MR imaging (MRI) as a noninvasive, painless and radiation free method, is being used in the diagnosis of the knee lesions recently. From January 1993 to September 1995 one hundred eighty patients (one hundred eighty-five knees) who had knee problems were examined by MRI. The mean age was 33.2 years, 121 were men and 59 were women. Arthroscopy was performed within two months of MRI examination and findings were compared. The accuracy of MRI in our series was 80% for medial meniscal tears, 90.8% for lateral meniscal tears, 97.8% for ACL lesions. Because of the excellent diagnostic correlation of MRI and Arthroscopy in meniscal and ligament tears and in differentiation of other lesions of the knee, we believe that MRI is a safe and noninvasive method in diagnosis of the knee problems., Manyetik Rezonans Görüntüleme (MRG) iyonizan radyasyon içermemesi, noninvaziv ve ağrısız bir yöntem olması nedeni ile diz lezyonlarının tanısında son yıllarda sıkça kullanılmaya başlanmıştır. Ocak 1993-Eylül 1995 tarihleri arasında diz problemi olan 180 hastanın 185 dizi MRG ve artroskopi yapılarak, MRG ve artroskopi bulguları karalaştırılmıştır. Olguların 121'i erkek, 59'u kadın olup ortalama yaş 33.2 idi. Çalışmamızda, MRG'nin doğruluk oranları, Medial menisküs yırtıkları için %80, lateral menisküs için %90.8, Ön çapraz bağ için %97.8 olarak bulunmuştur. Menisküs yırtıklarının ve diz bağ yaralanmalarının tanısında MRG ve artroskopi uyumunun iyi olması nedeni ile MRG'nin diz problemlerinin tanısında güvenle kullanılabilecek noninvazif bir yöntem olduğunu düşünüyoruz.

The circular frame of Ilizarov fixator could be modified due to anatomical variations and treatment aims. But these modifications in the frame system cannot always obtain the mechanical performance of the standard frame system. A standard system has two rings in each bone fragment which are connected by four longitudinal bars on each side of the ring. In this study, mechanical performance of one standard and eight modified frame systems were compared. Each system was loaded on a Material Testing Machine with axial compression, four point bending and torsion, and overall stiffness were tested. As a result, systems that were modified with drop wires and Schanz screws 45¡ oblique to the wires on the proximal ring, provided a closer mechanical performance to standard system., Ilizarov frame sistemi, anatomik bölgeye, yapılara ve tedavi amacına göre değişik modifikasyonlarla dü-zenlenebilmektedir. Ancak frame konfigürasyonunda yapılan değişiklikler standart frame sisteminin (her ke-mik fragmanında ikişer tam halka ve 4 rodla tesbit) mekanik performansım her zaman sağlayamamaktadır. Bu çalışmada biri standart ve sekizi standart üzerinde yapılan değişikliklerle hazırlanmış toplam dokuz fra-me sistemi mekanik performansları açısından karşılaştırılmıştır. Materyal test makinesi ile yapılan aksiyel kompresyon, dört nokta bükülme, torsiyon testleri sonucu, proksimal halkadaki tellere 45° açıyla uygulanan drop tel veya Schanz vidası ile kurulan sistemlerin standart frame sistemine yakın sıkılık sağladığı görülmüş-tür.

Tibialarında defekt oluşturduğumuz erişkin tavşanlarda kaynamanın ve uzunluğun elde edilebilmesi amacıyla eksternal fiksatörler kullanarak kompresyon-distraksiyon ve kemik kaydırma yöntemleri uygulandı. Elde edilen sonuçlar klinik, histolojik ve radyolojik oiarak değerlendirildi. Sonuçta kompresyon-distraksiyon yapılan deneklerde ve kemik kaydırması yapılan deneklerin defekt proksimalinaeki kaydırma bölgesinde yeterli kaynama gorülürken, kaydırma yapılan bazı deneklerin defekt distalinde kaynama yetersiz bulundu., In order to obtain union and lengthening in the tibial defects of adult rabbits; comparison-distraction and bone transport methods were performed by using external fixator. Clinical, histological and radiological evaluation of the results were done. Sufficient union was observed in all rabbits treated with compression-distraction method. While the union in the proximal site of the defect in rabbits treated with transport method was sufficient, the union in the distal site was not sufficient.

From April 1994 to August 1995, thirty consecutive patients who underwent total hip arthroplasty were managed with enoxaparine; in order to evaluate the prophylactic effect of low molecular weight heparin in high risk patients. Patients who were not having appropriate criteria for the usage of enoxaparine excluded from the study. There were 12 (%40) females and 18 (60) males with a mean age of 50.38 (range 40-78). Primary osteoarthritis in 22, rheumatoid arthritis in 6, and dysplastic hip in 2 patients were the indications of total hip replacement. Age, sex, height, weight, total biochemical analysis of each patient and schedule for drug were noted. First dose of enoxaparine was administered 12 hours before the operation and 0.4 ml (40 mg) daily dose was continued up to postoperative eleventh day. Pain, local allergic reactions, haematoma in the injection site and abnormal bleeding from the operation site were followed. No DVT or complication of injection was observed. As a result, prophylaxis of DVT with daily 40 mg of enoxaparine is a feasible, safe and effective procedure in high risk patients who underwent total hip arthroplasty., Derin ven trombozu (DVT) riski taşıyan ve total kalça artroplastisi gibi majör bir ameliyatla bu riskin arttığı hastalarda düşük molekül ağırlıklı heparinin proflaktik etkisini araştırmak amacıyla Nisan, 1994-Ağustos 1995 tarihleri arasında total kalça protezi uygulanan 30 hasta enoksaparin proflaksisine alındı. Hastalarımızın 12 (%40)'sı kadın, 18 (%60)’ı erkek olup yaş ortalaması 50.38 (40-78) idi. 22 hastaya primer osteoartrit, 6 hastaya romatoid artrit. 2 hastaya displazik kalça nedeniyle total kalça protezi uygulaması yapıldı. Hastaların yaş, cins, boy, kilo, tüm kan biyokimyası ile ilacın başlama, dozaj ve kullanım şemaları kaydedildi. Enoksaparin uygulaması operasyondan 12 saat önce başlatıldı ve 0.4 ml (40 mg) gün pozolojisi'nde 11 gün süreyle uygulandı. Hastalar enjeksiyon bölgesinde ağrı, lokal allerjik reaksiyonlar, hematom varlığı, peroperatif ve postoperatif anormal kanamalar açısın-dan takip edildi. Hiçbir hastada derin ven trombozu ya da enjeksiyona bağlı komplikasyon gelişmedi. Sonuç olarak 40 mg/gün enoksaparin ile yapılan derin ven tromboz proflaksisinin total kalça protezi uygulanan hastalar-da kolay uygulanabilen, güvenli ve etkili bir yöntem olduğu sonucuna varıldı.

Fifty-two children with acute septic arthritis of the hip treated from 1976 until 1996 in our clinic were reviewed. Patients were evaluated due to age, infectious agent, duration of symptoms and sequelaes. Joint aspiration material seems to be the most accurate diagnostic parameter. Spesific diagnostic criterias for each age group were determined. The outcome was satisfactory in cases treated within four days of symptoms. Worse prognosis was prominent in cases with sequelaes like septic dislocation and proximal femoral osteomyelitis. Staphylococcus aureus infected hips were having poorer prognosis than other bacteria species. The most important prognostic criterias are age and duration between the initial symptoms and the surgical treatment. An inevitable hip joint destruction could be prevented by early diagnosis, prompt surgical drainage and use of appropriate antibiotics., 1976-1996 yılları arasında kliniğimize başvuran ve kalçada septik artrit tanısı koyularak tedavi edilen 52 hasta çeşitli yönleri ile incelendi. Hastalar yaş grupları, enfeksiyöz ajan, tedavi zamanı ve sökeller açısından değerlendirildi. Tanıda en belirleyici faktörün eklem mayi incelemesi olduğu saptanırken, her yaş grubuna özgü klinik ve laboratuar bulgular belirlendi, ilk 4 gün içinde tedavi edilen hastalarda başarılı sonuçlar alındığı saptandı. Septik çıkık veya proksimal femoral osteomyelit gelişen hastalarda prognoz daha da kötüydü. Staphylococcus aureus ile oluşan enfeksiyonlarda prognoz diğerlerine göre daha kötüydü. En önemli prognostik faktörün yaş ve semptomların başlangıç zamanı ile tedavi arası süre olduğu görüldü. Akut kalça septik artritinde tanının erken konulmasıyla yapılan cerrahi drenaj, uygun antibiyotik tedavisiyle olası bir kalça harabiyetinin önlendiği saptandı.
