in millimeters from the external edge of the ethmoidal roof to the anterior ethmoidal fissure (LE). Descriptive and inferential statistics were performed. RESULTS: A total of 250 computed tomographies were reviewed, of which 180 were included; 50% were male. Mean of LE of the male sex: right side 6.83 mm and left side 6.83 mm. Average LE of the female sex: right side 6.69 mm and the left 6.67 mm. In the comparison of LE between both sides and both sexes a statistically significant difference was not obtained. CONCLUSIONS: We did not find a statistically significant difference between the values obtained, so the measure can be considered reliable regardless of sex, age and side. [ABSTRACT FROM AUTHOR]

Superior vena cava syndrome is the clinical outcome of the obstruction (partial or total) of said vein, mainly due to neoplasic etiology. Because of this blockage, an atypical collateral venous circulation is developed. The clinical severity and the collateral circulation development depend on the level and percentage of obstruction. The syndrome includes a clinical triad of neck and facial swelling, cyanosis and thoracobrachial collateral circulation. Jugular veins distention is the most frequent sign. Diagnosis of the syndome is based on signs and symptoms, and supported in diagnostic imaging, mainly front and side X-ray, computed tomography (CT) or magnetic resonance imaging (MRI). We present a superior vena cava syndrome case, about a thirty year-old woman who presents with mediastinal localization lymphoma. The obstruction is inferior to the azygos vein draining in the superior vena cava. This blocks the superior venous flow, generating a worse clinical scenario, which diagnostic imaging shows as great development of thoracobrachial collateral circulation. We emphasize in the importance of dignostic imaging which allows to locate the level of obstruction and so relate it to clinical evolution. [ABSTRACT FROM AUTHOR]

Objetivo: En los últimos años, se han incrementado significativamente las peticiones de tomografía computada (TC) para la evaluación preoperatoria de los dientes supernumerarios (SN) en niños. El objetivo de nuestro trabajo es describir de forma detallada y concisa los dientes SN, basándonos en los pacientes diagnosticados en nuestro centro. Materiales y métodos: Se realizó un análisis retrospectivo de los estudios de TC maxilofacial, realizados entre los años 2011 y 2015. Recopilamos datos de los pacientes (género y edad), motivos de solicitud y hallazgos tomográficos. Los dientes SN fueron clasificados según la enumeración dental de la Federación Dental Internacional (FDI), describiendo la variable LEOMA (localización, emplazamiento, orientación, morfología y angulación) y las alteraciones asociadas (heterotopia, inclusión, anquilosis u odontoma). Resultados: Se registraron 36 pacientes con 49 SN en total. La petición más frecuente de TC fue la evaluación preoperatoria de los dientes SN (21 casos). En su mayoría, la localización fue mesiodiente (30 SN), posterior respecto al diente más próximo (25 SN), con orientación vertical (19 SN), con morfología rudimentaria cónica (26 SN) y con angulación anteroposterior. Conclusión: La realización de una adecuada descripción de los dientes SN ayuda al radiólogo a redactar un ordenado informe y al cirujano maxilofacial a realizar la planificación preoperatoria.

Objetivo: Establecer la concordancia de la tomografía por emisión de positrones (PET) con 18-flúor colina y la tomografía computada (TC) para la reestadificación ganglionar (N) y metastásica (M) del TNM en la recaída bioquímica del cáncer de próstata. Materiales y métodos: Se revisaron retrospectivamente historias clínicas de pacientes atendidos en Imágenes moleculares. En cada método establecimos la clasificación TNM ganglionar y metastásica. Se utilizó el índice de concordancia Kappa, categorizando los resultados según lo propuesto por Landis y Koch. Resultados: De los 32 pacientes con PET-colina y TC, en la clasificación ganglionar con PET-colina, 19 (59,4%) fueron N0 y 13 (40,6%), N1; mientras que en la TC, 28 (87,5%) fueron N0 y 4 (12,5%), N1. En la clasificación metastásica, el método PET-colina identificó M0 en 17 (53,1%) pacientes, M1a en 1 (3,1%), M1b en 5 (15,6%), M1c en 1 (3,1%), M1a + M1b en 7 (21,9%), M1b + M1c en 1 (3,1%); mientras que la TC reportó M0 en 23 (71,9%), M1a en 2 (6,25%), M1a + M1b en 2 y M1b + M1c en 5 (15,6%). La concordancia en la clasificación TNM ganglionar y metastásica entre PET-colina y TC fue de 71,88% con 0,3455 de Kappa (error estándar: 0,1336; p= 0,0049) y de 62,5% con 0,3725 de Kappa (error estándar: 0,0847; p = 0,0001), respectivamente. Discusión: El método PET-colina demostró alta exactitud diagnóstica en la extensión de la enfermedad, con respecto a los métodos convencionales. Conclusión: La concordancia entre el método PET-colina y la TC para la clasificación TNM ganglionar y metastásica no es buena.

Objetivo: Determinar la relación entre la angiografía por tomografía computada (angio-TC) coronaria y el desarrollo de un evento coronario agudo en los primeros 6 meses en pacientes adultos con dolor torácico agudo, atendidos en el servicio de urgencias de una clínica de nivel IV de atención. Materiales y métodos: Entre febrero de 2012 y de 2014, se realizó un estudio descriptivo y retrospectivo en pacientes adultos que consultaron al servicio de urgencias por un dolor torácico agudo. Todos (n = 62) fueron estudiados por angio-TC coronaria en un tomógrafo Siemens de 40 detectores. Del total, se incluyeron para el análisis a 46 pacientes con resultado negativo o estenosis menor del 50%, a los que se les realizó un seguimiento telefónico o por historia clínica de 6 meses de duración. Se excluyeron los casos a los que no se les pudo hacer un control en los 6 meses posteriores o que tuvieron una estenosis mayor del 50%. Resultados: En la angio-TC se encontraron coronarias sanas en 37 (80%) pacientes y placas no obstructivas en 9 (20%). De los 46 casos incluidos, ninguno presentó un evento coronario agudo o muerte secundaria a este durante los 6 meses de seguimiento. Conclusión: La angio-TC coronaria es una ayuda diagnóstica útil para la valoración de pacientes con dolor torácico agudo. Constituye una herramienta segura y de rápida realización, que permite definir los resultados negativos o la existencia de placas no obstructivas desde el mismo servicio de urgencias.

Se revisa la iconografía de los hallazgos por tomografía computada (TC) y resonancia magnética (RM) de la cefalea, según nuestra experiencia. De acuerdo con la base MESH, esta entidad se define como un dolor craneano, que puede ser de ocurrencia benigna o la manifestación de una amplia gama de desórdenes. Las cefaleas se clasifican por su evolución temporal (aguda o crónica), presentación (en estallido, gravativa, etc.) o coexistencia de síntomas asociados, como auras, convulsiones o déficits focales. También se dividen en primarias o secundarias, según la existencia o no de una patología subyacente. Las primarias pueden tener manifestaciones clínicas definidas, pero en las secundarias ciertos signos y síntomas deben alertar sobre la presencia de una patología estructural. En este caso, las neuroimágenes tienen un rol esencial al detectar las causantes del cuadro. Nuestros hallazgos correspondieron a cefaleas primarias (p. ej: infarto migrañoso) y a etiologías orgánicas, entre las que se destacaron causas vasculares, como patología venosa (trombosis), vasoespasmo y leucoencefalopatía posterior reversible; hemorragias intraparenquimatosas y extraaxiales; cefaleas postraumáticas y posquirúrgicas; y causas infecciosas y tumorales (apoplejía hipofisaria e hipertensión endocraneana). Además, hubo malformaciones (Arnold-Chiari, p.ej.) y otras como hipotensión endocraneana. En algunos casos inicialmente se realizó una TC y luego una RM, mientras que en otros la RM fue el método de elección. Las neuroimágenes facilitan el estudio de la cefalea, caracterizando la afección en primaria o secundaria. En el segundo caso permiten, a su vez, clasificar los hallazgos.

El linfoma primario del sistema nervioso central (LPSNC) tiene una presentación predecible en las imágenes convencionales, tanto en pacientes inmunocompetentes como inmunodeprimidos. Analizamos las características imagenológicas que nos permiten reconocer ambos casos y realizamos una revisión de las formas clínicas más relevantes del LPSNC, así como su aspecto en las imágenes obtenidas con técnicas convencionales del Diagnóstico por Imágenes, basándonos en un análisis retrospectivo de nuestros archivos institucionales y la literatura actual. La revisión abarca todas las presentaciones relevantes de esta enfermedad poco frecuente para permitir un diagnóstico temprano, crucial para el adecuado tratamiento.

El tejido graso es una parte importante de la conformación estructural del cuerpo humano. Cuando su crecimiento se produce de manera excesiva, genera un espectro imagenológico diverso, que depende de su localización, tamaño y características de benignidad o malignidad. Si bien suelen ser lesiones benignas, la invariabilidad de sus elementos constituyentes contrasta con la multiplicidad de formas de presentación, variantes histológicas e implicancias clínicas. Por ello, resulta importante estar familiarizados con su espectro radiológico para establecer el diagnóstico, tratamiento y seguimiento, sobre todo en las lesiones infrecuentes. Estas pueden producir complicaciones graves, como hemorragias, necrosis o hasta obstrucciones que terminan en algunos casos con la resección del órgano sobre el cual asientan.

Los hematomas intramurales intestinales espontáneos son una causa altamente infrecuente del dolor abdominal agudo, pero es un diagnóstico probable en pacientes anticoagulados o con enfermedad hematológica de base. Si bien el tiempo medio de presentación desde el inicio de la anticoagulación es de 24,4 meses, puede llegar a aparecer tan solo 10 días después de la instauración del tratamiento. Sus hallazgos clínicos son muy inespecíficos (p. ej. dolor abdominal y náuseas), y aunque no siempre, es común que sean similares a los de un cuadro de obstrucción intestinal, pudiendo provocar fácilmente errores diagnósticos. El tratamiento del hematoma intramural espontáneo es conservador en la mayoría de los casos y, por ello, un diagnóstico equivocado puede conducir a intervenciones quirúrgicas innecesarias. En este punto, la ecografía y fundamentalmente la tomografía computada (TC) desarrollan un papel crucial en la caracterización de esta patología y su diagnóstico correcto y precoz. Describimos la semiología en imágenes de 5 pacientes con clínica abdominal aguda y diagnóstico final de hematoma intramural espontáneo, y también revisamos la literatura sobre el tema. Los principales signos radiológicos fueron el engrosamiento de la pared intestinal, la hiperdensidad intramural en la TC sin contraste intravenoso y la presencia de hemoperitoneo.

Hay una gran variedad de artefactos en imágenes que se producen por la interacción entre los equipos y el paciente. Reconocerlos es importante, ya que pueden inducir informes erróneos o encubrir una patología. Por ello, una vez detectados, es necesario emplear técnicas para su eliminación. Describimos los artefactos más frecuentes en tomografía computada y resonancia magnética.

La embolia grasa (EG) es una obstrucción de los vasos sanguíneos por glóbulos de grasa. Ha sido descrita en la circulación pulmonar con una gran variedad de asociaciones, pero las más comunes e importantes se dan con fracturas de huesos largos y daño de tejido blando debido a traumatismo grave. Por su parte, el síndrome de embolia grasa (SEG) es una manifestación poco frecuente, aunque grave, del fenómeno de embolia grasa, que se caracteriza clínicamente por la tríada disnea, petequias y confusión mental. La razón de la discrepancia entre la presencia de embolia grasa y el desarrollo del síndrome no es clara. Presentamos el caso de un hombre de 41 años que, inmediatamente después del trauma, presentó en la tomografía computada (TC) inicial defectos de llenado de atenuación grasa dentro de las arterias pulmonares. Estos fueron interpretados como múltiples embolias grasas macroscópicas. Los valores de atenuación de la tomografía computada multidetector (TCMD) de las embolias pulmonares deben examinarse cuidadosamente en el contexto de un traumatismo agudo o después de procedimientos ortopédicos, debido a la complicación rara pero potencialmente fatal del SEG. En los centros de trauma, la TCMD es una herramienta esencial para hacer el diagnóstico de la embolia grasa macroscópica, así como también para descartar otras causas de hipoxia, como la embolia pulmonar trombótica. La reconstrucción de cortes finos debe utilizarse para la evaluación detallada de la vasculatura pulmonar.

Se presenta el caso de un niño de 14 años que hizo una consulta de control por un encondroma en el fémur izquierdo. La resonancia magnética (RM) de caderas reveló, accidentalmente, un quiste gigante de la vesícula seminal derecha. Si bien se indicó una urografía por RM, esta fue suspendida por un cuadro de claustrofobia del paciente y se realizó una tomografía computada abdómino-pélvica con y sin contraste endovenoso. La patología de las vesículas seminales (VS) puede clasificarse en congénita o adquirida. El primer tipo tiene baja prevalencia (siendo el quiste y la agenesia los más comunes en la práctica diaria) y puede coexistir, a menudo, con anomalías ipsilaterales del tracto urinario superior y genital, debido a la estrecha relación en los hombres de los sistemas reproductor y urinario durante la embriogénesis. También se ha descrito su vinculación con anomalías óseas y vasculares. La escasa frecuencia de presentación y el amplio espectro de potenciales hallazgos asociados suelen dificultar el diagnóstico. Lo habitual es iniciar la evaluación con una ecografía abdominal o transrectal, según la edad y tolerancia del paciente, y continuar con una RM, aunque para confirmar los hallazgos pueden ser necesarios otros procedimientos, como la vesículo-deferentografía (VDG). Esta fue tradicionalmente el método de referencia para el diagnóstico, pero en la actualidad se aplica en casos seleccionados. El tratamiento de las malformaciones está restringido a pacientes sintomáticos y usualmente consiste en una vesiculectomía, con o sin extirpación del riñón displásico o hipoplásico.

Background: Common variable immunodeficiency (CVID) is one of the most common antibody deficiencies in adults. The prevalence of respiratory infections have been reported in 75% vs 2-16% in general population. Chronic rhinosinusitis is an inflammatory disease affecting paranasal sinuses and nose. Objective: To show the prevalence of chronic rhinosinusitis, affected sinus and severity by CT scan in adult patients with CVID. Material and method: A descriptive, observational and cross sectional study was made including 21 patients with CVID. Paranasal sinus CT scan was performed to all patients and was assessed by Otolaryngologist. The chronic rhinosinusitis severity was assessed with Lund-Mackay score. Results: Out of 21 patients, 15 were women (71.5%); average age was of 40 years (±13), 12 patients had chronic rhinosinusitis; according to Lund-Mackay score, 2 (17.8%) had the severe form, 3 (7.7%) had the moderate and 7 (54.5%) the mild one. The most affected sinus was bilateral maxilar sinus (n=7, 33%), ethmoid (n=6, 26%), unilateral maxilar sinus (n=4, 19%), frontal and sphenoid (n=3, 13%) each one. The following anatomical variations were found: deviated septum (n=6, 28.5%), maxillary septum (n=3, 14.2%), bilateral maxillary sinus hypoplasia (n=2, 10%), unilateral concha bullosa (n=2, 10%) and pneumatized Agger Nasi cell (n=1, 4.7%). Conclusions: 52% of patients with common variable immunodeficiency (CVID) presented chronic rhinosinusitis, 58% mild according to Lund- Mackay score; the main affected sinus was bilateral maxillary sinus (33%). Due to the prevalence of chronic rhinosinusitis in patients with common variable immunodeficiency, paranasal sinus CT scan should be performed in all patients to establish diagnosis and treatment.

Introduction: Transparietal lung biopsy with a Tru-cut type needle of quick core (Quick-Core ®), guided by computed tomography, is a useful technique for the histological study of pulmonary lesions associated with masses suspected of malignancy. Objective: To describe an experience in using pulmonary thoracic transparietal biopsy with a Tru-cut needle guided by computed tomography as part of the diagnostic algorithm of intrathoracic pulmonary masses. Materials and methods: A cross-sectional analysis, descriptive and prospective, was done of transparietal biopsies taken using lung tomography between January 2016 and November 2018. Results: The patients' average age was 50.8 years, with males' predominance in 70% of the cases. Sixty percent of lung lesions were located in the right lung, and 40% in the left lung. Ninety percent of the biopsies had enough samples. Primary lung tumors such as adenocarcinoma, carcinoma squamous or epidermoid, adenocarcinoma bronchiole-alveolar, and myxoid liposarcoma low-grade malignancy were diagnosed in 67% of cases. On the other hand, 33% of the patients had a history of secondary tumors or metastatic disease. Conclusion: Transparietal biopsy with a Tru-cut needle guided by tomography, is a safe and accurate method for the diagnosing of pulmonary intrathoracic masses due to its non-invasive nature, low complication rate, and lower cost to make a histopathological diagnosis reason why it should be integrated as part of the diagnostic algorithm. The trachea's morphological study can be performed by different methods, including scopic examination during autopsies, endoscopy and diagnostic imaging. Multislice computed tomography (MSCT) is the reference method in medical imaging. Knowledge and analysis of the trachea's morphology is not only relevant to basic science, it is also important to applied science. It has been suggested that it is the most relevant variable in intubation injuries. Therefore, our goal is to determine Mackenzie's tracheal morphology distribution in Instituto Oncológico "Ángel Roffo" (oncological institute-UBA) by MSCT. Tracheal morphology was reviewed in 68 patients to determine prevalence according to morphological variables defined by Mackenzie and other authors, finding a mode in the U category, place that the C category occupies in other studies. Furthermore, our results were analyzed through a comparison of proportions statistic test with a signification of 0,05 or less. Statistically significant differences were found between the authors of reference and the obtained results, except for a few categories. In conclusion, Mackenzie's tracheal morphology distribution in Instituto Oncológico "Ángel Roffo" can be determined by computed tomography, finding out statistically significant differences with other authors. Neck lymph nodes are grouped into levels and sub-levels, through the classification proposed by the American Academy of Otolaryngology- Head and Neck Surgery Foundation (AAO-HNSF). However, this classification does not establish criteria to identify them in a computed tomography. There is an alternative classification in imaging studies that allows to determine which level a cervical nodule belongs to in a computed tomography, but it does not propose a practical way to achieve it, since it requires permanent consultation of the limits of each level and sub-level. The objective of this work is to generate an anatomical algorithm that solves this problem, finding in a fast, simple and precise way, the level or sub-level a lymph node belongs to in an axial tomography of the neck. To achieve this purpose, an observational, prospective and cross-sectional study was performed using a non-statistical sample of Multislice Computed Tomography (MSCT) of the neck obtained between March and April of 2018 in the Department of Diagnostic Imaging of the Institute of Oncology "Ángel H. Roffo". We were able to perform an effective anatomical algorithm to identify each level and sublevel of the lymph nodes in the MSCT in axial sections of the neck. Therefore, a resource of extreme utility in everyday medical care practice, since the parameters needed for its application are easy to identify in the image (carotid artery, sternocleidomastoid muscle, etc.) and only require between two to four steps to determine the level or sublevel of the neck lymph nodes. All in all, we hope that this algorithm has an important impact in the study of lymph nodes in axial TCMS of the neck, increasing the effectiveness in identifying them, and decreasing the error margin in reports of complementary studies of medical diagnosis. Background: The medical literature talks about complications and pulmonary, pleural and mediastinal comorbidity in patients with Systemic Lupus Erythematosus, however, there is no consensus that supports simple chest CT as a screening method for such disease. Objective: To identify the most common pulmonary, pleural and mediastinal findings and morbidities that may be found in the Mexican population having long standing SLE. To discuss the importance and usefulness of CT scan in these patients. Material and method: A cross-sectional descriptive study was conducted at the Instituto Nacional de Cardiología Ignacio Chávez from April to June 2017, including patients diagnosed with Systemic Lupus Erythematosus of more than 10 years who had a simple chest CT to assess pulmonary, pleural, cardiovascular and abdominal findings, as well as in bones and soft tissue. Results: Forty-one patients diagnosed with SLE (>4 ACR criteria) were included, 38 (93%) females and 3 (7%) males. Pulmonary findings were predominant, found in 82% of patients, 17% with nodules. In the cardiovascular system, 43.9% had systemic atherosclerotic disease, of these, 21% had a positive Ca Score, and 17% had pulmonary hypertension. In abdomen, splenic granulomas predominated. Discussion: Chest CT is useful in patients with SLE for prevention of comorbidities, mainly cardiovascular and pulmonary, since they are more prevalent in the Mexican population. Si bien la mayoría de los lipomas intestinales son asintomáticos y se diagnostican de manera casual, en algunas ocasiones pueden tener síntomas, como sangrado, diarrea o, con menor frecuencia, obstrucción o invaginación intestinal. Esta última en el adulto es una patología poco común y por lo general tiene una presentación subaguda crónica, cuyo diagnóstico es complicado. La principal técnica de imagen para el diagnóstico de la invaginación intestinal en adultos es la tomografía computada (TC). Solamente un 20% de las invaginaciones son colo-cólicas y en su mayoría son secundarias a un carcinoma. No obstante, en algunas pocas oportunidades pueden ser secundarias a una causa benigna, siendo el lipoma la más usual.

La cantidad de estudios de tomografía computada (TC) que se llevan a cabo por año en el mundo crece de manera exponencial, fundamentalmente por la incorporación de la tomografía computada multicorte (TCMC) que permite la realización de estudios en pocos segundos. Sin embargo, a pesar del beneficio diagnóstico que tienen los pacientes, la dosis de radiación recibida con este tipo de prácticas es una preocupación en la comunidad profesional, debido a que es necesario disminuirla tanto como sea razonablemente posible. El presente artículo describe las principales unidades dosimétricas utilizadas en la tomografía computada, con el fin de poder trabajar con ellas de manera sencilla, utilizando los valores de los equipos modernos y los de referencia conocidos internacionalmente.

La neumonía herpética es una rara enfermedad que afecta casi con exclusividad a personas con un déficit de inmunidad. Con frecuencia tiene un desenlace fatal. Su aparición está descrita en los primeros 2 meses luego del trasplante pulmonar, pero en nuestra experiencia se evidenció después de los 10 meses, 1 año y medio, 8 y 19 años. Describimos los hallazgos tomográficos detectados en 4 pacientes con trasplantes pulmonares que cursaron neumonía herpética. Presentaban fiebre, tos, expectoración y disnea, con empeoramiento progresivo de su clase funcional, y todos fallecieron tras la aparición de la enfermedad. El diagnóstico histológico se realiza mediante lavado bronquioalveolar o biopsia transbronquial, con posterior tinción con hematoxilina-eosina y/o marcación inmunohistoquímica. Los hallazgos tomográficos destacados fueron: opacidades en vidrio esmerilado de distribución parcheada, consolidación del espacio aéreo, derrame pleural y bronquiectasias. Debido a su baja frecuencia y mal pronóstico, es importante conocer y tener presente esta entidad en personas trasplantadas de pulmón para no demorar el diagnóstico y actuar lo más rápidamente posible.
