Abstract Treatment of ocular tumors on dedicated scattering-based proton therapy systems is standard afforded due to sharp lateral and distal penumbras. However, most newer proton therapy centers provide pencil beam scanning treatments. In this paper, we present a pencil beam scanning (PBS)-based ocular treatment solution. The design, commissioning, and validation of an applicator mount for a conventional PBS snout to allow for ocular treatments are given. In contrast to scattering techniques, PBS-based ocular therapy allows for inverse planning, providing planners with additional flexibility to shape the radiation field, potentially sparing healthy tissues. PBS enables the use of commercial Monte Carlo algorithms resulting in accurate dose calculations in the presence of heterogeneities and fiducials. The validation consisted of small field dosimetry measurements of point doses, depth doses, and lateral profiles relevant to ocular therapy. A comparison of beam properties achieved through the applicator against published literature is presented. We successfully showed the feasibility of PBS-based ocular treatments.

The children’s opera Brundibár received fame through performances by Jewish children in the Theresienstadt ghetto from 1943 to 1944. Since its revival in the 1970s, the work has been performed around the world in multiple languages and has been transformed into a best-selling book by Tony Kushner and Maurice Sendak. Used as a tool for Holocaust education, many modern productions emphasize a narrative of cultural resistance as a way of reading the work, transforming Brundibár’s Brechtian agitprop plot of collective action profoundly. In the Sydney production of 2014, a decision was made to stay faithful to the original motives of the composer and librettist, and the production was shaped by ethnographic testimony of those who had witnessed the original performances. This article examines how the historical narrative interacts with the ethnographic and personal encounters in the interpretation and realization of this work. Burdened by a responsibility to historical context, how does ethnography assist in bringing nuance and multi-vocality? In what way does an empathic imperative inform these processes?

Treatment of ocular tumors on dedicated scattering-based proton therapy systems is standard afforded due to sharp lateral and distal penumbras. However, most newer proton therapy centers provide pencil beam scanning treatments. In this paper, we present a pencil beam scanning (PBS)-based ocular treatment solution. The design, commissioning, and validation of an applicator mount for a conventional PBS snout to allow for ocular treatments are given. In contrast to scattering techniques, PBS-based ocular therapy allows for inverse planning, providing planners with additional flexibility to shape the radiation field, potentially sparing healthy tissues. PBS enables the use of commercial Monte Carlo algorithms resulting in accurate dose calculations in the presence of heterogeneities and fiducials. The validation consisted of small field dosimetry measurements of point doses, depth doses, and lateral profiles relevant to ocular therapy. A comparison of beam properties achieved through the applicator against published literature is presented. We successfully showed the feasibility of PBS-based ocular treatments.

- Abstract
This chapter investigates the songs in Yiddish and Polish remembered by survivors of the łódz ghetto. It draws on interviews with two teenage survivors of the łódz ghetto who settled in Australia after the war in order to document and preserve personal musical experiences and memories of Jewish Holocaust survivors. It also references long and established literatures on examining witnesses and testifiers in Holocaust and trauma studies that speaks at length of delicate dynamics and ethical responsibilities of representation. The chapter analyzes the claim that sonic experiences remain in memories of people and travel with them throughout their lives, providing moments of nostalgia, evocations of past connections, ties to culture, friends, and family, and frames of reference. It explains how memories of dark, distant, and problematic times are enabled and returned to resonate in the present lives of testifiers and witnesses.

Purpose Our group previously introduced an in vivo proton range verification methodology in which a silicon diode array system is used to correlate the dose rate profile per range modulation wheel cycle of the detector signal to the water-equivalent path length (WEPL) for passively scattered proton beam delivery. The implementation of this system requires a set of calibration data to establish a beam-specific response to WEPL fit for the selected ‘scout’ beam (a 1 cm overshoot of the predicted detector depth with a dose of 4 cGy) in water-equivalent plastic. This necessitates a separate set of measurements for every ‘scout’ beam that may be appropriate to the clinical case. The current study demonstrates the use of Monte Carlo simulations for calibration of the time-resolved diode dosimetry technique. Methods Measurements for three ‘scout’ beams were compared against simulated detector response with Monte Carlo methods using the Tool for Particle Simulation (TOPAS). The ‘scout’ beams were then applied in the simulation environment to simulated water-equivalent plastic, a CT of water-equivalent plastic, and a patient CT data set to assess uncertainty. Results Simulated detector response in water-equivalent plastic was validated against measurements for ‘scout’ spread out Bragg peaks of range 10 cm, 15 cm, and 21 cm (168 MeV, 177 MeV, and 210 MeV) to within 3.4 mm for all beams, and to within 1 mm in the region where the detector is expected to lie. Conclusion Feasibility has been shown for performing the calibration of the detector response for three ‘scout’ beams through simulation for the time-resolved diode dosimetry technique in passive scattered proton delivery.

In 2010, all young patients treated for intrathoracic Hodgkin lymphoma (HL) at one of 10 radiotherapy centers in the province of Quebec received 3D conformal photon therapy. These patients may now be at risk for late effects of their treatment, notably secondary malignancies and cardiac toxicity. We hypothesized that more complex radiotherapy, including intensity‐modulated proton therapy (IMPT) and possibly IMRT (in the form of helical tomotherapy (HT)), could benefit these patients. With institutional review board approval at 10 institutions, all treatment plans for patients under the age of 30 treated for HL during a six‐month consecutive period of 2010 were retrieved. Twenty‐six patients were identified, and after excluding patients with extrathoracic radiation or treatment of recurrence, 20 patients were replanned for HT and IMPT. Neutron dose for IMPT plans was estimated from published measurements. The relative seriality model was used to predict excess risk of cardiac mortality. A modified linear quadratic model was used to predict the excess absolute risk for induction of lung cancer and, in female patients, breast cancer. Model parameters were derived from published data. Predicted risk for cardiac mortality was similar among the three treatment techniques (absolute excess risk of cardiac mortality was not reduced for HT or IMPT (p>0.05,p>0.05) as compared to 3D CRT). Ever since the Australian Research Council (ARC) recognised “non-traditional research outputs” (NTROs) as valid expressions of research, developing benchmarks to measure the quality of artistic research has been of increasing interest to the Australian higher education sector. Because research quality (as measured through the triennial Excellence in Research Australia exercise) is a driver for block funding to higher education institutions, decisions about the quality of “artistic research” need to be transparent, based on peer review and justifiable to government auditors. With these requirements in mind, in 2014 the University of Sydney adopted University Guidelines for Non- Traditional Research Outputs. Following the framework and terminology developed by the ARC, the Guidelines recognise that: [r]esearch output may consist of any form of publicly available, assessable materials embodying research, whether produced by writing, making, composing, designing, performing, or curating. (The University of Sydney, 2014) Artistic research (“creative work as research”) is placed within a conceptual framework embracing not only standard print research outputs and but also other “non-traditional” outputs of traditional research (scholarly translations, critical editions, technical standards, exhibitions of archaeological or scientific objects, and research reports for external bodies). Complementing the general criteria and principles, the University Guidelines also include specific criteria and output weightings for each type of NTRO. Recognising the inherently unsatisfactory nature of any system of quality metrics, the presentation will discuss issues arising from the development and implementation of the guidelines within the Sydney Conservatorium of Music. Too often, musical testimony from the Holocaust does not receive the sort of careful attention and openness that characterises the act of listening. For the philosopher Jean-Luc Nancy, listening (écouter) is to be distinguished from entendre, the latter having the dual meanings of to hear and to understand. This paper examines my own ethnographic project of 5 years, interviewing approximately one hundred Holocaust survivors, talking about individual musical experience and memory from time spent in ghettos, camps, in hiding and in partisan groups. The kind of careful attention and openness that characterises the act of listening in Nancy's analysis is not the kind of attention that has been brought to bear on the place of music in Holocaust survivor narratives. Rather, the phenomenon of music in the Holocaust has too often been figured as a means to serve as an archival documentary function (or at worst, a 'garnish' for a historical narrative), where specific songs/music and their content are employed as a means through re-creations to come to (at best) an understanding of the truth of the Holocaust experience. As I understand his writing, for Nancy the critical attention figured by listening (écouter) opens up a very different kind of attentive space in which objects of study can be approached differently. It's obviously suitable that a critical approach based on listening be used when talking about music, yet this is arguably not what has happened, especially in the field of Holocaust studies. Was there something in musical experience at that time that helped people create their own intimate spaces? If there was, then the listening critic has a place, rather than coming to such experiences from the perspective of a petrified structure (i.e. music was not functioning as a political record, so it cannot be accused of working as such). This is the crucial turning that I wish to bring to bear on this area - a different mode of sensibility so that the discourse is dislodged (albeit momentarily) from the aforementioned means (however important or inevitable it might be). Nancy expresses the current problem when he writes that “what truly betrays music and diverts or perverts the movement of its modern history is the extent to which it is indexed to a mode of signification and not to a mode of sensibility.” In re-examining my previous work through Nancy's concepts, I imagine the site of listening as an act of care within the scholarly space.

48. SU-E-T-524: In-Vivo Diode Dosimetry Proton Therapy Range Verification Validation Study for Pediatric CSI

Purpose: With the aim of reducing acute esophageal radiation toxicity in pediatric patients receiving craniospinal irradiation (CSI), we investigated the implementation of an in-vivo, adaptive proton therapy range verification methodology. Simulation experiments and in-phantom measurements were conducted to validate the range verification technique for this clinical application. Methods: A silicon diode array system has been developed and experimentally tested in phantom for passively scattered proton beam range verification for a prostate treatment case by correlating properties of the detector signal to the water equivalent path length (WEPL). We propose to extend the methodology to verify range distal to the vertebral body for pediatric CSI cases by placing this small volume dosimeter in the esophagus of the anesthetized patient immediately prior to treatment. A set of calibration measurements was performed to establish a time signal to WEPL fit for a “scout” beam in a solid water phantom. Measurements are compared against Monte Carlo simulation in GEANT4 using the Tool for Particle Simulation (TOPAS). Results: Measurements with the diode array in a spread out Bragg peak of 14 cm modulation width and 15 cm range (177 MeV passively scattered beam) in solid water were successfully validated against proton fluence rate simulations in TOPAS. The resulting calibration curve allows for a sensitivity analysis of detector system response with dose rate in simulation and with individual diode position through simulation on patient CT data. Conclusion: Feasibility has been shown for the application of this range verification methodology to pediatric CSI. An in-vivo measurement to determine the WEPL to the inner surface of the esophagus will allow for personalized adjustment of the treatment plan to ensure sparing of the esophagus while confirming target coverage. The implementation of this system requires a set of calibration measurements to establish a beam-specific diode response to WEPL fit for the selected ‘scout’ beam in a solid water phantom. This process is both tedious, as it necessitates a separate set of measurements for every ‘scout’ beam that may be appropriate to the clinical case, as well as inconvenient due to limited access to the clinical beamline. The diode response to WEPL relationship for a given ‘scout’ beam may be determined within a simulation environment, facilitating the applicability of this dosimetry technique. Measurements for three ‘scout’ beams were compared against simulated detector response with Monte Carlo methods using the Tool for Particle Simulation (TOPAS). Results: Detector response in water equivalent plastic was successfully validated against simulation for spread out Bragg peaks of range 10 cm, 15 cm, and 21 cm (168 MeV, 177 MeV, and 210 MeV) with adjusted R2 of 0.998. Conclusion: Feasibility has been shown for performing calibration of detector response for a given ‘scout’ beam through simulation for the time resolved diode dosimetry technique.
