Aim:Around 40% of non-small cell lung cancers have stage 3b or 4 disease at the time of diagnosis. In the treatment, platinum-based therapy can still be used in patients who do not carry a driver mutation or who are not suitable for immunotherapy with advanced non-small cell lung cancers (NSCLC). The objective of this study was to compare the effectiveness of weekly carboplatin-paclitaxel regimen with triweekly carboplatin-paclitaxel regimen.Materials and Methods:This is a retrospectively structured study. Patients who were followed-up and treated for lung cancer in Adana City Training and Research Hospital’s Oncology Department between January 1, 2017 and July 1, 2021 were included.Results:Out of the 104 patients, 52 (50%) patients received weekly carboplatin-paclitaxel (C-P), and 52 (50%) received C-P every 3 weeks. The mean overall survival was 19.64±2.53 months in the weekly C-P group and 17.47±1.64 months in the triweekly C-P group (p=0.675). The mean progression-free survival (PFS) was 8.5±1.01 months in the weekly C-P group and 5.76±0.61 months in the triweekly C-P group (p=0.017).Conclusion:We demonstrated that weekly C-P treatment, which is known to have fewer toxicity in NSCLC, provided better PFS compared to triweekly treatment.

Purpose: Testicular cancer is the most common solid tumor in young men between the ages of 20-35 and has excellent clinical outcomes with appropriate treatment. Neutrophil-to-lymphocyte-ratio (NLR) is related with prognosis and relapse in different solid tumors. We aimed to investigate the relation of presence and size of regional lymph node metastasis with NLR in testicular cancer. Materials and Methods: A total of 72 testicular cancer patients were included in the study. All medical records were retrospectively collected. NLR was calculated as the count of neutrophil divided by the count of lymphocyte. Chest and abdomen computed tomograpy (CT) scans of all patients were evaluated for metastases. Results: The mean age was 32.18±8.89 years. The pathologic classification was seminoma in 43% and nonseminoma in 57% of patients. Of 72 patients, 32 (44.4%) had lymph node positivity and 40 (55.6%) hadnot. The mean NLR value was 2.63±1.79 for all cohort. The mean NLR was significantly lower in patients with lymph node negative disease (for lymph node positive and negative disease 3.19±2.32 and 2.25±1.06, respectively.). In the ROC analysis, 2.5 was determined as the cut-off value for NLR to assess lymph node status. Conclusion NLR is a cheap and readily available index that can be used to determine lymph node metastases in testicular GCTs.

Amaç: Bu çalışmanın amacı kanser hastalarında koronavirus hastalığı 2019 (coronavirus disease 2019, COVID-19) bilgisayarlı tomografi (BT), klinik ve laboratuvar bulgularının değerlendirilmesi ve polimeraz zincir reaksiyonu (polymerase chain reaction, PCR) pozitif ve negatif hastaların bulgularının karşılaştırılmasıdır. Gereç ve Yöntemler: Çalışmaya PCR testi pozitif olan 23, klinik ve radyolojik bulgularla COVID-19 tanısı almış 22 kanser hastası alındı. Hastaların BT görüntüleri iki radyolog tarafından eşzamanlı değerlendirildi. Komorbid hastalık varlığı, semptomlar ve laboratuvar değerleri değerlendirildi. Bulgular: En sık BT tutulum paterni %88,9 (n=40) ile periferaldi. Bilateral akciğer tutulumu oranı %57,8 (n=26) idi. En sık saptanan bulgu buzlu cam dansiteleri idi (n=38, %84,5). Bunların %35,5 (n=16)’ine konsolidasyon da eşlik etmekteydi. Vakaların %62,2 (n=28)’sinde multifokal tutulum mevcuttu. En sık tutulan loblar alt loblar idi. Diğer nispeten sık bulgular septal kalınlaşma, subplevral çizgilenme ve hava bronkogramı idi. Hastaların ortanca nötrofil, lenfosit, D-dimer, prokalsitonin, C-reaktif protein ve laktat dehidrogenaz değerleri sırasıyla 2000 mm3, 1200 mm3, 1990 ng/mL, 30.7 µg/L 15.8 mg/dl, 161 IU/L idi. Sonuç: Multifokal ve bilateral tutulum ile buzlucam dansiteleri en sık bulgulardı. Ancak genelde daha az saptanan septal kalınlaşmanın yüksek saptanması kanser hastalarında bulguların daha ciddi olabileceğini düşündürmektedir. PCR negatif vakalarda inflamatuvar markerların birçoğu daha yüksekti. Çok merkezde daha fazla hasta ile yapılacak çalışmalar kanser hastalarındaki bulguları genel popülasyonla daha iyi karşılaştırmayı sağlayacaktır.

Amaç: Bu çalışmada bir Üniversite Dahiliye Yoğun Bakım Ünitesinde (DYBÜ) trombositopeni sıklığı, nedenleri, klinik sonuçları ile mortalite ve morbiditeye olan katkısını saptamayı amaçladık.Gereç ve Yöntemler: Temmuz 2011-Ağustos 2012 tarihleri arasında yoğun bakımda takip edilen vetrombosit sayısı 150.000/uL’nin altında olan 165 hasta çalışmaya dahil edildi. Trombositopeni nedenleri araştırıldı. Trombositopenisi olan ve trombositopenisi olmayan hastaların mortalite ve yoğun bakım yatış sürelerikarşılaştırıldı.Bulgular: Çalışma döneminde DYBÜ’de hastaların %30,1’inde trombositopeni gelişti (165/548). Hastaların 30 (%18)’unda trombositopeni nedeni sepsis, 18 (%10,9)’inde dissemine intravasküler koagülasyon (DİK) idi. Çalışma süresince 165 trombositopenik hastanın 115’i (%69,7), trombositopenik olmayan 383 hastanın 173’ü (%45,1) kaybedildi. Trombositopenisi olan hastalarda mortalite daha fazlaydı. Hastalarda trombosit düzeyi düştükçe mortalitenin istatistiksel olarak anlamlı düzeyde arttığı gözlendi. Aynı APACHE II skoruna sahip hasta gruplarında trombosit sayısı düştükçe mortalitenin istatistiksel olarak anlamlı olarak arttığı saptandı.Sonuç: Trombositopeninin en sık sebepleri sepsis ve DİK olup trombositopeni yoğun bakımda mortaliteyi arttıran bir etkendir.

(1) Background: The introduction of novel therapies has led to a considerable evolution in the management of Multiple Myeloma, and chromosomal abnormalities predict the success of treatment. We aimed to characterize cytogenetic abnormalities for risk stratification in the patient population and to evaluate the predictive and prognostic value of the specified abnormalities in distinct treatment modalities. (2) Methods: This study included patients with Multiple Myeloma who applied to the Internal Medicine Clinic of the Cukurova University Faculty of Medicine. Between 2010 and 2023, 98 cases with cytogenetic abnormality data were identified. We analysed the effects of cytogenetic abnormalities on survival and response rates to first chemotherapies. (3) Results: P53 del was the most prevalent abnormality, and t(11;14) was the most common translocation. There was no significant difference in the mean survival and treatment response rates for specific cytogenetic abnormalities. When chemotherapies based on lenalidomide were initiated, patients’ life-death statuses differed significantly from those of treatments without lenalidomide. Regardless of the type of chromosomal aberration, lenalidomide-based treatments independently enhanced average survival 14-fold, while there was no significant difference in overall survival among treatments. (4) Conclusions: In individuals with cytogenetic abnormalities, lenalidomide-based treatments should be started regardless of the chemotherapy to be used for the condition.

Background and Objective: This study evaluated the relationship between microsatellite status (MSI) and pan-immune-inflammation score (PIV) in tumor response to neoadjuvant chemotherapy (NAC) in patients with clinical stage III gastric cancer (cStage III GC). Materials and Methods: Microsatellite instability (MSI) status was evaluated based on pathology preparations. Pan-immune-inflammation score (PIV) was obtained from pre-treatment blood tests. The relationship of both parameters with pathological complete response (pCR) was evaluated. Results: A total of 104 patients were included in this study. All the patients were stage III GC patients receiving perioperative treatment. There were 13 patients in total who achieved a pCR response. While CNS was detected in 11 of the patients who achieved a pCR, the MSI status of the other two patients was unknown. No pCR was observed in any patient with MSI-H. According to the cut-off value for PIV, 25 (24%) patients were in the PIV-low (≤53.9) group, while 79 (76%) were in the PIV-high (>53.9) group. Based on univariate analysis, a higher PIV was associated with worse outcomes for pathological response, disease recurrence, and survival (p < 0.05). Conclusions: In patients with clinically stage III GC, the presence of MSI-H may predict no benefit from perioperative treatment. Conversely, a pre-treatment PIV score using specific cut-off values may provide a positive prediction of pathological response and survival.
