Abstract

In this paper, we study the geometry of trisections on certain rational elliptic surfaces. We utilize Mumford representations of semi-reduced divisors in order to construct trisections and related plane curves with interesting properties explicitly. As a result we are able to construct new examples of Zariski pairs. Especially, we show the existence of a family of Zariski pairs that degenerate to the same conic-line arrangement., Comment: 23 pages. Comments are welcome

Abstract

There is a close relationship between the embedded topology of complex plane curves and the (group-theoretic) arithmetic of elliptic curves. In a recent paper, we studied the topology of some arrangements of curves which include a special smooth component, via the torsion properties induced by the divisors in the special curve associated to the remaining components, which is an arithmetic property. When this special curve has maximal flexes, there is a natural isomorphism between its Jacobian variety and the degree zero part of its Picard group. In this paper we consider curve arrangements which contain a special smooth component with a maximal flex and exploit these properties to obtain Zariski tuples which show the interplay between topology, geometry and arithmetic., Comment: 26 pages

Abstract

In this paper we study the embedded topology of reducible plane curves having a smooth irreducible component. In previous studies, the relation between the topology and certain torsion classes in the Picard group of degree zero of the smooth component was implicitly considered. We formulate this relation clearly and give a criterion for distinguishing the embedded topology in terms of torsion classes. Furthermore, we give a method of systematically constructing examples of curves where our criterion is applicable, and give new examples of Zariski tuples., Comment: 20 pages

Abstract

Background: At the time of AtTEnd trial design, standard treatment for advanced or recurrent endometrial cancer included carboplatin and paclitaxel chemotherapy. This trial assessed whether combining atezolizumab with chemotherapy might improve outcomes in this population. Methods: AtTEnd was a multicentre, double-blind, randomised, placebo-controlled, phase 3 trial done in 89 hospitals in 11 countries across Europe, Australia, New Zealand, and Asia. Enrolled patients were aged 18 years or older, and had advanced or recurrent endometrial carcinoma or carcinosarcoma, an Eastern Cooperative Oncology Group performance status of 0–2, and received no previous systemic chemotherapy for recurrence. Patients were randomly assigned (2:1) using an interactive web response system (block size of six) to either atezolizumab 1200 mg or placebo given intravenously with chemotherapy (carboplatin at area under the curve of 5 or 6 and paclitaxel 175 mg/m2 intravenously on day 1 every 21 days) for 6–8 cycles, then continued until progression. Stratification factors were country, histological subtype, advanced or recurrent status, and mismatch repair (MMR) status. Participants and treating clinicians were masked to group allocation. The hierarchically tested co-primary endpoints were progression-free survival (in patients with MMR-deficient [dMMR] tumours, and in the overall population) and overall survival (in the overall population). Primary analyses were done in the intention-to-treat population, defined as all randomly assigned patients who gave their full consent to participation in the study and data processing. Safety was assessed in all patients included in the intention-to-treat population who received at least one dose of study treatment. Here, we report the primary progression-free survival and the interim overall survival results. This study is ongoing and is registered with ClinicalTrials.gov, NCT03603184. Globally irreducible nodes (i.e. nodes whose branches belong to the same irreducible component) have mild effects on the most common topological invariants of an algebraic curve. In other words, adding a globally irreducible node (simple nodal degeneration) to a curve should not change them a lot. In acquired TTP, especially in secondary TTP with various underlying diseases, the diagnosis is difficult because there are many cases that do not exhibit severe deficiency of ADAMTS13 or raised levels of ADAMST13 inhibitors. It is well known that collagen disease, malignancy, and hematopoietic stem cell transplantation can be underlying conditions that induce TTP. However, TTP induced by acute pancreatitis, as experienced by our patient, has rarely been reported. Our patient completely recovered with treatments using steroids and plasma exchange (PE) only. In cases where patients develop acute pancreatitis with no apparent causes for hemolytic anemia and thrombocytopenia, the possibility of TTP should be considered. Abstract

Objective: The authors investigated the relationship between the usefulness of CPT-11 + PLD combination therapy and the UGT1A1 genotype. Materials and Methods: Forty-one patients who provided informed consent were divided into the following two groups according to UGT1A1 genotypes: wild type and non-wild type. Adverse events, antitumor effect, and outcomes were compared between these two groups. Results: Twenty-three patients were wild type and 18 were non-wild type for UGT1A1. A total of 94 and 73 treatment cycles were prescribed to the wild-type and non-wild-type groups, respectively. No significant differences in the incidence of any grade 3 or higher adverse events were observed between the two groups. However, the next treatment cycle was postponed in 9.6% of the wildtype group and 12.3% of the non-wild-type group (p = 0.891). The antitumor effects as assessed by response rate were 26.1% in the wildtype group and 55.6% in the non-wild-type group (p = 0.054). The median observation period was 13 months. The median progression-free survival was three months in the wild-type group vs. five months in non-wild-type group (p = 0.913), while the median overall survival was 24 vs. 22 months (p = 0.535). Conclusions: This study did not demonstrate a statistically significant difference in the usefulness of CPT-11 + PLD combination therapy for recurrent ovarian cancer between the two groups of UGT1A1 genotypes. This study was considered to have significance as the first study conducted in Japan to prospectively evaluate the relationship between the usefulness of CPT-11-based chemotherapy and the UGT1A1 genotype for recurrent ovarian cancer.
