A 50-year-old male, presented with a two-months history of ascending paresthesias, with continuous twitchings over the body, associated with insomnia. His electromyography (EMG) revealed neuromyotonia and was diagnosed as a case of peripheral nerve hyperexcitability (PNH) syndrome due to Leucin-rich glioma-inactivated 1 (LGI1) antibody. Ankylosing spondylitis (AS) is a chronic systemic inflammatory disorder. It primarily affects the axial skeleton through involvement of the peripheral joint scan occurs. Temporomandibular joint (TMJ) involvement in AS varies from 4% to 35%. Here, we present a case of a 35-year-old male, follow-up of AS from last 8 years on nonsteroidal anti-inflammatory drugs, presented with fresh complaints of painfully restricted movements of jaw during swallowing. Tropical pyomyositis is a suppurative disease of the skeletal muscles. The most common site of the infection being muscles of the thigh. Staphylococcus aureus is the most common organism isolated. Aeromonas is one of the rarest organisms described as the etiologic agent for this disease. Rarely, cardiac involvement in pyomyositis has been described in the form of myocarditis and pyopericardium. Pancarditis is a very rare event in the natural course of pyomyositis. To the best of our knowledge, there is a single case report in world literature regarding involvement of all the layers of the heart in pyomyositis. Cardiovascular events are the leading cause of fatal outcome from tricyclic antidepressant (TCA) overdose; cardiotoxicity as dysrhythmias and hypotension. Electrocardiogram (ECG) abnormalities usually precede the development of significant, clinically evident cardiovascular disease, and so the ECG has manifests clinically emerged as a valuable tool in the assessment of TCA toxicity. Cardiac toxicity secondary to TCAs is due mainly to sodium-channel blockade and slowing of phase 0 depolarization of the action potential resulting in slowing of conduction through the His-Punkinje system and myocardium. Sinus tachycardia is the most common manifestation seen, other ECG changes, which are seen are premature ventricular contractions, ventricular tachycardia and fibrillation, supraventricular tachycardia with aberrancy, sinus arrest, idioventricular rhythm, pulseless electrical activity, QRS/QT/PR prolongation, rightward terminal QRS axis, increased amplitude of RaVR, atrioventricular blocks, and uncommonly as bundle branch block. Thyroid disorders, both hyperthyroidism and hypothyroidism, have been known to cause many cardiovascular effects however; hyperthyroidism has been known to be an important factor in the aetiology of paroxysmal supraventricular tachycardias. Here, we report a case of young female who was a known case of hypothyroidism for one year, on thyroxin 25 mcg once a day, presented with acute onset palpitations and diaphoresis with electrocardiography (ECG) findings suggestive of Paroxysmal Supraventricular Tachycardia (PSVT). Splenic abscess is a rare clinical entity as described in literature. The incidence is in the range of 0.14–0.7% and it has a high mortality rate. Hence, it is important to know its clinical presentation and complications, so that it can be treated early. We report a 40-year-old diabetic man who presented with fever with chills and rigor for the last 9 days and heaviness in the left hypochondrium for the last 6 days. He was initially diagnosed as having splenomegaly due to Plasmodium vivax (P. vivax), but was later found to have a splenic abscess due to Escherichia coli (E. coli). The article discusses the case of a 34-year-old male patient suffering from scalp swelling and diagnosed with small cell lung carcinoma. Topics discussed include the other symptoms exhibited by the patient, the results of the fine needle aspiration cytology (FNAC) examination of the scalp swelling and the eventual death of the patient due to respiratory failure. Also discussed is the mean survival rate of patients with lung carcinoma who exhibit cutaneous metastasis.

The association of acute hepatitis E viral (HEV) infection with glucose-6-phosphate dehydrogenase (G6PD) deficiency leading to extensive intravascular haemolysis is a very rare clinical entity. Here we discuss such a patient, who presented with acute HEV illness, developed severe intravascular haemolysis and unusually high levels of bilirubin, complicated by acute renal failure (ARF), and was later on found to have a deficiency of G6PD. The article presents a case study of an 18-year-old female suffering from esophageal cysts. Topics discussed include complaints of fever, cough, and breathlessness in the patient; diagnosis of leathery crepitations in the left lung; and increase in the leukocyte count. Her chest radiography revealed bronchiectatic lesions for which she was treated with antibiotics and underwent surgery of asymptomatic esophageal cyst.

Both dengue and typhoid fever have emerged as major public health problems in India. Coinfection with both these diseases is rarely reported. A lot of autoimmune conditions have been found to be associated with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). There are multiple cases of autoimmune and autoinflammatory diseases reported after the coronavirus disease 2019 (COVID-19) pandemic. Autoimmune neurological presentations have also been reported but are very rare. Morvan's syndrome is a life-threatening autoimmune disease and is very rare. Morvan's syndrome is classically characterized by neuromyotonia, dysautonomia, insomnia, and neuropsychiatric symptoms. Leucine-rich glioma-inactivated protein 1 (LGI1) and contactin-associated protein 2 (CASPR2) autoimmunity has also been associated with Morvan's syndrome. We hereby report an interesting case of a male patient in his 40s, who had a history of COVID-19 infection, following which he had bilateral lower-limb weakness and pain, with profuse sweating, and was later diagnosed as a case of SARS-CoV-2-associated Morvan's syndrome., (© Journal of the Association of Physicians of India 2024.)
